- The challenges of today's society call for more knowledge about how to maintain all aspects of cognitive health, such as speed/attention, memory/learning, visuospatial ability, language, executive capacity and social cognition during the life course.Medical advances have improved treatments of numerous diseases, but the cognitive implications have not been sufficiently addressed. Disability induced by cognitive dysfunction is also a major issue in groups of patients not suffering from Alzheimer's disease or related disorders. Recent studies indicate that several negative lifestyle factors can contribute to the development of cognitive impairment, but intervention and prevention strategies have not been implemented. Disability due to cognitive failure among the workforce has become a major challenge. Globally, the changing aging pyramid results in increased prevalence of cognitive disorders, and the diversity of cultures influences the expression, manifestation and consequences of cognitive dysfunction.Major tasks in the field of cognitive medicine are basic neuroscience research to uncover diverse disease mechanisms, determinations of the prevalence of cognitive dysfunction, health-economical evaluations, and intervention studies. Raising awareness for cognitive medicine as a clinical topic would also highlight the importance of specialized health care units for an integrative approach to the treatment of cognitive dysfunctions.

Formativ utvärdering av Projekt neuropsykiatri i närsjukvård, Nordöstra Skånes sjukvårdsdistrikt : tentativa resultat

- Syftet med utvärderingen har varit att beskriva hur Projekt neuropsykiatri i närsjukvård har genomförts under dess första två år. I arbetet med utvärderingen har speciellt fokus lagts på en del av projektet; endagarsutbildningarna. Hur har dessa upplevts av deltagarna? Har de fått de önskade effekterna? Vad kunde ha gjorts annorlunda? Projektets andra delar har mer indirekt berörts i utvärderingen.Som grund för insamling av data har observationer i samband med endagarsutbildningarna använts, liksom kursutvärderingar och informella samtal med såväl kursdeltagare som personal från Minnesmottagningen. Dessutom har strukturerade intervjuer genomförts med tre sjuksköterskor.Nyckelinformanter är personalen på Minnesmottagningen, distriktsläkare, sjuksköterskor samt biståndshandläggare. Dessa upplever utbildningarna som positiva och lärorika och att de kan leda till en ökad kunskap om demens. Samverkan mellan kommun och Region Skåne upplevs ha ökat till följd av utbildningarna. En ursprunglig förhoppning fanns att utbildningarna skulle leda till att kunskapen sprids vidare ut i organisationerna. Resultatet av utvärderingen tyder emellertid på att detta inte har skett, åtminstone inte i den utsträckningen som önskats av Minnesmottagningens personal. En orsak till den begränsade kunskapsspridningen kan vara att utbildningsdeltagarna ej har någon pedagogisk grund att stå på. En annan faktor som kan ha verkat hindrande för kunskapsspridningen är att det inom huvudmannaorganisationerna saknas en gemensam värdegrund för synen på demens.De tentativa resultaten av utvärderingen tyder på att projektet i stort har fått en rad positiva följder för vårdpersonalen. Förutom en ökad samverkan mellan kommun och Region Skåne har samverkan med olika enheter inom Centralsjukhuset Kristianstad ökat, liksom med Neuropsykiatriska kliniken i Malmö. I studien framkommer att personal från såväl hälso- och sjukvården som från den kommunala omsorgen vet att de med förtroende kan vända sig till Minnesmottagningen, något som visar sig ha lett till en ökad trygghet. Projektet har även fått till följd att antalet diagnoser samt användningen av bromsmediciner har ökat.Utvärderingen identifierar ett antal möjliga hinder för en god demensvård, bland annat vårdens hierarkiska struktur, bristen på gemensamt förhållningssätt gentemot demenssjuka samt att tilliten till experterna på Minnesmottagningen riskerar att öka deras arbetsbörda i stället för den önskade motsatta utvecklingen.Denna utvärdering har fokuserat på hur vårdpersonalen upplever projektets verksamhet. I en eventuell fortsatt utvärdering av verksamheten föreslås att fokus läggs på hur kunskap och erfarenheter sprids i verksamheterna samt hur projektet påverkar patienterna och deras anhöriga.

