  • Overview of the JET preparation for deuterium-tritium operation with the ITER like-wall

    • For the past several years, the JET scientific programme (Pamela et al 2007 Fusion Eng. Des. 82 590) has been engaged in a multi-campaign effort, including experiments in D, H and T, leading up to 2020 and the first experiments with 50%/50% D-T mixtures since 1997 and the first ever D-T plasmas with the ITER mix of plasma-facing component materials. For this purpose, a concerted physics and technology programme was launched with a view to prepare the D-T campaign (DTE2). This paper addresses the key elements developed by the JET programme directly contributing to the D-T preparation. This intense preparation includes the review of the physics basis for the D-T operational scenarios, including the fusion power predictions through first principle and integrated modelling, and the impact of isotopes in the operation and physics of D-T plasmas (thermal and particle transport, high confinement mode (H-mode) access, Be and W erosion, fuel recovery, etc). This effort also requires improving several aspects of plasma operation for DTE2, such as real time control schemes, heat load control, disruption avoidance and a mitigation system (including the installation of a new shattered pellet injector), novel ion cyclotron resonance heating schemes (such as the three-ions scheme), new diagnostics (neutron camera and spectrometer, active Alfven eigenmode antennas, neutral gauges, radiation hard imaging systems...) and the calibration of the JET neutron diagnostics at 14 MeV for accurate fusion power measurement. The active preparation of JET for the 2020 D-T campaign provides an incomparable source of information and a basis for the future D-T operation of ITER, and it is also foreseen that a large number of key physics issues will be addressed in support of burning plasmas.

  • Systemutveckling för riskbaserad tillsyn : Hur verksamhetsanalys på praktikteoretisk grund kan användas för kravfångst

    • Denna studie har utförts vid en myndighet som bedriver statlig tillsyn över det svenska järnvägssystemet. När myndigheten beslutade att deras tillsyn skulle vara riskbaserad uppstod behov att se över och eventuellt förändra myndighetens verksamhetsprocesser utifrån ett riskbaserat synsätt. Arbetet med denna licentiatavhandling, som har skett i form av praktikforskning, har sin grund i två förändringsprojekt som var starkt påverkade av den bärande idén om riskbaserad tillsyn. Av denna anledning fanns ett behov att karaktärisera riskbaserad tillsyn. Detta skedde med hjälp av verksamhetsanalys på praktikteoretisk grund vars resultat formulerades i en verksamhetsteori för riskbaserad tillsyn. På ett övergripande plan handlar riskbaserad tillsyn om att integrera riskhantering i myndighetens tillsynsprocesser.Denna licentiatavhandling är skriven inom ämnet informationssystemsutveckling och det problem som undersöks är hur man kan fånga kraven på ett verksamhetsstödjande informationssystem med hjälp av en verksamhetsanalys på praktikteoretisk grund. Tesen för arbetet är att de krav som identifieras i en verksamhetsanalys på praktikteoretisk grund bör kunna användas i det fortsatta kravarbetet. Därför är spårbarhet mellan verksamhetsanalysens dokumentation och den fortsatta kravhanteringen i ett systemutvecklingsprojekt en viktig egenskap. Spårbarheten undersöks genom att två av myndighetens utvecklingsprojekt som var inriktade på att utveckla både verksamheten och nya informationssystem analyseras.Att uppnå spårbarhet är en del av tillvägagångssättet när informationssystem utvecklas. I denna licentiatavhandling beskrivs tillvägagångssättet utifrån tre metodegenskaper: arbetssätt (vilka frågor som ställs), notation (hur kraven dokumenteras) och arbetsformer (vilka tekniker för datainsamling som används). Det är alltså inte bara spårbarhet (som ingår i metodegenskapen notation) som är intressant att studera närmare utan hela tillvägagångssättet i verksamhetsanalysen.Studien har bedrivits i form av praktikforskning och det innebär att tre bidrag har producerats: 1) ett bidrag till den lokala praktiken, dvs. till myndigheten; 2) ett bidrag till den generella praktiken, dvs. till andra verksamheter som bedriver statlig tillsyn men även praktiker som arbetar med verksamhets- respektive systemutveckling inkluderas i den generella praktiken; samt 3) ett bidrag till akademin. Bidraget till den lokala praktiken är genomförandet av utvecklingsprojekten och deras resultat. Bidraget till den generella praktiken inom verksamhetsutveckling är verksamhetsanalysen som resulterar i en verksamhetsteori för riskbaserad tillsyn. Bidraget till praktiker inom systemutveckling är kravanalysen som visar hur en verksamhetsanalys på praktikteoretisk grund kan användas för att fånga krav på ett verksamhetsstödjande informationssystem. Kravanalysen öppnar även för möjligheter att söka efter krav på ett informationssystem utan att verksamhetsanalysen fokuserar informationssystemet. Bidraget till akademin består också av kravanalysen som med hjälp av fyra kravkategorier härleder spårbarhet mellan verksamhetsteorin för riskbaserad tillsyn och kravdokumentation från de studerade projekten. Det andra bidraget till akademin är ett önskvärt tillvägagångssätt för att genomföra en praktikteoretisk verksamhetsanalys för riskbaserad tillsyn.

