  • Using quality improvement methods to implement guidelines to decrease the proportion of urinary retention in orthopaedic care

    • In patients treated with indwelling urethral catheter (IUC), complications such as catheter associated urinary tract infections are common, while underuse of IUC may cause harmful urinary retention (UR). A quality improvement (QI) program called ‘Indwelling urethral catheter (IUC) - only when needed’ was developed in Jönköping County Council, Sweden, aiming at creating a new approach: hospital staff should be able to identify and manage patients with risk of UR, prevent UR or treat UR without delay, and only use urinary IUCs on appropriate indications. The aim of this study was to describe the process of application of the quality improvement program. The Model for Improvement was used, and process coaches were appointed in the participating units. Their training was based on clinical issues and facts about UR, IUCs, guidelines, QI methods and measurements. Data were collected through prospective and retrospective patient record reviews, and differences were analyzed by inferential statistics.Before the intervention, only two patients out of 296 were cared for following the guidelines perfectly. During the intervention, adherence to guidelines showed a rising trend, and reached a new stable level, with an average of 67% adherence to guidelines. A systematic improvement program supported by coaches and improvement tools can increase the adherence to new guidelines and incorporate them into local practice. This study also shows that adherence to guidelines can improve patient safety, in this case a decreased risk for and incidence of UR in an orthopaedic patient population.

  • Prediction of Severe Acute Pancreatitis at Admission to Hospital Using Artificial Neural Networks.

    • Background/Aims: Artificial neural networks (ANNs) are non-linear pattern recognition techniques, which can be used as a tool in medical decision-making. The aim of this study was to construct and validate an ANN model for early prediction of the severity of acute pancreatitis (AP). Methods: Patients treated for AP from 2002 to 2005 (n = 139) and from 2007 to 2009 (n = 69) were analyzed to develop and validate the ANN model. Severe AP was defined according to the Atlanta criteria. Results: ANNs selected 6 of 23 potential risk variables as relevant for severity prediction, including duration of pain until arrival at the emergency department, creatinine, hemoglobin, alanine aminotransferase, heart rate, and white blood cell count. The discriminatory power for prediction of progression to a severe course, determined from the area under the receiver-operating characteristic curve, was 0.92 for the ANN model, 0.84 for the logistic regression model (p = 0.030), and 0.63 for the APACHE II score (p < 0.001). The numbers of correctly classified patients for a sensitivity of 50 and 75% were significantly higher for the ANN model than for logistic regression (p = 0.002) and APACHE II (p < 0.001). Conclusion: The ANN model identified 6 risk variables available at the time of admission, including duration of pain, a finding not being presented as a risk factor before. The severity classification developed proved to be superior to APACHE II. and IAP.

  • T A microarray analysis of the murine macrophage response to infection with Francisella tularensis LVS

    • The response of cells of the mouse macrophage cell line J774 to infection with Francisella tularensis LVS was analysed by means of a DNA microarray representing approximately 18 500 genes (20 600 clones). The adaptive response was modest at all time points, and at most, 81 clones were differentially regulated from the time point of uptake of bacteria (0 min) up to 240 min later. For all five time points, 229 clones fulfilled the criteria of being differentially regulated, i.e. the ratio between infected versus non-infected cells was at least 1.7-fold up- or down-regulated and P <0.05. It was found that many of the differentially regulated genes are known to respond to stress in general and to oxidative stress specifically. However, at 120 min it was observed that genes that lead to depletion of glutathione were upregulated. Possibly, this was a result of mechanisms induced by F. tularensis. Generally, there was a conspicuous lack of inflammatory responses and, for example, although tumour necrosis factor alpha (TNF-α) was upregulated at 0 min, a significant down-regulation was noted at all subsequent time points. When cells were treated with an inhibitor of inducible nitric oxide synthase (iNOS) or the antioxidant N-acetylcysteine (NAC), the infection-induced cytopathogenic effect was significantly inhibited. Together, the results suggest that F. tularensis LVS infection confers an oxidative stress upon the target cells and that many of the host-defence mechanisms appear to be intended to counteract this stress. The infection is characterized by a very modest inflammatory response.

    • To measure the effect of quality improvement interventions, it is appropriate to use analysis methods that measure data over time. Examples of such methods include statistical process control analysis and interrupted time series with segmented regression analysis. This article compares the use of statistical process control analysis and interrupted time series with segmented regression analysis for evaluating the longitudinal effects of quality improvement interventions, using an example study on an evaluation of a computerized decision support system.

  • Kartläggning av ägarskiften i företag : Utveckling och dokumentation av dataunderlag

