  • Systematic Coronary Risk Evaluation estimated risk and prevalent subclinical atherosclerosis in coronary and carotid arteries: A population-based cohort analysis from the Swedish Cardiopulmonary Bioimage Study

    • Background It is not clear if the European Systematic Coronary Risk Evaluation algorithm is useful for identifying prevalent subclinical atherosclerosis in a population of apparently healthy individuals. Our aim was to explore the association between the risk estimates from Systematic Coronary Risk Evaluation and prevalent subclinical atherosclerosis. Design The design of this study was as a cross-sectional analysis from a population-based study cohort. Methods From the general population, the Swedish Cardiopulmonary Bioimage Study randomly invited individuals aged 50-64 years and enrolled 13,411 participants mean age 57 (standard deviation 4.3) years; 46% males between November 2013-December 2016. Associations between Systematic Coronary Risk Evaluation risk estimates and coronary artery calcification and plaques in the carotid arteries by using imaging data from a computed tomography of the heart and ultrasonography of the carotid arteries were examined. Results Coronary calcification was present in 39.5% and carotid plaque in 56.0%. In men, coronary artery calcium score >0 ranged from 40.7-65.9% and presence of carotid plaques from 54.5% to 72.8% in the age group 50-54 and 60-65 years, respectively. In women, the corresponding difference was from 17.1-38.9% and from 41.0-58.4%. A doubling of Systematic Coronary Risk Evaluation was associated with an increased probability to have coronary artery calcium score >0 (odds ratio: 2.18 (95% confidence interval 2.07-2.30)) and to have >1 carotid plaques (1.67 (1.61-1.74)). Conclusion Systematic Coronary Risk Evaluation estimated risk is associated with prevalent subclinical atherosclerosis in two major vascular beds in a general population sample without established cardiovascular disease or diabetes mellitus. Thus, the Systematic Coronary Risk Evaluation risk chart may be of use for estimating the risk of subclinical atherosclerosis.

  • Fathers' sense of security during the first postnatal week-A qualitative interview study in Sweden

    • Background father's sense of security in the early postnatal period is important for the whole family. An instrument, which measures Parents' Postnatal Sense of Security (the PPSS instrument), is under development.Objective to explore and describe factors, which influence fathers' sense of security during the first postnatal week.Methods an explorative design with a qualitative approach was used. Thirteen fathers from three hospital uptake areas in Southern Sweden were interviewed using focus group discussions and individual interviews. Analysis was carried out using qualitative content analysis.Findings participation in the processes of pregnancy birth and early parenthood emerged as the main category for fathers' postnatal sense of security. The emergent categories were; ‘willingness to participate and take responsibility’, ‘being given the opportunity to take responsibility’, ‘being assured about mother's and baby's well-being’, ‘having someone to turn to—knowing who to ask’, ‘being met as an individual’ and ‘being met by competent and supporting staff’.Key conclusions and implications for practice new and specific items of importance when investigating fathers' sense of security during the early postnatal period have been pinpointed. Fathers' sense of early postnatal security may be enhanced by giving them a genuine opportunity to participate in the whole process and by giving them the opportunity to stay overnight at the hospital after the birth. Midwives and care organisations need to give clear information about where competent help and advice can be obtained at all hours. Midwives should strengthen the fathering role by acknowledging and listening to the father as an individual person.

    • Defining psychiatric and mental health nursing has been a challenge for decades, and it is still difficult to find a comprehensive definition. We have identified a possibility to clarify psychiatric and mental health nursing based on humanistic philosophy in a general psychiatric care context. The aim was therefore to identify and synthesize the theoretical frameworks from which psychiatric and mental health nursing models are developed. We systematically collected and evaluated articles based on Grounded Theory (GT) methodology regarding psychiatric or mental health nursing. The PRISMA statement for systematic reviews was used and the formal process of synthesis, as a three-step process of identifying first -, second - and third-order themes following the examples of Howell Major and Savin-Baden. The synthesis resulted in a model describing five core elements of psychiatric and mental health nursing: 'professional nursing', 'therapeutic relationships' and 'honest engagement', with time as the all-encompassing theme, including the patients' 'lifetime perspective'. Psychiatric and mental health nursing is a caring support towards recovery, where the patient's lifetime perspective must be in focus during the caring process with a relationship built on an honest engagement. Time is therefore essential for psychiatric and mental health nursing.

