    • Number and procedures of involuntary hospital admissions vary in Europe according to the different socio-cultural contexts. The European Commission has funded the EUNOMIA study in 12 European countries in order to develop European recommendations for good clinical practice in involuntary hospital admissions. The recommendations have been developed with the direct and active involvement of national leaders and key professionals, who worked out national recommendations, subsequently summarized into a European document, through the use of specific categories. The need for standardizing the involuntary hospital admission has been highlighted by all centers. In the final recommendations, it has been stressed the need to: providing information to patients about the reasons for hospitalization and its presumable duration; protecting patients’ rights during hospitalization; encouraging the involvement of family members; improving the communication between community and hospital teams; organizing meetings, seminars and focus-groups with users; developing training courses for involved professionals on the management of aggressive behaviors, clinical aspects of major mental disorders, the legal and administrative aspects of involuntary hospital admissions, on communication skills. The results showed the huge variation of involuntary hospital admissions in Europe and the importance of developing guidelines on this procedure.

    • Constant observation is a method used to insure the safety of suicidal inpatients. It involves structure and control as well as flexibility and the development of a relationship between the observer and the patient. It has been found that important observations may go unnoticed by the observer or fail to be communicated to the multidisciplinary team because of a lack of sufficient training and systematic documentation. We therefore conducted a Delphi survey to collect opinions on what would be important to observe during constant observation of suicidal patients. A panel of experienced clinicians, service users and researchers reached consensus on 37 of 40 observation items (92%). Of these, 28 were rated as the most important. As a result, we developed a form for systematic observer documentation in clinical practice, the Suicidal Patient Observation Chart. The Suicidal Patient Observation Chart includes the 28 items and covers 24 separate observation periods.

    • BACKGROUND AND AIM: Persistent organochlorine pollutants (POPs) have in epidemiological studies been associated with health hazards. The exposure window might be crucial and it is, accordingly, important to analyze exposure concentrations in samples collected at the window of interest. This makes the use of stored blood samples extremely interesting. However, small blood volumes from stored samples in biobanks means that the researchers in epidemiological studies most often have to select a limited number of biomarkers representing the POP exposure and abstain from analyses of lipid concentrations. This raises the question of the necessity of lipid adjustment. The aim of the present study was to evaluate the correlation between fresh weight and lipid adjusted concentrations. METHODS: We included data from studies were we have used the PCB congener 2,2',4,4',5,5'-hexachlorobiphenyl (PCB-153) as a biomarker for POP exposure and where fresh weight as well as lipid adjusted PCB-153 concentrations are known (n=1628). The variety of the cohort members in the different studies gave us the possibility to better understand how well these measures correlate. RESULTS: When all individuals were included in the analyses a correlation coefficient of 0.95 between fresh weight and lipid adjusted serum concentrations was obtained. Restricting the analyses to specific subgroups (based on gender, fasting status, age and BMI) gave correlation coefficients between 0.85 and 0.97. CONCLUSIONS: This study does strongly support the use of samples from biobanks in epidemiological studies for analyzing PCBs, at least regarding PCB-153, even though it is not possible to lipid adjust the results.

  • CD11b(+)/Gr-1(+) immature myeloid cells mediate suppression of T cells in mice bearing tumors of IL-1 beta-secreting cells

    • Tumor cells secreting IL-1 beta are invasive and metastatic, more than the parental line or control mock-transfected cells, and concomitantly induce in mice general immune suppression of T cell responses. Suppression strongly correlates with accumulation in the peripheral blood and spleen of CD11b(+)/Gr-1(+) immature myeloid cells and hematological alterations, such as splenomegaly, leukocytosis, and anemia. Resection of large tumors of IL-1 beta-secreting cells restored immune reactivity and hematological alterations within 7-10 days. Treatment of tumor-bearing mice with the physiological inhibitor of IL-1, the IL-1R antagonist, reduced tumor growth and attenuated the hematological alterations. Depletion of CD11b(+)/Gr-1(+) immature myeloid cells from splenocytes of tumor-bearing mice abrogated suppression. Despite tumor-mediated suppression, resection of large tumors of IL-1 beta-secreting cells, followed by a challenge with the wild-type parental cells, induced resistance in mice; protection was not observed in mice bearing tumors of mock-transfected fibrosarcoma cells. Altogether, we show in this study that tumor-derived IL-1 beta, in addition to its proinflammatory effects on tumor invasiveness, induces in the host hematological alterations and tumor-mediated suppression. Furthermore, the antitumor effectiveness of the IL-1R antagonist was also shown to encompass restoration of hematological alterations, in addition to its favorable effects on tumor invasiveness and angiogenesis that have previously been described by us.
