    • The aim of this dissertation is to study the conditions of and changes in sociative democracy processes at club level. One sports club is studied, Örebro Sportklubb (ÖSK), from its foundation in 1908 up to 1989. The main sources are club minutes, member magazines and annual reports. Democracy, and its twofolded relation to sport and economy processes, is the main problem area of the study. The specific question is how aspirations for economic effectiveness and sporting success influenced the democracy processes in ÖSK between 1908 and 1989.The Swedish sports movement has been described as a democratic movement. But the same movement has also been portrayed as an undemocratic movement made of men, for men. The study is based on a broad understanding of the democracy concept where issues of representativeness, influence, participation and knowledge are prominent. At a club level, the study is analysing the contents of the Swedish sports movement's democracy and its change during the 1900s. The thesis also illustrates how the pursuit of economic efficiency affected the associative democracy. These efficiencies were visible already in the 1920s, but was deepened during the 1970s. In short, the democratic range decreased, and successful sections became less and less motivated to finance the deficits of other sections.But the increased market orientation did not only represent a threat to the associative democracy. Marketisation and commercialization also preconditioned the democracy. At the club arena (Eyravallen), the members met in the clubhouse and café which, in turn, deepened the social capital and friendships within the club.

  • On the Validity of Some Markovian Models in ATM Traffic Modelling

    • Markov modulated models are often used to model traffic in a network. How well the processes really capture the essentials of real traffic is less known. We review some of these models and apply a number of associated fitting methods to an Ethernet traffic in different timescales. The fitted models are then used as input to a buffer where buffer occupancies are used as a measure of the goodness of the models. Also, the ability of a fitting method to correctly model the traffic when applied to traffic generated from a model that the fitting method was intended for is examined.

    • BACKGROUND: In clinical praxis it is important to be able to assess the effect of rehabilitation in patients with chronic heart failure (CHF). There is therefore a need for simple clinical tests that are objective, reliable and valid measures. AIM: The aim of this study was to examine the reliability of three muscle endurance tests for patients with CHF and to compare the results to a healthy control group. METHOD: The study included 20 patients (7 women) with CHF and 20 healthy persons. Dynamic endurance of the shoulder and calf muscle was measured as well as static shoulder endurance. All three tests were performed twice, within 5-10 days. RESULTS: The reliability (rs) of the tests for the patients with CHF was between 0.90-0.99 and for the healthy persons between 0.79-0.98. There was also a significant difference in dynamic endurance in shoulder flexion and in heel-rise left leg test between patients with CHF and healthy persons. CONCLUSION: Tests of dynamic and static endurance of the shoulder muscle and dynamic calf muscle endurance are reliable assessment tools for use in clinical practice. Patients with CHF have reduced isotonic muscular endurance compared with healthy persons.

    • BACKGROUND: Development of left ventricular outflow tract obstruction (LVOTO) in patients with hypertrophic cardiomyopathy (HCM) is important for explaining symptoms and designing management. LVOTO is mostly caused by a combination of septal hypertrophy and systolic anterior movement of the mitral valve (SAM). The aim of the present study was to determine predictors of exercise induced LVOTO in a group of HCM patients.METHODS: We performed supine exercise Doppler echocardiography, including measurements of LV morphology and function and anterior mitral leaflet length, in 51 mildly symptomatic HCM (septal thickness≥15mm) and compared them with 50 healthy controls. Measurements were made at 1) rest, 2) Valsalva maneuver, 3) peak exercise and 4) post exercise. LVOTO was diagnosed as a LVOT gradient of >30mmHg at rest, after Valsalva and after exercise or ≥50mmHg at peak exercise.RESULTS: All patients stopped exercise because of exhaustion. 35% of the patients had resting LVOTO and 48% during Valsalva. At peak exercise, only 37% had LVOTO, who increased to 64% post exercise. Patients who developed LVOTO at peak exercise were more prone to continue having it post exercise (p<0.001), to have attenuated systolic blood pressure rise (p=0.011) and to have long anterior mitral valve leaflets (p<0.001). Backward multiple regression analysis showed the anterior mitral leaflet length as the strongest single independent predictor (β=0.36, p=0.010) for increased LVOT velocities, followed by basal septal thickness.CONCLUSION: In patients with HCM, LV outflow tract obstruction seems to be relatively uncommon during exercise but rather occurring minutes after stopping exercise. Exercise LVOTO seems to be determined by long anterior mitral leaflets in addition to the well established septal hypertrophy.

