  • Cardio-Hepatic Interaction in Cardiac Amyloidosis

    • Background: Congestion is associated with poor prognosis in cardiac amyloidosis (CA). The cardio-hepatic interaction and the prognostic impact of secondary liver affection by cardiac congestion in CA are poorly understood and require further characterisation. Methods: Participants of the amyloidosis cohort study AmyKoS at the Interdisciplinary Amyloidosis Centre of Northern Bavaria with proven transthyretin (ATTR-CA) and light chain CA (AL-CA) underwent serial work-up including laboratory tests, echocardiography, and in-depth hepatic assessment by vibration-controlled transient elastography (VCTE) and 13C-methacetin breath test. Results: In total, 74 patients with AL-CA (n = 17), ATTR-CA (n = 26) and the controls (n = 31) were analysed. ATTR-CA patients showed decreased microsomal liver function expressed by maximal percentage of dose rate (PDRpeak) related to hepatic congestion. Reduced PDRpeak in AL-CA could result from altered pharmacokinetics due to changed hepatic blood flow. Liver stiffness as a combined surrogate of chronic liver damage and congestion was identified as a predictor of all-cause mortality. Statistical modelling of the cardio-hepatic interaction revealed septum thickness, NT-proBNP and PDRpeak as predictors of liver stiffness in both CA subtypes; dilatation of liver veins and the fibrosis score FIB-4 were only significant for ATTR-CA. Conclusions: Non-invasive methods allow us to characterise CA-associated hepatic pathophysiology. Liver stiffness might be promising for risk stratification in CA.

  • Incremental prognostic utility of congestion markers in cardiac transthyretin amyloidosis

    • Background/aims: Congestion is prognostically relevant in cardiac transthyretin amyloidosis (ATTR-CA), but whether congestion has an incremental prognostic value beyond the well-established, congestion-sensitive NT-proBNP is unknown. Therefore, we aimed to comparatively evaluate the prognostic utility of several congestion surrogates over NT-proBNP. Methods: We estimated hazard ratios by Cox proportional hazards regressions with time-varying covariates from a panel data set of the local amyloidosis cohort study AmyKoS. Different models were compared by using chi(χ)2-statistics measuring overall model significance. Results/conclusion: 131 ATTR-CA patients (wild-type 84.0%, hereditary 6.9%, without genetic testing 9.2%; median age 78.7 (quartiles 73.3, 82.1) years; 85.5% male) with 566 observations across a median follow-up of 38.2 (30.6; 48.2) months were analyzed. 83.2% received disease-modifying treatment; 20.6% participated concurrently in placebo-controlled gene silencer trials. Information on congestion improved biomarker-driven risk stratification and identified patients at the highest risk. Echocardiographic congestion markers performed better than clinical findings and daily diuretic use/dosage. Relevant adjusters were daily diuretic dosage, disease-modifying treatment, eGFR, and right atrial volume. NT-proBNP and the tricuspid regurgitation peak velocity (tr-vmax) provided an easy-to-use stratification with overall model performance similar to NAC and Mayo staging systems. Further analyses are necessary for validation and to identify the optimal cut points of the congestion markers. Graphical abstract: (Figure presented.)

  • Liver stiffness as a prognostic parameter and tool for risk stratification in advanced cardiac transthyretin amyloidosis

    • Background: In light of increasing therapeutic options, risk stratification of advanced cardiac transthyretin amyloidosis (ATTR-CA) is gaining clinical importance to avoid ineffective treatments. Liver stiffness as a marker of hypervolemia and hepatic congestion might predict mortality in advanced ATTR-CA and allow to identify patients at highest risk. Methods: Proven ATTR-CA patients underwent repeated vibration-controlled transient elastography (VTCE) and standardized serial workup within the local amyloidosis cohort study AmyKoS. Spearman correlation analyses and Cox regressions were performed to evaluate the prognostic value. Results: 41 patients with ATTR-CA were included with median age of 76.6 (55.1–89.1) years, of which 90.2% were male and > 92% wild-type ATTR-CA. In total, 85 VCTE examinations were performed. Median follow-up was 43.7 (2.4–75.6) months; 26.8% of the patients died. At the first clinical evaluation, median left ventricular (LV) absolute global longitudinal strain (GLS) was 11.4 (5.2–19.0) % and median liver stiffness was 6.3 (2.4–22.9) kPa, both significantly correlated with mortality. NT-proBNP possessed statistically significant predictive power in ATTR-CA with more preserved LV function (absolute GLS ≥ 10), whereas stiffness seemed to be more discriminative for ATTR-CA with absolute GLS < 10. The use of alternative congestion surrogates such as liver vein dilation and tricuspid regurgitation peak velocity (tr-vmax) showed congruent results. Conclusion: Liver stiffness shows prognostic value regarding all-cause mortality and allows risk stratification in advanced ATTR-CA, particularly in those with markedly impaired longitudinal LV function. These results are transferable to other congestion surrogates.

    • SN 2019neq was a very fast evolving superluminous supernova. At a redshift z = 0.1059, its peak absolute magnitude was −21.5 ± 0.2 mag in g band. In this work, we present data and analysis from an extensive spectrophotometric follow-up campaign using multiple observational facilities. Thanks to a nebular spectrum of SN 2019neq, we investigated some of the properties of the host galaxy at the location of SN 2019neq and found that its metallicity and specific star formation rate are in a good agreement with those usually measured for SLSNe-I hosts. We then discuss the plausibility of the magnetar and the circumstellar interaction scenarios to explain the observed light curves, and interpret a nebular spectrum of SN 2019neq using published SUMO radiative-transfer models. The results of our analysis suggest that the spin-down radiation of a millisecond magnetar with a magnetic field B ≃ 6×1014 G could boost the luminosity of SN 2019neq.

