  • Techniques for mesoappendix transection and appendix resection: insights from the ESTES SnapAppy study

    • Introduction: Surgically managed appendicitis exhibits great heterogeneity in techniques for mesoappendix transection and appendix amputation from its base. It is unclear whether a particular surgical technique provides outcome benefit or reduces complications. Material and methods: We undertook a pre-specified subgroup analysis of all patients who underwent laparoscopic appendectomy at index admission during SnapAppy (ClinicalTrials.gov Registration: NCT04365491). We collected routine, anonymized observational data regarding surgical technique, patient demographics and indices of disease severity, without change to clinical care pathway or usual surgeon preference. Outcome measures of interest were the incidence of complications, unplanned reoperation, readmission, admission to the ICU, death, hospital length of stay, and procedure duration. We used Poisson regression models with robust standard errors to calculate incident rate ratios (IRRs) and 95% confidence intervals (CIs). Results: Three-thousand seven hundred sixty-eight consecutive adult patients, included from 71 centers in 14 countries, were followed up from date of admission for 90days. The mesoappendix was divided hemostatically using electrocautery in 1564(69.4%) and an energy device in 688(30.5%). The appendix was amputated by division of its base between looped ligatures in 1379(37.0%), with a stapler in 1421(38.1%) and between clips in 929(24.9%). The technique for securely dividing the appendix at its base in acutely inflamed (AAST Grade 1) appendicitis was equally divided between division between looped ligatures, clips and stapled transection. However, the technique used differed in complicated appendicitis (AAST Grade 2 +) compared with uncomplicated (Grade 1), with a shift toward transection of the appendix base by stapler (58% vs. 38%; p < 0.001). While no statistical difference in outcomes could be detected between different techniques for division of appendix base, decreased risk of any [adjusted IRR (95% CI): 0.58 (0.41–0.82), p = 0.002] and severe [adjusted IRR (95% CI): 0.33 (0.11–0.96), p = 0.045] complications could be detected when using energy devices. Conclusions: Safe mesoappendix transection and appendix resection are accomplished using heterogeneous techniques. Technique selection for both mesoappendix transection and appendix resection correlates with AAST grade. Higher grade led to more ultrasonic tissue transection and stapled appendix resection. Higher AAST appendicitis grade also correlated with infection-related complication occurrence. Despite the overall well-tolerated heterogeneity of approaches to acute appendicitis, increasing disease acuity or complexity appears to encourage homogeneity of intraoperative surgical technique toward advanced adjuncts.

    • Introduction: The COVID-19 (SARS-CoV-2) pandemic drove acute care surgeons to pivot from long established practice patterns. Early safety concerns regarding increased postoperative complication risk in those with active COVID infection promoted antibiotic-driven non-operative therapy for select conditions ahead of an evidence-base. Our study assesses whether active or recent SARS-CoV-2 positivity increases hospital length of stay (LOS) or postoperative complications following appendectomy. Methods: Data were derived from the prospective multi-institutional observational SnapAppy cohort study. This preplanned data analysis assessed consecutive patients aged ≥ 15years who underwent appendectomy for appendicitis (November 2020–May 2021). Patients were categorized based on SARS-CoV-2 seropositivity: no infection, active infection, and prior infection. Appendectomy method, LOS, and complications were abstracted. The association between SARS-CoV-2 seropositivity and complications was determined using Poisson regression, while the association with LOS was calculated using a quantile regression model. Results: Appendectomy for acute appendicitis was performed in 4047 patients during the second and third European COVID waves. The majority were SARS-CoV-2 uninfected (3861, 95.4%), while 70 (1.7%) were acutely SARS-CoV-2 positive, and 116 (2.8%) reported prior SARS-CoV-2 infection. After confounder adjustment, there was no statistically significant association between SARS-CoV-2 seropositivity and LOS, any complication, or severe complications. Conclusion: During sequential SARS-CoV-2 infection waves, neither active nor prior SARS-CoV-2 infection was associated with prolonged hospital LOS or postoperative complication. Despite early concerns regarding postoperative safety and outcome during active SARS-CoV-2 infection, no such association was noted for those with appendicitis who underwent operative management.