  • Sublingual grass allergen specific immunotherapy : a retrospective study of clinical outcome and discontinuation

    • Background: Sublingual immunotherapy (SLIT) is effective, tolerable, and convenient for many allergic patients. Still, real-world evidence is scarce and the aim of this study is to assess the patient reported outcome of treatment with SLIT against grass pollen allergy in a consecutive patient population.Methods: Patients (n = 329) who were confirmed to be allergic to timothy grass and had been prescribed SLIT were consecutively enrolled in the study and completed a questionnaire online or in hard copy.Results: 207 (62.9%) patients responded to the questionnaire. The female/male ratio was 105/102 with a mean age of 39 ± 11 years (range 19-70 years). 113 (55%) patients reported they had completed the full 3-year treatment period, 49 (24%) were still on treatment, and 45 (22%) had discontinued treatment prematurely. Respondents who had completed the full treatment period reported that their allergy symptoms in the most recent grass pollen season had improved to a larger extent than subjects still on treatment or discontinuing the treatment prematurely. Improvement of asthma was twice as common among patients who completed compared to discontinued treatment (42 vs. 20%). Younger age (37 ± 12 vs. 41 ± 11 years, p < 0.001) and a higher prevalence of reported oral and/or gastrointestinal side effects (49 vs. 24%, p = 0.02) characterised the group that terminated SLIT. Forgetfulness was the most commonly reported specific reason.Conclusion: Treatment perseverance resulted in improved patient reported outcome. Forgetfulness was the most frequently reported reason for discontinuing SLIT treatment against grass pollen allergy.

    • BackgroundThe SQ tree sublingual immunotherapy (SLIT)-tablet is authorised for treatment of allergic rhinoconjunctivitis with or without asthma in trees of the birch homologous group in 21 European countries. The primary objective of this study was to explore the safety in real-life.MethodsIn a prospective, non-interventional post-authorisation safety study (EUPAS31470), adverse events (AEs) and adverse drug reactions (ADRs) at first administration and follow-up visits, symptoms, medication use, and pollen food syndrome were recorded by physicians in 6 European countries during the first 4-6 months of treatment.ResultsADRs with the SQ tree SLIT-tablet were reported in 57.7% of 1069 total patients (median age 36.0 years, 53.7% female) during the entire observation period (severity, mild-to-moderate: 70.1%, severe: 4.7%, serious: 0.7%) and in 45.9% after first administration. ADRs were not increased with pollen exposure at first administration. With coadministration of the SQ tree and grass SLIT-tablet AEs were reported in 73.8% of patients and in 52.8% with the SQ tree SLIT-tablet alone. Nasal and eye symptoms improved in 86.9% and 80.9% of patients and use of symptomatic medication in 76.0%. PFS with symptoms was reported in 43.0% of patients at baseline and in 4.3% at the individual last visit.ConclusionsThe results of this non-interventional safety study with the SQ tree SLIT-tablet confirm the safety profile from placebo-controlled clinical trials and support effectiveness in real-life according to the published efficacy data. Safety was not impaired by pollen exposure at first administration or co-administration with other SLIT-tablets.

    • Activation of the beta-receptor for platelet-derived growth factor (PDGF) by its ligand leads to autophosphorylation on a number of tyrosine residues. Here we show that Tyr763 in the kinase insert region is a novel autophosphorylation site, which after phosphorylation binds the protein tyrosine phosphatase SHP-2. SHP-2 has also previously been shown to bind to phosphorylated Tyr1009 in the PDGF beta-receptor. Porcine aortic endothelial (PAE) cells transfected with a PDGF beta-receptor in which Tyr763 and Tyr1009 were mutated to phenylalanine residues failed to associate with SHP-2 after ligand stimulation. Moreover, PDGF-BB-induced Ras GTP-loading and Erk2 activation were severely compromised in the receptor mutant. Whereas the mitogenic response to PDGF-BB remained at the same level as in cells expressing wild-type PDGF beta-receptor, chemotaxis induced by PDGF-BB was significantly decreased in the case of the Y763F/Y1009F mutant cells, suggesting an important role for SHP-2 in chemotactic signaling.