    • Myndigheten för tillväxtpolitiska utvärderingar och analyser, Tillväxtanalys, har haft i uppdrag av regeringen att genomföra kartläggning och analys av ägarskiften i företag. Uppdraget omfattade även att göra jämförelser mellan generationsskiften och andra typer av ägarskiften som inte är åldersbaserade samt belysa möjligheten att följa upp företagens skifte och dynamik över tid. Tillväxtanalys lämnade i oktober 2013 en delrapportering av uppdraget som avser dels en kartläggning av åldersstrukturen i företagsstocken, dels en fördjupad analys av ägarskiften. Denna rapport är en redovisning av uppdragets sista del som innebär utveckling och dokumentation av dataunderlag för identifiering av ägarförändringar.SCB har på uppdrag av Tillväxtanalys undersökt möjligheten att bättre identifiera ägarskiften genom att integrera och göra en matchningskontroll av kompletterande uppgifter gällande delägare i fåmansaktiebolag som kan hämtas från Skatteverket, SKV, med redan befintlig information om företagsdynamik, vilken finns i Företagens och arbetsställenas dynamik, FAD, och den registerbaserade arbetsmarknadsstatisken, RAMS, på SCB. För att göra detta studeras företagens ägarskiften mellan åren 2010 och 2011. Uppdraget omfattar även att beskriva FAD gällande syfte, metodik och innehåll.Cirka 80 procent av samtliga företag oberoende av bolagsform överlevde mellan åren 2010 och 2011. Resterande företag har på något sätt förändrats. Företag läggs ned och andra startar som en del i strukturomvandlingen. Dessutom ser vi att många företag och arbetsställen byter ägare. Näringslivet visar på en stor dynamik och dynamiken omfattar många olika typer av förändringar.Bearbetningarna visar att det med hjälp av RAMS och SKV:s register går att följa och framställa uppgifter om ägarförändringar på ett bra sätt. Detta gäller både för fåmansaktiebolag, där det går att följa individuellt ägande, och för övriga juridiska former där förändringar i organisationsnummer kan spåras, vilket indikerar ändrade ägarförhållanden. Analysen visar att SKV:s register är ett bra komplement till RAMS och FAD och tillför ytterligare information. Samkörningen av dessa registerdata ger mer information än vad som kan fås ut av respektive registerkällor separat. Det går dessutom att koppla på annan information om anställda, omsättning och olika mått på lönsamhet från andra register.Tillväxtanalys rekommenderar därför att SCB får tillgång till register gällande ägarandelar från Skatteverket under en längre tidsperiod än enbart de undersökta åren. Detta för att kunna sammanställa en databas som möjliggör bättre och mer precisa analyser av olika dimensioner av näringslivsdynamik och ägarförändringar. Det vore dessutom önskvärt att denna databas görs tillgänglig för forskning och studier som ökar kunskapen om olika dimensioner av näringslivets strukturomvandling.

  • Trafiksäkerhetspåverkan vid omkörning av 30-metersfordon

    • Trafikverket överväger att tillåta längre och tyngre fordon på vägarna förutsatt att de inte påverkar trafiksäkerheten negativt. Syftet med studien var att undersöka säkerhetseffekten av fordonslängd, speciellt med avseende på olycksrisken vid omkörningar. Intervjuade förare av en 30-meters timmerbil hade inte upplevt de farhågor som förare av normallånga lastbilar uttryckt i samband med trånga rondeller och korsningar, men de nämner betydelsen av stödjande åkeri, arbetsmiljö och fordonsutrustning. En simulatorstudie studerade bilförares omkörningar av ett 30- och ett 18,75-metersfordon på en 2+1-väg i situationen då två körfält går ihop till ett. Tidluckan till ett återstående körfält var i genomsnitt 0,2 s (sign.) kortare efter omkörningar av 30-metersfordonet i situationer då bakänden var i samma relativa position som för 18,75-metersfordonet vid början av omkörningen. En fältstudie analyserade videoinspelade omkörningar av en 30- och en 24-meters timmerbil på en 2+1-väg och en tvåfältig väg. Ingen signifikant skillnad i tidluckor kunde påvisas mellan omkörningar av de två fordonen för någon av vägtyperna. Det senare resultatet ska dock tolkas med försiktighet på grund av ojämnt distribuerad data som insamlats under specifika förhållanden. Slutsatserna är att det finns en liten tendens till negativ säkerhetseffekt vid omkörningar av längre fordon, och att fler fältstudier är nödvändiga.

  • High-accuracy sampling of saproxylic diversity indicators at regional scales with pheromones : The case of Elater ferrugineus (Coleoptera, Elateridae)

    • The rare beetle Elater ferrugineus was sampled at 47 sites in the county of Östergötland, Sweden by means of pheromone-baited traps to assess its value as an indicator species for hollow oak stands rich in rare saproxylic beetle species. In addition, Osmoderma eremita was also sampled with pheromone baits. These data were then compared against species survey data collected at the same sites by pitfall and window traps. Both species co-occur with many Red Listed saproxylic beetles, with E. ferrugineus being a somewhat better indicator for the rarest species. The conservation value of a site (measured as Red List points or number of Red Listed species) increased with the number of specimens of E. ferrugineus and O. eremita caught. Accuracy of sampling by means of pheromone trapping turned out to be radically different for the two model species. E. ferrugineus traps put out during July obtained full accuracy after only 6. days, whereas O. eremita traps needed to be out from early July to mid-August in order to obtain full accuracy with one trap per site. By using E. ferrugineus, or preferably both species, as indicator species, accuracy would increase and costs decrease for saproxylic biodiversity sampling, monitoring and identification of hotspots. © 2014 Elsevier Ltd.

    • This paper presents a 3rd-order, 3-bit continuous time (CT) Delta-Sigma modulator for an LTE radio receiver. By adopting a return-to-zero (RZ) pulse in the innermost DAC, the modulator shows a reduced sensitivity to loop-delay variations, and the additional loop delay compensation usually needed in CT modulators can be omitted. The modulator has been implemented in a 65nm CMOS process, where it occupies an area of 0.2mmx0.4mm. It achieves an SNR of 71dB and an SNDR of 69dB over a 9MHz bandwidth with an oversampling ratio of 16. Power consumption is 7.5mW from a 1.2V supply, for a figure-of-merit of 181fJ/conversion.

  • A Filtering Delta Sigma ADC for LTE and Beyond

    • Abstract in UndeterminedThis paper presents a filtering ADC for the LTE communication standard, where a second-order Delta-Sigma modulator (DSM) is incorporated into the third-order Chebychev channel-select filter (CSF) of the radio receiver. The CSF introduces an additional third-order suppression of both thermal and quantization DSM noise, while the CSF transfer function is maintained. A design method for the filtering ADC accounting for unavoidable DSM-DAC delays is developed and experimentally demonstrated. The 65 nm CMOS prototype is clocked at 576/288 MHz with an 18.5/9.0 MHz LTE bandwidth, has an in-band gain of 26 dB, an SNDR of 56.4/58.1 dB, an input-referred noise of 5 nV/root Hz, and an out-of-band (half-duplex) IIP3 of 20/12 dBV(rms), with a power consumption of 7.9/5.4 mW and an overall state-of-the art performance.

