    • Speech comprehension depends on the successful operation of a network of brain regions. Processing of degraded speech is associated with different patterns of brain activity in comparison with that of high-quality speech. In this exploratory study, we studied whether processing degraded auditory input in daily life because of hearing impairment is associated with differences in brain volume. We compared TI-weighted structural magnetic resonance images of 17 hearing-impaired (HI) adults with those of 17 normal-hearing (NH) controls using a voxel-based morphometry analysis. HI adults were individually matched with NH adults based on age and educational level. Gray and white matter brain volumes were compared between the groups by region-of-interest analyses in structures associated with speech processing, and by whole-brain analyses. The results suggest increased gray matter volume in the right angular gyrus and decreased white matter volume in the left fusiform gyrus in HI listeners as compared with NH ones. In the HI group, there was a significant correlation between hearing acuity and cluster volume of the gray matter cluster in the right angular gyrus. This correlation supports the link between partial hearing loss and altered brain volume. The alterations in volume may reflect the operation of compensatory mechanisms that are related to decoding meaning from degraded auditory input.

  • Extending Subcortical EEG Responses to Continuous Speech to the Sound-Field

    • The auditory brainstem response (ABR) is a valuable clinical tool for objective hearing assessment, which is conventionally detected by averaging neural responses to thousands of short stimuli. Progressing beyond these unnatural stimuli, brainstem responses to continuous speech presented via earphones have been recently detected using linear temporal response functions (TRFs). Here, we extend earlier studies by measuring subcortical responses to continuous speech presented in the sound-field, and assess the amount of data needed to estimate brainstem TRFs. Electroencephalography (EEG) was recorded from 24 normal hearing participants while they listened to clicks and stories presented via earphones and loudspeakers. Subcortical TRFs were computed after accounting for non-linear processing in the auditory periphery by either stimulus rectification or an auditory nerve model. Our results demonstrated that subcortical responses to continuous speech could be reliably measured in the sound-field. TRFs estimated using auditory nerve models outperformed simple rectification, and 16 minutes of data was sufficient for the TRFs of all participants to show clear wave V peaks for both earphones and sound-field stimuli. Subcortical TRFs to continuous speech were highly consistent in both earphone and sound-field conditions, and with click ABRs. However, sound-field TRFs required slightly more data (16 minutes) to achieve clear wave V peaks compared to earphone TRFs (12 minutes), possibly due to effects of room acoustics. By investigating subcortical responses to sound-field speech stimuli, this study lays the groundwork for bringing objective hearing assessment closer to real-life conditions, which may lead to improved hearing evaluations and smart hearing technologies.

  • Toward EEG-Assisted Hearing Aids: Objective Threshold Estimation Based on Ear-EEG in Subjects With Sensorineural Hearing Loss

    • Electrophysiological feedback on activity in the auditory pathway may potentially advance the next generation of hearing aids. Conventional electroencephalographic (EEG) systems are, however, impractical during daily life and incompatible with hearing aids. Ear-EEG is a method in which the EEG is recorded from electrodes embedded in a hearing aid like earpiece. The method therefore provides an unobtrusive way of measuring neural activity suitable for use in everyday life. This study aimed to determine whether ear-EEG could be used to estimate hearing thresholds in subjects with sensorineural hearing loss. Specifically, ear-EEG was used to determine physiological thresholds at 0.5, 1, 2, and 4 kHz using auditory steady-state response measurements. To evaluate ear-EEG in relation to current methods, thresholds were estimated from a concurrently recorded conventional scalp EEG. The threshold detection rate for ear-EEG was 20% lower than the detection rate for scalp EEG. Thresholds estimated using in-ear referenced ear-EEG were found to be elevated at an average of 5.9, 2.3, 5.6, and 1.5 dB relative to scalp thresholds at 0.5, 1, 2, and 4 kHz, respectively. No differences were found in the variance of means between in-ear ear-EEG and scalp EEG. In-ear ear-EEG, auditory steady-state response thresholds were found at 12.1 to 14.4 dB sensation level with an intersubject variation comparable to that of behavioral thresholds. Collectively, it is concluded that although further refinement of the method is needed to optimize the threshold detection rate, ear-EEG is a feasible method for hearing threshold level estimation in subjects with sensorineural hearing impairment.

