  • Gastric Bypass Surgery Elevates NT-ProBNP Levels

    • BackgroundBrain natriuretic peptide (BNP) is produced in the heart in response to stretching of the myocardium. BNP levels are negatively correlated to obesity, and in obese subjects, a reduced BNP responsiveness has been described. Diet-induced weight loss has been found to lower or to have no effect on BNP levels, whereas gastric banding and gastric bypass have reported divergent results. We studied obese patients undergoing gastric bypass (GBP) surgery during follow-up of 1 year.MethodsTwenty patients, 18 women, mean 41 (SD 9.5) years old, with a mean preoperative BMI of 44.6 (SD 5.5) kg/m2 were examined. N-terminal pro-brain natriuretic peptide (NT-ProBNP), glucose and insulin were measured preoperatively, at day 6 and months 1, 6 and 12. In 14 of the patients, samples were also taken at days 1, 2 and 4.ResultsThe NT-ProBNP levels showed a marked increase during the postoperative week (from 54 pg/mL preop to 359 pg/mL on day 2 and fell to 155 on day 6). At 1 year, NT-ProBNP was 122 pg/mL (125 % increase, p = 0.01). Glucose, insulin and HOMA indices decreased shortly after surgery without correlation to NT-ProBNP change. Mean BMI was reduced from 44.6 to 30.5 kg/m2 at 1 year and was not related to NT-ProBNP change.ConclusionsThe data indicate that GBP surgery rapidly alters the tone of BNP release, by a mechanism not related to weight loss or to changes in glucometabolic parameters. The GBP-induced conversion of obese subjects, from low to high NT-ProBNP responders, is likely to influence the evaluation of cardiac function in GBP operated individuals.

  • GLP1 analogs as treatment of postprandial hypoglycemia following gastric bypass surgery : a potential new indication?

    • Objective: The number of morbidly obese subjects submitted to bariatric surgery is rising worldwide. In a fraction of patients undergoing gastric bypass (GBP), episodes with late postprandial hypoglycemia (PPHG) develop 1-3 years after surgery. The pathogenesis of this phenomenon is not fully understood; meal-induced rapid and exaggerated increases of circulating incretins and insulin appear to be at least partially responsible. Current treatments include low-carbohydrate diets, inhibition of glucose intestinal uptake, reduction of insulin secretion with calcium channel blockers, somatostatin analogs, or diazoxide, a KATP channel opener. Even partial pancreatectomy has been advocated. In type 2 diabetes, GLP1 analogs have a well-documented effect of stabilizing glucose levels without causing hypoglycemia. Design: We explored GLP1 analogs as open treatment in five consecutive GBP cases seeking medical attention because of late postprandial hypoglycemic symptoms. Results: Glucose measured in connection with the episodes in four of the cases had been 2.7, 2.5, 1.8, and 1.6 mmol/l respectively. The patients consistently described that the analogs eliminated their symptoms, which relapsed in four of the five patients when treatment was reduced/discontinued. The drug effect was further documented in one case by repeated 24-h continuous glucose measurements. Conclusion: These open, uncontrolled observations suggest that GLP1 analogs might provide a new treatment option in patients with problems of late PPHG.

  • Hypoglycemia in everyday life after gastric bypass and duodenal switch

    • Design: Gastric bypass (GBP) and duodenal switch (DS) in morbid obesity are accompanied by marked metabolic improvements, particularly in glucose control. In recent years, episodes of severe late postprandial hypoglycemia have been increasingly described in GBP patients; data in DS patients are scarce. We recruited three groups of subjects; 15 GBP, 15 DS, and 15 non-operated overweight controls to examine to what extent hypoglycemia occurs in daily life. Methods: Continuous glucose monitoring (CGM) was used during 3 days of normal activity. The glycemic variability was measured by mean amplitude of glycemic excursion and continuous overall net glycemic action. Fasting blood samples were drawn, and the patients kept a food and symptom log throughout the study. Results: The GBP group displayed highly variable CGM curves, and 2.9% of their time was spent in hypoglycemia (< 3.3 mmol/l, or 60 mg/dl). The DS group had twice as much time in hypoglycemia (5.9%) and displayed CGM curves with little variation as well as lower HbA1c levels (29.3 vs 35.9 mmol/mol, P < 0.05). Out of a total of 72 hypoglycemic episodes registered over the 3-day period, 70 (97%) occurred in the postprandial state and only about one-fifth of the hypoglycemic episodes in the GBP and DS groups were accompanied by symptoms. No hypoglycemias were seen in controls during the 3-day period. Conclusion: Both types of bariatric surgery induce marked, but different, changes in glucose balance accompanied by frequent, but mainly unnoticed, hypoglycemic episodes. The impact and mechanism of hypoglycemic unawareness after weight-reduction surgery deserves to be clarified.