    • Low-density polyethylene (LDPE) is the preferred type of polyolefin for many medical and electrical applications because of its superior purity and cleanliness. However, the inferior thermo-mechanical properties as compared to, for example, high-density polyethylene (HDPE), which arise because of the lower melting temperature of LDPE, constitute a significant drawback. Here, we demonstrate that the addition of minute amounts of HDPE to a LDPE resin considerably improves the mechanical integrity above the melting temperature of LDPE. A combination of dynamic mechanical analysis and creep experiments reveals that the addition of as little as 1 to 2 wt% HDPE leads to complete form stability above the melting temperature of LDPE. The investigated LDPE/HDPE blend is found to be miscible in the melt, which facilitates the formation of a solid-state microstructure that features a fine distribution of HDPE-rich lamellae. The absence of creep above the melting temperature of LDPE is rationalized with the presence of tie chains and trapped entanglements that connect the few remaining crystallites.

  • Dielectric loss determination of fine residual waste electrical and electronic equipment for understanding of heat development during microwave pyrolysis

    • Waste electrical and electronic equipment (WEEE) contains rare and valuable metals which are important to recycle, but it also has high organic content. The conventional methods used today for treatment of WEEE cannot recycle 100%; they generate valuable fine residual fractions. For further processing of these residues, microwave pyrolysis has shown to be promising since it reduces the organic content and liberate enclosed metal pieces. However, to fully control the process, the dielectric properties of the materials need to be known but are difficult to determine due to the complex composition and structure of WEEE. This paper aims to describe a suitable procedure for determination of dielectric loss in six WEEE residual fine fractions and to correlate the results to heating behaviour during microwave pyrolysis. Three fractions are "dusts" (light, medium and heavy dust) and three fractions are called "particulate materials" and contain smaller particles (0-7 mm, 0-20 mm and 7-12 mm). The method chosen was Vector Network Analyzer (VNA) using waveguide WR430 equipped with polymer foam to contain the samples which consisted of 1 dl of WEEE fine fraction. The scattering parameters were measured and dielectric loss in the materials was calculated. The measurements were performed in frequency region 1.3-2.7 GHz to include the industrial frequency 2.45 GHz. Differences between dielectric losses for the materials were determined by ANOVA. It was shown that the scattering parameter measurements were rather stable for the dust materials and the losses could be determined. For the larger sized particulate materials a lot of reflection was observed during the measurements, due to fine metal wires in the material. The losses could be determined after removing these wires. The previously observed higher heating rate and shorter time for mass reduction of dusts could not be explained by differences in dielectric loss. However, since VNA measurements indicate that metal wires in the material disturb the field; presence of wires in the particulate materials might be one cause for slower heating rate and observed temperature drops during pyrolysis.

  • Dielectric strength of γ-radiation cross-linked, high vinyl-content polyethylene

    • We explore γ-radiation cross-linking of high vinyl-content low-density polyethylene (LDPE) and its potential use as a high-voltage insulation material. Of the three investigated resins containing 1, 0.5 and 0.17 vinyl groups per 1000 carbons, respectively, only the highest vinyl-content material featured a sufficiently high gel content of more than 70% and hot-set elongation below 175%, when cross-linked with a γ-radiation dose of at least 68 kGy. Differential scanning calorimetry (DSC) and small-angle X-ray scattering (SAXS) reveal that neither the crystallinity nor the lamellar thickness of the highest vinyl-content LDPE are negatively affected by γ-radiation cross-linking. As a result, we find that the dielectric strength, as characterized by electrical tree initiation experiments, can be maintained upon γ-radiation cross-linking.

    • Objective: The main objective was to report mechanisms and precursors for post-endovascular aneurysm repair (EVAR) rupture. The second was to apply a structured protocol to explore whether these factors were identifiable on follow up computed tomography (CT) prior to rupture. The third objective was to study the incidence, treatment, and outcome of post-EVAR rupture. Methods: This was a multicentre, retrospective study of patients treated with standard EVAR at five Swedish hospitals from 2008 to 2018. Patients were identified from the Swedvasc registry. Medical records were reviewed up to 2020. Index EVAR and follow up data were recorded. The primary endpoint was post-EVAR rupture. CT at follow up and at post-EVAR rupture were studied, using a structured protocol, to determine rupture mechanisms and identifiable precursors. Results: In 1 805 patients treated by EVAR, 45 post-EVAR ruptures occurred in 43 patients. The cumulative incidence was 2.5% over a mean follow up of 5.2 years. The incidence rate was 4.5/1 000 person years. Median time to post-EVAR rupture was 4.1 years. A further six cases of post-EVAR rupture in five patients found outside the main cohort were included in the analysis of rupture mechanisms only. The rupture mechanism was type IA in 20 of 51 cases (39%), IB in 20 of 51 (39%) and IIIA/B in 11 of 51 (22%). One of these had type IA + IB combined. One patient had an aortoduodenal fistula without another mechanism being identified. Precursors had been noted on CT follow up prior to post-EVAR rupture in 16 of 51 (31%). Retrospectively, using the structured protocol, precursors could be identified in 43 of 51 (84%). In 17 of 27 (63%) cases missed on follow up but retrospectively identifiable, the mechanisms were type IB/III. Overall, the 30 day mortality rate after post-EVAR rupture was 47% (n = 24/51) and the post-operative mortality rate was 21% (n = 7/33). Conclusions: Most precursors of post-EVAR rupture are underdiagnosed but identifiable before rupture using a structured follow up CT protocol. Precursors of type IB and III failures caused the majority of post-EVAR ruptures.