  • Spectrotemporal Modulation Sensitivity as a Predictor of Speech-Reception Performance in Noise With Hearing Aids

    • The audiogram predicts amp;lt;30% of the variance in speech-reception thresholds (SRTs) for hearing-impaired (HI) listeners fitted with individualized frequency-dependent gain. The remaining variance could reflect suprathreshold distortion in the auditory pathways or nonauditory factors such as cognitive processing. The relationship between a measure of suprathreshold auditory function-spectrotemporal modulation (STM) sensitivity-and SRTs in noise was examined for 154 HI listeners fitted with individualized frequency-specific gain. SRTs were measured for 65-dB SPL sentences presented in speech-weighted noise or four-talker babble to an individually programmed master hearing aid, with the output of an ear-simulating coupler played through insert earphones. Modulation-depth detection thresholds were measured over headphones for STM (2cycles/octave density, 4-Hz rate) applied to an 85-dB SPL, 2-kHz lowpass-filtered pink-noise carrier. SRTs were correlated with both the high-frequency (2-6 kHz) pure-tone average (HFA; R-2 = .31) and STM sensitivity (R-2 = .28). Combined with the HFA, STM sensitivity significantly improved the SRT prediction (Delta R-2 = .13; total R-2 = .44). The remaining unaccounted variance might be attributable to variability in cognitive function and other dimensions of suprathreshold distortion. STM sensitivity was most critical in predicting SRTs for listenersamp;lt;65 years old or with HFA amp;lt;53 dB HL. Results are discussed in the context of previous work suggesting that STM sensitivity for low rates and low-frequency carriers is impaired by a reduced ability to use temporal fine-structure information to detect dynamic spectra. STM detection is a fast test of suprathreshold auditory function for frequencies amp;lt;2 kHz that complements the HFA to predict variability in hearing-aid outcomes for speech perception in noise.

  • Internet-Based Interventions for Adults With Hearing Loss, Tinnitus, and Vestibular Disorders : A Systematic Review and Meta-Analysis

    • Internet-based interventions have been developed to improve access to audiovestibular health care. This review aimed to identify outcomes of Internet interventions for adults with hearing loss, tinnitus, and vestibular disorders. Electronic databases and manual searches were performed to identify studies meeting eligibility for inclusion. Fifteen studies (1,811 participants) met the inclusion criteria, with nine studies targeting tinnitus distress, five considering hearing loss, and one for vestibular difficulties. Only the tinnitus and hearing loss Internet intervention studies were eligible for data synthesis. Internet-based interventions for hearing loss were diverse. Overall, they showed no significant effects, although a statistically significant moderate effect (d = 0.59) was found after removing the study with the highest risk of bias (as a result of high attrition). Most Internet-based interventions for tinnitus provided cognitive behavioural therapy. They yielded statistically significant mean effect sizes for reducing tinnitus distress compared with both inactive (d = 0.59) and active controls (d = 0.32). Significant effects were also present for the secondary outcomes of anxiety, depression, insomnia, and quality of life (combined effect d = 0.28). Only Internet-based interventions for tinnitus evaluated the 1-year postintervention effects indicated that results were maintained long term (d = 0.45). Scientific study quality was appraised using the Grading of Recommendations Assessment, Development and Evaluation approach and found to vary from very low to moderate. This review indicates the potential of Internet interventions for tinnitus to provide evidence-based accessible care. There is a need for additional high-quality evidence before conclusive results can be established regarding the effects of audiovestibular Internet interventions.