  • On the Impact of Bariatric Surgery on Glucose Homeostasis

    • Obesity has grown to epidemic proportions, and in lack of efficient life-style and medical treatments, the bariatric surgeries are performed in rising numbers. The most common surgery is the Gastric Bypass (GBP) surgery, with the Biliopancreatic diversion with duodenal switch (DS) as an option for the most extreme cases with a BMI>50 kg/m2.In paper I 20 GBP-patients were examined during the first post-operative year regarding the natriuretic peptide, NT-ProBNP, which is secreted from the cardiac ventricles. Levels of NT-ProBNP quickly increased during the first post-surgery week, and later established itself on a higher level than pre-surgery.In paper II we report of 5 patient-cases after GBP-surgery with severe problems with postprandial hypoglycaemia that were successfully treated with GLP-1-analogs. The effect of treatment could be observed both symptomatically and in some cases using continuous glucose measuring systems (CGMS).In paper III three groups of subjects; 15 post-GBP patients, 15 post-DS, and 15 obese controls were examined for three days using CGMS during everyday life. The post-GBP group had high glucose variability as measured by MAGE and CONGA, whereas the post-DS group had low variability. Both post-operative groups exhibited significant time in hypoglycaemia, about 40 and 80 minutes per day <3.3mmol/l and 20 and 40 minutes < 2.8mmol/l, respectively, longer time for DS-group. Remarkably, only about 20% of these hypoglycaemic episodes were accompanied with symptoms.In Paper IV the hypoglycaemia counter regulatory system was investigated; 12 patients were examined before and after GBP-surgery with a stepped hypoglycaemic hyperinsulinemic clamp. The results show a downregulation of symptoms, counter regulatory hormones (glucagon, cortisol, epinephrine, norepinephrine, growth hormone), incretin hormones (GLP-1 and GIP), and sympathetic nervous response.In conclusion patients post bariatric surgery exhibit a downregulated counter regulatory response to hypoglycaemia, accompanied by frequent asymptomatic hypoglycaemic episodes in everyday life. Patients suffering from severe hypoglycaemic episodes can often be treated successfully with GLP-1-analogues.

    • Roadgrip is an important parameter for vehicle testing and road maintenance. Therefore, an evaluation of the velocity and curvature effects on roadgrip measurement was performed on asphalt roads and on two ice tracks using the continuous roadgrip apparatus RT3 Curve. The aim was to find suitable driving patterns for measurements on public roads and test tracks to ensure the repeatability of roadgrip measurements. During the evaluation, it was concluded that in order to achieve a reliable roadgrip value, regardless of road conditions, the radius of curvature should not be less than 20 m. The velocity dependency of the RT3 Curve is different for the two road conditions, with the measurements on ice being much more sensitive to velocity changes than the measurements on the dry asphalt.

  • Investigation of ice surface change during vehicle testing

    • Today, there are a lot of vehicles and tyre testing carried out on lake ice surfaces. Thus, it is important to have knowledge about parameters that affect roadgrip. The thesis within this paper is that the liquid like layer which appears due to increasing temperature can be reduced by manipulating the ice roughness. This in turn should decrease the temperature dependence of the roadgrip in temperatures around 0°C. In order to investigate this, measurements of temperature, surface roughness and hardness and roadgrip were performed on three outdoor ice surfaces using an IR thermometer, an optical sensor with three IR-diodes, a steel ball drop indentation test and an RT3 curve, respectively. Additional ice roughness measurements were also made on two tempered ice surfaces in an ice hall. Results show a clear connection between ice temperature and roadgrip, unfortunately the created ice roughness was too small to influence the change in roadgrip

  • Direct low lateral slip roadgrip measurement compared with surface reflection of three laser beams

    • Road grip on roads depend on many factors and different techniques are available to estimate available road grip. To ensure that entrepreneurs have successfully restored the road grip to an acceptable level both entrepreneurs and road keepers need tools to evaluate present road grip. Information about road grip can also be utilized directly in a vehicle if available, for systems such as ABS, traction control, etc. To evaluate a non-contact method based on three laser beams with different frequency this equipment was mounted on a car equipped with a fifth wheel behind the car. The wheel is angled about 1.5 degrees compared with the centreline of travel. A force sensor in the hub measures forces acting on a standard winter tire also mounted to the hub, this is considered to be one measurement of road grip. The main reason is to see how well one can estimate road grip with a low cost non contact device made with Laser beams in combination with a photodiode.

