  • Design and Baseline Characteristics of the Finerenone in Reducing Cardiovascular Mortality and Morbidity in Diabetic Kidney Disease Trial

    • <b><i>Background:</i></b> Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. <b><i>Patients and</i></b> <b><i>Methods:</i></b> The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate ≥25 mL/min/1.73 m<sup>2</sup> and albuminuria (urinary albumin-to-creatinine ratio ≥30 to ≤5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level α = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. <b><i>Conclusions:</i></b> FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.   • Roadmap for naming uncultivated Archaea and Bacteria

    • The assembly of single-amplified genomes (SAGs) and metagenome-assembled genomes (MAGs) has led to a surge in genome-based discoveries of members affiliated with Archaea and Bacteria, bringing with it a need to develop guidelines for nomenclature of uncultivated microorganisms. The International Code of Nomenclature of Prokaryotes (ICNP) only recognizes cultures as 'type material', thereby preventing the naming of uncultivated organisms. In this Consensus Statement, we propose two potential paths to solve this nomenclatural conundrum. One option is the adoption of previously proposed modifications to the ICNP to recognize DNA sequences as acceptable type material; the other option creates a nomenclatural code for uncultivated Archaea and Bacteria that could eventually be merged with the ICNP in the future. Regardless of the path taken, we believe that action is needed now within the scientific community to develop consistent rules for nomenclature of uncultivated taxa in order to provide clarity and stability, and to effectively communicate microbial diversity. In this Consensus Statement, the authors discuss the issue of naming uncultivated prokaryotic microorganisms, which currently do not have a formal nomenclature system due to a lack of type material or cultured representatives, and propose two recommendations including the recognition of DNA sequences as type material.

  • Pheromone-Induced Morphogenesis Improves Osmoadaptation Capacity by Activating the HOG MAPK Pathway

    • Environmental and internal conditions expose cells to a multiplicity of stimuli whose consequences are difficult to predict. We investigate the response to mating pheromone of yeast cells adapted to high osmolarity. Events downstream of pheromone binding involve two mitogen-activated protein kinase (MAPK) cascades: the pheromone response (PR) and the cell wall integrity (CWI) response. Although the PR MAPK pathway shares components with a third MAPK pathway, the high osmolarity (HOG) response, each one is normally only activated by its cognate stimulus, a phenomenon called insulation. We found that in cells adapted to high osmolarity, PR activated the HOG pathway in a pheromone- and osmolarity-dependent manner. Activation of HOG by the PR was not due to loss of insulation, but rather a response to a reduction in internal osmolarity, which resulted from an increase in glycerol release caused by the PR. By analyzing single-cell time courses, we found that stimulation of HOG occurred in discrete bursts that coincided with the "shmooing" morphogenetic process. Activation required the polarisome, the CWI MAPK Slt2, and the aquaglyceroporin Fps1. HOG activation resulted in high glycerol turnover, which improved adaptability to rapid changes in osmolarity. Our work shows how a differentiation signal can recruit a second, unrelated sensory pathway to fine-tune yeast response in a complex environment.

  • Simplifying prediction of disease progression in pre-symptomatic type 1 diabetes using a single blood sample