  • Characteristics of Bone-Conduction Devices Simulated in a Finite-Element Model of a Whole Human Head

    • Nowadays, many different kinds of bone-conduction devices (BCDs) are available for hearing rehabilitation. Most studies of these devices fail to compare the different types of BCDs under the same conditions. Moreover, most results are between two BCDs in the same subject, or two BCDs in different subjects failing to provide an overview of the results between several of the BCDs. Another issue is that some BCDs require surgical procedures that prevent comparison of the BCDs in the same persons. In this study, four types of skin-drive BCDs, three direct-drive BCDs, and one oral device were evaluated in a finite-element model of the human head that was able to simulate all BCDs under the same conditions. The evaluation was conducted using both a dynamic force as input and an electric voltage to a model of a BCD vibrator unit. The results showed that the direct-drive BCDs and the oral device gave vibration responses within 10 dB at the cochlea. The skin-drive BCDs had similar or even better cochlear vibration responses than the direct-drive BCDs at low frequencies, but the direct-drive BCDs gave up to 30 dB higher cochlear vibration responses at high frequencies. The study also investigated the mechanical point impedance at the interface between the BCD and the head, providing information that explains some of the differences seen in the results. For example, when the skin-drive BCD attachment area becomes too small, the transducer cannot provide an output force similar to the devices with larger attachment surfaces.

  • The Development of Sound Localization Latency in Infants and Young Children with Normal Hearing

    • With the advances in eye tracking, saccadic reflexes towards auditory stimuli have become an easily accessible behavioral response. The present study investigated the development of horizontal sound localization latency quantified by saccadic reflexes in infants and young children with normal hearing (0.55 to 5.6 years, n = 22). The subject was seated in front of an array of 12 loudspeaker/display-pairs arranged equidistantly in an arc from −55 to + 55° azimuth. An ongoing auditory-visual stimulus was presented at 63 dB SPL and shifted to another randomly selected pair at 24 occasions. At each shift, the visual part of the stimulus was blanked for 1.6 s providing auditory-only localization cues. A sigmoid model was fitted to the gaze samples following the azimuthal sound shifts. The overall sound localization latency (SLL) for a subject was defined as the mean of the latencies for all trials included by objective criteria. The SLL was assessed in 21 of 22 children with a mean of 6.1 valid trials. The SLL ranged 400 to 1400 ms (mean = 860 ms). An inverse model demonstrated a significant relationship between SLL and age ( R2 = 0.79, p < 0.001), reflecting a distinct reduction of latency with increasing age. No partial correlation between SLL and sound localization accuracy was found when controlling for age ( p = 0.5), suggesting that localization latency may provide diagnostic value beyond accuracy.

  • Improving Speech Intelligibility by Hearing Aid Eye-Gaze Steering: Conditions With Head Fixated in a Multitalker Environment

    • The behavior of a person during a conversation typically involves both auditory and visual attention. Visual attention implies that the person directs his or her eye gaze toward the sound target of interest, and hence, detection of the gaze may provide a steering signal for future hearing aids. The steering could utilize a beamformer or the selection of a specific audio stream from a set of remote microphones. Previous studies have shown that eye gaze can be measured through electrooculography (EOG). To explore the precision and real-time feasibility of the methodology, seven hearing-impaired persons were tested, seated with their head fixed in front of three targets positioned at -30 degrees, 0 degrees, and +30 degrees azimuth. Each target presented speech from the Danish DAT material, which was available for direct input to the hearing aid using head-related transfer functions. Speech intelligibility was measured in three conditions: a reference condition without any steering, a condition where eye gaze was estimated from EOG measures to select the desired audio stream, and an ideal condition with steering based on an eye-tracking camera. The "EOG-steering" improved the sentence correct score compared with the "no-steering" condition, although the performance was still significantly lower than the ideal condition with the eye-tracking camera. In conclusion, eye-gaze steering increases speech intelligibility, although real-time EOG-steering still requires improvements of the signal processing before it is feasible for implementation in a hearing aid.

