    • Background Colorectal cancer is the third leading cause of cancer deaths worldwide. Given the recent increasing trends in colorectal cancer incidence globally, up-to-date information on the colorectal cancer burden could guide screening, early detection, and treatment strategies, and help effectively allocate resources. We examined the temporal patterns of the global, regional, and national burden of colorectal cancer and its risk factors in 204 countries and territories across the past three decades. Methods Estimates of incidence, mortality, and disability-adjusted life years (DALYs) for colorectal cancer were generated as a part of the Global Burden of Diseases, Injuries and Risk Factors Study (GBD) 2019 by age, sex, and geographical location for the period 1990-2019. Mortality estimates were produced using the cause of death ensemble model. We also calculated DALYs attributable to risk factors that had evidence of causation with colorectal cancer. Findings Globally, between 1990 and 2019, colorectal cancer incident cases more than doubled, from 842 098 (95% uncertainty interval [UI] 810 408-868 574) to 2.17 million (2.00-2.34), and deaths increased from 518 126 (493 682-537 877) to 1.09 million (1.02-1.15). The global age-standardised incidence rate increased from 22.2 (95% UI 21.3-23.0) per 100 000 to 26.7 (24.6-28.9) per 100 000, whereas the age-standardised mortality rate decreased from 14.3 (13.5-14.9) per 100 000 to 13.7 (12.6-14.5) per 100 000 and the age-standardised DALY rate decreased from 308.5 (294.7-320.7) per 100 000 to 295.5 (275.2-313.0) per 100 000 from 1990 through 2019. Taiwan (province of China; 62.0 [48.9-80.0] per 100 000), Monaco (60.7 [48.5-73.6] per 100 000), and Andorra (56.6 [42.8-71.9] per 100 000) had the highest age-standardised incidence rates, while Greenland (31.4 [26.0-37.1] per 100 000), Brunei (30.3 [26.6-34.1] per 100 000), and Hungary (28.6 [23.6-34.0] per 100 000) had the highest age-standardised mortality rates. From 1990 through 2019, a substantial rise in incidence rates was observed in younger adults (age <50 years), particularly in high Socio-demographic Index (SDI) countries. Globally, a diet low in milk (15.6%), smoking (13.3%), a diet low in calcium (12.9%), and alcohol use (9.9%) were the main contributors to colorectal cancer DALYs in 2019. Interpretation The increase in incidence rates in people younger than 50 years requires vigilance from researchers, clinicians, and policy makers and a possible reconsideration of screening guidelines. The fast-rising burden in low SDI and middle SDI countries in Asia and Africa calls for colorectal cancer prevention approaches, greater awareness, and cost-effective screening and therapeutic options in these regions.     • In 2016, the dominant greenhouse gases released into Earth's atmosphere-carbon dioxide, methane, and nitrous oxide-continued to increase and reach new record highs. The 3.5 +/- 0.1 ppm rise in global annual mean carbon dioxide from 2015 to 2016 was the largest annual increase observed in the 58-year measurement record. The annual global average carbon dioxide concentration at Earth's surface surpassed 400 ppm (402.9 +/- 0.1 ppm) for the first time in the modern atmospheric measurement record and in ice core records dating back as far as 800000 years. One of the strongest El Nino events since at least 1950 dissipated in spring, and a weak La Nina evolved later in the year. Owing at least in part to the combination of El Nino conditions early in the year and a long-term upward trend, Earth's surface observed record warmth for a third consecutive year, albeit by a much slimmer margin than by which that record was set in 2015. Above Earth's surface, the annual lower troposphere temperature was record high according to all datasets analyzed, while the lower stratospheric temperature was record low according to most of the in situ and satellite datasets. Several countries, including Mexico and India, reported record high annual temperatures while many others observed near-record highs. A week-long heat wave at the end of April over the northern and eastern Indian peninsula, with temperatures surpassing 44 degrees C, contributed to a water crisis for 330 million people and to 300 fatalities. In the Arctic the 2016 land surface temperature was 2.0 degrees C above the 1981-2010 average, breaking the previous record of 2007, 2011, and 2015 by 0.8 degrees C, representing a 3.5 degrees C increase since the record began in 1900. The increasing temperatures have led to decreasing Arctic sea ice extent and thickness. On 24 March, the sea ice extent at the end of the growth season saw its lowest maximum in the 37-year satellite record, tying with 2015 at 7.2% below the 1981-2010 average. The September 2016 Arctic sea ice minimum extent tied with 2007 for the second lowest value on record, 33% lower than the 1981-2010 average. Arctic sea ice cover remains relatively young and thin, making it vulnerable to continued extensive melt. The mass of the Greenland Ice Sheet, which has the capacity to contribute similar to 7 m to sea level rise, reached a record low value. The onset of its surface melt was the second earliest, after 2012, in the 37-year satellite record. Sea surface temperature was record high at the global scale, surpassing the previous record of 2015 by about 0.01 degrees C. The global sea surface temperature trend for the 21st century-to-date of +0.162 degrees C decade(-1) is much higher than the longer term 1950-2016 trend of +0.100 degrees C decade(-1). Global annual mean sea level also reached a new record high, marking the sixth consecutive year of increase. Global annual ocean heat content saw a slight drop compared to the record high in 2015. Alpine glacier retreat continued around the globe, and preliminary data indicate that 2016 is the 37th consecutive year of negative annual mass balance. Across the Northern Hemisphere, snow cover for each month from February to June was among its four least extensive in the 47-year satellite record. Continuing a pattern below the surface, record high temperatures at 20-m depth were measured at all permafrost observatories on the North Slope of Alaska and at the Canadian observatory on northernmost Ellesmere Island. In the Antarctic, record low monthly surface pressures were broken at many stations, with the southern annular mode setting record high index values in March and June. Monthly high surface pressure records for August and November were set at several stations. During this period, record low daily and monthly sea ice extents were observed, with the November mean sea ice extent more than 5 standard deviations below the 1981-2010 average. These record low sea ice values contrast sharply with the record high values observed during 2012-14. Over the region, springtime Antarctic stratospheric ozone depletion was less severe relative to the 1991-2006 average, but ozone levels were still low compared to pre-1990 levels. Closer to the equator, 93 named tropical storms were observed during 2016, above the 1981-2010 average of 82, but fewer than the 101 storms recorded in 2015. Three basins-the North Atlantic, and eastern and western North Pacific-experienced above-normal activity in 2016. The Australian basin recorded its least active season since the beginning of the satellite era in 1970. Overall, four tropical cyclones reached the Saffir-Simpson category 5 intensity level. The strong El Nino at the beginning of the year that transitioned to a weak La Nina contributed to enhanced precipitation variability around the world. Wet conditions were observed throughout the year across southern South America, causing repeated heavy flooding in Argentina, Paraguay, and Uruguay. Wetter-than-usual conditions were also observed for eastern Europe and central Asia, alleviating the drought conditions of 2014 and 2015 in southern Russia. In the United States, California had its first wetter-than-average year since 2012, after being plagued by drought for several years. Even so, the area covered by drought in 2016 at the global scale was among the largest in the post-1950 record. For each month, at least 12% of land surfaces experienced severe drought conditions or worse, the longest such stretch in the record. In northeastern Brazil, drought conditions were observed for the fifth consecutive year, making this the longest drought on record in the region. Dry conditions were also observed in western Bolivia and Peru; it was Bolivia's worst drought in the past 25 years. In May, with abnormally warm and dry conditions already prevailing over western Canada for about a year, the human-induced Fort McMurray wildfire burned nearly 590000 hectares and became the costliest disaster in Canadian history, with $3 billion (U.S. dollars) in insured losses.