  • Highly Insulating Polyethylene Blends for High-Voltage Direct-Current Power Cables

    • The insulation of state-of-the-art extruded high-voltage direct-current (HVDC) power cables is composed of cross-linked low-density polyethylene. Driven by the search for sustainable energy solutions, concepts that improve the ability to withstand high electrical fields and, ultimately, the power transmission efficiency are in high demand. The performance of a HVDC insulation material is limited by its residual electrical conductivity. Here, we demonstrate that the addition of small amounts of high-density polyethylene (HDPE) to a low-density polyethylene (LDPE) resin results in a drastic reduction in DC conductivity. An HDPE content as low as 1 wt % is found to introduce a small population of thicker crystalline lamellae, which are finely distributed throughout the material. The change in nanostructure correlates with a decrease in DC conductivity by approximately 1 order of magnitude to about 10(-15) S m(-1) at high electric fields of 30 and 40 kV mm(-1) and elevated temperature of 70 degrees C. This work opens up an alternative design concept for the insulation of HVDC power cables.

  • Influence of Molecular Weight on the Creep Resistance of Almost Molten Polyethylene Blends

    • The most common route to improve the creep resistance of low density polyethylene (LDPE) is crosslinking, which however results in volatile decomposition products that must be removed. Blends of LDPE and an additive-like amount of a linear polyethylene are found to offer improved creep resistance. Above the melting temperature of LDPE, T m ≈ 111 °C, a load-bearing network of higher-melting crystallites—connected through tie chains and trapped entanglements—provides additional form stability. The molecular weight of the linear polyethylene is found to be critical for the ability to arrest creep, which is correlated with the probability of tie chain formation as well as cocrystallization of the two polyethylenes. A number of high-density polyethylenes (HDPE) and one ultrahigh molecular weight polyethylene (UHMW-PE) are explored. For blends of LDPE and 2 wt% of the linear polyethylene, an HDPE with a weight-average molecular weight M w of 16 kg mol −1 is found to be sufficient to arrest creep at 115 °C. Further improvement in terms of creep resistance is obtained in case of UHMW-PE with creep fracture occurring only at a stress of 12 kPa at 115 °C. (Figure presented.).

  • Intrinsic and extrinsic influences on the temperature dependence of mobility in conjugated polymers

    • The temperature dependence of charge carrier mobility in conjugated polymers and their blends with fullerenes is investigated with different electrical methods, through field effect transistor (FET), space charge limited current (SCLC) and charge extraction (CELIV) measurements. Simple models, such as the Gaussian disorder model (GDM), are shown to accurately predict the temperature behavior, and a good correlation between the different measurement methods is obtained. Inconsistent charge carrier concentrations in the modeling are explained through intrinsic non-equilibrium effects, and are responsible for the limited applicability of existing numerical models. A severe extrinsic influence from water in FETs with a hydrophilic insulator interface is also demonstrated. The presence of water leads to a significant overestimate of the disorder in the materials from measurements close to room temperature and erratic behavior in the 150-350 K range. To circumvent this problem it is shown to be necessary to measure under ultra high vacuum (UHV) conditions. © 2008 Elsevier B.V. All rights reserved.

    • Blending of polyethylenes permits to combine the superior mechanical properties of high-density material with the higher purity that is associated with low-density resins. Mixing different polyethylene architectures offers a lot of advantages, but for electrical applications it is important that there is no detrimental effect on the resulting dielectric strength. Here, the nanostructure of crosslinked blends that comprise low-and high-density polyethylene (LDPE and HDPE) is explored. Despite the presence of higher-melting lamellae the formation of electrical trees under alternating current (AC) conditions is found to be invariant for the investigated HDPE content of 1 to 10 wt%. This observation suggests that the use of polyethylene blends is feasible for AC electrical applications.

    • Background: The FUS-DDIT3 fusion oncogene encodes an abnormal transcription factor that has a causative role in the development of myxoid/round-cell liposarcomas (MLS/RCLS). We have previously identified FLT1 (VEGFR1) as a candidate downstream target gene of FUS-DDIT3. The aim of this study was to investigate expression of FLT1 and its ligands in MLS cells. Methods: HT1080 human fibrosarcoma cells were transiently transfected with FUS-DDIT3-GFP variant constructs and FLT1 expression was measured by quantitative real-time PCR. In addition, FLT1, PGF, VEGFA and VEGFB expression was measured in MLS/RCLS cell lines, MLS/RCLS tumors and in normal adiopocytes. We analyzed nine cases of MLS/RCLS and one cell line xenografted in mice for FLT1 protein expression using immunohistochemistry. MLS/RCLS cell lines were also analyzed for FLT1 by immunofluorescence and western blot. MLS/RCLS cell lines were additionally treated with FLT1 tyrosine kinase inhibitors and assayed for alterations in proliferation rate. Results: FLT1 expression was dramatically increased in transfected cells stably expressing FUS-DDIT3 and present at high levels in cell lines derived from MLS. The FLT1 protein showed a strong nuclear expression in cells of MLS tissue as well as in cultured MLS cells, which was confirmed by cellular fractionation. Tissue array analysis showed a nuclear expression of the FLT1 protein also in several other tumor and normal cell types including normal adipocytes. The FLT1 ligand coding gene PGF was highly expressed in cultured MLS cells compared to normal adipocytes while the other ligand genes VEGFA and VEGFB were expressed to lower levels. A more heterogeneous expression pattern of these genes were observed in tumor samples. No changes in proliferation rate of MLS cells were detected at concentrations for which the kinase inhibitors have shown specific inhibition of FLT1. Conclusions: Our results imply that FLT1 is induced as an indirect downstream effect of FUS-DDIT3 expression in MLS. This could be a consequence of the ability of FUS-DDIT3 to hijack parts of normal adipose tissue development and reprogram primary cells to a liposarcoma-like phenotype. The findings of nuclear FLT1 protein and expression of corresponding ligands in MLS and normal tissues may have implications for tissue homeostasis and tumor development through auto- or intracrine signaling.