    • Dissatisfaction with the sound of ones own voice is common among hearing-aid users. Little is known regarding how hearing impairment and hearing aids separately affect own-voice perception. This study examined own-voice perception and associated issues before and after a hearing-aid fitting for new hearing-aid users and refitting for experienced users to investigate whether it was possible to differentiate between the effect of (unaided) hearing impairment and hearing aids. Further aims were to investigate whether First-Time and Experienced users as well as users with dome and mold inserts differed in the severity of own-voice problems. The study had a cohort design with three groups: First-Time hearing-aid users going from unaided to aided hearing (n = 70), Experienced hearing-aid users replacing their old hearing aids (n = 70), and an unaided control group (n = 70). The control group was surveyed once and the hearing-aid users twice; once before hearing-aid fitting/refitting and once after. The results demonstrated that own-voice problems are common among both First-Time and Experienced hearing-aid users with either dome- or mold-type fittings, while people with near-normal hearing and not using hearing aids report few problems. Hearing aids increased ratings of own-voice problems among First-Time users, particularly those with mold inserts. The results suggest that altered auditory feedback through unaided hearing impairment or through hearing aids is likely both to change own-voice perception and complicate regulation of vocal intensity, but hearing aids are the primary reason for poor perceived sound quality of ones own voice.

  • Measuring the Impact of Tinnitus on Aided Listening Effort Using Pupillary Response

    • Tinnitus can have serious impact on a persons life and is a common auditory symptom that is especially comorbid with hearing loss. This study investigated processing effort required for speech recognition in a group of hearing-impaired people with tinnitus and a control group (CG) of hearing-impaired people without tinnitus by means of pupillary response. Furthermore, the relationship between the pupillary response, self-rating measures of tinnitus severity, and fatigue was examined. Participants performed a speech-in-noise task with a competing four-talker babble at two speech intelligibility levels (50% and 95%) with either an active or inactive noise-reduction scheme while the pupillary response was recorded. Tinnitus participants showed significantly smaller time-dependent pupil dilations and significantly higher fatigue ratings. No correlation was found for the tinnitus severity and pupillary response, but a significant correlation was found between the tinnitus severity and fatigue. As participants with tinnitus generally reported higher fatigue and showed smaller task-evoked pupil dilations, it was speculated that this may suggest an increased activity of the parasympathetic nervous system, which governs the bodily response during rest. The finding that tinnitus participants showed higher fatigue has clinical implications, highlighting the importance of taking steps to decrease the risk of developing long-term fatigue. Finally, the tinnitus participants showed reduced pupillary responses when noise reduction was activated, suggesting a reduced effort from hearing aid signal processing.

    • This study evaluated how hearing-impaired listeners perceive native (Swedish) and nonnative (English) speech in the presence of noise- and speech maskers. Speech reception thresholds were measured for four different masker types for each target language. The maskers consisted of stationary and fluctuating noise and two-talker babble in Swedish and English. Twenty-three hearing-impaired native Swedish listeners participated, aged between 28 and 65 years. The participants also performed cognitive tests of working memory capacity in Swedish and English, nonverbal reasoning, and an English proficiency test. Results indicated that the speech maskers were more interfering than the noise maskers in both target languages. The larger need for phonetic and semantic cues in a nonnative language makes a stationary masker relatively more challenging than a fluctuating-noise masker. Better hearing acuity (pure tone average) was associated with better perception of the target speech in Swedish, and better English proficiency was associated with better speech perception in English. Larger working memory and better pure tone averages were related to the better perception of speech masked with fluctuating noise in the nonnative language. This suggests that both are relevant in highly taxing conditions. A large variance in performance between the listeners was observed, especially for speech perception in the nonnative language.

  • Diagnostic Criteria for Somatosensory Tinnitus : A Delphi Process and Face-to-Face Meeting to Establish Consensus.

