    • BACKGROUND: Subarachnoid hemorrhage is a condition with high rates of mortality and morbidity. The diagnosis requires an urgent cerebral computed tomography scan and also a lumbar puncture if the scan fails to demonstrate intracranial blood. In Sweden the cerebrospinal fluid (CSF) is analyzed by spectrophotometric scanning for the presence of hemoglobin and bilirubin. The aim of the study was to develop a quantitative diazo reagent based analysis of cerebrospinal fluid bilirubin as a replacement for spectrophotometric scanning. METHODS: The CSF bilirubin assay on an Architect C8000 chemistry analyzer was compared with spectrophotometry using patient samples. RESULTS: The method correlates with spectrophotometry, has a good linearity and precision. CONCLUSIONS: Quantitative bilirubin measurement offers shorter turnaround times, simplifies the interpretation of the results and reduces work load in comparison with spectrophotometry.

  • Healthy ecosystems for human and animal health : Science diplomacy for responsible development in the Arctic

    • Climate warming is occurring most rapidly in the Arctic, which is both a sentinel and a driver of further global change. Ecosystems and human societies are already affected by warming. Permafrost thaws and species are on the move, bringing pathogens and vectors to virgin areas. During a five-year project, the CLINF - a Nordic Center of Excellence, funded by the Nordic Council of Ministers, has worked with the One Health concept, integrating environmental data with human and animal disease data in predictive models and creating maps of dynamic processes affecting the spread of infectious diseases. It is shown that tularemia outbreaks can be predicted even at a regional level with a manageable level of uncertainty. To decrease uncertainty, rapid development of new and harmonised technologies and databases is needed from currently highly heterogeneous data sources. A major source of uncertainty for the future of contaminants and infectious diseases in the Arctic, however, is associated with which paths the majority of the globe chooses to follow in the future. Diplomacy is one of the most powerful tools Arctic nations have to influence these choices of other nations, supported by Arctic science and One Health approaches that recognise the interconnection between people, animals, plants and their shared environment at the local, regional, national and global levels as essential for achieving a sustainable development for both the Arctic and the globe.

  • An oral antisense oligonucleotide for PCSK9 inhibition

    • Inhibitors of proprotein convertase subtilisin/kexin type 9 (PCSK9) reduce low-density lipoprotein (LDL) cholesterol and are used for treatment of dyslipidemia. Current PCSK9 inhibitors are administered via subcutaneous injection. We present a highly potent, chemically modified PCSK9 antisense oligonucleotide (ASO) with potential for oral delivery. Past attempts at oral delivery using earlier-generation ASO chemistries and transient permeation enhancers provided encouraging data, suggesting that improving potency of the ASO could make oral delivery a reality. The constrained ethyl chemistry and liver targeting enabled by N-acetylgalactosamine conjugation make this ASO highly potent. A single subcutaneous dose of 90 mg reduced PCSK9 by >90% in humans with elevated LDL cholesterol and a monthly subcutaneous dose of around 25 mg is predicted to reduce PCSK9 by 80% at steady state. To investigate the feasibility of oral administration, the ASO was coformulated in a tablet with sodium caprate as permeation enhancer. Repeated oral daily dosing in dogs resulted in a bioavailability of 7% in the liver (target organ), about fivefold greater than the plasma bioavailability. Target engagement after oral administration was confirmed by intrajejunal administration of a rat-specific surrogate ASO in solution with the enhancer to rats and by plasma PCSK9 and LDL cholesterol lowering in cynomolgus monkey after tablet administration. On the basis of an assumption of 5% liver bioavailability after oral administration in humans, a daily dose of 15 mg is predicted to reduce circulating PCSK9 by 80% at steady state, supporting the development of the compound for oral administration to treat dyslipidemia.

  • Unbroken Digital Data Flow In The Built Environment Process : A Case Study In Sweden

    • An unbroken digital data flow would save substantial resources in the built environment process. In this study, which is part of a larger Swedish project, data delivery specifications and methods to integrate BIM and geodata are developed and tested with the aim to facilitate such an unbroken data flow. The main focus areas of the study are: (1) specifications that enables building permission applications based on BIM data to automate the building permission process, (2) reuse of as-built BIM models to update geodata when a building is constructed and (3) a national Swedish CityGML ADE for buildings. The study shows that building permission applications can be partly automated even though the delivery specifications were in the early stages of development at the time of a performed test case. With fully implemented delivery specifications more regulations can be checked. Furthermore, the study demonstrates how a BIM model can be georeferenced with a standard deviation of the transformation of 3 cm compared to field measurements performed with a total station. The georeferenced BIM model can then be converted to a LOD2 geodata building model to update existing geodata. Finally, a proposal for a national Swedish CityGML ADE for buildings is presented.