  • Improvement in indices of cellular protection after psychological treatment for social anxiety disorder

    • Telomere attrition is a hallmark of cellular aging and shorter telomeres have been reported in mood and anxiety disorders. Telomere shortening is counteracted by the enzyme telomerase and cellular protection is also provided by the antioxidant enzyme glutathione peroxidase (GPx). Here, telomerase, GPx, and telomeres were investigated in 46 social anxiety disorder (SAD) patients in a within-subject design with repeated measures before and after cognitive behavioral therapy. Treatment outcome was assessed by the Liebowitz Social Anxiety Scale (self-report), administered three times before treatment to control for time and regression artifacts, and posttreatment. Venipunctures were performed twice before treatment, separated by 9 weeks, and once posttreatment. Telomerase activity and telomere length were measured in peripheral blood mononuclear cells and GPx activity in plasma. All patients contributed with complete data. Results showed that social anxiety symptom severity was significantly reduced from pretreatment to posttreatment (Cohen’s d = 1.46). There were no significant alterations in telomeres or cellular protection markers before treatment onset. Telomere length and telomerase activity did not change significantly after treatment, but an increase in telomerase over treatment was associated with reduced social anxiety. Also, lower pretreatment telomerase activity predicted subsequent symptom improvement. GPx activity increased significantly during treatment, and increases were significantly associated with symptom improvement. The relationships between symptom improvement and putative protective enzymes remained significant also after controlling for body mass index, sex, duration of SAD, smoking, concurrent psychotropic medication, and the proportion of lymphocytes to monocytes. Thus, indices of cellular protection may be involved in the therapeutic mechanisms of psychological treatment for anxiety.

    • Stress is associated with poorer physical and mental health. To improve our understanding of this link, we performed genome-wide association studies (GWAS) of depressive symptoms and genome-wide by environment interaction studies (GWEIS) of depressive symptoms and stressful life events (SLE) in two UK population-based cohorts (Generation Scotland and UK Biobank). No SNP was individually significant in either GWAS, but gene-based tests identified six genes associated with depressive symptoms in UK Biobank (DCC, ACSS3, DRD2, STAG1, FOXP2 and KYNU; p < 2.77 × 10−6). Two SNPs with genome-wide significant GxE effects were identified by GWEIS in Generation Scotland: rs12789145 (53-kb downstream PIWIL4; p = 4.95 × 10−9; total SLE) and rs17070072 (intronic to ZCCHC2; p = 1.46 × 10−8; dependent SLE). A third locus upstream CYLC2 (rs12000047 and rs12005200, p < 2.00 × 10−8; dependent SLE) when the joint effect of the SNP main and GxE effects was considered. GWEIS gene-based tests identified: MTNR1B with GxE effect with dependent SLE in Generation Scotland; and PHF2 with the joint effect in UK Biobank (p < 2.77 × 10−6). Polygenic risk scores (PRSs) analyses incorporating GxE effects improved the prediction of depressive symptom scores, when using weights derived from either the UK Biobank GWAS of depressive symptoms (p = 0.01) or the PGC GWAS of major depressive disorder (p = 5.91 × 10−3). Using an independent sample, PRS derived using GWEIS GxE effects provided evidence of shared aetiologies between depressive symptoms and schizotypal personality, heart disease and COPD. Further such studies are required and may result in improved treatments for depression and other stress-related conditions.