    • Since somatic or somatosensory tinnitus (ST) was first described as a subtype of subjective tinnitus, where altered somatosensory afference from the cervical spine or temporomandibular area causes or changes a patient's tinnitus perception, several studies in humans and animals have provided a neurophysiological explanation for this type of tinnitus. Due to a lack of unambiguous clinical tests, many authors and clinicians use their own criteria for diagnosing ST. This resulted in large differences in prevalence figures in different studies and limits the comparison of clinical trials on ST treatment. This study aimed to reach an international consensus on diagnostic criteria for ST among experts, scientists and clinicians using a Delphi survey and face-to-face consensus meeting strategy. Following recommended procedures to gain expert consensus, a two-round Delphi survey was delivered online, followed by an in-person consensus meeting. Experts agreed upon a set of criteria that strongly suggest ST. These criteria comprise items on somatosensory modulation, specific tinnitus characteristics, and symptoms that can accompany the tinnitus. None of these criteria have to be present in every single patient with ST, but in case they are present, they strongly suggest the presence of ST. Because of the international nature of the survey, we expect these criteria to gain wide acceptance in the research field and to serve as a guideline for clinicians across all disciplines. Criteria developed in this consensus paper should now allow further investigation of the extent of somatosensory influence in individual tinnitus patients and tinnitus populations.

  • A Glimpse of Memory Through the Eyes : Pupillary Responses Measured During Encoding Reflect the Likelihood of Subsequent Memory Recall in an Auditory Free Recall Test

    • The aim of the current study was to investigate whether task-evoked pupillary responses measured during encoding, individual working memory capacity and noise reduction in hearing aids were associated with the likelihood of subsequently recalling an item in an auditory free recall test combined with pupillometry. Participants with mild to moderately severe symmetrical sensorineural hearing loss (n = 21) were included. The Sentence-final Word Identification and Recall (SWIR) test was administered in a background noise composed of sixteen talkers with noise reduction in hearing aids activated and deactivated. The task-evoked peak pupil dilation (PPD) was measured. The Reading Span (RS) test was used as a measure of individual working memory capacity. Larger PPD at a single trial level was significantly associated with higher likelihood of subsequently recalling a word, presumably reflecting the intensity of attention devoted during encoding. There was no clear evidence of a significant relationship between working memory capacity and subsequent memory recall, which may be attributed to the SWIR test and RS test being administered in different modalities, as well as differences in task characteristics. Noise reduction did not have a significant effect on subsequent memory recall. This may be due to the background noise not having a detrimental effect on attentional processing at the favorable signal-to-noise ratio levels at which the test was conducted.

  • Objective Assessment of Listening Effort : Coregistration of Pupillometry and EEG

    • Listening to speech in noise is effortful, particularly for people with hearing impairment. While it is known that effort is related to a complex interplay between bottom-up and top-down processes, the cognitive and neurophysiological mechanisms contributing to effortful listening remain unknown. Therefore, a reliable physiological measure to assess effort remains elusive. This study aimed to determine whether pupil dilation and alpha power change, two physiological measures suggested to index listening effort, assess similar processes. Listening effort was manipulated by parametrically varying spectral resolution (16- and 6-channel noise vocoding) and speech reception thresholds (SRT; 50% and 80%) while 19 young, normal-hearing adults performed a speech recognition task in noise. Results of off-line sentence scoring showed discrepancies between the target SRTs and the true performance obtained during the speech recognition task. For example, in the SRT80% condition, participants scored an average of 64.7%. Participants true performance levels were therefore used for subsequent statistical modelling. Results showed that both measures appeared to be sensitive to changes in spectral resolution (channel vocoding), while pupil dilation only was also significantly related to their true performance levels (%) and task accuracy (i.e., whether the response was correctly or partially recalled). The two measures were not correlated, suggesting they each may reflect different cognitive processes involved in listening effort. This combination of findings contributes to a growing body of research aiming to develop an objective measure of listening effort.