    • The human chymase (HC) is a major granule constituent of mast cells (MCs) residing in the connective tissue and the sub-mucosa. Although many potential substrates have been described for this important MC enzyme, its full range of in vivo substrates has most likely not yet been identified. A major step toward a better understanding of the function of the HC is therefore to determine its extended cleavage specificity. Using a phage-displayed random nonapeptide library, we show that the HC has a rather stringent substrate recognition profile. Only aromatic amino acids (aa) are accepted in position P1, with a   strong preference for Tyr and Phe over Trp. Aliphatic aa are preferred in positions P2 to P4 N-terminal of the cleaved bond. In the P1' position C-terminal of the cleaved bond, Ser is clearly over-represented and acidic aa Asp and Glu are strongly preferred in the P2' position. In P3', the small aliphatic aa Ala, Val and Gly were frequently observed. The consensus sequence, from P4 to P3': Gly/Leu/Val-Val/Ala/Leu-Ala/Val/Leu-Tyr/Phe-Ser-Asp/Glu-Ala/Val/Gly,   provides an instrument for the identification of novel in vivo substrates for the HC. Interestingly, a very similar cleavage specificity was recently reported for the major chymase in mouse connective tissue mast cells (CTMCs), the beta-chymase mouse mast cell   protease-4, suggesting functional homology between these two enzymes. This indicates that a rather stringent chymotryptic substrate recognition profile has been evolutionary conserved for the dominant CTMC chymase in mammals.

    • Different mixtures of identically substituted C60 and C70 based fullerens have been used as acceptors in three polymer: fullerene systems that strongly express various performance limiting aspects of bulk heterojunction solar cells. Results are correlated with, and discussed in terms of e.g. morphology, charge separation, and charge transport. In these systems, there appears to be no relevant differences in either mobility or energy level positions between the identically substituted C60 and C70 based fullerenes tested. Examples of how fullerene mixtures influence the nano-morphology of the active layer are given. An upper limit to the open circuit voltage that can be obtained with fullerenes is also suggested.

    • A recently developed variant of quartz crystal microbalance (QCM) called QCM-with dissipation monitoring (QCM-D) allows simultaneous and simple measurements of changes in adsorbed mass as well as the viscoelastic property (D-factor) of deposited protein layers on the sensor surface. We have taken the QCM-D technology a step further and demonstrated its advantages in the study of protein assembly as a consequence of surface induced immune complement activation, or contact activated blood coagulation. In the present study we have continued our QCM-D investigations of surface assembly of fibrin clot formation and complement activation and incubated differently modified quartz sensor surfaces in blood plasma and sera. Polymer surfaces used were spin-coated polyethylene, poly(ethylene terephtalate), poly(methylmetacrylate) and poly(dimethylsiloxane). Also used were sputtered titanium and heparin grafted surfaces. In this investigation we found that we could describe the surface induced coagulation with four independent parameters: (1) Time of onset of coagulation, (2) fibrin deposition rate, (3) total frequency shift at stable plateau, and (4) fibrin clot density. The most important finding was that the blood plasma clot density can be assessed with the use of D determinations and that the clot density varied significantly with the chemical composition of the surface. However, the D-factor did not give any new analytical information about the possible complement activation mechanisms. Nevertheless, the QCM-D was found to be a reliable tool for the analysis of surface induced complement activation. We also compared the QCM-D technique with traditional enzyme immuno assay (EIA) measurements of soluble products from the surface activation of the complement and coagulation systems. We found that the results from EIA and QCM-D measurements corresponded well for the complement activation but not for the coagulation, probably due to the biological complexity of the coagulation system.

    • This paper presents a 3rd-order, 3-bit continuous-time (CT) \Updelta\Upsigma Δ Σ modulator for an LTE radio receiver. A return-to-zero (RZ) pulse, centered in the sampling period by a quadrature clock, is used in the innermost DAC to reduce the sensitivity to loop-delay variations in the modulator, and omit implementing the additional loop delay compensation usually needed in CT modulators. The performance and stability of the NRZ/NRZ/RZ feedback scheme is thoroughly analysed using a discrete-time model. The modulator has been implemented in a 65 nm CMOS process, where it occupies an area of 0.2 × 0.4 mm2. It achieves an SNR of 71 dB and an SNDR of 69 dB over a 9 MHz bandwidth with an oversampling ratio of 16, and a power consumption of 7.5 mW from a 1.2 V supply.

    • Survivin is known as an inhibitor of apoptosis and a positive regulator of cell division. We have recently identified survivin as a predictor of joint destruction in patients with rheumatoid arthritis (RA). Flt3 ligand (Flt3L) is expressed in the inflamed joints and has adjuvant properties in arthritis. Studies on 90 RA patients (median age 60.5 years [range, 24-87], disease duration 10.5 years [range, 0-35]) show a strong positive association between the levels of survivin and Flt3L in blood. Here, we present experimental evidence connecting survivin and Flt3L signaling. Treatment of BALB/c mice with Flt3L led to an increase of survivin in the bone marrow and in splenic dendritic cells. Flt3L changed the profile of survivin splice variants, increasing transcription of the short survivin40 in the bone marrow. Treatment with an Flt3 inhibitor reduced total survivin expression in bone marrow and in the dendritic cell population in spleen. Inhibition of survivin transcription in mice, by shRNA lentiviral constructs, reduced the gene expression of Flt3L. We conclude that expression of survivin is a downstream event of Flt3 signaling, which serves as an essential mechanism supporting survival of leukocytes during their differentiation, and maturation of dendritic cells, in RA.

  • CODUSA - Customize Optimal Donor Using Simulated Annealing In Heart Transplantation.

    • In heart transplantation, selection of an optimal recipient-donor match has been constrained by the lack of individualized prediction models. Here we developed a customized donor-matching model (CODUSA) for patients requiring heart transplantations, by combining simulated annealing and artificial neural networks. Using this approach, by analyzing 59,698 adult heart transplant patients, we found that donor age matching was the variable most strongly associated with long-term survival. Female hearts were given to 21% of the women and 0% of the men, and recipients with blood group B received identical matched blood group in only 18% of best-case match compared with 73% for the original match. By optimizing the donor profile, the survival could be improved with 33 months. These findings strongly suggest that the CODUSA model can improve the ability to select optimal match and avoid worst-case match in the clinical setting. This is an important step towards personalized medicine.