    • Background: Efforts to establish the 2015 baseline and monitor early implementation of the UN Sustainable Development Goals (SDGs) highlight both great potential for and threats to improving health by 2030. To fully deliver on the SDG aim of “leaving no one behind”, it is increasingly important to examine the health-related SDGs beyond national-level estimates. As part of the Global Burden of Diseases, Injuries, and Risk Factors Study 2017 (GBD 2017), we measured progress on 41 of 52 health-related SDG indicators and estimated the health-related SDG index for 195 countries and territories for the period 1990–2017, projected indicators to 2030, and analysed global attainment. Methods: We measured progress on 41 health-related SDG indicators from 1990 to 2017, an increase of four indicators since GBD 2016 (new indicators were health worker density, sexual violence by non-intimate partners, population census status, and prevalence of physical and sexual violence [reported separately]). We also improved the measurement of several previously reported indicators. We constructed national-level estimates and, for a subset of health-related SDGs, examined indicator-level differences by sex and Socio-demographic Index (SDI) quintile. We also did subnational assessments of performance for selected countries. To construct the health-related SDG index, we transformed the value for each indicator on a scale of 0–100, with 0 as the 2·5th percentile and 100 as the 97·5th percentile of 1000 draws calculated from 1990 to 2030, and took the geometric mean of the scaled indicators by target. To generate projections through 2030, we used a forecasting framework that drew estimates from the broader GBD study and used weighted averages of indicator-specific and country-specific annualised rates of change from 1990 to 2017 to inform future estimates. We assessed attainment of indicators with defined targets in two ways: first, using mean values projected for 2030, and then using the probability of attainment in 2030 calculated from 1000 draws. We also did a global attainment analysis of the feasibility of attaining SDG targets on the basis of past trends. Using 2015 global averages of indicators with defined SDG targets, we calculated the global annualised rates of change required from 2015 to 2030 to meet these targets, and then identified in what percentiles the required global annualised rates of change fell in the distribution of country-level rates of change from 1990 to 2015. We took the mean of these global percentile values across indicators and applied the past rate of change at this mean global percentile to all health-related SDG indicators, irrespective of target definition, to estimate the equivalent 2030 global average value and percentage change from 2015 to 2030 for each indicator. Findings: The global median health-related SDG index in 2017 was 59·4 (IQR 35·4–67·3), ranging from a low of 11·6 (95% uncertainty interval 9·6–14·0) to a high of 84·9 (83·1–86·7). SDG index values in countries assessed at the subnational level varied substantially, particularly in China and India, although scores in Japan and the UK were more homogeneous. Indicators also varied by SDI quintile and sex, with males having worse outcomes than females for non-communicable disease (NCD) mortality, alcohol use, and smoking, among others. Most countries were projected to have a higher health-related SDG index in 2030 than in 2017, while country-level probabilities of attainment by 2030 varied widely by indicator. Under-5 mortality, neonatal mortality, maternal mortality ratio, and malaria indicators had the most countries with at least 95% probability of target attainment. Other indicators, including NCD mortality and suicide mortality, had no countries projected to meet corresponding SDG targets on the basis of projected mean values for 2030 but showed some probability of attainment by 2030. For some indicators, including child malnutrition, several infectious diseases, and most violence measures, the annualised rates of change required to meet SDG targets far exceeded the pace of progress achieved by any country in the recent past. We found that applying the mean global annualised rate of change to indicators without defined targets would equate to about 19% and 22% reductions in global smoking and alcohol consumption, respectively; a 47% decline in adolescent birth rates; and a more than 85% increase in health worker density per 1000 population by 2030. Interpretation: The GBD study offers a unique, robust platform for monitoring the health-related SDGs across demographic and geographic dimensions. Our findings underscore the importance of increased collection and analysis of disaggregated data and highlight where more deliberate design or targeting of interventions could accelerate progress in attaining the SDGs. Current projections show that many health-related SDG indicators, NCDs, NCD-related risks, and violence-related indicators will require a concerted shift away from what might have driven past gains—curative interventions in the case of NCDs—towards multisectoral, prevention-oriented policy action and investments to achieve SDG aims. Notably, several targets, if they are to be met by 2030, demand a pace of progress that no country has achieved in the recent past. The future is fundamentally uncertain, and no model can fully predict what breakthroughs or events might alter the course of the SDGs. What is clear is that our actions—or inaction—today will ultimately dictate how close the world, collectively, can get to leaving no one behind by 2030.

  • Population and fertility by age and sex for 195 countries and territories, 1950–2017: a systematic analysis for the Global Burden of Disease Study 2017

