    • I ett sameuropeiskt Leonardo da Vinci-projekt har utvecklats ett internationellt utbildnings- och praktikpa- ket för blivande sjukhusfysiker [1-3]. Utbildningspaketet innehåller praktiska och kliniska övningsuppgifter där delta- garna tränas i för en sjukhusfysiker vanligt förekommande arbetsuppgifter. Målet har varit att det skall ge sjukhusfysi- kern den praktiska kompetens som krävs av det europeiska regelverket inom ämnesområdena magnetresonans (MRI), ultraljud, röntgendiagnostik, nuklearmedicin och strålbe- handling. Utbildningsmaterialet är webbaserat med en stor bilddata- bas och används redan i nära 70 länder runt om i världen. EMIT-projektet (European Medical Imaging Technology Training) belönades i december 2004 med det första Leonar- do da Vinci priset som delades ut till de tre bästa av totalt 4000 EU-projekt inom praktisk yrkesrelaterad utbildning. I projektet har vi konfronterats med den pedagogiska ut- maningen att förmedla praktisk kunskap, i sjukhusmiljöer med mycket olika förutsättningar beroende på vilket land deltagarna arbetar i. En omöjlig uppgift kan tyckas, men eftersom materialet är sammanställt av några av Europas starkaste forsknings- och utbildningsgrupper inom respekti- ve område så har det visat sig vara en stor tillgång och varje användare utnyttjar materialet efter sina egna behov. En an- nan erfarenhet är samarbetet över nationsgränser där vi har utnyttjat och konfronterats med likheter och oliktänkande inte minst vad gäller pedagogik och didaktik. Vi har dessut- om fått ett stort kontaktnät och många goda vänner. Den här presentationen beskriver delar av utbildningspaketet och erfarenheter av samarbetsprojektet.

  • Quality assurance in radiotherapy: the importance of medical physics staffing levels. Recommendations from an ESTRO/EFOMP joint task group

    • The safe application of ionising radiation for diagnosis and therapy requires a high level of knowledge of the underlying processes and of quality assurance. Sophisticated modern equipment can be used effectively for complicated diagnostic and therapeutic techniques only with adequate physics support. In the light of recent analyses and recommendations by national and international societies a joint working group of representatives from ESTRO (European Society for Therapeutic Radiology and Oncology) and from EFOMP (European Federation of Organisations for Medical Physics) was set up to assess the necessary staffing levels for physics support to radiotherapy. The method used to assess the staffing levels, the resulting recommendations and examples of their practical application are described.

    • Background: Ratios of values of brachytherapy source strengths, as measured by hospitals and vendors, comprise constant differences as, e.g., systematic errors in ion chamber calibration factors and measurement setup. Such ratios therefore have the potential to reveal the systematic changes in routines or calibration services at either the hospital or the vendor laboratory, which could otherwise be hidden by the uncertainty in the source strength values. Methods: The RAKR of each new source in 13 afterloading units at five hospitals were measured by well-type ion chambers and compared to values for the same source stated on vendor certificates. Results: Differences from unity in the ratios of RAKR values determined by hospitals and vendors are most often small and stable around their mean values to within +/- 11.5%. Larger deviations are rare but occur. A decreasing ratio, seen at two hospitals for the same source, was useful in detecting an erroneous pressure gauge at the vendors site. Conclusions: Establishing a mean ratio of RAKR values, as measured at the hospital and supplied on the vendor certificate, and monitoring this as a function of time are an easy way for the early detection of problems with equipment or routines at either the hospital or the vendor site.