    • We have examined the global gene expression profile of small intestinal carcinoids by microarray analysis. High expression of a number of genes was found including amyloid precursor-like protein 1 (APLP1). Quantitative real-time PCR and western blot analysis demonstrated higher expression of APLP1 in carcinoid metastases relative to primary tumours indicating a role of APLP1 in tumour dissemination. Tissue microarray analysis of gastroentero-pancreatic tumours demonstrated a high frequency of APLP1 expression and a low frequency of APLP2 expression in neuroendocrine (NE) tumours when compared with non-NE tumours at the same sites. Meta-analysis of gene expression data from a large number of tumours outside the gastrointestinal tract confirmed a correlation between APLP1 expression and NE phenotype where high expression of APLP1 was accompanied by downregulation of APLP2 in NE tumours. Cellular localization of APLP1, APLP2 and amyloid precursor protein (APP) in carcinoid cells (GOT1) by confocal microscopy demonstrated partial co-localization with synaptophysin. This suggests that the APP family of proteins is transported to the cell membrane by synaptic microvesicles and that they may influence tumour cell adhesion and invasiveness. We conclude that APLP1 is differentially upregulated in gastrointestinal NE tumours and that APLP1 may be important for the dissemination of small intestinal carcinoids. Identification of APLP1 in NE tumours offers a novel target for treatment and may also serve as a tumour-specific marker.

    • I detta uppdrag undersöker vi kemiska tillsatser i plaster som försvårar eller utgör hinder för återvinning av materialet. I denna rapport avser kemiska tillsatser additiv som medvetet introducerats i produkten eller materialet, och det är dessa kemiska ämnens natur som avgör problematiken ur återvinningssynpunkt. Vi fokuserar på byggsektorn eftersom denna sektor använder stora mängder plast av hög kvalité. Trots detta är återvinningsgraden för plast låg och potentialen att öka återvinningsgraden är därmed stor. Ett materials potential för återvinning bestäms av flera faktorer, varav kemikalieinnehåll är en. Det är viktigt att tänka på vilken eller vilka produkter plasten är lämplig att återvinnas till, och hänsyn måste alltid tas till gällande kemikalielagstiftning för just de produkttyperna. Kemiska ämnen som kan vara skadliga för människan och/eller miljön är särskilt viktiga att utreda, men det finns också andra tillsatser i material och produkter som försvårar återvinningsprocessen eller påverkar kvalitén på den återvunna plasten negativt så att marknaden för det återvunna materialet blir begränsad. Ytterligare en faktor att ta i beaktande är hur exponeringen för kemikalierna ser ut, om de är bundna i plasten eller kan emitteras och utsätta människor och miljön för direkta risker. De stora kategorierna i denna kartläggning har varit golv, rör och rördelar, kablar, profiler och lister, isoleringsmaterial, samt tätningsskikt. För dessa produktgrupper dominerar polymertyperna PVC, PE av olika densitetsgrad och comonomer-innehåll, PP (homo- och copolymer), PS och PUR. Då härdplaster, där även PUR ingår, förekommer i form av lacker, adhesiv och ytbeläggningar i byggprodukter behandlas dessa också övergripande. Många materialströmmar finns tillgängliga för återvinning inom kategorin byggplast generellt sett, men möjligheterna och incitamenten att sortera ut dessa i sina ursprungliga fraktioner är låg. Detta beror antingen på att efterfrågan på mekaniskt återvunnet material i dessa produktkategorier inte är stor nog, som för PEX och vissa typer av rör, eller på att volymerna är för låga för lönsamhet. Ett exempel på det sistnämnda är profiler och lister av PVC där etablerad cirkulär återvinning finns ute i Europa, men produktkategorin är för liten i Sverige för att drivkraften ska uppstå. Eftersom flera av de polymermaterial och produkter vi kartlagt i denna rapport har en historisk användning av idag reglerade, eller till och med förbjudna kemiska ämnen, kompliceras återvinningen av byggplast i att de inkommande avfallsströmmarna är av mycket varierande ålder. För att möjliggöra en högre återvinningsgrad och bättre kvalitet krävs därför utökad och mer noggrann sortering så att problematiska, och i vissa fall hälso- och miljöfarliga, innehållsämnen inte följer med i den mekaniska återvinningen, men inte heller så att kvalitativa fraktioner av en viss produkt- och polymertyp avvisas från återvinning av säkerhetsskäl. Ett axplock av problematiska tillsatser är tungmetallstabilisatorer och mjukgörare i produkter av PVC, flamskyddsmedel i isolering av EPS och XPS, samt silanförnätad PEX som innehåller tennkatalysator. Kontaminering i form av härdplastrester, felsorterad PEX i PE-recyklat, samt tejper och fogar på tätskikt utgör de mer oavsiktliga, fysiska hindren för kvalitetsmässig återvinning tillsammans med faktumet att en stor del av kablar och rör helt enkelt inte utvinns ur marken efter sin användningstid. Sammanfattningsvis skulle fler fraktioner av byggplast kunna återvinnas mekaniskt genom att stärka infrastrukturen kring insamling och sortering, men för detta krävs ökad efterfrågan och långsiktig lönsamhet. Kemiska återvinningsmetoder seglar upp som en möjlig lösning för flera av de hinder som identifieras i denna studie, till exempel tvärbundna material, material med hög andel fyllmedel, eller för avskiljning av oönskade tillsatser likt tungmetaller och ftalater. Kartläggningen av detta område får därför ses som en intressant frågeställning för ytterligare arbete.