  • Hypothesis-driven genome-wide association studies provide novel insights into genetics of reading disabilities

    • Reading Disability (RD) is often characterized by difficulties in the phonology of the language. While the molecular mechanisms underlying it are largely undetermined, loci are being revealed by genome-wide association studies (GWAS). In a previous GWAS for word reading (Price, 2020), we observed that top single-nucleotide polymorphisms (SNPs) were located near to or in genes involved in neuronal migration/axon guidance (NM/AG) or loci implicated in autism spectrum disorder (ASD). A prominent theory of RD etiology posits that it involves disturbed neuronal migration, while potential links between RD-ASD have not been extensively investigated. To improve power to identify associated loci, we up-weighted variants involved in NM/AG or ASD, separately, and performed a new Hypothesis-Driven (HD)–GWAS. The approach was applied to a Toronto RD sample and a meta-analysis of the GenLang Consortium. For the Toronto sample (n = 624), no SNPs reached significance; however, by gene-set analysis, the joint contribution of ASD-related genes passed the threshold (p~1.45 × 10–2, threshold = 2.5 × 10–2). For the GenLang Cohort (n = 26,558), SNPs in DOCK7 and CDH4 showed significant association for the NM/AG hypothesis (sFDR q = 1.02 × 10–2). To make the GenLang dataset more similar to Toronto, we repeated the analysis restricting to samples selected for reading/language deficits (n = 4152). In this GenLang selected subset, we found significant association for a locus intergenic between BTG3-C21orf91 for both hypotheses (sFDR q < 9.00 × 10–4). This study contributes candidate loci to the genetics of word reading. Data also suggest that, although different variants may be involved, alleles implicated in ASD risk may be found in the same genes as those implicated in word reading. This finding is limited to the Toronto sample suggesting that ascertainment influences genetic associations.

    • Understanding the nature of the magnetic-field-induced precipitation behaviors represents a major step forward towards unravelling the real nature of interesting phenomena in Fe-based alloys and especially towards solving the key materials problem for the development of fusion energy. Experimental results indicate that the a pplied high magnetic field effectively promotes the precipitation of M 23 C 6 carbides. We build an integrated method, which breaks through the limitations of zero temperature and zero external field, to concentrate on the dependence of the stability induced by the magnetic effect, excluding the thermal effect. We investigate the intimate relationship between the external field and the origins of various magnetics structural characteristics, which are derived from the interactions among the various Wyckoff sites of iron atoms, antiparallel spin of chromium and Fe-C bond distances. The high-magnetic-field-induced exchange coupling increases with the strength of the external field, which then causes an increase in the parallel magnetic moment. The stability of the alloy carbide M 23 C 6 is more dependent on external field effects than thermal effects, whereas that of M 2 C, M 3 C and M 7 C 3 is mainly determined by thermal effects.

  • Alternative futures for global biological invasions

    • Scenario analysis has emerged as a key tool to analyze complex and uncertain future socio-ecological developments. However, currently existing global scenarios (narratives of how the world may develop) have neglected biological invasions, a major threat to biodiversity and the economy. Here, we use a novel participatory process to develop a diverse set of global biological invasion scenarios spanning a wide range of plausible global futures through to 2050. We adapted the widely used two axes scenario analysis approach to develop four families of four scenarios each, resulting in 16 scenarios that were later clustered into four contrasting sets of futures. Our analysis highlights that socioeconomic developments and technological innovation have the potential to shape biological invasions, in addition to well-known drivers, such as climate and human land use change and global trade. Our scenarios partially align with the shared socioeconomic pathways created by the climate change research community. Several factors that drive differences in biological invasions were underrepresented in the shared socioeconomic pathways; in particular, the implementation of biosecurity policies. We argue that including factors related to public environmental awareness and technological and trade development in global scenarios and models is essential to adequately consider biological invasions in global environmental assessments and thereby obtain a more integrative picture of future social-ecological developments.

    • Domestic dogs have been central to life in the North American Arctic for millennia. The ancestors of the Inuit were the first to introduce the widespread usage of dog sledge transportation technology to the Americas, but whether the Inuit adopted local Palaeo-Inuit dogs or introduced a new dog population to the region remains unknown. To test these hypotheses, we generated mitochondrial DNA and geometric morphometric data of skull and dental elements from a total of 922 North American Arctic dogs and wolves spanning over 4500 years. Our analyses revealed that dogs from Inuit sites dating from 2000 BP possess morphological and genetic signatures that distinguish them from earlier Palaeo-Inuit dogs, and identified a novel mitochondrial clade in eastern Siberia and Alaska. The genetic legacy of these Inuit dogs survives today in modern Arctic sledge dogs despite phenotypic differences between archaeological and modern Arctic dogs. Together, our data reveal that Inuit dogs derive from a secondary pre-contact migration of dogs distinct from Palaeo-Inuit dogs, and probably aided the Inuit expansion across the North American Arctic beginning around 1000 BP.