    • Background: Population estimates underpin demographic and epidemiological research and are used to track progress on numerous international indicators of health and development. To date, internationally available estimates of population and fertility, although useful, have not been produced with transparent and replicable methods and do not use standardised estimates of mortality. We present single-calendar year and single-year of age estimates of fertility and population by sex with standardised and replicable methods. Methods: We estimated population in 195 locations by single year of age and single calendar year from 1950 to 2017 with standardised and replicable methods. We based the estimates on the demographic balancing equation, with inputs of fertility, mortality, population, and migration data. Fertility data came from 7817 location-years of vital registration data, 429 surveys reporting complete birth histories, and 977 surveys and censuses reporting summary birth histories. We estimated age-specific fertility rates (ASFRs; the annual number of livebirths to women of a specified age group per 1000 women in that age group) by use of spatiotemporal Gaussian process regression and used the ASFRs to estimate total fertility rates (TFRs; the average number of children a woman would bear if she survived through the end of the reproductive age span [age 10–54 years] and experienced at each age a particular set of ASFRs observed in the year of interest). Because of sparse data, fertility at ages 10–14 years and 50–54 years was estimated from data on fertility in women aged 15–19 years and 45–49 years, through use of linear regression. Age-specific mortality data came from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2017 estimates. Data on population came from 1257 censuses and 761 population registry location-years and were adjusted for underenumeration and age misreporting with standard demographic methods. Migration was estimated with the GBD Bayesian demographic balancing model, after incorporating information about refugee migration into the model prior. Final population estimates used the cohort-component method of population projection, with inputs of fertility, mortality, and migration data. Population uncertainty was estimated by use of out-of-sample predictive validity testing. With these data, we estimated the trends in population by age and sex and in fertility by age between 1950 and 2017 in 195 countries and territories. Findings: From 1950 to 2017, TFRs decreased by 49·4% (95% uncertainty interval [UI] 46·4–52·0). The TFR decreased from 4·7 livebirths (4·5–4·9) to 2·4 livebirths (2·2–2·5), and the ASFR of mothers aged 10–19 years decreased from 37 livebirths (34–40) to 22 livebirths (19–24) per 1000 women. Despite reductions in the TFR, the global population has been increasing by an average of 83·8 million people per year since 1985. The global population increased by 197·2% (193·3–200·8) since 1950, from 2·6 billion (2·5–2·6) to 7·6 billion (7·4–7·9) people in 2017; much of this increase was in the proportion of the global population in south Asia and sub-Saharan Africa. The global annual rate of population growth increased between 1950 and 1964, when it peaked at 2·0%; this rate then remained nearly constant until 1970 and then decreased to 1·1% in 2017. Population growth rates in the southeast Asia, east Asia, and Oceania GBD super-region decreased from 2·5% in 1963 to 0·7% in 2017, whereas in sub-Saharan Africa, population growth rates were almost at the highest reported levels ever in 2017, when they were at 2·7%. The global average age increased from 26·6 years in 1950 to 32·1 years in 2017, and the proportion of the population that is of working age (age 15–64 years) increased from 59·9% to 65·3%. At the national level, the TFR decreased in all countries and territories between 1950 and 2017; in 2017, TFRs ranged from a low of 1·0 livebirths (95% UI 0·9–1·2) in Cyprus to a high of 7·1 livebirths (6·8–7·4) in Niger. The TFR under age 25 years (TFU25; number of livebirths expected by age 25 years for a hypothetical woman who survived the age group and was exposed to current ASFRs) in 2017 ranged from 0·08 livebirths (0·07–0·09) in South Korea to 2·4 livebirths (2·2–2·6) in Niger, and the TFR over age 30 years (TFO30; number of livebirths expected for a hypothetical woman ageing from 30 to 54 years who survived the age group and was exposed to current ASFRs) ranged from a low of 0·3 livebirths (0·3–0·4) in Puerto Rico to a high of 3·1 livebirths (3·0–3·2) in Niger. TFO30 was higher than TFU25 in 145 countries and territories in 2017. 33 countries had a negative population growth rate from 2010 to 2017, most of which were located in central, eastern, and western Europe, whereas population growth rates of more than 2·0% were seen in 33 of 46 countries in sub-Saharan Africa. In 2017, less than 65% of the national population was of working age in 12 of 34 high-income countries, and less than 50% of the national population was of working age in Mali, Chad, and Niger. Interpretation: Population trends create demographic dividends and headwinds (ie, economic benefits and detriments) that affect national economies and determine national planning needs. Although TFRs are decreasing, the global population continues to grow as mortality declines, with diverse patterns at the national level and across age groups. To our knowledge, this is the first study to provide transparent and replicable estimates of population and fertility, which can be used to inform decision making and to monitor progress.     • Background The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2017 comparative risk assessment (CRA) is a comprehensive approach to risk factor quantification that offers a useful tool for synthesising evidence on risks and risk-outcome associations. With each annual GBD study, we update the GBD CRA to incorporate improved methods, new risks and risk-outcome pairs, and new data on risk exposure levels and risk- outcome associations. Methods We used the CRA framework developed for previous iterations of GBD to estimate levels and trends in exposure, attributable deaths, and attributable disability-adjusted life-years (DALYs), by age group, sex, year, and location for 84 behavioural, environmental and occupational, and metabolic risks or groups of risks from 1990 to 2017. This study included 476 risk-outcome pairs that met the GBD study criteria for convincing or probable evidence of causation. We extracted relative risk and exposure estimates from 46 749 randomised controlled trials, cohort studies, household surveys, census data, satellite data, and other sources. We used statistical models to pool data, adjust for bias, and incorporate covariates. Using the counterfactual scenario of theoretical minimum risk exposure level (TMREL), we estimated the portion of deaths and DALYs that could be attributed to a given risk. We explored the relationship between development and risk exposure by modelling the relationship between the Socio-demographic Index (SDI) and risk-weighted exposure prevalence and estimated expected levels of exposure and risk-attributable burden by SDI. Finally, we explored temporal changes in risk-attributable DALYs by decomposing those changes into six main component drivers of change as follows: (1) population growth; (2) changes in population age structures; (3) changes in exposure to environmental and occupational risks; (4) changes in exposure to behavioural risks; (5) changes in exposure to metabolic risks; and (6) changes due to all other factors, approximated as the risk-deleted death and DALY rates, where the risk-deleted rate is the rate that would be observed had we reduced the exposure levels to the TMREL for all risk factors included in GBD 2017.

  • Lunar occultation of the diffuse radio sky : LOFAR measurements between 35 and 80 MHz

    • We present radio observations of the Moon between 35 and 80 MHz to demonstrate a novel technique of interferometrically measuring large-scale diffuse emission extending far beyond the primary beam (global signal) for the first time. In particular, we show that (i) the Moon appears as a negative-flux source at frequencies 35 < ν < 80 MHz since it is ‘colder’ than the diffuse Galactic background it occults, (ii) using the (negative) flux of the lunar disc, we can reconstruct the spectrum of the diffuse Galactic emission with the lunar thermal emission as a reference, and (iii) that reflected RFI (radio-frequency interference) is concentrated at the centre of the lunar disc due to specular nature of reflection, and can be independently measured. Our RFI measurements show that (i) Moon-based Cosmic Dawn experiments must design for an Earth-isolation of better than 80 dB to achieve an RFI temperature <1 mK, (ii) Moon-reflected RFI contributes to a dipole temperature less than 20 mK for Earth-based Cosmic Dawn experiments, (iii) man-made satellite-reflected RFI temperature exceeds 20 mK if the aggregate scattering cross-section of visible satellites exceeds 175 m2 at 800 km height, or 15 m2 at 400 km height. Currently, our diffuse background spectrum is limited by sidelobe confusion on short baselines (10–15 per cent level). Further refinement of our technique may yield constraints on the redshifted global 21 cm signal from Cosmic Dawn (40 > z > 12) and the Epoch of Reionization (12 > z > 5).