    • The Swedish road administration requested that an independent organization facilitate friction tests with several types of friction measuring devices. These tests were done on the following three surfaces: macro rough and micro smooth ice, macro- and micro smooth ice and macroand micro rough ice. The objective was to evaluate lateral low slip measuring devices against traditional longitudinal high slip devices. This was done by evaluating how they measure road grip, i.e. friction differences between road surfaces and the measuring tire. Tests were performed on Lake Kakel near Arjeplog, northern Sweden on March 18th, 2008. Weather conditions were stable with ice temperatures between -4°C (24.8°F) and -3°C (26.6°F). Tests showed that changes on the ice surfaces were readily detected with all four of the lateral low slip friction measurement devices and the two longitudinal high slip devices. Friction resolutions were higher with low lateral slip devices compared to high longitudinal slip devices. The tires on the devices were of different types and are the main reason for the difference in friction resolution. One major difference was detected; the lateral low slip method measured higher friction between its regular winter tire and the macro rough, micro smooth surfacecompared to the friction on the macro- and micro rough ice. Longitudinal high slip devices measured higher friction between its industrial friction tire and macro- and micro rough ice. Capacity to measure friction in curves was only displayed by the high longitudinal slip devices.

  • Road grip test in Arjeplog

    • The Swedish road administration sees a need to improve the road grip estimation capacity for the Swedish road system. The challenge is to find methods to measure road grip fast and reliable. There where six different system types at the tests in Arjeplog, three continuous, two system measuring road grip through deceleration and one system based on GPS and accelerometers. Two system types used air craft runway tires. The other systems used either studded winter tires or friction winter tires. Test runs where performed and road grip where recorded for three different surface types, old system 2000 ice, polished ice and new system 2000 ice. Weather data was collected during the three days. It is clear that all systems can detect changes in road grip. It is clear that continuous systems hold an information advantage over the sequence oriented systems. For measurements of road grip on winter roads, we recommend that rubber compounds adapted for cold conditions are used, as the resolution of the road grip measurement where better. In extreme condition like on polished ice, studded winter tires increase the road grip significantly.

    • Circulating proteins are vital in human health and disease and are frequently used as biomarkers for clinical decision-making or as targets for pharmacological intervention. Here, we map and replicate protein quantitative trait loci (pQTL) for 90 cardiovascular proteins in over 30,000 individuals, resulting in 451 pQTLs for 85 proteins. For each protein, we further perform pathway mapping to obtain trans-pQTL gene and regulatory designations. We substantiate these regulatory findings with orthogonal evidence for trans-pQTLs using mouse knockdown experiments (ABCA1 and TRIB1) and clinical trial results (chemokine receptors CCR2 and CCR5), with consistent regulation. Finally, we evaluate known drug targets, and suggest new target candidates or repositioning opportunities using Mendelian randomization. This identifies 11 proteins with causal evidence of involvement in human disease that have not previously been targeted, including EGF, IL-16, PAPPA, SPON1, F3, ADM, CASP-8, CHI3L1, CXCL16, GDF15 and MMP-12. Taken together, these findings demonstrate the utility of large-scale mapping of the genetics of the proteome and provide a resource for future precision studies of circulating proteins in human health.

  • Automotive winter testing with path-following steering robot

    • When testing modern active safety systems on cars, it is essential to test vehicle response to steering inputs. This paper highlights the use of a path-following steering robot for repetitive car testing in winter conditions. Experiments have been made with commonly used test sequences, e.g., lane-change, double lane-change, constant radius circle, and handling circuit. The steering input from the human driver is replaced by a steering robot and a path-following algorithm. The main focus for this paper is to describe what happens when one pushes the path-following system to, and beyond, the physical limitations of road grip when cornering at high vehicle speed. It shows that, with an appropriate tuning of the path-following parameters, the system performs predictably and satisfactorily. The overall conclusion is that a path-following steering robot is indeed useful for repetitive test in winter conditions with poor road grip.

