  • Expectation and experiences of childbirth in primiparae with caesarean section

    • Objective: The aim of this study was to examine the expectations and experiences in women undergoing a caesarean section on maternal request and compare these with women undergoing caesarean section with breech presentation as the indication and women who intended to have vaginal delivery acting as a control group. A second aim was to study whether assisted delivery and emergency caesarean section in the control group affected the birth experience. Design: A prospective group-comparison cohort study. Setting: Danderyd Hospital, Stockholm, Sweden. Sample: First-time mothers (n = 496) were recruited to the study in week 37-39 of gestation and follow up was carried out 3 months after delivery. Comparisons were made between 'caesarean section on maternal request', 'caesarean section due to breech presentation' and 'controls planning a vaginal delivery'. Methods: The instrument used was the Wijma Delivery Expectancy/Experience Questionnaire (W-DEQ). Main outcome measures: Expectations prior to delivery and experiences at 3 months after birth. Results: Mothers requesting a caesarean section had more negative expectations of a vaginal delivery (P < 0.001) and 43.4% in this group showed a clinically significant fear of delivery. Mothers in the two groups expecting a vaginal delivery, but having an emergency caesarean section or an assisted vaginal delivery had more negative experiences of childbirth (P < 0.001). Conclusions: Women requesting caesarean section did not always suffer from clinically significant fear of childbirth. The finding that women subjected to complicated deliveries had a negative birth experience emphasises the importance of postnatal support. © 2008 The Authors.

    • BACKGROUND: This study examines relations between regional cerebral blood flow (rCBF) and neuropsychological test results, age at onset and duration of disease in patients with frontotemporal-type dementia (FTD).METHOD: Sixteen patients with a diagnosis of probable FTD were examined using single photon emission computerised tomography (SPECT) with 99mTc-HMPAO as the tracer. The rCBF of 14 regions of interest relative to cerebellar blood flow was calculated. Psychological tests assessing language, verbal fluency, memory and visuospatial constructive ability were given.RESULTS: Correlations were demonstrated between a global impairment score and relative blood flow in lateral frontal, medial frontal and left orbital frontal areas. Verbal fluency scores correlated with left lateral frontal, medial frontal and left anterior inferior temporal blood flow. No relationships between decrease in CBF and age at onset or duration of disease, or between impaired cognitive function and age at onset or duration of disease were found.CONCLUSIONS: The present study demonstrates a close coupling between reduced rCBF and specific neuropsychological deficits in FTD.

    • This chapter analyses how secondhand consumption and vintage fashion retailers are incorporated into Swedish urban life and the retail planning context. The chapter is part of an ongoing research project, studying retail planning and vintage fashion consumption practices. Key concepts when discussing retail and localization are attractiveness and accessibility. Cities are often characterised by their unique qualities and the spirit of creativity. In the “experience society”, retail and (fashion) consumption are regarded as everyday activities for identity development and communication. Goods/services have become value-creating components of a hedonistic fashion consumption culture that is characterised by an increasingly more knowledgeable and demanding fashion consumer. Secondhand and vintage fashion represents an economic grey area and an important part of the informal economy. The existence of both types of retailer within an urban space can communicate specific values and imagery. Stores, which sell pre-owned goods and dedicate themselves to re-use and charity, have their own specific historiography. In the Western welfare context, however, re-use and secondhand goods obtain a specific significance via the fashion transformation of the pre-owned, which took place during the post-war era. In place development and retail planning, there is trust in the potential of fashion, sustainability and creativity. This finds specific expression when it comes to the production of urban fashion retail space. This chapter sheds light on the complex relationship between retail planning and the production of green vintage fashion space. By identifying how some Swedish secondhand and vintage fashion markets are organised, the study contributes to an understanding of the conditions under which secondhand and vintage fashion markets operate.

    • Mass spectrometric experiments show that protonated mixed ammonia/water clusters predominant exist in three forms namely H+(NH3)(4)(H2O)(n), H+(NH3)(5)(H2O)(n), and H+(NH3)(6)(H2O)(n) (n = 1-25). For the first two series the collisional activation mass spectra are dominated by loss of water, whereas ions of the latter series preferably lose ammonia. The quantitative characteristics of these observations are reproduced by quantum chemical calculations that also provide insight into the geometrical structures of the clusters. Although the experiments and the calculations agree that clusters with five ammonia are thermodynamically preferred, this does not indicate a rigid tetrahedral structure with one central ammonium covered with an inner solvation shell of four ammonia molecules, with water outside, Instead, water and ammonia have comparable affinities to the binding sites of the first shell, with a preference for ammonia for the first two sites, and water for the last two. The "leftover" ammonia molecules bind equally strong as water molecules to sites in the second shell due to synergistic hydrogen binding. Finally, it is discussed whether the observation of enhanced stability of the H+(NH3)(5)(H2O)(20) in terms of magic numbers and associated geometries may be related to a tetrahedral ammonium core encapsulated in a dodecahedral (H2O)(20) structure, typically found in clathrates.

    • Here we report on the gas-phase interactions between protonated enantiopure amino acids (l- and d-enantiomers of Met, Phe, and Trp) and chiral target gases [(R)- and (S)-2-butanol, and (S)-1-phenylethanol] in 0.1-10.0 eV low-energy collisions. Two major processes are seen to occur over this collision energy regime, collision-induced dissociation and ion-molecule complex formation. Both processes were found to be independent of the stereo-chemical composition of the interacting ions and targets. These data shed light on the currently debated mechanisms of gas-phase chiral selectivity by demonstrating the inapplicability of the three-point model to these interactions, at least under single collision conditions.

DesignQualitative study of nulliparous pregnant womens narratives before and after CBT. SettingThe first ICBT programme for treating severe fear of childbirth. SampleFifteen nulliparous pregnant Swedish women with severe fear of childbirth participating in an ICBT self-help programme. MethodsSemi-structured open-ended questions over the internet before and after 8weeks of ICBT. The data were analysed using thematic analysis. Main outcome measuresThe participants narratives pertaining to five different situations during labour and delivery before and after ICBT. ResultsAfter therapy, participants described a more realistic attitude towards imminent childbirth, more self-confidence and more active coping strategies. They perceived their partners and the staff as more supportive. They were more aware of the approaching meeting with their baby when giving birth. ConclusionsFollowing the ICBT programme, participants changed their attitude towards imminent childbirth from negative to more positive. This was manifested in positive and more realistic expectations regarding themselves, their partner and the staff that would look after them.

    • In patients with severe head injuries raised intracranial pressure (ICP) constitutes the most important cause of mortality. Several new therapies for increased ICP have recently been suggested and it is of importance to study the physiological effects of these treatments in animal experiments during steady state conditions. A porcine model for evaluation of cerebral haemodynamics and metabolism during increased ICP is presented. Intracranial hypertension was induced by inflation of two tonometric gastric balloons placed extradurally covering a major part of the parietooccipital region bilaterally. The distribution of the blood flow supplied by the carotid artery used for the cerebral blood flow (CBF) measurements was studied by intraarterial (i.a.) injection of 99mTc-HMPAO. The measurements showed that following ligation of the external carotid and the occipital artery no accumulation of tracer substance occurred in extracranial tissues during normal or increased ICP. Cerebral physiological variables (CBF, Cavo2, and ICP) were measured 5, 20 and 60 min after induction of intracranial hypertension. The results confirm that the experimental situation gives a reproducible increase in ICP (25-28 mm Hg) and that the physiological variables remain stable during the period of intracranial hypertension. We conclude that the model simulates the effects of an acute intracranial focal mass and is well suited for the evaluation of different pharmacological therapies of increased ICP.