Everyday activities in older adults readmitted to hospital

- The coordination of health care and social care at discharge are particularly important for older adults with complex health and social needs, as approximately 30 % of these persons are readmitted to hospital on an unplanned basis within three months of discharge. In order to provide adequate care for this group of older adults, we need a deeper understanding of their experiences regarding their functional ability at hospital and at home.The overall aim of this thesis was to describe older readmitted adults in terms of their abilities at hospital, everyday activities at home and of healthcare professionals’ conception of the care of older adults. Four studies were conducted using descriptive design. Data were gathered using four different methods of data collection. In this thesis, older readmitted adults are, defined as persons aged 75 years or older who have been readmitted to hospital on an unplanned basis three or more times within 12 months. In Study I, older adults readmitted to hospital (n=60), were assessed regarding personal activities of daily living, cognitive function, risk of falls, pressure sores and malnutrition. They were compared with samples from the general population. In order to describe everyday activities at home, semi-structured interviews were carried out with older readmitted adults (n=16) in Study II, and with close relatives (n=20) in Study III. In Study IV, focus group interviews were performed with healthcare professionals (n=29) about care at discharge and which measures that may be appropriate in order to prevent readmission.The major findings were that the participants had a higher risk of pressure sores, falls and dependency on ADL at hospital compared to age-matched samples. 40% of the participants had a cognitive limitation. At home, older adults strived to perform everyday activities but the ability was limited. Close relatives described that social relations were fundamental to perform everyday activities and social activities at home. Healthcare professionals stated that older readmitted adults were vulnerable. Multidisciplinary teamwork and geriatric expertise were fundamental but were perceived as lacking. Medical resources and rehabilitation resources should be developed and include rapid follow-up appointments at home in order to prevent readmission.Overall, health care and social care must identify this group and offer preventive assessments and interventions.

- Angiotensin II has been suggested to influence central dopamine and serotonin turnover. Since the angiotensin-converting enzyme (ACE) plays a key role in angiotensin regulation by converting inactive angiotensin 1 to active angiotensin II, we hypothesised that the functional insertion/deletion (I/D) polymorphism in the ACE gene, which has previously been suggested to be associated with, depression and panic disorder, may influence monoamine activity. A well-established technique for assessing brain monoamine turnover in humans is to measure concentrations of monoamine metabolites in the cerebrospinal fluid (CSF). We thus investigated possible associations between the ACE I/D polymorphism and CSF monoamine metabolite concentrations in a population of healthy male subjects. After having found such an association between the ACE I/D polymorphism and CSF levels of the dopamine metabolite homovanillic acid and the serotonin metabolite 5-hydroxyindoleacetic acid in this sample, I carriers displaying lower levels, we tried to replicate this observation in a population of violent male offenders from which also both CSF and DNA were available. Also in this sample, the same associations were found. Our results suggest that the ACE I/D polymorphism may play a role in the modulation of serotonergic and dopaminergic turnover in men. - A randomized trial was conducted comparing 10 individual weekly sessions of cognitive behaviour therapy for panic disorder (PD) with or without agoraphobia with a 10-module self-help program on the Internet. After confirming the PD diagnosis with an in-person structured clinical interview (SCID) 49 participants were randomized. Overall, the results suggest that Internet-administered self-help plus minimal therapist contact via e-mail can be equally effective as traditional individual cognitive behaviour therapy. Composite within-group effect sizes were high in both groups, while the between-group effect size was small (Cohen's d=16). One-year follow-up confirmed the results, with a within-group effect size of Cohen's d=0.80 for the Internet group and d=0.93 for the live group. The results from this study generally provide evidence to support the continued use and development of Internet-distributed self-help programs.