    • Objectives: Dental caries is a health problem that can be prevented. The aim of this study is to analyse if the quality of leadership, in Swedish Public Dental Health clinics, influences the extent to which patients with caries receive preventive care, and if any such effect is mediated through a collaborative work climate, clear role expectations and a low average level of burnout among staff.Methods: The multilevel cross-sectional design includes work environment data from surveys of 75 general public dental clinics, register-based data on preventive measures provided to 5398 patients who received a dental filling due to a caries diagnosis, and patient demographics. Using a multilevel path analysis with logistic regression, we tested a model with one direct and three indirect pathways, controlling for the potential confounding effect of patient demographic factors.Results: Leadership quality, as assessed by the staff at the clinic, was associated with increased odds of patients with caries receiving prevention, controlling for patient demographic factors. Leadership quality was also positively related to a collaborative work climate, clear role expectations and a low average level of burnout among staff. Against expectations, however, no indirect effect from leadership quality on prevention through the other work environment factors was found.Conclusions: In conclusion, the quality of leadership in Swedish Public Dental Health clinics was positively related to a good work environment for staff and to delivery of preventive care to patients experiencing caries.

  • Genetic effects influencing risk for major depressive disorder in China and Europe

    • Major depressive disorder (MDD) is a common, complex psychiatric disorder and a leading cause of disability worldwide. Despite twin studies indicating its modest heritability (~30–40%), extensive heterogeneity and a complex genetic architecture have complicated efforts to detect associated genetic risk variants. We combined single-nucleotide polymorphism (SNP) summary statistics from the CONVERGE and PGC studies of MDD, representing 10 502 Chinese (5282 cases and 5220 controls) and 18 663 European (9447 cases and 9215 controls) subjects. We determined the fraction of SNPs displaying consistent directions of effect, assessed the significance of polygenic risk scores and estimated the genetic correlation of MDD across ancestries. Subsequent trans-ancestry meta-analyses combined SNP-level evidence of association. Sign tests and polygenic score profiling weakly support an overlap of SNP effects between East Asian and European populations. We estimated the trans-ancestry genetic correlation of lifetime MDD as 0.33; female-only and recurrent MDD yielded estimates of 0.40 and 0.41, respectively. Common variants downstream of GPHN achieved genome-wide significance by Bayesian trans-ancestry meta-analysis (rs9323497; log10 Bayes Factor=8.08) but failed to replicate in an independent European sample (P=0.911). Gene-set enrichment analyses indicate enrichment of genes involved in neuronal development and axonal trafficking. We successfully demonstrate a partially shared polygenic basis of MDD in East Asian and European populations. Taken together, these findings support a complex etiology for MDD and possible population differences in predisposing genetic factors, with important implications for future genetic studies.

    • Preventing SARS-CoV-2 infection by modulating viral host receptors, such as angiotensin-converting enzyme 2 (ACE2)1, could represent a new chemoprophylactic approach for COVID-19 that complements vaccination2,3. However, the mechanisms that control the expression of ACE2 remain unclear. Here we show that the farnesoid X receptor (FXR) is a direct regulator of ACE2 transcription in several tissues affected by COVID-19, including the gastrointestinal and respiratory systems. We then use the over-the-counter compound z-guggulsterone and the off-patent drug ursodeoxycholic acid (UDCA) to reduce FXR signalling and downregulate ACE2 in human lung, cholangiocyte and intestinal organoids and in the corresponding tissues in mice and hamsters. We show that the UDCA-mediated downregulation of ACE2 reduces susceptibility to SARS-CoV-2 infection in vitro, in vivo and in human lungs and livers perfused ex situ. Furthermore, we reveal that UDCA reduces the expression of ACE2 in the nasal epithelium in humans. Finally, we identify a correlation between UDCA treatment and positive clinical outcomes after SARS-CoV-2 infection using retrospective registry data, and confirm these findings in an independent validation cohort of recipients of liver transplants. In conclusion, we show that FXR has a role in controlling ACE2 expression and provide evidence that modulation of this pathway could be beneficial for reducing SARS-CoV-2 infection, paving the way for future clinical trials.