  • Surface topography characterization of high-quality PE coated paperboard using confocal chromatic microscope and 3D SEM stereo-photogrammetry technique

    • Coating paperboard enhances printability and optical quality of the product as well as other important properties like packaging performance and shelf-life. To obtain high quality products a quality control of the manufacturing process requires identifying those manufacturing parameters that affect it. Roughness measurement and characterization of coating thickness are examples of these control parameters. Optical instruments measuring these quantities range from laboratory equipment to in-line and on-line sensors. However, the variety of instruments and sometimes misunderstanding of their limitations generate uncorrelated measurements, which are no longer valid to their comparison. The new ISO 25178 standard for surface texture provides guidelines to metrologists to address some this problem. Here, we report a case study for surface characterization of high quality printing polyethylene (PE) coated paperboard for high quality printing, where surface roughness is a key parameter. Two imaging methods to create topographic measurements will be compared, i.e. a confocal chromatic microscope and a scanning electron microscope (SEM). For the latter, stereo photogrammetry is used and 3D topographic profiles are obtained from Alicona MeX software. Leach and Haitjema [Leach, R., & Haitjema, H. (2010). Bandwidth characteristics and comparisons of surface texture measuring instruments. Measurement Science and Technology, 21(3), 032001] addressed the topic on how to design comparisons when using different instruments for areal texture measurement. We use their bandwidth matching concept, since it provides an extension to the ISO 25178 guidelines, ensuring that the instrumentation used to characterize the samples are within its measuring limits and further analysis of the results can be correlated. It is important to adopt a good metrology practice in order to translate these parameters into our future work. We expect to extend these findings into a real-time optical sensor, which later can be implemented in an industrial manufacturing environment for high optical quality paper and paperboard.

    • A conjugated polymer with a low band gap of 1.21 eV, i.e., absorbing infrared light, is demonstrated as active material in field-effect transistors (FETs). The material consists of alternating fluorene units and low band gap segments with electron donor-acceptor-donor units composed of two electron-donating thiophene rings attached on both sides of a thiadiazolo-quinoxaline electron-acceptor group. The polymer is solution-processable and air-stable; the resulting FETs exhibit typical p-channel characteristics and field-effect mobility of 0.03 cm2 V−1 s−1.

    • A new alternating polyfluorene copolymer poly[2,7-(9,9-dioctylfluoren)-alt-5,5-(5',8'-di-2-thienyl-(2',3'-bis-(3''-octyloxyphenyl)-quinoxaline))] (APFO-15), which has electron donor-acceptor-donor units in between the fluorene units, is synthesized and characterized. This polymer has a strong absorption and emission in the visible range of the solar spectrum. Its electroluminescence and photolumin escence emissions extend from about 560 to 900 nm. Moreover, solar cells with efficiencies in excess of 3.5 % have been realized from blends of APFO-15 and an electron acceptor mol., a mathanofullerene [6,6]-phenyl-C61-butyric acid Me ester (PCBM). It has also been obsd. that electron and hole transport is balanced both in the pure polymer phase and in polymer/PCBM bulk heterojunction films, which makes this material quite attractive for applications in opto-electronic devices.     • Background: Olfactory dysfunction together with neurological and cognitive symptoms are common after COVID-19. We aimed to study whether performance on olfactory and neuropsychological tests following infection predict post-COVID condition (PCC), persisting symptoms, and reduced health-related quality of life.Methods: Both hospitalized (N = 10) and non-hospitalized individuals (N = 56) were enrolled in this prospective cohort study. Participants were evaluated 1–3 months after infection with an olfactory threshold test and neuropsychological tests, which was used as predictors of PCC. A questionnaire outlining persisting symptoms and the validated instrument EuroQol five-dimension five-level for health-related quality of life assessment were used as outcome data 1 year after infection (N = 59). Principal component analysis was used to identify relevant predictors for PCC at 1 year.Results: Objectively assessed olfactory dysfunction at 1–3 months post infection, but not subjective olfactory symptoms, predicted post-COVID condition with reduced health-related quality of life (PCC+) at 1 year. The PCC+ group scored more often below the cut off for mild cognitive impairment on the Montreal Cognitive Assessment (61.5% vs. 21.7%) and higher on the Multidimensional Fatigue Inventory-20, compared to the group without PCC+.Conclusion: Our results indicate that objectively assessed, olfactory dysfunction is a predictor for PCC+. These findings underscore the importance of objective olfactory testing. We propose that olfactory screening in the early post-acute phase of COVID-19 infection might identify individuals that are at higher risk of developing long-term health sequalae.

    • FUS (also called TLS), EWSR1 and TAF15 (also called TAF2N) are related genes involved in tumor type-specific fusion oncogenes in human malignancies. The FUS-DDIT3 fusion oncogene results from a t(12;16)(q13;p11) chromosome translocation and has a causative role in the initiation of myxoid/round cell liposarcomas (MLS/RCLS). The FUS-DDIT3 protein induces increased expression of the CAAT/enhancer-binding protein (C/EBP) and nuclear factor-kappaB (NF-kappaB)-controlled gene IL8, and the N-terminal FUS part is required for this activation. Chromatin immunoprecipitation analysis showed that FUS-DDIT3 binds the IL8 promoter. Expression studies of the IL8 promoter harboring a C/EBP-NF-kappaB composite site pinpointed the importance of NF-kappaB for IL8 expression in FUS-DDIT3-expressing cells. We therefore probed for possible interaction of FUS-DDIT3 with members of the NF-kappaB family. The nuclear factor NFKBIZ colocalizes with FUS-DDIT3 in nuclear structures, and immunoprecipitation experiments showed that FUS-DDIT3 binds the C-terminal of NFKBIZ. We also report that additional NF-kappaB-controlled genes are upregulated at the mRNA level in FUS-DDIT3-expressing cell lines and they can be induced by NFKBIZ. Taken together, the results indicate that FUS-DDIT3 deregulates some NF-kappaB-controlled genes through interactions with NFKBIZ. Similar mechanisms may be a part of the transformation process in other tumor types carrying FUS, EWSR1 and TAF15 containing fusion oncogenes.

