    • Over the last decades the weight has increased in the Norwegian population. Report no 16 to the Storting, “Prescription for a healthier Norway”, expresses concern for this development and defines important areas of commitment to prevent a further increase in health problems like diabetes 2 and other lifestyle related diseases. The foci described above are all justified by figures and statistical surveys. The dominant approach towards lifestyle related diseases is closely connected with Western medicine. The main tendency in the recommendations is primarily presenting overweight and obesity as something negative and problematic. This study focuses on self-assessment and self-perception of people that are overweight as well as those of normal weight. The aim is to reveal the collective understanding in a local community that struggles to avoid a further weight-gain in its population. This thesis seeks to, shed light on how the weight- evolvement in the Norwegian society influence peoples individual self-image and body perception. A local community in Norway (“Libygda”) defined by media, some years ago, as the most overweight local community in the country, constitutes the basis for this thesis.Methods like fieldwork, multistage focus group interviews and individual interviews (all carried out in “Libygda”) were used. The main focus in all the approaches is the bottom up strategy gained through the research approach Co-operative Inquiry which implies that both researcher and participants together search to gain insight and new experiential knowledge. In this case about individual health perception, body and identity. The data material is analyzed mainly through Life Mode-Theory, and theories of social capital (and other theories from the Social Science field).Methodological and theoretical approaches within Public Health Work are mainly based in Health Science, whereas this thesis has Social Science (Sociology and Social Anthropology) as its point of departure. The thesis will by its perspective aim to complement the traditional Health Science and offer a supplemental approach to the Science of Public Health.This thesis describes how experience-based perspectives on health and body exist side by side with the medical and Public Health Science perspective. This implies that there exists a parallel understanding of reality, which is rarely focused on by health professionals. If Public Health Work should gain a greater impact on peoples’ everyday life, these experience based and folk perspectives must be taken into consideration and accepted by professionals as relevant.

    • Avhandlingen, baserad på intervjuer med 58 studenter vid tre olika program vid Högskolan i Halmstad, behandlar de skäl olika studentgrupper beskriver för att börja studera, deras upplevelser av mötet med högskolemiljön, och vilken betydelse de en lokal högskola kan ha. Trots olika åtgärder var den sociala snedrekryteringen till högre utbildning i stort sett oförändrad mellan 1960 och 1990. Under 1990-talet skedde en utjämning, men 1999 kom fortfarande bara 24% av studenterna vid svenska universitet från arbetarklasshem, trots att de utgjorde 35% av hela befolkningen. Studenter med högutbildade föräldrar uppger att de egentligen inte träffar ett aktivt val att börja högre studier, det uppfattas snarare som en naturlig del av vuxenblivandet. Barn från hem utan utbildningstraditioner ger en annan beskrivning. De beskriver valet att utbilda sig som ett val bland flera andra, och att de fått stöd från föräldrarna oavsett vad de valt. För barn till lågutbildade var det ofta fråga om en särskild händelse som gjorde att utbildning blev ett aktuellt alternativ. I studien har det varit möjligt att jämföra hur både klassresenärer och arvtagare beskriver mötet med och vistelsen på högskolan. Båda grupperna beskriver hur de upplever miljön som förvirrande och otrygg. Arvtagarna utvecklar emellertid strategier för att hantera situationen, eftersom den är svårare att ifrågasätta för dessa - utbildningen är ju en naturlig del av vuxenblivandet. Klassresenärerna är däremot mer benägna att ifrågasätta studierna. Klassresenärerna beskriver hur de upplever att de förändras ifråga om språk och förhållningssätt under studierna, männen i mer positiva ordalag, kvinnor med en viss tvehågsenhet.

    • This thesis focuses on the work in primary health care enterprises within two municipals which have organised their services after new organisational models attuned to New Public Management ideologies. The organisational model, i.e. the purchaser – provider model, had features in common with tayloristic principles, including the separation between planning and execution of work, and it brought about a loss of opportunities for the employee’s flexibility. For this reason, the employee's control over their own work was focus of the study. Method The data are based on a comparative intensive case study of four organizations, i.e. enterprises that performed nursing and care services. The respondents in the study were home assistants, care workers, auxiliary nurses and registered nurses; all of which performed the work, i.e. practical assistance and primary health care services. The data was collected using participant observation, individual interviews and focus group interviews. Findings Employees often did more work than was expected by organisational standards. This work is called “hidden services” and categorised as organisational misbehaviour, since it was neither expected nor desired by the organisation. Five types of work were performed in addition to their expected work: surplus work, additional work, forbidden work, inappropriate basic work and collective work. The rationale for doing so diverged among occupations, since different occupations called upon dissimilar types of rules to legitimize their misbehaviour. Three types of rules legitimized the overriding of organisational rules, all based on distinctive work relations among employees, including employees’ work relations to clients. First, there were situational rules based on informal work relations with clients – situational work relations. Although these rules were established across all occupations in the study, situated rules were most active in long-lasting work relations between employees and clients. Second, there were collective rules, developed in consequence of employees’ social position at the workplace, bringing employees together in work teams and thereby establishing collective work relations. Collective rules included rules that modified organizational rules on how to provide service to clients, and rules that legitimized the breaking of administrative rules. Finally, professional rules, as a result of professionals’ socialisation through formal education and work relations among professionals at the workplace, provided the last distinct type of rules to legitimize organisational misbehaviour. Conclusion The thesis concludes that there are distinct work relations in the social structure of organisations that explains employees' execution of hidden services. Formal, private, collective and professional work relations are part of the social structure in organisations. These work relations generates mechanisms, i.e. norms, that modifies and legitimises the work in primary health care services

  • Negotiating needs : Processing older persons as home care recipients in gerontological social work practices

