  • PRINciples of optimal antithrombotiC therapy and coagulation managEment during elective fenestrated and branched EndovaScular aortic repairS (PRINCE2SS) : An International Expert Based Delphi Consensus Study

    • ObjectiveManagement of antithrombotic therapy in patients undergoing elective fenestrated branched endovascular aortic repair (F-BEVAR) is not standardised, nor are there any recommendations from current guidelines. By designing an international expert based Delphi consensus, the study aimed to create recommendations on the pre-, intra-, and post-operative management of antithrombotic therapy in patients scheduled for elective F-BEVAR in high volume centres.MethodsEight facilitators created appropriate statements regarding the study topic that were voted on, using a four point Likert scale, by a selected panel of international experts using a three round modified Delphi consensus process. Based on the experts’ responses, only those statements reaching Grade A (full agreement ≥ 75%) or B (overall agreement ≥ 80% and full disagreement < 5%) were included in the final document. The round answers’ consistency was graded using Cohen’s k, the intraclass correlation coefficient, and, in case of double re-submission, the Fleiss k.ResultsSixty-seven experts were included in the final analysis and voted the initial 43 statements related to pre- (n = 15), intra- (n = 10), and post-operative (n = 18) management of antithrombotic drugs. At the end of the process, six statements (13%) were rejected, 20 statements (44%) received a Grade B consensus, and 18 statements (40%) reached a Grade A consensus. Most statements (27; 71%) exhibited very high or high consistency grades, and 11 (29%) a fair or poor grading. The intra-operative statements mostly concentrated on threshold for and monitoring of proper heparinisation. The pre- and post-operative statements mainly focused on indications for dual antiplatelet therapy and its management, considering the possible need for cerebrospinal fluid drainage.ConclusionBased on the elevated strength and high consistency of this international expert based Delphi consensus, most of the statements might guide current clinical management of antithrombotic therapy for elective F-BEVAR. Future studies are needed to clarify the debated issues.

  • On electric resistance effects of non-homogeneous GDL deformation in a PEM fuel cell

    • The electric resistance is very important for the performance of a proton exchange membrane (PEM) fuel cell. However, the performance analysis is more complex as the cell operates under assembly conditions. At such conditions, the mass transfer is deteriorated but the electric conductivity is favored. In this paper, the electric resistance of a cell is evaluated by application of a recently developed method in the through-plane direction of the electrodes, together with consideration of the contact resistance between the gas diffusion layer (GDL) and bi-polar plates (BPP) for various assembly pressures. The predicted electric resistance and deformed GDL were implemented in an existing CFD code for evaluation of the PEM fuel cell performance. It is found that the electric current is distributed in a narrow area in the GDL under the shoulders and then redistributed into the BPP above the channels for all cases. The channel/rib structure promotes a non-homogeneous electric conductivity along the cell in the in-plane direction and a concentrated area of the current flow around the corner of the BPP close to the channels as the cell is subject to an assembly pressure. Additional contact areas are created between the GDL and BPP at the vertical interface when the cell operates at an assembly pressure above 2 . MPa. Therefore, both the corner of a BPP close to the channel and the GDL region become the dominating zones, where the electric current under the middle of the channel must cross over a longer distance due to the intrusion of the GDL into the BPP. In addition, the optimized cell performance is obtained as the cell is operating below 1 . MPa assembly pressure. The findings are useful for proper design of PEM fuel cells.

  • Personal and demographic factors and change of subjective indoor air quality reported by school children in relation to exposure at Swedish schools : A 2-year longitudinal study

    • This paper studies changes in subjective indoor air quality (SIAQ) among school children and relates these data to repeated exposure measurements during a two-year follow-up period. Data on SIAQand demographic information were gathered by a questionnaire sent to 1476 primary and secondary school pupils in 39 randomly selected schools at baseline and after two years (follow-up). Exposure measurements were applied after questionnaire data were collected at baseline and follow-up in approximately 100 classrooms. The arithmetic mean values for baseline and follow-up were: for indoor air temperature 23.6 degrees C and 21.8 degrees C and for outdoor air flow rate 5.4 L/s and 7.9 L/s. Older children, those with atopy at baseline, and those in larger schools reported impaired SIAQ during follow-up. Installation of new ventilation systems, higher personal air flow rate and air exchange rate, and better illumination were associated with improved SIAQ, Higher CO2 levels were associated with impaired SIAQ, In conclusion, sufficient ventilation and illumination in classrooms are essential for the perception of good indoor air quality.     • Syftet med denna lav- och svampinventering har varit att undersöka om förekomsten av hotade eller skyddsvärda arter kan motivera en reservatsbildning av de undersökta områdena Fastnäs, Mjönäs, Elindebol, Lernäs, Fiskenäsängen och Äftnäs mellan Ekshärad och Stöllet, men också att generellt ge en ökad kunskap om lav- och svampfloran i Klarälvdalens näs-skogar.Då inventeringen skett under endast en säsong är artlistorna långt ifrån kompletta, men indikerar i alla fall att höga naturvärden finns längs Klarälvdalen och att flera skyddsvärda lavar och svampar här kan ha sina främsta förekomster i Sverige.Lavfloran på näsen är artrik och flera intressanta arter har påträffats. Sammanlagt 176 arter redovisas i rapporten, inklusive en del lavväxande svampar (parasiter eller parasymbionter). Av lavarna är 7 arter klassificerade som hotade (Databanken 1991). Dessutom har några andra mycket sällsynta och skyddsvärda arter påträffats.Lavfloran präglas av flera nordliga arter, t ex Calicium adaequatum, mörkhövdad spiklav, och Physconia detersa. Inga utpräglade sydliga arter är påträffade. Av övriga arter märks bl a en lavväxande Arthonia-art som är ny för vetenskapen.Svampfloran är också artrik och särpräglad med 302 registrerade arter varav 13 är hotklassificerade.Arter med både sydlig och nordlig prägel förekommer.Av tickorna kan nämnas fynden av mycket sällsynta arter som Perenniporia subacida och Trametes suaveolens, sydlig anisticka. En annan ticka är den i Sverige nyligen upptäckta Antrodiella americana som noterats från flera näs.Bland marksvamparna kan nämnas många fynd av den sällsynta larvklubban Cordyceps bifusispora och broskskivlingen Marasmius siccus, veckbrosking, båda med tidigare endast få fynd i Sverige.En del insamlat material återstår ännu att bestämma.Vi anser att samtliga sex näs är skyddsvärda. Av dessa har de fyra näsen Mjönäs, Lernäs, Fiskenäsängen och Äftnäs mycket stort skyddsvärde, medan Elindebol har stort skyddsvärde.

    • The gamma p -> K-0 Sigma(+) reaction has been measured from threshold to E-gamma = 1.45 GeV (W-CM = 1.9 GeV) using the Crystal Ball and TAPS multiphoton spectrometers together with the photon tagging facility at the Mainz Microtron MAMI. In the present experiment, this reaction was searched for in the 3 pi(0)p final state, by assuming K-S(0) -> pi(0)pi(0) and Sigma(+) -> pi(0)p. The experimental results include total and differential cross sections as well as the polarization of the recoil hyperon. The new data significantly improve empirical knowledge about the gamma p -> K-0 Sigma(+) reaction in the measured energy range. The results are compared to previous measurements and model predictions. It is demonstrated that adding the present gamma p -> K-0 Sigma(+) results to existing data allowed a better description of this reaction with various models.