  • Comparison of Gated Audiovisual Speech Identification in Elderly Hearing Aid Users and Elderly Normal-Hearing Individuals: Effects of Adding Visual Cues to Auditory Speech Stimuli

    • The present study compared elderly hearing aid (EHA) users (n = 20) with elderly normal-hearing (ENH) listeners (n = 20) in terms of isolation points (IPs, the shortest time required for correct identification of a speech stimulus) and accuracy of audiovisual gated speech stimuli (consonants, words, and final words in highly and less predictable sentences) presented in silence. In addition, we compared the IPs of audiovisual speech stimuli from the present study with auditory ones extracted from a previous study, to determine the impact of the addition of visual cues. Both participant groups achieved ceiling levels in terms of accuracy in the audiovisual identification of gated speech stimuli; however, the EHA group needed longer IPs for the audiovisual identification of consonants and words. The benefit of adding visual cues to auditory speech stimuli was more evident in the EHA group, as audiovisual presentation significantly shortened the IPs for consonants, words, and final words in less predictable sentences; in the ENH group, audiovisual presentation only shortened the IPs for consonants and words. In conclusion, although the audiovisual benefit was greater for EHA group, this group had inferior performance compared with the ENH group in terms of IPs when supportive semantic context was lacking. Consequently, EHA users needed the initial part of the audiovisual speech signal to be longer than did their counterparts with normal hearing to reach the same level of accuracy in the absence of a semantic context.

We measured the accuracy with which the target speech stimuli were identified, as well as the isolation points (IPs: the shortest duration, from onset, required to correctly identify the speech target). The relationships between working memory capacity, the IPs, and speech accuracy were also measured. Twenty-four EHA users (with mild to moderate hearing impairment) and 24 ENH individuals participated in the present study. Despite the use of their regular hearing aids, the EHA users had delayed IPs and were less accurate in identifying consonants and words compared with the ENH individuals. The EHA users also had delayed IPs for final word identification in sentences with lower predictability; however, no significant between-group difference in accuracy was observed. Finally, there were no significant between-group differences in terms of IPs or accuracy for final word identification in highly predictable sentences. Our results also showed that, among EHA users, greater working memory capacity was associated with earlier IPs and improved accuracy in consonant and word identification. Together, our findings demonstrate that the gated speech perception ability of EHA users was not at the level of ENH individuals, in terms of IPs and accuracy. In addition, gated speech perception was more cognitively demanding for EHA users than for ENH individuals in the absence of semantic context.

    • The present study aimed to investigate the changing relationship between aided speech recognition and cognitive function during the first six months of hearing aid use. Twentyseven first-time hearing aid users with symmetrical mild to moderate sensorineural hearing loss were recruited. Aided speech recognition thresholds in noise (SRTs) were obtained in the hearing aid fitting session as well as at three and six months post-fitting. Cognitive abilities were assessed using a reading span test, which is a measure of working memory capacity, and a cognitive test battery. Results showed a significant correlation between reading span and SRT during the hearing aid fitting session. This relation was significantly weakened over the first six months of hearing aid use. Multiple regression analysis showed that reading span was the main predictor of SRT when hearing aids were first fitted, but that pure-tone average hearing threshold (PTA) was the main predictor six months later. This indicates that working memory capacity plays a more important role in speech recognition in noise before than after six months of use. We argue that new hearing aid users engage working memory capacity to recognize unfamiliar processed speech signals but that as familiarization proceeds, engagement of working memory capacity is reduced.

    • Bone conduction sound transmission in humans has been extensively studied using cochlear promontory vibrations. These studies use vibration data collected from measurements in live humans, whole cadavers, and severed cadaver heads, with stimulation applied either at an implant in the skull bone or directly on the skin. Experimental protocols, methods, and preparation of cadavers or cadaver heads vary among the studies, and it is currently unknown to what extent the aforementioned variables affect the outcome of those studies. The current study has two aims. The first aim is to review and compare available experimental data and assess the effects of the experimental protocol and methods. The second aim is to investigate similarities and differences found between the experimental studies based on simulations in a finite element model, the LiUHead. With implant stimulation, the average cochlear promontory vibration levels were within 10 dB, independent of the experimental setup and preparations of the cadavers or cadaver heads. With on-skin stimulation, the results were consistent between cadaver heads and living humans. Partial or complete replacement of the brain with air does not affect the cochlear promontory vibration, whereas replacing the brain with liquid reduces the vibration level by up to 5 dB. An intact head-neck connection affects the vibration of the head at frequencies below 300-400 Hz with a significant vibration reduction at frequencies below 200 Hz. Removing all soft tissue, brain tissue, and intracranial fluid from the head increases the overall cochlear promontory vibration level by around 5 dB.