    • Aims. This study aims to characterise the polarized foreground emission in the ELAIS-N1 field and to address its possible implications or extracting of the cosmological 21 cm signal from the LOw-Frequency ARray - Epoch of Reionization (LOFAR-EoR) data Methods. We used the high band antennas of LOFAR to image this region and RM-synthesis to unravel structures of polarized emission at high Galactic latitudes. Results. The brightness temperature of the detected Galactic emission is on average similar to 4 K in polarized intensity and covers the range from -10 to +13 rad m(-2) in Faraday depth, The total polarized intensity and polarization angle show a wide range of morphological features. We have also used the Westerbork Synthesis Radio Telescope (WSRT) at 350 MHz to image the same region. The LOFAR and WSRT images show a similar complex morphology at comparable brightness levels, but their spatial correlation is very low. The fractional polarization at 150 MHz, expressed as a percentage of the total intensity, amounts to approximate to 1.5%. There is no indication of diffuse emission in total intensity in the interferometric data. in line with results at higher frequencies Conclusions. The wide frequency range. high angular resolution, and high sensitivity make LOFAR an exquisite instrument for studying Galactic polarized emission at a resolution of similar to 1-2 rad m(-2) in Faraday depth. The different polarized patterns observed at 150 MHz and 350 MHz are consistent with different source distributions along the line of sight wring in a variety of Faraday thin regions of emission. The presence of polarized foregrounds is a serious complication for epoch of reionization experiments. To avoid the leakage of polarized emission into total intensity, which can depend on frequency, we need to calibrate the instrumental polarization across the field of view to a small fraction of 1%.

  • Initial deep LOFAR observations of epoch of reionization windows I. The north celestial pole

    • Aims. The aim of the LOFAR epoch of reionization (EoR) project is to detect the spectral fluctuations of the redshifted HI 21 cm signal. This signal is weaker by several orders of magnitude than the astrophysical foreground signals and hence, in order to achieve this, very long integrations, accurate calibration for stations and ionosphere and reliable foreground removal are essential. Methods. One of the prospective observing windows for the LOFAR EoR project will be centered at the north celestial pole (NCP). We present results from observations of the NCP window using the LOFAR highband antenna (HBA) array in the frequency range 115 MHz to 163 MHz. The data were obtained in April 2011 during the commissioning phase of LOFAR. We used baselines up to about 30 km. The data was processed using a dedicated processing pipeline which is an enhanced version of the standard LOFAR processing pipeline. Results. With about 3 nights, of 6 h each, effective integration we have achieved a noise level of about 100 mu Jy/PSF in the NCP window. Close to the NCP, the noise level increases to about 180 mu Jy/PSF, mainly due to additional contamination from unsubtracted nearby sources. We estimate that in our best night, we have reached a noise level only a factor of 1.4 above the thermal limit set by the noise from our Galaxy and the receivers. Our continuum images are several times deeper than have been achieved previously using the WSRT and GMRT arrays. We derive an analytical explanation for the excess noise that we believe to be mainly due to sources at large angular separation from the NCP. We present some details of the data processing challenges and how we solved them. Conclusions. Although many LOFAR stations were, at the time of the observations, in a still poorly calibrated state we have seen no artefacts in our images which would prevent us from producing deeper images in much longer integrations on the NCP window which are about to commence. The limitations present in our current results are mainly due to sidelobe noise from the large number of distant sources, as well as errors related to station beam variations and rapid ionospheric phase fluctuations acting on bright sources. We are confident that we can improve our results with refined processing.

    • We investigate the feasibility of detecting 21 cm absorption features in the afterglow spectra of high redshift long Gamma Ray Bursts (GRBs). This is done employing simulations of cosmic reionization, together with estimates of the GRB radio afterglow flux and the instrumental characteristics of the LOw Frequency ARray (LOFAR). We find that absorption features could be marginally (with a S/N larger than a few) detected by LOFAR at z greater than or similar to 7 if the GRB is a highly energetic event originating from Pop III stars, while the detection would be easier if the noise were reduced by one order of magnitude, i.e. similar to what is expected for the first phase of the Square Kilometre Array (SKA1-low). On the other hand, more standard GRBs are too dim to be detected even with ten times the sensitivity of SKA1-low, and only in the most optimistic case can a S/N larger than a few be reached at z greater than or similar to 9.

    • We report on the use of extreme ultraviolet (XUV, 30.3 nm) radiation from the Free-electron LASer in Hamburg (FLASH) and visible (Vis, 405 nm) photons from an optical laser to investigate the relaxation and fragmentation dynamics of fluorene ions. The ultrashort laser pulses allow to resolve the molecular processes occurring on the femtosecond timescales. Fluorene is a prototypical small polycyclic aromatic hydrocarbon (PAH). Through their infrared emission signature, PAHs have been shown to be ubiquitous in the universe, and they are assumed to play an important role in the chemistry of the interstellar medium. Our experiments track the ionization and dissociative ionization products of fluorene through time-of-flight mass spectrometry and velocity-map imaging. Multiple processes involved in the formation of each of the fragment ions are disentangled through analysis of the ion images. The relaxation lifetimes of the excited fluorene monocation and dication obtained through the fragment formation channels are reported to be in the range of a few tens of femtoseconds to a few picoseconds.

    • Polycyclic aromatic hydrocarbons (PAHs) play an important role in interstellar chemistry and are subject to high energy photons that can induce excitation, ionization, and fragmentation. Previous studies have demonstrated electronic relaxation of parent PAH monocations over 10-100 femtoseconds as a result of beyond-Born-Oppenheimer coupling between the electronic and nuclear dynamics. Here, we investigate three PAH molecules: fluorene, phenanthrene, and pyrene, using ultrafast XUV and IR laser pulses. Simultaneous measurements of the ion yields, ion momenta, and electron momenta as a function of laser pulse delay allow a detailed insight into the various molecular processes. We report relaxation times for the electronically excited PAH*, PAH(+*) and PAH(2+*) states, and show the time-dependent conversion between fragmentation pathways. Additionally, using recoil-frame covariance analysis between ion images, we demonstrate that the dissociation of the PAH(2+) ions favors reaction pathways involving two-body breakup and/or loss of neutral fragments totaling an even number of carbon atoms. Polycyclic aromatic hydrocarbons play an important role in interstellar chemistry, where interaction with high energy photons can induce ionization and fragmentation reactions. Here the authors, with XUV-IR pump-probe experiments, investigate the ultrafast photoinduced dynamics of fluorene, phenanthrene and pyrene, providing insight into their preferred reaction channels.

    • Aims. This paper discusses the spectral occupancy for performing radio astronomy with the Low-Frequency Array (LOFAR), with a focus on imaging observations.Methods. We have analysed the radio-frequency interference (RFI) situation in two 24-h surveys with Dutch LOFAR stations, covering 30-78 MHz with low-band antennas and 115-163 MHz with high-band antennas. This is a subset of the full frequency range of LOFAR. The surveys have been observed with a 0.76 kHz/1 s resolution.Results. We measured the RFI occupancy in the low and high frequency sets to be 1.8% and 3.2% respectively. These values are found to be representative values for the LOFAR radio environment. Between day and night, there is no significant difference in the radio environment. We find that lowering the current observational time and frequency resolutions of LOFAR results in a slight loss of flagging accuracy. At LOFAR's nominal resolution of 0.76 kHz and 1 s, the false-positives rate is about 0.5%. This rate increases approximately linearly when decreasing the data frequency resolution.Conclusions. Currently, by using an automated RFI detection strategy, the LOFAR radio environment poses no perceivable problems for sensitive observing. It remains to be seen if this is still true for very deep observations that integrate over tens of nights, but the situation looks promising. Reasons for the low impact of RFI are the high spectral and time resolution of LOFAR; accurate detection methods; strong filters and high receiver linearity; and the proximity of the antennas to the ground. We discuss some strategies that can be used once low-level RFI starts to become apparent. It is important that the frequency range of LOFAR remains free of broadband interference, such as DAB stations and windmills.