    • Epigenetic processes, including DNA methylation (DNAm), are among the mechanisms allowing integration of genetic and environmental factors to shape cellular function. While many studies have investigated either environmental or genetic contributions to DNAm, few have assessed their integrated effects. Here we examine the relative contributions of prenatal environmental factors and genotype on DNA methylation in neonatal blood at variably methylated regions (VMRs) in 4 independent cohorts (overall n = 2365). We use Akaike’s information criterion to test which factors best explain variability of methylation in the cohort-specific VMRs: several prenatal environmental factors (E), genotypes in cis (G), or their additive (G + E) or interaction (GxE) effects. Genetic and environmental factors in combination best explain DNAm at the majority of VMRs. The CpGs best explained by either G, G + E or GxE are functionally distinct. The enrichment of genetic variants from GxE models in GWAS for complex disorders supports their importance for disease risk.

    • Psychiatric disorders are thought to have a complex genetic pathology consisting of interplay of common and rare variation. Traditionally, pedigrees are used to shed light on the latter only, while here we discuss the application of polygenic risk scores to also highlight patterns of common genetic risk. We analyze polygenic risk scores for psychiatric disorders in a large pedigree (n ~ 260) in which 30% of family members suffer from major depressive disorder or bipolar disorder. Studying patterns of assortative mating and anticipation, it appears increased polygenic risk is contributed by affected individuals who married into the family, resulting in an increasing genetic risk over generations. This may explain the observation of anticipation in mood disorders, whereby onset is earlier and the severity increases over the generations of a family. Joint analyses of rare and common variation may be a powerful way to understand the familial genetics of psychiatric disorders.

  • Late Ordovician Faunal Distribution and Ecospace Partitioning in Marine Impact Craters : The Aftermath of the Lockne and Tvären Events

    • In the Middle to Late Ordovician a boost of marine biodiversity occurred which is regarded as the most rapid diversity in Earth’s history, and termed the Great Ordovician Biodiversification Event. This time is also unique in that at least four marine meteorite craters with a good record of post-impact sediments are preserved in Baltoscandia. Catastrophic impacts can serve as constructive events and produce wide-ranging environments providing new ecological niches for a diverse biota to occupy. Additionally, they generate distinctive patterns of biological destruction and recovery. This, and the study of distribution and ecospace utilisation of Late Ordovician faunas, has been analysed in two almost contemporary (around 455 million years ago) meteorite craters (Lockne and Tvären, Sweden). Within the confined space of the impact craters environments varied from shallow and reef-like to over 200 m in depth and from well oxygenated to hypoxic. These types of environments favored colonization of different individual groups. In Tvären rhynchonelliformean brachiopod assemblages from the shallow crater rim include a range of morphotypes, not established elsewhere in the crater. Within the crater depression rhynchonelliformean brachiopods were not established until the upper third of the remaining crater fill. Colonization of post-impact faunas varies dependent on topography, depth and susbstrate within the impact craters. This is recognised for scolecodonts in Tvären and for gastropod-like mollusks, linguliform and craniiform brachiopods in both of the craters, as they inhabit a wide range of ecospace. A succession of different taxa is observed from the deepest part of each crater and upwards towards inferably more shallow, higher energy, water settings. The development of new community types and narrowly-defined niches in the craters helped further drive both α and β biodiversity during a critical phase of the Great Ordovician Biodiversification Event.