    • AIMS/HYPOTHESIS: Accurate prediction of disease progression in individuals with pre-symptomatic type 1 diabetes has potential to prevent ketoacidosis and accelerate development of disease-modifying therapies. Current tools for predicting risk require multiple blood samples taken during an OGTT. Our aim was to develop and validate a simpler tool based on a single blood draw.METHODS: Models to predict disease progression using a single OGTT time point (0, 30, 60, 90 or 120 min) were developed using TrialNet data collected from relatives with type 1 diabetes and validated in independent populations at high genetic risk of type 1 diabetes (TrialNet, Diabetes Prevention Trial-Type 1, The Environmental Determinants of Diabetes in the Young [1]) and in a general population of Bavarian children who participated in Fr1da.RESULTS: Cox proportional hazards models combining plasma glucose, C-peptide, sex, age, BMI, HbA1c and insulinoma antigen-2 autoantibody status predicted disease progression in all populations. In TrialNet, the AUC for receiver operating characteristic curves for models named M60, M90 and M120, based on sampling at 60, 90 and 120 min, was 0.760, 0.761 and 0.745, respectively. These were not significantly different from the AUC of 0.760 for the gold standard Diabetes Prevention Trial Risk Score, which requires five OGTT blood samples. In TEDDY, where only 120 min blood sampling had been performed, the M120 AUC was 0.865. In Fr1da, the M120 AUC of 0.742 was significantly greater than the M60 AUC of 0.615.CONCLUSIONS/INTERPRETATION: Prediction models based on a single OGTT blood draw accurately predict disease progression from stage 1 or 2 to stage 3 type 1 diabetes. The operational simplicity of M120, its validity across different at-risk populations and the requirement for 120 min sampling to stage type 1 diabetes suggest M120 could be readily applied to decrease the cost and complexity of risk stratification.

  • Coinfections identified from metagenomic analysis of cervical lymph nodes from tularemia patients

    • Background: Underlying coinfections may complicate infectious disease states but commonly go unnoticed because an a priori clinical suspicion is usually required so they can be detected via targeted diagnostic tools. Shotgun metagenomics is a broad diagnostic tool that can be useful for identifying multiple microbes simultaneously especially if coupled with lymph node aspirates, a clinical matrix known to house disparate pathogens. The objective of this study was to analyze the utility of this unconventional diagnostic approach (shotgun metagenomics) using clinical samples from human tularemia cases as a test model. Tularemia, caused by the bacterium Francisella tularensis, is an emerging infectious disease in Turkey. This disease commonly manifests as swelling of the lymph nodes nearest to the entry of infection. Because swollen cervical nodes are observed from many different types of human infections we used these clinical sample types to analyze the utility of shotgun metagenomics.Methods: We conducted an unbiased molecular survey using shotgun metagenomics sequencing of DNA extracts from fine-needle aspirates of neck lymph nodes from eight tularemia patients who displayed protracted symptoms. The resulting metagenomics data were searched for microbial sequences (bacterial and viral).Results: F. tularensis sequences were detected in all samples. In addition, we detected DNA of other known pathogens in three patients. Both Hepatitis B virus (HBV) and Human Parvovirus B-19 were detected in one individual and Human Parvovirus B-19 alone was detected in two other individuals. Subsequent PCR coupled with Sanger sequencing verified the metagenomics results. The HBV status was independently confirmed via serological diagnostics, despite evading notice during the initial assessment.Conclusion: Our data highlight that shotgun metagenomics of fine-needle lymph node aspirates is a promising clinical diagnostic strategy to identify coinfections. Given the feasibility of the diagnostic approach demonstrated here, further steps to promote integration of this type of diagnostic capability into mainstream clinical practice are warranted.

    • Prompted by an increased interest of both research participants and the patient advocacy community in obtaining information about research outcomes and on the use of their biological samples; the international community has begun to debate the emergence of an ethical 'duty' to return research results to participants. Furthermore, the use of new technologies (e.g., whole-genome and -exome sequencing) has revealed both genetic data and incidental findings with possible clinical significance. These technologies together with the proliferation of biorepositories, provide a compelling rationale for governments and scientific institutions to adopt prospective policies. Given the scarcity of policies in the context of stem cell research, a discussion on the scientific, ethical and legal implications of disclosing research results for research participants is needed. We present the International Stem Forum Ethics Working Party's Policy Statement and trust that it will stimulate debate and meet the concerns of researchers and research participants alike.

  • Gridmapping the Northern Plains of Mars: A New Overview of Recent Water‐ and Ice‐Related Landforms in Acidalia Planitia.