We investigated these ultrafast fragmentation reac-tions after ionization of the PAHs phenanthrene, fluorene and pyrene at a wavelength of 30.3 nm using pump probe spectroscopy at a free electron laser. We observe double ionization and afterwards hydrogen abstraction and acetylene loss with characteristic time scales for the reaction processes below one hundred femtoseconds.

    • Background The rapid spread of COVID-19 renewed the focus on how health systems across the globe are financed, especially during public health emergencies. Development assistance is an important source of health financing in many low-income countries, yet little is known about how much of this funding was disbursed for COVID-19. We aimed to put development assistance for health for COVID-19 in the context of broader trends in global health financing, and to estimate total health spending from 1995 to 2050 and development assistance for COVID-19 in 2020. Methods We estimated domestic health spending and development assistance for health to generate total health-sector spending estimates for 204 countries and territories. We leveraged data from the WHO Global Health Expenditure Database to produce estimates of domestic health spending. To generate estimates for development assistance for health, we relied on project-level disbursement data from the major international development agencies' online databases and annual financial statements and reports for information on income sources. To adjust our estimates for 2020 to include disbursements related to COVID-19, we extracted project data on commitments and disbursements from a broader set of databases (because not all of the data sources used to estimate the historical series extend to 2020), including the UN Office of Humanitarian Assistance Financial Tracking Service and the International Aid Transparency Initiative. We reported all the historic and future spending estimates in inflation-adjusted 2020 US$, 2020 US$ per capita, purchasing-power parity-adjusted US$ per capita, and as a proportion of gross domestic product. We used various models to generate future health spending to 2050. Findings In 2019, health spending globally reached $8. 8 trillion (95% uncertainty interval [UI] 8.7-8.8) or $1132 (1119-1143) per person. Spending on health varied within and across income groups and geographical regions. Of this total, $40.4 billion (0.5%, 95% UI 0.5-0.5) was development assistance for health provided to low-income and middle-income countries, which made up 24.6% (UI 24.0-25.1) of total spending in low-income countries. We estimate that $54.8 billion in development assistance for health was disbursed in 2020. Of this, $13.7 billion was targeted toward the COVID-19 health response. $12.3 billion was newly committed and $1.4 billion was repurposed from existing health projects. $3.1 billion (22.4%) of the funds focused on country-level coordination and $2.4 billion (17.9%) was for supply chain and logistics. Only $714.4 million (7.7%) of COVID-19 development assistance for health went to Latin America, despite this region reporting 34.3% of total recorded COVID-19 deaths in low-income or middle-income countries in 2020. Spending on health is expected to rise to $1519 (1448-1591) per person in 2050, although spending across countries is expected to remain varied. Interpretation Global health spending is expected to continue to grow, but remain unequally distributed between countries. We estimate that development organisations substantially increased the amount of development assistance for health provided in 2020. Continued efforts are needed to raise sufficient resources to mitigate the pandemic for the most vulnerable, and to help curtail the pandemic for all. Copyright (C) 2021 The Author(s).     • Aims: This study aims to characterize linear polarization structures in LOFAR observations of the interstellar medium (ISM) in the 3C 196 field, one of the primary fields of the LOFAR-Epoch of Reionization key science project.Methods: We have used the high band antennas (HBA) of LOFAR to image this region and rotation measure (RM) synthesis to unravel the distribution of polarized structures in Faraday depth.Results: The brightness temperature of the detected Galactic emission is 5-15 K in polarized intensity and covers the range from -3 to +8 rad m(-2) in Faraday depth. The most interesting morphological feature is a strikingly straight filament at a Faraday depth of +0.5 rad m(-2) running from north to south, right through the centre of the field and parallel to the Galactic plane. There is also an interesting system of linear depolarization canals conspicuous in an image showing the peaks of Faraday spectra. We used the Westerbork Synthesis Radio Telescope (WSRT) at 350 MHz to image the same region. For the first time, we see some common morphology in the RM cubes made at 150 and 350 MHz. There is no indication of diffuse emission in total intensity in the interferometric data, in line with results at higher frequencies and previous LOFAR observations. Based on our results, we determined physical parameters of the ISM and proposed a simple model that may explain the observed distribution of the intervening magneto-ionic medium.Conclusions: The mean line-of-sight magnetic field component, B-parallel to, is determined to be 0.3 +/- 0.1 mu G and its spatial variation across the 3C 196 field is 0.1 mu G. The filamentary structure is probably an ionized filament in the ISM, located somewhere within the Local Bubble. This filamentary structure shows an excess in thermal electron density (n(e)B(parallel to) > 6.2 cm(-3) mu G) compared to its surroundings.

    • Using a combination of N-body simulations, semi-analytic models and radiative transfer calculations, we have estimated the theoretical cross-power spectrum between galaxies and the 21 cm emission from neutral hydrogen during the epoch of reionization. In accordance with previous studies, we find that the 21 cm emission is initially correlated with haloes on large scales (greater than or similar to 30 Mpc), anticorrelated on intermediate (similar to 5 Mpc) and uncorrelated on small (less than or similar to 3 Mpc) scales. This picture quickly changes as reionization proceeds and the two fields become anticorrelated on large scales. The normalization of the cross-power spectrum can be used to set constraints on the average neutral fraction in the intergalactic medium and its shape can be a powerful tool to study the topology of reionization. When we apply a drop-out technique to select galaxies and add to the 21 cm signal the noise expected from the LOw Frequency ARray (LOFAR) telescope, we find that while the normalization of the cross-power spectrum remains a useful tool for probing reionization, its shape becomes too noisy to be informative. On the other hand, for an Ly alpha Emitter (LAE) survey both the normalization and the shape of the cross-power spectrum are suitable probes of reionization. A closer look at a specific planned LAE observing program using Subaru Hyper-Suprime Cam reveals concerns about the strength of the 21 cm signal at the planned redshifts. If the ionized fraction at z similar to 7 is lower than the one estimated here, then using the cross-power spectrum may be a useful exercise given that at higher redshifts and neutral fractions it is able to distinguish between two toy models with different topologies.