    • Purpose: To provide a guideline curriculum covering theoretical and practical aspects of education and training for medical physicists in radiotherapy within Europe. Material and methods: Guidelines have been developed for the specialist theoretical knowledge and practical experience required to practice as a medical physicist in radiotherapy. It is assumed that the typical entrant into training will have a good initial degree in the physical sciences, therefore these guidelines also require that and are additional to it. National training programmes of medical physics, radiation physics and radiotherapy physics from a range of European countries and from North America were reviewed by an expert panel set up by the European Society of Therapeutic Radiology and Oncology (ESTRO) and the European Federation of Organisations for Medical Physics (EFOMP). A draft document prepared by this group was circulated, via the EFOMP infrastructure, among national professional medical physics societies in Europe for review and comment and was also discussed in an education session in the May 2003 EFOMP scientific meeting in Eindhoven. Results: The resulting guideline curriculum for education and training of medical physicists in radiotherapy within Europe discusses the EFOMP terms, qualified medical physicist (QMP) and specialist medical physicist (SMP), and the group's view of the links to the EU (Directive 97/43) term, medical physics expert (MPE). The minimum level expected in each topic in the theoretical knowledge and practical experience sections is intended to bring trainees up to the requirements of a QMP. The responses from the circulation of the document to national societies and its discussion were either to agree its content, with no changes required, or to suggest changes, which were taken into account after consideration by the expert group. Following this the guidelines have been endorsed by the parent organisations. Conclusions: This new joint ESTRO/EFOMP European guideline curriculum is a first step to harmonise specialist training of medical physicists in radiotherapy within Europe. It provides a common framework for national medical physics societies to develop or benchmark their own curricula, but is also flexible enough to suit different situations of initial physics qualifications, medical physics training programmes, accreditation structures, etc. The responsibility for the implementation of these standards and guidelines will lie with the national training bodies and authorities.

    • In 2007, the European Commission (EC) commissioned a group of experts to undertake the revision of Report Radiation Protection (RP 91) oCriteria for acceptability of radiological (including radiotherapy) and nuclear medicine installations' written in 1997(( 1)). The revised draft report was submitted to the EC in 2010, who issued it for public consultation. The EC has commissioned the same group of experts to consider the comments of the public consultation for further improvement of the revised report. The EC intends to publish the final report under its Radiation Report Series as RP 162. This paper describes the background to the selection of the key performance parameters for radiotherapy equipment and sets out the sources of their criteria of acceptability including suspension levels for a wide range of radiotherapy equipment.

    • Vid överföring av information kan man karakterisera ett överföringssystem på olika sätt. Man kan t.ex. ange hur en signal förstärks, hur den förvr~ngs (distorderas), hur mycket brus systemet innehåller etc. Ett bildgivande system kan i princip karakteriseras av ett objekt som ger en insignal till en detektor vars utsignal antagligen innehåller signal + brus. För att beskriva hur utsignalen ser ut i jämförelse med insignalen används flera begrepp som: Punktspridningsfunktionen (PSF) Linjespridningsfunktionen (LSF) Halvvärdesbredden (FWHM) Inom bildöverförande system använder man sig av begreppet MTF (MTF= Modulation Transfer Eunction) . Vi skall i det följande gå igenom grunderna för MTF och även ge exempel på hur man beräknar MTF. Genom att införa Fourier-transformen kan man lättare beräkna MTF och vi kommer därför att beskriva denna. För mätning av MTF för olika system inom röntgendiagnostiken hänvisas-till litteraturen.

This paper presents a selection of practical applications of suspension criteria scenarios in radiotherapy, mostly in brachytherapy, with special emphasis on the critical roles and responsibilities of qualified radiotherapy staff (radiation oncologists, medical physicists and radiotherapy technicians).

    • Subcutaneous metastases from an oesophageal carcinoma were irradiated using different schedules. The results have to be evaluated with greatest caution but indicate that with the same CRE value, few fractions caused less skin reactions than several, and the size of the shoulder of the cell survival curve was of the order of 0.7 Gy.

  • Development of educational image databases and e-books for medical physics training

    • Medical physics education and training requires the use of extensive imaging material and specific explanations. These requirements provide an excellent background for application of e-Learning. The EU projects Consortia EMERALD and EMIT developed five volumes of such materials, now used in 65 countries. EMERALD developed e-Learning materials in three areas of medical physics (X-ray diagnostic radiology, nuclear medicine and radiotherapy). EMIT developed e-Learning materials in two further areas: ultrasound and magnetic resonance imaging. This paper describes the development of these e-Learning materials (consisting of e-books and educational image databases). The e-books include tasks helping studying of various equipment and methods. The text of these PDF e-books is hyperlinked with respective images. The e-books are used through the readers' own Internet browser. Each Image Database (IDB) includes a browser, which displays hundreds of images of equipment, block diagrams and graphs, image quality examples, artefacts, etc. Both the e-books and IDB are engraved on five separate CD-ROMs. Demo of these materials can be taken from www.emerald2.net.
