    • Introduction Technical and logical breakthroughs have provided new opportunities in medicine to use knowledge bases and large-scale clinical data (real-world) at point-of-care as part of a learning healthcare system to diminish the knowledge-practice gap. Areas covered The article is based on presentations, discussions and recommendations from an international scientific workshop. Value, research needs and funding avenues of knowledge bases and access to real-world data as well as transparency and incorporation of patient perspectives are discussed. Expert opinion Evidence-based, publicly funded, well-structured and curated knowledge bases are of global importance. They ought to be considered as a public responsibility requiring transparency and handling of conflicts of interest. Information has to be made accessible for clinical decision support systems (CDSS) for healthcare staff and patients. Access to rich and real-world data is essential for a learning health care ecosystem and can be augmented by data on patient-reported outcomes and preferences. This field can progress by the establishment of an international policy group for developing a best practice guideline on the development, maintenance, governance, evaluation principles and financing of open-source knowledge bases and handling of real-world data.

  • Segmentation of glioblastomas in early post-operative multi-modal MRI with deep neural networks.

    • Extent of resection after surgery is one of the main prognostic factors for patients diagnosed with glioblastoma. To achieve this, accurate segmentation and classification of residual tumor from post-operative MR images is essential. The current standard method for estimating it is subject to high inter- and intra-rater variability, and an automated method for segmentation of residual tumor in early post-operative MRI could lead to a more accurate estimation of extent of resection. In this study, two state-of-the-art neural network architectures for pre-operative segmentation were trained for the task. The models were extensively validated on a multicenter dataset with nearly 1000 patients, from 12 hospitals in Europe and the United States. The best performance achieved was a 61% Dice score, and the best classification performance was about 80% balanced accuracy, with a demonstrated ability to generalize across hospitals. In addition, the segmentation performance of the best models was on par with human expert raters. The predicted segmentations can be used to accurately classify the patients into those with residual tumor, and those with gross total resection.

    • Here we show that the TATA-binding protein (TBP) is localized in the nucleoplasm and in the nucleolus of mammalian cells, consistent with its known involvement in transcription by RNA polymerase I, II, and III. In the nucleolus of actively growing cells, TBP colocalizes with upstream binding factor (UBF) and RNA polymerase I at the sites of rRNA transcription. During mitosis, when rRNA synthesis is down-regulated, TBP colocalizes with TBP-associated factors for RNA polymerase I (TAF(I)s), UBF, and RNA polymerase I on the chromosomal regions containing the rRNA genes. Treatment of cells with a low concentration of actinomycin D inhibits rRNA synthesis and causes a redistribution of the rRNA genes that become concentrated in clusters at the periphery of the nucleolus. A similar redistribution was observed for the major components of the rRNA transcription machinery (i.e., TBP, TAF(I)s, UBF, and RNA polymerase I), which still colocalized with each other. Furthermore, anti-TBP antibodies are shown to coimmunoprecipitate TBP and TAF(I)63 in extracts prepared from untreated and actinomycin D-treated cells. Collectively, the data indicate that in vivo TBP/promoter selectivity factor, UBF, and RNA polymerase I remain associated with both active and inactive rRNA genes.

    • Ram pressure stripping is perhaps the most efficient mechanism for removing gas and quenching galaxies in dense environments, as they move through the intergalactic medium. Extreme examples of on-going ram pressure stripping are known as jellyfish galaxies, characterized by a tail of stripped material that can be directly observed in multiple wavelengths. Using the largest homogeneous broad-band optical jellyfish candidate sample in local clusters known to date, we measure the angle between the direction of the tails visible in the galaxies, and the direction towards the host cluster centre. We find that 33 per cent of the galaxy tails point away from the cluster centre, 18 per cent point towards the cluster centre, and 49 per cent point elsewhere. Moreover, we find stronger signatures of ram pressure stripping happening on galaxies with a tail pointing away and towards the cluster centre, and larger velocity dispersion profiles for galaxies with tails pointing away. These results are consistent with a scenario where ram pressure stripping has a stronger effect for galaxies following radial orbits on first infall. The results also suggest that in many cases, radially infalling galaxies are able to retain their tails after pericenter and continue to experience significant on-going ram pressure stripping. We further constrain the lifespan of the optical tails from the moment they first appear to the moment they disappear, by comparing the observed tail directions with matched N-body simulations through Bayesian parameter estimation. We obtain that galaxy tails appear for the first time at ∼ 1.16R200 and disappear ∼ 660 Myr after pericenter.