    • The structural and magnetic properties of the Dy2CoMnO6, Dy2NiMnO6 and Gd2CoMnO6 double perovskites are investigated using X-ray powder diffraction and squid magnetometry. The materials adopt an orthorhombic structure (space ground Pnma) with disordered Co(Ni)/Mn cations, and exhibit ferrimagnetic transitions near T(C)85, 95, and 115K respectively. T-C was found to monotonously depend on the orthorhombic distortion (a-c)/(a+c) of the compounds. The crystal structure of the compounds was investigated as a function of temperature (16-1100K range), evidencing changes in the BO6 octahedron near T-C. The magnetic entropy changes are estimated for comparison of the magnetocaloric properties to those from literature.

    • DDIT3, also known as GADD153 or CHOP, encodes a basic leucine zipper transcription factor of the dimer forming C/EBP family. DDIT3 is known as a key regulator of cellular stress response, but its target genes and functions are not well characterized. Here, we applied a genome wide microarray based expression analysis to identify DDIT3 target genes and functions. By analyzing cells carrying tamoxifen inducible DDIT3 expression constructs we show distinct gene expression profiles for cells with cytoplasmic and nuclear localized DDIT3. Of 175 target genes identified only 3 were regulated by DDIT3 in both cellular localizations. More than two thirds of the genes were downregulated, supporting a role for DDIT3 as a dominant negative factor that could act by either cytoplasmic or nuclear sequestration of dimer forming transcription factor partners. Functional annotation of target genes showed cell migration, proliferation and apoptosis/survival as the most affected categories. Cytoplasmic DDIT3 affected more migration associated genes, while nuclear DDIT3 regulated more cell cycle controlling genes. Cell culture experiments confirmed that cytoplasmic DDIT3 inhibited migration, while nuclear DDIT3 caused a G1 cell cycle arrest. Promoters of target genes showed no common sequence motifs, reflecting that DDIT3 forms heterodimers with several alternative transcription factors that bind to different motifs. We conclude that expression of cytoplasmic DDIT3 regulated 94 genes. Nuclear translocation of DDIT3 regulated 81 additional genes linked to functions already affected by cytoplasmic DDIT3. Characterization of DDIT3 regulated functions helps understanding its role in stress response and involvement in cancer and degenerative disorders.

  • Gamma spectroscopy methodology for large amounts of environmental samples in Sweden 30 years after the Chernobyl accident

    • In a Swedish citizen science project, more than 200 elementary school classes participated in collecting fungi, soil samples, and droppings from deer and wild boar, from all over Sweden. The samples have been sent to a laboratory at Uppsala University where they are being analyzed through gamma spectroscopy with a shielded HPGe detector. The main objective is to scan the samples for 137Cs from the Chernobyl accident and compare the data with measurements from 1986, but uptake of naturally occuring radionuclides like 40K and radon daughters will also be determined. Together with the soil samples, transfer factors will be derived, and correlations for these factors will be sought for different species of fungi and soil types. The potential for correlating the results with different biological processes will also be investigated, in part through the collected animal droppings. This is a work in progress where the present status of the experimental setup and methodology are presented. Issues with the initial approach for corrections are discussed and preliminary results are presented.

    • How 1,8-diiodooctane (DIO) enhances performance of polymer solar cells based on polymer HXS-1 and fullerene [6,6]-phenyl C(71)-butyric acid methyl ester (PC(71)BM) from 3.6% to 5.4% is scrutinized with several techniques by comparing devices or blend films spin-coated from dichlorobenzene (DCB) to those from DCB/DIO (97.5:2.5 v/v). Morphology of blend films is examined with atomic force microscopy (AFM), transmission electron microscopy (TEM) and electron tomography (3-D TEM), respectively. Charge generation and recombination is studied with photoluminescence, and charge transport with field effect transistors. The morphology with domain size in 10-20 nm and vertical elongated clusters formed in DIO system is supposed to facilitate charge transport and minimize charge carrier recombination, which are the main reasons for enhancing power conversion efficiency (PCE) from 3.6% (without DIO) to 5.4% (with DIO). Furthermore, a two year inspection shows no significant impact of DIO on the shelf-stability of the solar cells. No visible degradation in the second year indicates that the morphology of the active layers in the devices is relatively stable after initial relaxation in the first year.

    • In this study we report the synthesis, characterization. and photovoltaic properties of a series of six Conjugated polymers based on donor-acceptor-donor (DAD) structure. The polymers are obtained via Suzuki polymerization of different alkoxy-substituted DAD monomers together with a substituted fluorene or phenylene monomer. Application of polymers as light-harvesting and electron-donating materials in solar cells, in conjunction with both [60]PCBM and [70]PCBM as acceptors, show power-conversion efficiencies (PCEs) up to 2.9%, values obtained without extensive optimization work. Furthermore, atomic force microscopy and field-effect transistor (FET) mobility measurements of acceptor-polymer mixtures show that differences in substitution on the polymers affect morphology, mobility, and device performance. Within the series of polymers, all showing similar optical absorption and redox behavior, substituents play an important role in phase separation on a micrometer scale, which in turn has a large impact on device performance. The phase-separation behavior is clearly seen in [70]PCBM devices where the best-performing devices are obtained using the polymers with short alkoxy groups or no substituents together with a high speed of spin coating during device preparation.

    • The influence of the ignition quality of diesel-and gasoline-like fuels on the lift-off length of the jet were investigated in an optical heavy duty engine. The engine was operated at a load of 22 bar IMEPg and 1200 rpm. A production type injector with standard holes were used. The lift-off length was recorded with high speed video Different injection pressures and inlet temperatures were used to affect conditions that consequently affect the lift-off length. No matter which fuel used nor injection pressure or inlet temperature, all lift-off lengths showed equal or close to equal lift-off length when stabilized. The higher octane fuel had a longer ignition delay and therefore the fuel penetrate the combustion chamber before auto ignition. This gave a longer lift-off length at the initial stage of combustion before reaching the same stabilized lift-off length. These results indicate that the hot combustion gases are a dominant factor to the lift-off length. Also, that possible soot reductions using high octanes fuels are feasible because of a longer ignition delay that allow more premixing, and an initially longer lift-off length due to longer penetration into the combustion chamber.
