  • Successfully REBOA performance: does medical specialty matter? International data from the ABOTrauma Registry

Background: Resuscitative endovascular balloon occlusion of the aorta (REBOA) is a minimally invasive procedure being increasingly utilized to prevent patients with non-compressible torso hemorrhage from exsanguination. The increased use of REBOA is giving rise to discussion about “Who is and who should be performing it?” Methods: Data from the international ABO (aortic balloon occlusion) Trauma Registry from between November 2014 and April 2020 were analyzed concerning the question: By who, how, and where is REBOA being performed? The registry collects retrospective and prospective data concerning use of REBOA in trauma patients. Results: During the study period, 259 patients had been recorded in the registry, 72.5% (n = 188) were males with a median (range) age of 46 (10-96) years. REBOA was performed in the ER in 50.5%, in the OR in 41.5%, and in the angiography suite in 8% of patients. In 54% of the patients REBOA was performed by surgeons (trauma surgeons 28%, vascular surgeons 22%, general surgeons 4%) and in 46% of the patients by non-surgeons (emergency physicians 31%, radiologists 9.5%, anesthetists 5.5%). Common femoral artery (CFA) access was achieved by use of external anatomic landmarks and palpation alone in 119 patients (51%), by cutdown in 57 patients (24%), using ultrasound in 49 patients (21%), and by fluoroscopy in 9 patients (4%). Significant differences between surgeons and non-surgeons were found regarding patient’s age, injury severity, access methods, place where REBOA was performed, location patients were taken to from the emergency room, and mortality. Conclusion: A substantial number of both surgical and non-surgical medical disciplines are successfully performing REBOA to an almost equal extent. Surgical cutdown is used less frequently as access to the CFA compared with reports in older literature and puncture by use of external anatomic landmarks and palpation alone is used with a high rate of success. Instead of discussing “Who should be performing REBOA?” future research should focus on “Which patient benefits most from REBOA?”

    • Ultrahigh frequencies (UHF) have applications in signal and power electronics to minimize product sizes, increase production quantity and lower manufacturing cost. In the UHF range of 300 MHz to 3 GHz, ferrimagnetic iron oxides (ferrites) are especially useful because they combine the properties of a magnetic material with that of an electrical insulator. Ferrites have much higher electrical resistivity than metallic ferromagnetic materials, resulting in minimization of the eddy current losses, and total penetration of the electromagnetic (EM) field. Hence ferrites are frequently applied as circuit elements, magnetic storage media like read/write heads, phase shifters and Faraday rotators. The electromagnetic properties of ferrites are affected by operating conditions such as field strength, temperature and frequency. The spinel system ZnxCo1-xFe2O4 (x= 0.0, 0.25, 0.50 and 0.75) has been prepared by the standard solid state sintering method. X-ray and neutron powder diffraction measurements were performed at room temperature. Neutron diffraction data analysis confirms the cubic symmetry corresponding to the space group Fd3m. The distribution of three cations Zn2+, Co2+ and Fe3+ over the spinel lattice and other crystallographic parameters like lattice constant, oxygen position parameter, overall temperature factor and occupancies of different ions in different lattice sites for the samples have been determined from the analysis of neutron diffraction data.     • Construction graduates are currently in high demand by contractors in all types of construction work. The diversity of employment opportunities for construction management graduates, increase the need in having a portfolio for skills to work efficiently and effectively with other professions in the industry. Consequently, there are numerous TVET institutes that potentially produce local workforce as highly skilled workers in the construction industry. However, it is believed that, there is a persisting lack of generic competency among the construction graduates and an absence of strategic framework for Malaysian construction graduates. Therefore, this research investigates generic competencies required by construction graduates in the management scopes from the perspectives of the construction practitioners. Drawing on a construction graduates generic competencies and roles, and tasks framework, a structured questionnaire survey was used to elicit data from construction practitioners in the industry. The data was analyzed using Rasch Measurement Model with PCA, item and person reliability and item measure analysis. Result from this research indicates that industries require graduates to perform more on analytical and critical thinking in the project management phase. Moreover, in the construction management phase, it has been found that industries expect construction graduates to perform more on teamwork competency. Furthermore, this research identified that site management phase request more on problem-solving and teamwork competency from construction graduates. Therefore, it is believe that these research findings will be useful in assisting the universities and industries to improve the generic competency amongst construction students by providing valuable insight and expanding current knowledge-base of those elements and variables required by industries.