  • The Nile and the Grand Ethiopian Renaissance Dam: Is There a Meeting Point between Nationalism and Hydrosolidarity?

    • The soon-to-be completed Grand Ethiopian Renaissance Dam (GERD), which will be the largest hydroelectric power plant and among the largest reservoirs in Africa, has highlighted the need for expanding traditional integrated water resources management to better include the cultural, social, and political complexities of large water infrastructure in development projects. The GERD will store a maximum of 74 billion cubic meters of water corresponding to approximately the average annual outflow of the Nile from the Aswan high dam. Undoubtedly, the GERD will be vital for energy production and a key factor for food production, economic development, and poverty reduction in Ethiopia and the Nile Basin. However, the GERD is also a political statement that in one stroke has re-written the hydropolitical map of the Nile Basin. The GERD has become a symbol of Ethiopian nationalism or “renaissance” (hidase in Amharic). A contrasting concept to nationalism is hydrosolidarity. This concept has been put forward to better stress equitable use of water in international water management challenges that would lead to sustainable socioeconomic development. We use the opposing notions of nationalism and hydrosolidarity at three different scales, everyday politics, state policies, and interstate and global politics to analyse some aspects of the new hydropolitical map of the Nile Basin. We argue that nationalism and national interests are not necessarily negative standpoints but that there may instead be a meeting point where regional and national interests join with hydrosolidarity principles. We believe that this meeting point can maximize not only the common good, but also the good from a national interest point of view. For this, it is important not increase collaboration instead of being locked in to the historical narrative of nationalistic culture and historical discourse. This would benefit and improve future sustainability.

Vi har bara en femtedel kvar och det är bråttom att kraftigt reducera utsläppen. Det har Greta Thunberg och de strejkande ungdomarna förstått. Därför stödjer vi deras krav, skriver 270 klimatforskare.

    • Universitet och högskolor bidrar till samhällsutvecklingen genom att forskningsbaserad kunskap tillämpas av olika aktörer i samhället. För att möta dagens och morgondagens samhällsutmaningar utgör således kunskapsutbyte och samverkan mellan lärosäten och samhället en värdefull möjlighet. Att ha starka relationer med samhällets aktörer identifieras också som viktigt i utvecklingen av ett internationellt starkt lärosäte.Samverkan i högre utbildning främjar samhällets förändringsförmåga och stärker utbildningens kvalitet, men samverkan förbereder också studenterna för ett livslångt lärande och underlättar övergången mellan utbildning och arbetsliv. Ett lärosäte i nära samverkan med aktörer i samhället är ett relevant lärosäte, det vill säga ett lärosäte som är till nytta för sin omvärld och för sina medarbetare och studenter.Ett grundläggande problem vad gäller möjligheterna att integrera samverkan i akademisk utbildning står att finna i statens fördelning av medel till lärosätena. De statliga medlen utgår nämligen i två separata anslag, ett vardera för de båda huvuduppdragen utbildning och forskning. Samverkan kan sägas ingå i bägge uppdragen. Till skillnad från forskningssamverkan finns det för utbildningssamverkan ingen särskild uppföljning och inte heller några ekonomiska incitament. Utvärdering av forskning och utbildning bör således breddas så att kvalitetsstärkande samverkansinslag också inkluderas i resursfördelningsprinciperna.     • Det här är en kokbok för förändringsledare. Du kan använda denna bok om du ska leda  utvecklingsaktiviteter och letar efter bra och beprövade metoder för att leda grupper  och processer. Vi beskriver metoderna som vi har använt inom vårt projekt SMICE, i  vilket sammanhang metoderna använts och när de olika metoderna fungerat bra. Vår förhoppning med att nedteckna dessa metoder är att du ska inspireras och vågar  prova något nytt för att skapa nytta. Vi tror att ett detaljerat sätt att dokumentera  metoderna gör dem mer användbara för dig men också för oss själva.   Metoderna är graderade efter vilket behov av förkunskap och förberedelser du behöver,  från det enklaste, där du kan öppna kokboken och använda metoden på en gång, till  metoder som kräver utbildning eller specialistkompetens innan du sätter igång. Boken är indelad i fyra huvuddelar; Starta, Forma, Utveckla och förankra och Bygga vidare. Till dessa delar har vi identifierat metodstöd som kan användas vid en rad olika tillfällen;  vid idégenerering, vid idéutveckling, vid affärsutveckling, för att inspirera, för att skapa  samsyn och samverkan och för att mobilisera större grupper av människor och  organisationer. Något för alla, alltså. Avslutningsvis finner du tips och stöd för att  driva dessa utvecklingsprocesser på distans med digitala verktyg i digitala möten.  Ska vi sammanfatta någon lärdom av detta arbete så blir det våra nycklar som presenteras på nästa sida.   • Evidence for a spin-aligned neutron-proton paired phase from the level structure of 92Pd