    • Studiens syfte var att undersöka hur många föräldrar som efter förlossningen önskade kontinuitet mellan graviditet och nyföddhetsperiod genom ett hembesök av den barnmorska som man lärt känna under graviditeten, hur dessa kvinnor och män formulerade sina erfarenheter och behov, om de skilde sig från föräldrar som inte önskade hembesök och om resultatet kunde ge synpunkter på en individualiserad hälsovård i nyföddhetsperioden. Önskan om bekräftelse, traumatiska förlossningsupplevelser, osäkerhet i relationen till barnet, påfrestningar och krissituationer kunde avslöjas i probandgrup¬pen som önskade hembesök (15% av inskrivna vid en mödravårdscentral). I jämförelse med en kontrollgrupp förekom fler av annan nationalitet än svensk och fler belastande omständigheter i probandgruppen: fler kvinnor med kroppsliga och psykiska besvär och fler parförhållanden med bristande samförstånd. Metoden kan ge riktlinjer för individualiserad hälsovård med möjlighet att bekräfta och stödja föräldrar, utifrån deras egna önskemål, i det tidiga samspelet med barnet.

  • Korttidspsykoterapi med kvinnor och män i samband med graviditet och nyföddhetsperiod

    • Korttidspsykoterapi med sex kvinnor och sex män i samband med graviditets- och post partumperioden analyserades med narrativ ansats och med anknytningsteori och affektteori som grund. Syftet med studien var att skaffa kunskap om psykiska besvär hos blivande och nyblivna föräldrar och att undersöka om korttidspsykoterapi med kvinnor och män i samband med barnafödandet i förebyggande syfte kunde avlasta dem. Anknytningsmönster och affektiva tillstånd kartlades genom att, i den narrativa analysen, använda Adult Attachment Interview (AAI) respektive Affect Consciousness Interview (ACI). De perceptgenetiska testen DMT och PORT användes för att få en oberoende uppfattning om kvinnornas och männens anknytningsmönster och affektiva tillstånd. Samtliga kvinnor och män företedde brister i eget anknytningsmönster; traumatiska livshändelser avslöjades och ”skam-förödmjukelse” var den affekt som dominerade psykoterapiberättelserna både i början och slutet av terapin. Det fanns en tydlig skillnad i medvetenhet om och förmåga att uttrycka affekten i slutet av psykoterapin jämfört med i början av terapin. Antagandet om att det tematiska innehållet i psykoterapiberättelserna skulle domineras av parförhållandet, den egna identiteten, förhållandet till det kommande eller nyfödda barnet och till egna föräldrar bekräftades. Resultaten visade att korttidspsykoterapi i samband med barnafödandet möjliggjorde en ”reflekterande funktion” i relation till det tematiska innehållet i terapin, det egna anknytningsmönstret och livshändelser samt höjde den affektiva medvetenhetsnivån vilket bör öka möjligheten till och gynna förutsättningen för anknytningen till och det affektiva samspelet med barnet.

  • När kvinnor och män får barn : Ett psykologiskt och könsspecifikt betraktande av psykisk hälsa och ohälsa

    • The central issue is to increase knowledge of women's and men's mental wellbeing when their child is about to be born, as seen from four aspects relevant to maternity and child health care centres. Major psychological theories are the attachment theory and the affect theory, health psychology with theories on nervous tension, vulnerability, coping and individual resources, clinical psychology to understand mental divergencies, crisis theory to understand reactions, taking the individual's subjective experience and personal definitions of reactions and needs into consideration. Major concepts are the reflective function, RF, empowerment, positioning and intersubjectivity. Study I: Epistemological approach. Gender similarities are: Mental condition post-partum, practical arrangements, organizing the first days at home after delivery, pregnancy issues in the last trimester, existential issues to come to an understanding of what is happening. Gender differences: women ask about pains at giving birth and about breastfeeding, men ask about the child and its development, the role of the man at delivery and as father, the pair relationship and parenthood. Study II: Professional support. Fifteen percent took advantage of an offered home visit after delivery, by a midwife familiar from pregnancy. The study demonstrates a model for individualized healthcare. The experiment group differed from the control group in that more individuals had a mother tongue other than Swedish, more dysfunctional pair relationships, more women with somatic difficulties and a tendency to more mental problems. The visit revealed a need for positive confirmation, traumatic experiences of giving birth, crises in the pair relationship, social isolation and distress caused by death of a close relative or friend. Study III: Mental crisis and incidence of emergency visits to psychiatric clinics. The incidence group (women and men expecting a child or with a child aged 0-17 months) constituted 1.23% of all emergency visits to psychiatric clinics and 1.1% of the total parents expecting or with a child 0-17 months. More women than men and more people with Swedish as their mother tongue received planned psychotherapy. More men were given pharmacological treatment. Study IV: Psychotherapy. Short-term psychotherapy results were studied, 6 women and 6 men, before and after childbirth. The percept-genetic tests PORT and DMT were used as control instruments for diagnostic purposes. The psychotherapy narratives were analysed from a gender perspective. Dominant themes for women were first the pair relationship, secondly their own identity. With the men their own identity came first, the pair relationship second. All participants showed an insecure attachment, confirmed by the percept-genetic results; 3 women and 3 men showed disorganized patterns. Attachment experiences differed between women and men. Shame and humiliation were dominant in therapies with both women and men. Significantly less negative expressions of affect were found in the later period of the therapies and an increased consciousness of affective expressions and ability to express emotions in a constructive way.