    • The first generation of redshifted 21 cm detection experiments, carried out with arrays like Low Frequency Array (LOFAR), Murchison Widefield Array (MWA) and Giant Metrewave Telescope (GMRT), will have a very low signal-to-noise ratio (S/N) per resolution element (less than or similar to 0.2). In addition, whereas the variance of the cosmological signal decreases on scales larger than the typical size of ionization bubbles, the variance of the formidable galactic foregrounds increases, making it hard to disentangle the two on such large scales. The poor sensitivity on small scales, on the one hand, and the foregrounds effect on large scales, on the other hand, make direct imaging of the Epoch of Reionization of the Universe very difficult, and detection of the signal therefore is expected to be statistical. Despite these hurdles, in this paper we argue that for many reionization scenarios low-resolution images could be obtained from the expected data. This is because at the later stages of the process one still finds very large pockets of neutral regions in the intergalactic medium, reflecting the clustering of the large-scale structure, which stays strong up to scales of approximate to 120 h(-1) comoving Mpc (approximate to 1 degrees). The coherence of the emission on those scales allows us to reach sufficient S/N (greater than or similar to 3) so as to obtain reionization 21 cm images. Such images will be extremely valuable for answering many cosmological questions but above all they will be a very powerful tool to test our control of the systematics in the data. The existence of this typical scale (approximate to 120 h(-1) comoving Mpc) also argues for designing future EoR experiments, e. g. with Square Kilometre Array, with a field of view of at least 4 degrees.

    • Women's childbirth experience is an outcome indicator for evaluating maternity care. This study evaluated the psychometric properties of the Persian version of the Childbirth Experience Questionnaire (P-CEQ). The study recruited two hundred and fifty primiparous postpartum women in the 1-3 months following birth from one private and three public hospitals in Mazandaran province, Iran. First, face validity and content validity were evaluated. Moreover, confirmatory factor analysis (CFA) was conducted, and discriminant validity was assessed by applying the known-groups method. Intraclass correlation coefficient (ICC) was measured to confirm the stability and Cronbach's alpha to confirm the internal consistency. CFA also confirmed the values of fit indices (RMSEA = 0.05, SRMSR = 0.06, CFI >0.93, chi(2)/df = 1.80). ICC was 0.88 and Cronbach's alpha for all items was 0.85. Furthermore, discriminant validity of the P-CEQ was approved given that it effectively differentiated women whose stay in the labor unit exceeded twelve hours from those with a shorter stay. The P-CEQ questionnaire is a valid and reliable tool for assessing childbirth experiences. It is an easy-to-use questionnaire that can be used for evaluating quality of care in terms of women's childbirth experience. It can be used in maternity services that aim to improve quality of care during labor and childbirth.

    • We investigated the dissociation of dications and trications of three polycyclic aromatic hydrocarbons (PAHs), fluorene, phenanthrene, and pyrene. PAHs are a family of molecules ubiquitous in space and involved in much of the chemistry of the interstellar medium. In our experiments, ions are formed by interaction with 30.3 nm extreme ultraviolet (XUV) photons, and their velocity map images are recorded using a PImMS2 multi-mass imaging sensor. Application of recoil-frame covariance analysis allows the total kinetic energy release (TKER) associated with multiple fragmentation channels to be determined to high precision, ranging 1.94-2.60 eV and 2.95-5.29 eV for the dications and trications, respectively. Experimental measurements are supported by Born-Oppenheimer molecular dynamics (BOMD) simulations.

    • The aim of this study was to evaluate the partial substitution of urea-formaldehyde (UF) adhesive by fungal biomass (FB) and soy flour (SF) for plywood (PW) fabrication. For this purpose, three substitute rates of 5%, 10%, and 15% based on the dry weight of UF were examined. The physical properties, functional groups, and thermo-stability of the adhesives were characterized as well as three-ply PW panels bonded with the adhesives were evaluated in terms of water absorption (WA), thickness swelling (TS), dry and wet shear strength, and formaldehyde emission (FE). The results show that FB and SF lead in higher viscosity and longer gel time than UF adhesive. The UF and UF modified with FB and SF showed similar Fourier-transform infrared spectroscopy (FT-IR) spectral patterns. In addition, fully cured FB-containing UF adhesive possessed higher thermal stability compared to UF and UF + SF adhesives. The results revealed that the partial replacement of UF adhesive with FB and SF has no significant negative effect on the dimensional stability of PW panels. The partial replacement of UF adhesive with FB and SF, particularly in substitute rate of 10% and 15%, did significantly reduce the FE while not significantly influencing the shear strength under both dry and wet conditions. Eventually, the FB-containing UF adhesive presented similar good performance and lower FE compared with UF and UF + SF adhesives. Hence, FB can be a promising natural raw material to use in wood adhesives with acceptable performance. © 2020, © 2020 Informa UK Limited, trading as Taylor & Francis Group.

    • One of the most promising ways to study the epoch of reionization (EoR) is through radio observations of the redshifted 21-cm line emission from neutral hydrogen. These observations are complicated by the fact that the mapping of redshifts to line-of-sight positions is distorted by the peculiar velocities of the gas. Such distortions can be a source of error if they are not properly understood, but they also encode information about cosmology and astrophysics. We study the effects of redshift space distortions on the power spectrum of 21-cm radiation from the EoR using large-scale N-body and radiative transfer simulations. We quantify the anisotropy introduced in the 21-cm power spectrum by redshift space distortions and show how it evolves as reionization progresses and how it relates to the underlying physics. We go on to study the effects of redshift space distortions on LOFAR observations, taking instrument noise and foreground subtraction into account. We find that LOFAR should be able to directly observe the power spectrum anisotropy due to redshift space distortions at spatial scales around k similar to 0.1 Mpc(-1) after greater than or similar to 1000 h of integration time. At larger scales, sample errors become a limiting factor, while at smaller scales detector noise and foregrounds make the extraction of the signal problematic. Finally, we show how the astrophysical information contained in the evolution of the anisotropy of the 21-cm power spectrum can be extracted from LOFAR observations, and how it can be used to distinguish between different reionization scenarios.

    • Background: Induction of cellular response after exposure to electromagnetic fields is limited to coherent fields. An incoherent noise field is supposed to suppress the bioeffects of regular RF electromagnetic fields. The purpose of this study was to investigate the effect of GSM mobile phone-induced radiofrequency (RF) on the induction of oxidative stress in SP2/0 cell line. Materials and Methods: This study was also an attempt to assess whether these RF-induced effects can be blocked by superposing the RF radiation and an incoherent magnetic noise. Three groups of cultured cells were used in this study. The cells in the first group were only exposed to RF radiation emitted from a mobile phone simulator. The second group was only exposed to an incoherent noise field and the third group was simultaneously exposed to RF radiation and incoherent noise field. The exposure duration in all groups was 2 hours. The level of ROS production in the cells was quantified by the CM-H2DCFDA fluorescence probe, using flow cytometry technique. Results: Although our results showed increased ROS production after exposure to 900 MHz RF radiation, superposition of 900 MHz RF and the incoherent noise fields did not lead to increased levels of ROS in any experiment. However, the differences between RF exposure group and superposition of RF and noise exposure group were not statistically significant. Conclusion: Altogether our results cannot support the neutralizing effect of noise theory but may confirm the concept that just the coherent fields can be bioeffective while the incoherent noise fields cannot cause any biological effects.