    • Non-alcoholic fatty liver disease (NAFLD) is the liver manifestation of the metabolic syndrome with global prevalence reaching epidemic levels. Despite the high disease burden in the population only a small proportion of those with NAFLD will develop progressive liver disease, for which there is currently no approved pharmacotherapy. Identifying those who are at risk of progressive NAFLD currently requires a liver biopsy which is problematic. Firstly, liver biopsy is invasive and therefore not appropriate for use in a condition like NAFLD that affects a large proportion of the population. Secondly, biopsy is limited by sampling and observer dependent variability which can lead to misclassification of disease severity. Non-invasive biomarkers are therefore needed to replace liver biopsy in the assessment of NAFLD. Our study addresses this unmet need. The LITMUS Imaging Study is a prospectively recruited multi-centre cohort study evaluating magnetic resonance imaging and elastography, and ultrasound elastography against liver histology as the reference standard. Imaging biomarkers and biopsy are acquired within a 100-day window. The study employs standardised processes for imaging data collection and analysis as well as a real time central monitoring and quality control process for all the data submitted for analysis. It is anticipated that the high-quality data generated from this study will underpin changes in clinical practice for the benefit of people with NAFLD.     • We report on three optically bright, ∼16th mag, point sources within 10 arcsec of each other that vanished within 1 h, based on two consecutive exposures at Palomar Observatory on 1952 July 19 (POSS I Red and Blue). The three point sources have continued to be absent in telescope exposures during 71 yr with detection thresholds of ∼21st mag. We obtained two deep exposures with the 10.4-m Gran Telescopio Canarias on 2023 April 25 and 27 in r and g band, both reaching magnitude 25.5 (3σ). The three point sources are still absent, implying they have dimmed by more than 10 mag within an hour back in 1952. When bright in 1952, the most isolated transient source has a profile nearly the same as comparison stars, implying the sources are subarcsec in angular size and they exhibit no elongation due to movement. This triple transient has observed properties similar to other cases where groups of transients ('multiple transients') have appeared and vanished in a small region within a plate exposure. The explanation for these three transients and the previously reported cases remains unclear. Models involving background objects that are optically luminous for less than 1 h coupled with foreground gravitational lensing seem plausible. If so, a significant population of massive objects with structure serving as the lenses, to produce three images, are required to explain the subhour transients.

  • A glint in the eye : Photographic plate archive searches for non-terrestrial artefacts

    • In this paper, we present a simple strategy to identify Non-Terrestrial artefacts [NTAs; Haqq-Misra and Kopparapu (2012)] in or near geosynchronous Earth orbits (GEOs). We show that even the small pieces of reflective debris in orbit around the Earth can be identified through searches for multiple transients in old photographic plate material exposed before the launch of first human satellite in 1957. In order to separate between possible false point-like sources on photographic plates from real reflections, we present calculations to quantify the associated probabilities of alignments. We show that in an image with nine "simultaneous transients" at least four or five point sources along a line within a 10 * 10 arcmin(2) image box are a strong indicator of NTAs, corresponding to significance levels of 2.5 to 3.9 sigma. This given methodology can then be applied to set an upper limit to the prevalence of NTAs with reflective surfaces in geosynchronous orbits.

  • Exploring nine simultaneously occurring transients on April 12th 1950

    • Nine point sources appeared within half an hour on a region within similar to 10 arcmin of a red-sensitive photographic plate taken in April 1950 as part of the historic Palomar Sky Survey. All nine sources are absent on both previous and later photographic images, and absent in modern surveys with CCD detectors which go several magnitudes deeper. We present deep CCD images with the 10.4-m Gran Telescopio Canarias, reaching brightness r similar to 26 mag, that reveal possible optical counterparts, although these counterparts could equally well be just chance projections. The incidence of transients in the investigated photographic plate is far higher than expected from known detection rates of optical counterparts to e.g. flaring dwarf stars, Fast Radio Bursts, Gamma Ray Bursts or microlensing events. One possible explanation is that the plates have been subjected to an unknown type of contamination producing mainly point sources with of varying intensities along with some mechanism of concentration within a radius of similar to 10 arcmin on the plate. If contamination as an explanation can be fully excluded, another possibility is fast (t <0.5 s) solar reflections from objects near geosynchronous orbits. An alternative route to confirm the latter scenario is by looking for images from the First Palomar Sky Survey where multiple transients follow a line.

  • Launching the VASCO Citizen Science Project

    • The Vanishing & Appearing Sources during a Century of Observations (VASCO) project investigates astronomical surveys spanning a time interval of 70 years, searching for unusual and exotic transients. We present herein the VASCO Citizen Science Project, which can identify unusual candidates driven by three different approaches: hypothesis, exploratory, and machine learning, which is particularly useful for SETI searches. To address the big data challenge, VASCO combines three methods: the Virtual Observatory, user-aided machine learning, and visual inspection through citizen science. Here we demonstrate the citizen science project and its improved candidate selection process, and we give a progress report. We also present the VASCO citizen science network led by amateur astronomy associations mainly located in Algeria, Cameroon, and Nigeria. At the moment of writing, the citizen science project has carefully examined 15,593 candidate image pairs in the data (ca. 10% of the candidates), and has so far identified 798 objects classified as "vanished". The most interesting candidates will be followed up with optical and infrared imaging, together with the observations by the most potent radio telescopes.