    • We used a grid‐mapping technique to analyze the distribution of 13 water‐ and ice‐related landforms in Acidalia Planitia as part of a joint effort to study the three main basins in the northern lowlands of Mars, that is, Acidalia, Utopia, and Arcadia Planitiae. The landforms were mapped at full Context Camera resolution along a 300‐km‐wide strip from 20°N to 84°N. We identified four landform assemblages: (1) Geologically recent polar cap (massive ice), which superposes the latitude‐dependent mantle (LDM) (LA1); (2) ice‐related landforms, such as LDM, textured terrain, small‐scale polygons, scalloped terrain, large‐scale viscous flow features, and gullies, which have an overlapping distribution (LA2); (3) surface features possibly related to water and subsurface sediment mobilization (LA3; kilometer‐scale polygons, large pitted mounds, small pitted mounds, thumbprint terrain); and (4) irregularly shaped pits with raised rims on equator‐facing slopes. Pits are likely the result of an energetic release of volatiles (H2O, CO2, and CH4), rather than impact‐, volcanism‐, or wind‐related processes. LDM occurs ubiquitously from 44°N to 78°N in Acidalia Planitia. Various observations suggest an origin of air fall deposition of LDM, which contains less ice in the uppermost tens of meters in Acidalia Planitia than in Arcadia and Utopia Planitiae. However, LDM may be thicker and more extended in the past in Acidalia Planitia. The transition between LDM‐free terrain and LDM is situated further north than in Utopia and Arcadia Planitiae, suggesting different past and/or present climatic conditions among the main basins in the northern lowlands.

    • A project of mapping ice‐related landforms was undertaken to understand the role of subsurface ice in the northern plains. This work is the first continuous regional mapping from CTX (ConTeXt Camera, 6 m/pixel; Malin et al., 2007) imagery in Arcadia Planitia along a strip 300 km across stretching from 30°N to 80°N centered on the 170°W line of longitude. The distribution and morphotypes of these landforms were used to understand the permafrost cryolithology. The mantled and textured signatures occur almost ubiquitously between 35°N and 78°N and have a positive spatial correlation with inferred ice stability based on thermal modeling, neutron spectroscopy, and radar data. The degradational features into the LDM (latitude‐dependent mantle) include pits, scallops, and 100‐m polygons and provide supporting evidence for subsurface ice and volatile loss between 35 and 70°N in Arcadia with the mantle between 70 and 78°N appearing much more intact. Pitted terrain appears to be much more pervasive in Arcadia than in Acidalia and Utopia suggesting that the Arcadia study area had more widespread near‐surface subsurface ice and thus was more susceptible to pitting or that the ice was less well buried by sediments. Correlations with ice stability models suggest that lack of pits north of 65–70°N could indicate a relatively young age (~1 Ma); however, this could also be explained through regional variations in degradation rates. The deposition of the LDM is consistent with an air fall hypothesis; however, there appears to be substantial evidence for fluvial processes in southern Arcadia with older, underlying processes being equally dominant with the LDM and degradation thereof in shaping the landscape.

    • Snow cover plays a major role in the climate, hydrological and ecological systems of the Arctic and other regions through its influence on the surface energy balance (e.g. reflectivity), water balance (e.g. water storage and release), thermal regimes (e.g. insulation), vegetation and trace gas fluxes. Feedbacks to the climate system have global consequences. The livelihoods and well-being of Arctic residents and many services for the wider population depend on snow conditions so changes have important consequences. Already, changing snow conditions, particularly reduced summer soil moisture, winter thaw events and rain-on-snow conditions have negatively affected commercial forestry, reindeer herding, some wild animal populations and vegetation. Reductions in snow cover are also adversely impacting indigenous peoples' access to traditional foods with negative impacts on human health and well-being. However, there are likely to be some benefits from a changing Arctic snow regime such as more even run-off from melting snow that favours hydropower operations.

    • Global mean near surface temperature change is the key metric by which our warming climate is monitored and for which international climate policy is set. At the end of each year the Met Office issues a global mean temperature forecast for the coming year. Following on from the new record in 2023, we predict that 2024 will likely (76% chance) be a new record year with a 1-in-3 chance of exceeding 1.5 degrees C above pre-industrial. Whilst a one-year temporary exceedance of 1.5 degrees C would not constitute a breach of the Paris Agreement target, our forecast highlights how close we are now to this. Our 2024 forecast is primarily driven by the strong warming trend of +0.2 degrees C/decade (1981-2023) and secondly by the lagged warming effect of a strong tropical Pacific El Ni & ntilde;o event. We highlight that 2023 itself was significantly warmer than the Met Office DePreSys3 forecast, with much of this additional observed warming coming from the southern hemisphere, the cause of which requires further understanding.