Thirty-three dental students administered an inferior alveolar nerve block injection in the model, followed by preparation on a tooth. The injection was evaluated by three observers, and the feedback from the model was registered. After completion of the practical session, the opinion of the dental students was explored with a ten-item questionnaire. Thirty dental students (91 percent) performed the injection correctly according to the feedback of the model, and twenty-eight students (85 percent) did so according to the expert opinion. The agreement between feedback from the training model and the expert opinion was high. The students were very satisfied with the opportunity to practice with the training model, as indicated by the high scores on each item of the questionnaire. These results suggest that use of this preclinical training model in anesthesia teaching may have beneficial effects on the administration of local anesthetics by dental students.

  • Importance of the micro-lattice structure of selective laser melting processed Mo/Mo(x)S(x+1) composite : Corrosion studies on the electrochemical performance in aqueous solutions

    • Selective laser melting (SLM) based processing of Mo-based samples is challenging due to solidification cracking. We here demonstrate that the addition of 2 wt% MoS2 to the Mo feedstock markedly improves crack mitigation of SLM-processed Mo/MoS2/Mo2S3 composite micro-lattice structures (SLM-Mo/ Mo(x)S(x+1)). Crack inhibition is suggested to be a result of Mo2S3 formation, decreased lattice strain (0.04 4%), and a decrease in accumulated residual stresses. The increased values of polarization resistance from 42.3 and 19.2 kU cm2 to 437 and 78.2 kU cm2, respectively verified the hindering effect of the composition on stress corrosion cracking (SCC) and surface oxidation cracking. However, an increased corrosion current density, from 1.22 to 10.2 mA/cm2, and cathodic Tafel constant, from 175 to 260.5 mV, confirmed the decreased polarization resistance and occurrence of different types of corrosion such as SCC and pitting. The strategy to add 2 wt% MoS2 to the Mo feedstock enables the fabrication of hightemperature micro-lattice structure components with improved corrosion resistance properties applicable in e.g., electronic, power semiconductor heat sinks, offshore-, aerospace-, defense-, or particularly novel sodium-ion energy storage applications.

  • Organizing principles and management climate in high-performing municipal elderly care

    • Purpose - Previous research has shown that user-oriented care predicts older persons' satisfaction with care. What is yet to be researched is how senior management facilitates the implementation of user-oriented care. The purpose of this study is to investigate the organizing principles and management climate characterizing successful elderly care. Design/methodology/approach - The department in one highly ranked municipality was selected and compared with a more average municipality. On-site in-depth semi-structured interviews with department managers and participatory observations at managers' meetings were conducted in both municipalities. Findings - Results revealed three key principles for successful elderly care: organizing care from the viewpoint of the older person; recruiting and training competent and autonomous employees; instilling a vision for the mission that guides operations at all levels in the organization. Furthermore, using climate theory to interpret the empirical material, in the highly successful municipality the management climate was characterized by affective support and cognitive autonomy, in contrast to a more instrumental work climate primarily focusing on organizational structure and doing the right things characterizing the more average municipality. Originality/value - The authors suggest that guiding organizing principles are intertwined with management climate and that there are multiple perspectives that must be considered by the management, that is, the views of the older persons, the co-workers and the mission. These results can guide future care quality developments, and increase the understanding of the importance of organizational climate at the senior management level.

  • Stimulus Salience as an Explanation for Imperfect Mimicry

    • The theory of mimicry explains how a mimic species gains advantage by resembling a model species [1-3]. Selection for increased mimic-model similarity should then result in accurate mimicry, yet there are many surprising examples of poor mimicry in the natural world [4-8]. The existence of imperfect mimics remains a major unsolved conundrum. We propose and experimentally test a novel explanation of the phenomenon. We argue that predators perceive prey as having several traits, but that the traits differ in their importance for learning. When predators learn to discriminate prey, high-salience traits overshadow other traits, leaving them under little or no selection for similarity, and allow imperfect mimicry to succeed. We tested this idea experimentally, using blue tits as predators and artificial prey with three prominent traits: color, pattern, and shape. We found that otherwise imperfect color mimics were avoided about as much as perfect mimics, whereas pattern and shape mimics did not gain from their similarity to the model. All traits could separately be perceived and learned by the predators, but the color trait was learned at a higher rate, implying that it had higher salience. We conclude that difference in salience between components of prey appearance is of major importance in explaining imperfect mimicry.

  • Development of antimicrobial gelatin-ulvan-beeswax composite films: Optimization of formulation using mixture design methodology

    • A new generation of antimicrobial film was developed by incorporation of ulvan extracted from Ulva intestinalis into gelatin from common carp scale and its water sensitivity was reduced with addition of beeswax. Optimum composition of gelatin (0–100%w/w), ulvan (0–100%w/w) and beeswax (0–10%w/w) for achieving composite films with minimum water solubility (S) and water vapor permeability (WVP) and maximum tensile strength (TS), elongation at break point (EAB) and antibacterial effect on E. coli (EC) were investigated using mixture design methodology. Both pure gelatin and ulvan films and their composites had relatively good mechanical and optical properties. Addition of ulvan to gelatin produced composite films with good antibacterial properties but water resistance of all the films was weak. Addition of beeswax up to ∼5 % improved the water resistance and mechanical properties of the films without jeopardizing their antibacterial properties. The final optimum formulation with a desirability of 0.709 was achieved as 52.18 % of gelatin, 40.83 % of ulvan and 6.97 % of beeswax resulting in a minimum possible S (40 %) and WVP (1.86 10−10 g/ms Pa) and maximum possible TS (6.23 MPa) and EAB (89 %) with good EC (7.66 mm). Finally, good mechanical, thermal and microstructural properties of the optimum composite film was confirmed. Altogether, a combination of ulvan and beeswax can be a promising solution for development of gelatin films with both antimicrobial properties and lower water sensitivity.

    • The use of auxotrophic Saccharomyces cerevisiae strains for improved production of a heterologous protein was examined. Two different marker genes were investigated, encoding key enzymes in the metabolic pathways for amino acid (LEU2) and pyrimidine (URA3) biosynthesis, respectively. Expression plasmids, carrying the partly defective selection markers LEU2d and URA3d, were constructed. Two CEN.PK-derived strains were chosen and insulin analogue precursor was selected as a model protein. Different truncations of the LEU2 and URA3 promoters were used as the mean to titrate the plasmid copy number and thus the recombinant gene dosage in order to improve insulin productivity. Experiments were initially carried out in batch mode to examine the stability of yeast transformants and to select high yielding mutants. Next, chemostat cultivations were run at high cell density to address industrial applicability and long-term expression stability of the transformants. We found that the choice of auxotrophic marker is crucial for developing a yeast expression system with stable heterologous protein production. The incremental truncation of the URA3 promoter led to higher plasmid copy numbers and IAP yields, whereas the truncation of the LEU2 promoter caused low plasmid stability. We show that the modification of the level of the recombinant gene dosage by varying the degree of promoter truncation can be a strong tool for optimization of productivity. The application of the URA3d-based expression systems showed a high potential for industrial protein production and for further academic studies.