    • Bone conduction (BC) stimulation has mainly been used for clinical hearing assessment and hearing aids where stimulation is applied at the mastoid behind the ear. Recently, BC has become popular for communication headsets where the stimulation position often is close to the anterior part of the ear canal opening. The BC sound transmission for this stimulation position is here investigated in 21 participants by ear canal sound pressure measurements and hearing threshold assessment as well as simulations in the LiUHead. The results indicated that a stimulation position close to the ear canal opening improves the sensitivity for BC sound by around 20 dB but by up to 40 dB at some frequencies. The transcranial transmission ranges typically between -40 and -25 dB. This decreased transcranial transmission facilitates saliency of binaural cues and implies that BC headsets are suitable for virtual and augmented reality applications. The findings suggest that with BC stimulation close to the ear canal opening, the sound pressure in the ear canal dominates the perception of BC sound. With this stimulation, the ear canal pathway was estimated to be around 25 dB greater than other contributors, like skull bone vibrations, for hearing BC sound in a healthy ear. This increased contribution from the ear canal sound pressure to BC hearing means that a position close to the ear canal is not appropriate for clinical use since, in such case, a conductive hearing loss affects BC and air conduction thresholds by a similar amount.

    • Affective processing appears to be altered in tinnitus, and the condition is to a large extent characterized by the emotional reaction to the phantom sound. Psychophysiological models of tinnitus and supporting brain imaging studies have suggested a role for the limbic system in the emergence and maintenance of tinnitus. It is not clear whether the tinnitus-related changes in these systems are specific for tinnitus only, or whether they affect emotional processing more generally. In this study, we aimed to quantify possible deviations in affective processing in tinnitus patients by behavioral and physiological measures. Tinnitus patients rated the valence and arousal of sounds from the International Affective Digitized Sounds database. Sounds were chosen based on the normative valence ratings, that is, negative, neutral, or positive. The individual autonomic response was measured simultaneously with pupillometry. We found that the subjective ratings of the sounds by tinnitus patients differed significantly from the normative ratings. The difference was most pronounced for positive sounds, where sounds were rated lower on both valence and arousal scales. Negative and neutral sounds were rated differently only for arousal. Pupil measurements paralleled the behavioral results, showing a dampened response to positive sounds. Taken together, our findings suggest that affective processing is altered in tinnitus patients. The results are in line with earlier studies in depressed patients, which have provided evidence in favor of the so-called positive attenuation hypothesis of depression. Thus, the current results highlight the close link between tinnitus and depression.

    • This study investigates the effect of spatial release from masking (SRM) in bilateral bone conduction (BC) stimulation at the mastoid. Nine adults with normal hearing were tested to determine SRM based on speech recognition thresholds (SRTs) in simulated spatial configurations ranging from 0 to 180 degrees. These configurations were based on nonindividualized head-related transfer functions. The participants were subjected to sound stimulation through either air conduction (AC) via headphones or BC. The results indicated that both the angular separation between the target and the masker, and the modality of sound stimulation, significantly influenced speech recognition performance. As the angular separation between the target and the masker increased up to 150 degrees, both BC and AC SRTs decreased, indicating improved performance. However, performance slightly deteriorated when the angular separation exceeded 150 degrees. For spatial separations less than 75 degrees, BC stimulation provided greater spatial benefits than AC, although this difference was not statistically significant. For separations greater than 75 degrees, AC stimulation offered significantly more spatial benefits than BC. When speech and noise originated from the same side of the head, the "better ear effect" did not significantly contribute to SRM. However, when speech and noise were located on opposite sides of the head, this effect became dominant in SRM.