    • Salmonella Typhimurium causes a self-limiting gastroenteritis that may lead to systemic disease. Bacteria invade the small intestine, crossing the intestinal epithelium from where they are transported to the mesenteric lymph nodes (MLNs) within migrating immune cells. MLNs are an important site at which the innate and adaptive immune responses converge but their architecture and function is severely disrupted during S. Typhimurium infection. To further understand host-pathogen interactions at this site, we used mass spectrometry imaging (MSI) to analyse MLN tissue from a murine model of S. Typhimurium infection. A molecule, identified as palmitoylcarnitine (PalC), was of particular interest due to its high abundance at loci of S. Typhimurium infection and MLN disruption. High levels of PalC localised to sites within the MLNs where B and T cells were absent and where the perimeter of CD169(+) sub capsular sinus macrophages was disrupted. MLN cells cultured ex vivo and treated with PalC had reduced CD4(+) CD25(+) T cells and an increased number of B220(+) CD19(+) B cells. The reduction in CD4(+) CD25(+) T cells was likely due to apoptosis driven by increased caspase-3/7 activity. These data indicate that PalC significantly alters the host response in the MLNs, acting as a decisive factor in infection outcome.

  • Palaeogenomic analysis of black rat (Rattus rattus) reveals multiple European introductions associated with human economic history

    • The distribution of the black rat (Rattus rattus) has been heavily influenced by its association with humans. The dispersal history of this non-native commensal rodent across Europe, however, remains poorly understood, and different introductions may have occurred during the Roman and medieval periods. Here, in order to reconstruct the population history of European black rats, we first generate a de novo genome assembly of the black rat. We then sequence 67 ancient and three modern black rat mitogenomes, and 36 ancient and three modern nuclear genomes from archaeological sites spanning the 1st-17th centuries CE in Europe and North Africa. Analyses of our newly reported sequences, together with published mitochondrial DNA sequences, confirm that black rats were introduced into the Mediterranean and Europe from Southwest Asia. Genomic analyses of the ancient rats reveal a population turnover in temperate Europe between the 6th and 10th centuries CE, coincident with an archaeologically attested decline in the black rat population. The near disappearance and re-emergence of black rats in Europe may have been the result of the breakdown of the Roman Empire, the First Plague Pandemic, and/or post-Roman climatic cooling. 'Archaeogenetic analysis of black rat remains reveals that this species was introduced into temperate Europe twice, in the Roman and medieval periods. This population turnover was likely associated with multiple historical and environmental factors.'

    • Modifiable lifestyle factors have been shown to promote healthy brain ageing. However, studies have typically focused on a single factor at a time. Given that lifestyle factors do not occur in isolation, multivariable analyses provide a more realistic model of the lifestyle-brain relationship. Here, canonical correlation analyses (CCA) examined the relationship between nine lifestyle factors and seven MRI-derived indices of brain structure. The resulting covariance pattern was further explored with Bayesian regressions. CCA analyses were first conducted on a Danish cohort of older adults (n = 251) and then replicated in a British cohort (n = 668). In both cohorts, the latent factors of lifestyle and brain structure were positively correlated (UK: r =.37, p < 0.001; Denmark: r =.27, p < 0.001). In the cross-validation study, the correlation between lifestyle-brain latent factors was r =.10, p = 0.008. However, the pattern of associations differed between datasets. These findings suggest that baseline characterisation and tailoring towards the study sample may be beneficial for achieving targeted lifestyle interventions.

    • A hybrid method combining first-principles calculations and Weiss molecular field theory with thermodynamic data has been implemented to explore the origin of magnetic-field-induced precipitation behaviors for alloy carbides. The paramagnetic Mo atom disturbed the order of magnetic moment and resulted in a decrease in the Curie temperature for alloy carbide Fe6-xMoxC. The temperature dependence of magnetic moment and saturation magnetization of Fe atoms at different Wyckoff positions, as well as the saturation or induced magnetization of Fe6-xMoxC, decreased with increasing temperature. The higher Fe content and external magnetic field greatly increased the magnetization of alloy carbides. Two kinds of stella quadrangula lattices were employed to account for the total magnetism which was derived from the contribution of different Wyckoff sites of Fe atoms and Fe-C distances. The calculated total free energy taking into account magnetic field, temperature and composition was sufficient to provide quantitative agreement with experiment. The investigation of the effects of external field on the carbide precipitation behaviors led to a better understanding of the magnetic-field-induced phase transformation mechanism in heat resistant steels.