    • In this work, we have investigated the structural and magnetic properties of GaP-based diluted magnetic semiconductors (DMSs). Based on first-principle density functional theory (DFT) calculations and using a full potential linearized augmented plane wave (FP-LAPW) method in generalized gradient approximation (GGA), some significant structural and magnetic properties of Transition Compounds-doped Gallium Phosphide (Ga1-XTCXP: TC = V, Mn, Fe, Co, Ni amp; Cu) as DMS are investigated. Then, a conventional gallium phosphide photovoltaic junction was simulated with a GaP absorber layer as reference cell. Last, a high efficiency gallium phosphide photovoltaic junction was proposed with a Ga1-XTCXP absorber layer. Simulation results showed that by using Ga1-XTCXP compound, the short-circuit current density (J(SC)) and the conversion efficiency of proposed solar cell increase impressively. Under global AM 1.5 conditions, the proposed cell structure has an open-circuit voltage (V-OC) of 1.01 V, J(SC) of 9.05 mA/cm(2) and a fill factor (FF) of 88%; all in all lead to total area conversion efficiency (eta) improved to 8.06% which increased about 5.93% compared with a reference cell.

    • Heat transfer in a turbocharger plays a crucial role in the optimization of turbocharger-engine matching process. Due to high temperature gradients between the hot exhaust gas compared to the compressor as well as the environment, it is well-known, that the heat loss from a turbocharger turbine is significant. Investigations of turbocharger performance are commonly done by quantifying the performance parameters under adiabatic conditions, following the paradigm of the first law of thermodynamics, based on the energy balance method. It turns out that an adiabatic assumption and the energy balance method is insufficient to provide a deep understanding about the aerothermodynamic effects on the turbine performance due to heat transfer. Based on the current state-of-the-art, this study aims to improve the characterization methods for passenger car turbocharger turbines, considering the impacts of heat transfer. Firstly, the turbocharger is measured on a hot gas test bench. Based on this experimental data, the turbine heat transfer is being quantified through implementing a new innovative power-based approach. Consequently, a heat loss free turbine performance map can be derived. Secondly, a CFD analysis is performed on selected operating points, taking turbine housing temperature measurements as boundary conditions. CFD results are verified and validated by using the experimental data, both at adiabatic and diabatic test conditions. Finally, a flow exergy-based method is being applied to the predicted 3D flow field from the CFD simulation. This approach allows to identify and quantify the aerothermodynamic impacts of heat transfer on the turbine performance for cases with and without heat loss, considering both first and second laws of thermodynamics. This study aims to enhance our understanding of the underlying thermo-fluid physics in a turbocharger turbine associated with heat loss. It will also demonstrate the potential of the application of flow exergy method to 3D CFD data, rather than limited to 1D adiabatic models in current engine research and development.

    • What is the recommended assessment and management of those with polycystic ovary syndrome (PCOS), based on the best available evidence, clinical expertise, and consumer preference?International evidence-based guidelines address prioritized questions and outcomes and include 254 recommendations and practice points, to promote consistent, evidence-based care and improve the experience and health outcomes in PCOS.The 2018 International PCOS Guideline was independently evaluated as high quality and integrated multidisciplinary and consumer perspectives from six continents; it is now used in 196 countries and is widely cited. It was based on best available, but generally very low to low quality, evidence. It applied robust methodological processes and addressed shared priorities. The guideline transitioned from consensus based to evidence-based diagnostic criteria and enhanced accuracy of diagnosis, whilst promoting consistency of care. However, diagnosis is still delayed, the needs of those with PCOS are not being adequately met, evidence quality was low and evidence-practice gaps persist.The 2023 International Evidence-based Guideline update reengaged the 2018 network across professional societies and consumer organizations with multidisciplinary experts and women with PCOS directly involved at all stages. Extensive evidence synthesis was completed. Appraisal of Guidelines for Research and Evaluation-II (AGREEII)-compliant processes were followed. The Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) framework was applied across evidence quality, feasibility, acceptability, cost, implementation and ultimately recommendation strength and diversity and inclusion were considered throughout.This summary should be read in conjunction with the full Guideline for detailed participants and methods. Governance included a six-continent international advisory and management committee, five guideline development groups, and paediatric, consumer, and translation committees. Extensive consumer engagement and guideline experts informed the update scope and priorities. Engaged international society-nominated panels included paediatrics, endocrinology, gynaecology, primary care, reproductive endocrinology, obstetrics, psychiatry, psychology, dietetics, exercise physiology, obesity care, public health and other experts, alongside consumers, project management, evidence synthesis, statisticians and translation experts. Thirty-nine professional and consumer organizations covering 71 countries engaged in the process. Twenty meetings and five face-to-face forums over 12 months addressed 58 prioritized clinical questions involving 52 systematic and 3 narrative reviews. Evidence-based recommendations were developed and approved via consensus across five guideline panels, modified based on international feedback and peer review, independently reviewed for methodological rigour, and approved by the Australian Government National Health and Medical Research Council (NHMRC).The evidence in the assessment and management of PCOS has generally improved in the past five years, but remains of low to moderate quality. The technical evidence report and analyses (∼6000 pages) underpins 77 evidence-based and 54 consensus recommendations, with 123 practice points. Key updates include: i) further refinement of individual diagnostic criteria, a simplified diagnostic algorithm and inclusion of anti-Müllerian hormone (AMH) levels as an alternative to ultrasound in adults only; ii) strengthening recognition of broader features of PCOS including metabolic risk factors, cardiovascular disease, sleep apnea, very high prevalence of psychological features, and high risk status for adverse outcomes during pregnancy; iii) emphasizing the poorly recognized, diverse burden of disease and the need for greater healthcare professional education, evidence-based patient information, improved models of care and shared decision making to improve patient experience, alongside greater research; iv) maintained emphasis on healthy lifestyle, emotional wellbeing and quality of life, with awareness and consideration of weight stigma; and v) emphasizing evidence-based medical therapy and cheaper and safer fertility management.Overall, recommendations are strengthened and evidence is improved, but remain generally low to moderate quality. Significantly greater research is now needed in this neglected, yet common condition. Regional health system variation was considered and acknowledged, with a further process for guideline and translation resource adaptation provided.The 2023 International Guideline for the Assessment and Management of PCOS provides clinicians and patients with clear advice on best practice, based on the best available evidence, expert multidisciplinary input and consumer preferences. Research recommendations have been generated and a comprehensive multifaceted dissemination and translation programme supports the Guideline with an integrated evaluation program.This effort was primarily funded by the Australian Government via the National Health Medical Research Council (NHMRC) (APP1171592), supported by a partnership with American Society for Reproductive Medicine, Endocrine Society, European Society for Human Reproduction and Embryology, and the Society for Endocrinology.