    • The position of a bone conduction (BC) transducer influences the perception of BC sound, but the relation between the stimulation position and BC sound perception is not entirely clear. In the current study, eleven participants with normal hearing were evaluated for their hearing thresholds and speech intelligibility for three stimulation positions (temple, mastoid, and condyle) and four types of ear canal occlusion produced by headphones. In addition, the sound quality for three types of music was rated with stimulation at the three positions. Stimulation at the condyle gave the best performance while the temple showed the worst performance for hearing thresholds, speech intelligibility, and sound quality. The in-ear headphones gave the highest occlusion effect while fully open headphones gave the least occlusion effect. BC stimulated speech intelligibility improved with greater occlusion, especially for the temple stimulation position. The results suggest that BC stimulation at the condyle is generally superior to the other positions tested in terms of sensitivity, clarity, and intelligibility, and that occlusion with ordinary headphones improves the BC signal.

    • Recently, the measurement of the pupil dilation response has been applied in many studies to assess listening effort. Meanwhile, the mechanisms underlying this response are still largely unknown. We present the results of a method that separates the influence of the parasympathetic and sympathetic branches of the autonomic nervous system on the pupil response during speech perception. This is achieved by changing the background illumination level. In darkness, the influence of the parasympathetic nervous system on the pupil response is minimal, whereas in light, there is an additional component from the parasympathetic nervous system. Nineteen hearing-impaired and 27 age-matched normal-hearing listeners performed speech reception threshold tests targeting a 50% correct performance level while pupil responses were recorded. The target speech was masked with a competing talker. The test was conducted twice, once in dark and once in a light condition. Need for Recovery and Checklist Individual Strength questionnaires were acquired as indices of daily-life fatigue. In dark, the peak pupil dilation (PPD) did not differ between the two groups, but in light, the normal-hearing group showed a larger PPD than the hearing-impaired group. Listeners with better hearing acuity showed larger differences in dilation between dark and light. These results indicate a larger effect of parasympathetic inhibition on the pupil dilation response of listeners with better hearing acuity, and a relatively high parasympathetic activity in those with worse hearing. Previously observed differences in PPD between normal and impaired listeners are probably not solely because of differences in listening effort.

    • The measurement of cognitive resource allocation during listening, or listening effort, provides valuable insight in the factors influencing auditory processing. In recent years, many studies inside and outside the field of hearing science have measured the pupil response evoked by auditory stimuli. The aim of the current review was to provide an exhaustive overview of these studies. The 146 studies included in this review originated from multiple domains, including hearing science and linguistics, but the review also covers research into motivation, memory, and emotion. The present review provides a unique overview of these studies and is organized according to the components of the Framework for Understanding Effortful Listening. A summary table presents the sample characteristics, an outline of the study design, stimuli, the pupil parameters analyzed, and the main findings of each study. The results indicate that the pupil response is sensitive to various task manipulations as well as interindividual differences. Many of the findings have been replicated. Frequent interactions between the independent factors affecting the pupil response have been reported, which indicates complex processes underlying cognitive resource allocation. This complexity should be taken into account in future studies that should focus more on interindividual differences, also including older participants. This review facilitates the careful design of new studies by indicating the factors that should be controlled for. In conclusion, measuring the pupil dilation response to auditory stimuli has been demonstrated to be sensitive method applicable to numerous research questions. The sensitivity of the measure calls for carefully designed stimuli.

    • Self-reports of subjective hearing difficulties by people with hearing loss may be a useful complement to audiometry in hearing aid rehabilitation. To be useful, such self-reports need to be reliable. This study investigated the reliability and the validity of the Swedish Hearing Handicap Inventory for the Elderly (Screening Version; HHIE-S). Sixty-nine participants completed a questionnaire before hearing aid rehabilitation. Of these individuals, 49 completed hearing aid rehabilitation (aged between 23 and 94 years), and 41 of these 49 participants completed the questionnaire after completing the rehabilitation. The Swedish HHIE-S exhibited good internal consistency (Cronbachs alpha coefficient of .77). The questionnaire was effective for evaluating hearing aid rehabilitation, and a statistically significant reduction in hearing difficulties was observed. The clinicians found the questionnaire easy to administer and effective in hearing aid rehabilitation. The findings from the study support the use of the HHIE-S in clinical practice.