Mötet mellan process och komponent : mot ett ramverk för en verksamhetsnära kravspecifikation vid anskaffning av komponentbaserade informationssystem

- Denna avhandling är en kvalitativ studie av hur organisationer kan förenkla och förbättra arbetet med kravspecificering vid anskaffning av komponentbaserade informationssystem. Avhandlingens utgångspunkt är att mer verksamhetsnära och användbara informationssystem kan anskaffas genom att använda en integrerad verksamhetsmodell där verksamhetsanalys och kravspecificering slås samman. Fyra kunskapsområden har studerats och analyserats för att generera ett ramverk som riktar uppmärksamhet mot relevant innehåll och utformning av en integrerad verksamhetsmodell, 1) verksamhetsprocesser 2) verksamhetsmodellering 3) mjukvarukomponenter och 4) specificering. Avhandlingens empiri representeras av en rekonstruktion av den komponent- och processorienterade anskaffning av ett komplett vårdinformationssystem som genomförts i Landstinget i Värmland. Analyser och respektive kunskapsområde har skett utifrån ansatsen Multi-Grounded Theory vilket innebär en reorigenerering utifrån en växelverkan mellan existerande teorier och empirisk grundning. Några exempel på kunskapsbidrag är ett vidareutvecklat processbegrepp samt en ny livscykelmodell för komponentbaserade informationssystem. Utifrån kunskapsbidragen inom respektive kunskapsområde har integrationssynpunkter identifierats som legat till grund för utveckling av ramverket för en verksamhetsnära kravspecifikation. I avhandlingen dras slutsatsen att genom att integrera arbetet med verksamhetsanalys och kravspecificering skapas bättre förutsättningar för att kravspecifikationer speglar de behov systemen är avsedda att uppfylla och att gapet mellan krav och lösning minskar genom ett bättre underlag för kommunikation mellan kund och systemleverantör. Ramverket pekar på att en verksamhetsnära kravspecifikation utformas i en integrerad processmodell som beskriver verksamhetsprocesser utifrån identifierade generiska processbegrepp samt ett scenario för användning och klassificering av funktionalitet. Kravspecifikationen bör, enligt ramverket, även innehålla en systemöversikt, kommersiella och leverantörsrelaterade krav, design- och arkitektoniska krav samt utförandekrav. Ramverket har prövats och vidareutvecklats utifrån en extern validering i form av en utvärderingsworkshop där ramverkets relevans och användbarhet diskuterats utifrån de erfarenheter och den dokumentation som genererats i Landstinget i Värmland.

- Background: Since giving birth is a profound life experience, it is important to understand the impact of the physical and psychosocial environment on women´s birth processes. As part of the Room4Birth research project, a birthing room at a labour ward in Sweden was redesigned to increase its potential to be adapted to personal needs, reduce stress, enable the endogenous oxytocin release and thereby facilitate a healthy labour and birth process. Aim: To explore the influence and meaning of the birthing room for nulliparous women. Methods: Ethnographic fieldwork was conducted, including five months of participant observations of nulliparous women (n = 16) labouring in either a regular birthing room (n = 8) or the redesigned birthing room (n = 8). Reflective memos and informal interviews with women, companions, and care providers were transcribed and in-depth interviews with eight of the women were conducted two to seven months after birth. An ethnographic iterative process was used to analyse the data. Results: We identified the birthing room as consisting of the physical space, the human interaction, and cultural practices within it. The Birth Manual was an identified analytic concept for managing birth according to the organisational authority that was incorporated into the birthplace. The conforming of this manual affected the atmosphere, birth practices, and role disposition in the room. The results describe how authoritative guidance of birth processes can disposition women as passive participants, but also how midwives can enable a birth environment where women experience safety and a sense of agency. Conclusions: Regardless of the design of the room, a power imbalance between birthing women’s needs and organisational demands was identified. This emphasises the need to modify the physical design of birth units, but more importantly, to incorporate a care philosophy that promotes childbirth physiology and agency of birthing women.