    • This report highlights the use of a steering robot with path-following for car testing in winter conditions. The steering robot was provided from Anthony Best Dynamics. Experiments have been made with a couple of commonly used test sequences (lane change, dual lane change, constant radius circle, handling), the result from these test are shown and discussed. The path-following algorithm gives steering commands to the car (replace the human driver), these commands affect how the car will move; hence, the car performance is affected by the behaviour of the path-following algorithm. The settings of the path-following algorithm greatly affect the car response in different situations. This is especially noticeable when the car starts to slide, a situation common when driving on a slippery winter road. The main focus for this report is to describe what happens when you push the path-following system to, and beyond, the physical limitations concerning road grip and vehicle speed. A theoretical analysis of the path-following algorithm and how it reacts on large lateral errors and high sideslip angles was done. As shown in the results, the evaluated path-following test sequences in general did work out well. Some characteristics of the algorithm, especially noticeable when driving in winter condition, were found and discussed. The overall impression is that the ABD steering robot with path-following can indeed be useful for repetitive test in winter conditions with bad road grip.

    • We have assessed the population trends for the 255 bird species breeding in Sweden (including distinct subspecies), based on data for the last 30 and 10 years, respectively. Over the past 30 years more species have decreased (38%) than increased (32%) in numbers. In particular, formerly common farmland species have fared poorly but this is also true for some forest species. Over the past 10 years there are more species with increasing trends (29%) than there are species with decreasing trends (19%). Trends for several species in long-term decline have levelled off and have in some cases even started to increase. It is not known whether this recent change is a result of conservation efforts or simply that population numbers have stabilised at lower levels now permitted by the environment. It is therefore essential to initiate research devoted to finding factors directly linked to ongoing population changes, particularly for species in longterm decline. To cover population trends for all Swedish species additional monitoring programmes are needed, in particular on owls and in mountain habitats.

    • BACKGROUND: Subarachnoid haemorrhage (SAH) is a devastating disease, with high mortality rate and substantial disability among survivors. Its causes are poorly understood. We aimed to investigate risk factors for SAH using a novel nationwide cohort consortium.METHODS: We obtained individual participant data of 949 683 persons (330 334 women) between 25 and 90 years old, with no history of SAH at baseline, from 21 population-based cohorts. Outcomes were obtained from the Swedish Patient and Causes of Death Registries.RESULTS: During 13 704 959 person-years of follow-up, 2659 cases of first-ever fatal or non-fatal SAH occurred, with an age-standardized incidence rate of 9.0 [95% confidence interval (CI) (7.4-10.6)/100 000 person-years] in men and 13.8 [(11.4-16.2)/100 000 person-years] in women. The incidence rate increased exponentially with higher age. In multivariable-adjusted Poisson models, marked sex interactions for current smoking and body mass index (BMI) were observed. Current smoking conferred a rate ratio (RR) of 2.24 (95% CI 1.95-2.57) in women and 1.62 (1.47-1.79) in men. One standard deviation higher BMI was associated with an RR of 0.86 (0.81-0.92) in women and 1.02 (0.96-1.08) in men. Higher blood pressure and lower education level were also associated with higher risk of SAH.CONCLUSIONS: The risk of SAH is 45% higher in women than in men, with substantial sex differences in risk factor strengths. In particular, a markedly stronger adverse effect of smoking in women may motivate targeted public health initiatives.

  • Glycemic Control and Metabolic Adaptation in Response to High-Fat versus High-Carbohydrate Diets-Data from a Randomized Cross-Over Study in Healthy Subjects.

    • Granular study of metabolic responses to alterations in the ratio of dietary macro-nutrients can enhance our understanding of how dietary modifications influence patients with impaired glycemic control. In order to study the effect of diets enriched in fat or carbohydrates, fifteen healthy, normal-weight volunteers received, in a cross-over design, and in a randomized unblinded order, two weeks of an iso-caloric high-fat diet (HFD: 60E% from fat) and a high-carbohydrate diet (HCD: 60E% from carbohydrates). A mixed meal test (MMT) was performed at the end of each dietary period to examine glucose clearance kinetics and insulin and incretin hormone levels, as well as plasma metabolomic profiles. The MMT induced almost identical glycemia and insulinemia following the HFD or HCD. GLP-1 levels were higher after the HFD vs. HCD, whereas GIP did not differ. The HFD, compared to the HCD, increased the levels of several metabolomic markers of risk for the development of insulin resistance, e.g., branched-chain amino acid (valine and leucine), creatine and α-hydroxybutyric acid levels. In normal-weight, healthy volunteers, two weeks of the HFD vs. HCD showed similar profiles of meal-induced glycemia and insulinemia. Despite this, the HFD showed a metabolomic pattern implying a risk for a metabolic shift towards impaired insulin sensitivity in the long run.