    • The study concerns the needs assessment processes that older persons undergo to gain access to home care. The participation of older persons, their relatives and municipal care managers was studied from a communicative perspective. The assessment meetings functions as formal problem-solving events. The older persons´ accounts are negotiated discursively in interaction. Various storylines are used by the older persons and their relatives whether they view home care as an intrusion, as a complement or as a right. In case of divergent opinions the older person has the final say as prescribed by the Swedish social service act. One conclusion is that the role of relatives is not defined and a family perspective is not present. In the study the institutional structure of the assessment process was also analyzed. Older persons are processed into clients; their needs are fitted within the framework of documentation and institutional categories. In the transfer of talk to text all the particulars are not reflected and two types of documentation was identified; a fact-oriented objective language or an event-oriented personal language. Care management models and a managerialist thinking has influenced the assessment process by bureaucratisation of older people trough people processing, which is in contradiction to the individual-centric perspective prescribed by the law. The introduction of care management models in gerontological social work has lead to an embedded contradiction and constitutes a welfare political dilemma. Improved communicative methods are needed in order to achieve a holistic assessment situation.

    • Temporary Work Agencies (TWAs) are an example of a trend towards flexibility in the labour market. By hiring staff from TWAs organisations can keep their staff numbers down. This however, refers to the client companies´ use or temps, and omits the intermediary, the TWA, which also needs various flexibility strategies in its activities. The main purpose of this thesis is to study the organising of TWAs supply of manpower to different clients companies, and how managers and temps experience their terms of employment, working conditions and relationships with regular employees at the client companies. Two qualitative case studies were conducted. In study one, interviews have been carried out with managers and temps in a TWA and a client company in Jämtland. In the second study, managers and temps in an international TWA in Sweden were interviewed. The results show that both managers and temps work under complex conditions and demands on flexibility. The managers shall, by flexible prioritising, satisfy demands from the TWA on profitability, demands from client companies´ for an adaptable workforce and temps need of employment and community. The results also indicate that temps have to balance between a position as stranger and outsider, and handle insecurity of the assignments duration and spatial location. They have to be able to manoeuvre between the power inequalities, stigmatisation and exclusion they encounter in working conditions and social interactions in client companies. To handle such situations the ideal temp and manager have to be able to vary between distance and nearness in spatiality and indifference and involvement in social relations.

  • Aktiv anpassning till förändrade villkor : Fem kvinnors erfarenheter av kroniskt nedsatt funktionsförmåga och återskapande av ett meningsfullt liv

    • Studiens övergripande syfte var att utveckla kunskap om kvinnors erfarenheter av att i vuxen ålder drabbas av och leva med kroniskt nedsatt fysisk funktionsförmåga. Utöver studiens beskrivande och kunskapsgenererande syfte och frågeställningar hade denna även en ambition att bidra till utvecklingen av forskningsområdet ”hälsa och livsstil”. Studien baserades på aktiva intervjuer med fem kvinnor som i vuxen ålder drabbats av nedsatt funktionsförmåga, på grund av kronisk sjukdom eller skada. Analysen skedde i flera steg som successivt gav en breddad och fördjupad förståelse av kvinnornas berättelser. I ett första steg genomfördes en tematisk analys av innehållet i kvinnornas berättelser. Den tematiska analysen sammanfattades av hur kvinnorna själva genom olika retoriska stilgrepp betonade särskilt viktiga delar av sina berättelser. Det andra steget av analysen belyste berättelserna utifrån tidigare forskning och den symboliska interaktionismens perspektiv. Att studiens resultat relaterades till tidigare forskning och ett teoretiskt sammanhang ökade dess överförbarhet ytterligare. Studiens resultat låg i linje med tidigare forskning, det bekräftade bl.a. tidens betydelse under omställningen till ett liv med nedsatt funktionsförmåga, där kontrollen över livet återskapades genom planering av tillvaron så att krafterna räckte till det som upplevdes som mest angeläget, eller genom att krympa tiden till ett här och nu, och ta en dag i sänder och avskärma sig från osäkerheten inför framtiden. Det fanns även fynd som gick utöver vad tidigare forskning rapporterat. De kvinnor som ingick i studien rekonstruerade ur tidigare erfarenheter en ny identitet, med en livsstil i balans mellan omgivningen och den faktiska funktionsförmågan. Förhållanden som bidrog till en hälsosam livsstil var organiseringen i stödjande nätverk, och engagemang i ideella organisationer i vilka kvinnorna kunde uppleva den medkänsla som de stundtals sade sig sakna i mötet med vårdgivare och myndigheter.De kvinnor som medverkade i studien framstod som mer aktiva och kreativa än som var vanligt bland kvinnor med nedsatt funktionsförmåga i tidigare studier. Kvinnorna i studien berättade hur de tog initiativ till, planerade och genomförde längre livsprojekt, som satsningar på studier och eget företagande, vilket inte rapporterats vara vanligt i tidigare studier. Några kvinnor visade sin självständighet genom att inte villkorslöst acceptera vårdapparatens och myndigheternas beslut, utan motsatte sig sådant som motverkade deras strävan att förverkliga personliga mål. Kvinnorna stärkte självkänslan och utvecklade alternativa livsstilar och identiteter genom att ta vara på tidigare erfarenheter och ur dem konstruera nya livsstilar, präglade av relativ hälsa och välmående. Det skedde t.ex. genom att utveckla ett idrottsintresse i gemenskap med andra inom handikappidrotten, eller genom att vårda ett fräscht och ungdomligt utseende och bekräftas genom att medverka som modell vid modevisningar. Studiens resultat har betydelse för att stärka unga kvinnor, som drabbats av nedsatt funktionsförmåga i vuxenålder, men även för yrkesverksamma som möter kvinnor i samma situation och deras anhöriga.