    • A combined approach to calculate the thermodynamic properties of iron and alloy carbides including the thermal and magnetic contribution is derived. Special emphasis is placed on the role of Fe and Mo to the Gibbs free energy. Lower Mo concentration in the carbides corresponds to a higher thermal Gibbs free energy change. The higher Fe content and external magnetic field greatly increase the induced magnetization, reducing the magnetic Gibbs free energy substantially and therefore increase the formation temperature. The stability of M 2 C and M 3 C are mainly determined by the thermal factors, whereas magnetic field has a predominant contribution for M 6 C.

    • Crisis, by its very nature, requires decisive intervention. However, important questions can be obscured by the very immediacy of the crisis condition.  What is the nature of the crisis? How it is defined (and by whom)?  And, subsequently, what forms of knowledge are deemed legitimate and authoritative for informing interventions?  As we see in the current pandemic, there is a desire for immediate answers and solutions during periods of uncertainty. Policymakers and publics grasp for techno-scientific solutions, as though the technical nature of the crisis is self-evident. What is often obscured by this impulse is the contingent, conjunctural and ultimately social nature of these crises.  The danger here is that by focussing on immediate technical goals, unanticipated secondary effects are produced.  These either exacerbate the existing crisis or else produce subsequent further crises.  Equally, these technical goals can conceal the varied, and often unjust, distribution of risk exposure and resources and capacities for mitigation present within and between societies.  These socio-political factors all have important functions in determining the effectiveness of interventions. As with climate change, the unfolding response to the COVID-19 pandemic underscores the importance of broadening the knowledge base beyond technical considerations.  Only by including social scientific knowledge is it possible to understand the social nature of the crises we face.  Only then is it possible to develop effective, just and legitimate responses.

  • A practicable and reliable test for metal powder spreadability : development of test and analysis technique

    • A crucial step in the powder bed metal additive manufacturing process is the formation of a thin layer of powder on top of the existing material. The propensity of the powder to form thin layers under the conditions used in additive manufacturing is critically important, but no test method has yet been established to measure this characteristic, which is sometimes referred to as spreadability. The current work spreads a single layer of powder using commercial equipment from the paint and food industries and derives the density of a layer of powder, which is of a similar thickness to that in additive manufacturing. Twenty-four powders from eight suppliers have been tested and the density of the layers has been measured as a function of various parameters. Twenty-two of the powders successfully form thin layers, with a density of at least 40% of each powder’s apparent density. Hall flow time did not correlate with the spread layer density, although the two powders that did not spread did not pass through the Hall funnel. The roughness of the plate onto which the powder was spread, the recoater speed, the layer thickness, particle size and aspect ratio all affect the measured layer density. Results of the new test are repeatable and reproducible. These findings can be used to develop a test for spreadability for metal powders that can be used for additive manufacturing, which will help to improve the quality of printed components.

    • Purpose: This study investigated the stability and growth of preschool language skills and explores latent class analysis as an approach for identifying children at risk of language impairment. Method: The authors present data from a large-scale 2-year longitudinal study, in which 600 children were assessed with a language-screening tool (LANGUAGE4) at age 4 years. Asubsample(n = 206) was assessed on measures of sentence repetition, vocabulary, and grammatical knowledge at ages 4, 5, and 6 years. Results: A global latent language factor showed a high degree of longitudinal stability in children between the ages of 4 to 6 years. A low-performing group showing a language deficit compared to their age peers at age 4 was identified on the basis of the LANGUAGE4. The growth-rates during this 2-year time period were parallel for the low-performing and 3 higher performing groups of children. Conclusions: There is strong stability in children’s language skills between the ages of 4 and 6 years. The results demonstrate that a simple language screening measure can successfully identify a low-performing group of children who show persistent language weaknesses between the ages of 4 and 6 years. © 2016 American Speech-Language-Hearing Association.

    • A novel, low-alloy steel has been designed for use in the oil and gas industry. Its high strength and hydrogen trapping potential are derived from a martensitic microstructure containing a dispersion of fine vanadium-molybdenum alloy carbides that evolve during tempering. In this second paper, the effect of quench rate from austenitisation and tempering conditions are investigated with respect to the microstructure. The alloy loses its tempering resistance following slow-cooling from austenitisation as a result of MC precipitation, leading to vanadium depletion and significant M2C coarsening. This is predicted using computer simulation and confirmed by high energy X-ray diffraction, combined with electron microscopy.