  • Causes of irregularities in trends of global mean surface temperature since the late 19th century

    • The time series of monthly global mean surface temperature (GST) since 1891 is successfully reconstructed from known natural and anthropogenic forcing factors, including internal climate variability, using a multiple regression technique. Comparisons are made with the performance of 40 CMIP5 models in predicting GST. The relative contributions of the various forcing factors to GST changes vary in time, but most of the warming since 1891 is found to be attributable to the net influence of increasing greenhouse gases and anthropogenic aerosols. Separate statistically independent analyses are also carried out for three periods of GST slowdown (1896–1910, 1941–1975, and 1998–2013 and subperiods); two periods of strong warming (1911–1940 and 1976–1997) are also analyzed. A reduction in total incident solar radiation forcing played a significant cooling role over 2001–2010. The only serious disagreements between the reconstructions and observations occur during the Second World War, especially in the period 1944–1945, when observed near-worldwide sea surface temperatures (SSTs) may be significantly warm-biased. In contrast, reconstructions of near-worldwide SSTs were rather warmer than those observed between about 1907 and 1910. However, the generally high reconstruction accuracy shows that known external and internal forcing factors explain all the main variations in GST between 1891 and 2015, allowing for our current understanding of their uncertainties. Accordingly, no important additional factors are needed to explain the two main warming and three main slowdown periods during this epoch.

  • High predictive skill of global surface temperature a year ahead

    • [1] We discuss 13 real-time forecasts of global annual-mean surface temperature issued by the United Kingdom Met Office for 1year ahead for 2000–2012. These involve statistical, and since 2008, initialized dynamical forecasts using the Met Office DePreSys system. For the period when the statistical forecast system changed little, 2000–2010, issued forecasts had a high correlation of 0.74 with observations and a root mean square error of 0.07°C. However, the HadCRUT data sets against which issued forecasts were verified were biased slightly cold, especially from 2004, because of data gaps in the strongly warming Arctic. This observational cold bias was mainly responsible for a statistically significant warm bias in the 2000–2010 forecasts of 0.06°C. Climate forcing data sets used in the statistical method, and verification data, have recently been modified, increasing hindcast correlation skill to 0.80 with no significant bias. Dynamical hindcasts for 2000–2011 have a similar correlation skill of 0.78 and skillfully hindcast annual mean spatial global surface temperature patterns. Such skill indicates that we have a good understanding of the main factors influencing global mean surface temperature.

    • The origin of eukaryotes represents an enigmatic puzzle, which is still lacking a number of essential pieces. Whereas it is currently accepted that the process of eukaryogenesis involved an interplay between a host cell and an alphaproteo-bacterial endosymbiont, we currently lack detailed information regarding the identity and nature of these players. A number of studies have provided increasing support for the emergence of the eukaryotic host cell from within the archaeal domain of life, displaying a specific affiliation with the archaeal TACK superphylum. Recent studies have shown that genomic exploration of yet-uncultivated archaea, the so-called archaeal 'dark matter', is able to provide unprecedented insights into the process of eukaryogenesis. Here, we provide an overview of state-of-the-art cultivation-independent approaches, and demonstrate how these methods were used to obtain draft genome sequences of several novel members of the TACK superphylum, including Lokiarchaeum, two representatives of the Miscellaneous Crenarchaeotal Group (Bathyarchaeota), and a Korarchaeum-related lineage. The maturation of cultivation-independent genomics approaches, as well as future developments in next-generation sequencing technologies, will revolutionize our current view of microbial evolution and diversity, and provide profound new insights into the early evolution of life, including the enigmatic origin of the eukaryotic cell.