    • Nyutveckling och förvaltning av ett informationssystem står ständigt inför nya krav och förutsättningar. Utvecklingen skall ske på kortare tid och med ökad produktivitet. Ur förvaltningssynpunkt skall IT-systemen snabbt kunna anpassas till förändringar i verksamhet och teknik, samtidigt som dessa IT-system även skall ha en hög kvalitet och säkerhet. Allt detta kräver nya sätt att arbeta och att organisera IT-verksamheten på. Ett av dessa nya arbetssätt gäller hantering av mjukvarukomponenter. Den grundläggande idén med detta arbetssätt är att utveckling och förvaltning av IT-system inte skall basera sig på nyutveckling av mjukvara, utan på återanvändning av befintliga mjukvarukomponenter.Forskningsprocessen har haft en kvalitativ ansats med induktiva och deduktiva inslag. Datainsamlingen har skett via källstudier och intervjuer. Hanteringen av mjukvarukomponenter har studerats på två interna IT-avdelningar hos två myndigheter. Syftet har varit att kartlägga vad komponenthantering innebär och på vilket sätt arbetet på IT-avdelningarna har förändrats. Komponenthanteringen beskrivs ur ett praktikperspektiv, vilket innebär att IT-verksamhetens förutsättningar, handlingar, resultat och klienter analyseras.Avhandlingens resultat utgörs av en praktikteori för komponenthantering. Praktiken ”Komponenthantering” består av fyra subpraktiker: komponentanskaffning, komponentförvaltning, komponentbaserad systemutveckling och komponentbaserad systemförvaltning. Produkten av denna praktik är användbara IT-system. I avhandlingen diskuteras olika sätt att organisera denna praktik, samt vilka grundläggande förutsättningar som behövs för att bedriva denna praktik. Syftet med den praktikteori som presenteras är att den skall visa på hur intern IT-verksamhet kan bedrivas för att kunna möta de nya krav på effektivitet, förändringsbarhet, kvalitet och säkerhet som ställs på verksamheten.

  • The Role of Staff-Assessed Care Quality in the Relationship between Job Demands and Stress in Human Service Work : The Example of Dentistry

    • The aim of this study was to investigate staff-assessed care quality at the clinic as a predictorof stress and as a moderator between job demands (quantitative demands and role conflict) and stressamong dental professionals as an example of human service workers. Cross-sectional questionnairedata from 1012 dental professionals (i.e., dentists, dental hygienists and dental nurses) working at99 clinics were analysed by confirmatory factor analysis and a two-level hierarchical linear model.Stress, quantitative demands and role conflict were measured by the Swedish standard version ofCOPSOQ III and care quality was measured by three proprietary items. The results showed thatstaff-assessed care quality at the clinic was of importance for the individual workers’ experiencesof stress. Furthermore, the staff’s joint assessment of the care quality at the clinic mitigated thenegative effect of role conflict on stress among dental nurses. These results indicate that a high levelof staff-assessed care quality at the clinic can contribute to reduced stress in dental professionals.

This study aims to report the 30-day mortality associated with perioperative infection of patients undergoing surgery for proximal femoral fractures and to examine the factors that influence mortality in a multivariate analysis.SettingProspective, international, multicentre, observational cohort study.ParticipantsPatients undergoing any operation for a proximal femoral fracture from 1 February to 30 April 2020 and with perioperative SARS-CoV-2 infection (either 7 days prior or 30-day postoperative).Primary outcome30-day mortality. Multivariate modelling was performed to identify factors associated with 30-day mortality.ResultsThis study reports included 1063 patients from 174 hospitals in 19 countries. Overall 30-day mortality was 29.4% (313/1063). In an adjusted model, 30-day mortality was associated with male gender (OR 2.29, 95% CI 1.68 to 3.13, p<0.001), age >80 years (OR 1.60, 95% CI 1.1 to 2.31, p=0.013), preoperative diagnosis of dementia (OR 1.57, 95% CI 1.15 to 2.16, p=0.005), kidney disease (OR 1.73, 95% CI 1.18 to 2.55, p=0.005) and congestive heart failure (OR 1.62, 95% CI 1.06 to 2.48, p=0.025). Mortality at 30 days was lower in patients with a preoperative diagnosis of SARS-CoV-2 (OR 0.6, 95% CI 0.6 (0.42 to 0.85), p=0.004). There was no difference in mortality in patients with an increase to delay in surgery (p=0.220) or type of anaesthetic given (p=0.787).ConclusionsPatients undergoing surgery for a proximal femoral fracture with a perioperative infection of SARS-CoV-2 have a high rate of mortality. This study would support the need for providing these patients with individualised medical and anaesthetic care, including medical optimisation before theatre. Careful preoperative counselling is needed for those with a proximal femoral fracture and SARS-CoV-2, especially those in the highest risk groups.Trial registration numberNCT04323644