    • Shell structure and magic numbers in atomic nuclei were generally explained by pioneering work(1) that introduced a strong spin-orbit interaction to the nuclear shell model potential. However, knowledge of nuclear forces and the mechanisms governing the structure of nuclei, in particular far from stability, is still incomplete. In nuclei with equal neutron and proton numbers (N = Z), enhanced correlations arise between neutrons and protons (two distinct types of fermions) that occupy orbitals with the same quantum numbers. Such correlations have been predicted to favour an unusual type of nuclear superfluidity, termed isoscalar neutron-proton pairing(2-6), in addition to normal isovector pairing. Despite many experimental efforts, these predictions have not been confirmed. Here we report the experimental observation of excited states in the N = Z = 46 nucleus Pd-92. Gamma rays emitted following the Ni-58(Ar-36,2n)Pd-92 fusion-evaporation reaction were identified using a combination of state-of-the-art high-resolution c-ray, charged-particle and neutron detector systems. Our results reveal evidence for a spin-aligned, isoscalar neutron-proton coupling scheme, different from the previous prediction(2-6). We suggest that this coupling scheme replaces normal superfluidity (characterized by seniority coupling(7,8)) in the ground and low-lying excited states of the heaviest N = Z nuclei. Such strong, isoscalar neutron-proton correlations would have a considerable impact on the nuclear level structure and possibly influence the dynamics of rapid proton capture in stellar nucleosynthesis.

  • Maternal Obesity and Risk of Preterm Delivery

    • Importance Preterm birth is a leading cause of infant mortality, morbidity, and long-term disability, and these risks increase with decreasing gestational age. Obesity increases the risk of preterm delivery, but the associations between overweight and obesity and subtypes of preterm delivery are not clear. Objective To study the associations between early pregnancy body mass index (BMI) and risk of preterm delivery by gestational age and by precursors of preterm delivery. Design, Setting, and Participants Population-based cohort study of women with live singleton births in Sweden from 1992 through 2010. Maternal and pregnancy characteristics were obtained from the nationwide Swedish Medical Birth Register. Main Outcomes and Measures Risks of preterm deliveries (extremely, 22-27 weeks; very, 28-31 weeks; and moderately, 32-36 weeks). These outcomes were further characterized as spontaneous (related to preterm contractions or preterm premature rupture of membranes) and medically indicated preterm delivery (cesarean delivery before onset of labor or induced onset of labor). Risk estimates were adjusted for maternal age, parity, smoking, education, height, mother's country of birth, and year of delivery. Results Among 1 599 551 deliveries with information on early pregnancy BMI, 3082 were extremely preterm, 6893 were very preterm, and 67 059 were moderately preterm. Risks of extremely, very, and moderately preterm deliveries increased with BMI and the overweight and obesity-related risks were highest for extremely preterm delivery. Among normal-weight women (BMI 18.5-<25), the rate of extremely preterm delivery was 0.17%. As compared with normal-weight women, rates (%) and adjusted odds ratios (ORs [95% CIs]) of extremely preterm delivery were as follows: BMI 25 to less than 30 (0.21%; OR, 1.26; 95% CI, 1.15-1.37), BMI 30 to less than 35 (0.27%; OR, 1.58; 95% CI, 1.39-1.79), BMI 35 to less than 40 (0.35%; OR, 2.01; 95% CI, 1.66-2.45), and BMI of 40 or greater (0.52%; OR, 2.99; 95% CI, 2.28-3.92). Risk of spontaneous extremely preterm delivery increased with BMI among obese women (BMI >= 30). Risks of medically indicated preterm deliveries increased with BMI among overweight and obese women. Conclusions and Relevance In Sweden, maternal overweight and obesity during pregnancy were associated with increased risks of preterm delivery, especially extremely preterm delivery. These associations should be assessed in other populations.

    • Discoveries of bacterial communities in environments that previously have been described as sterile have in recent years radically challenged the view of these environments. In this study we aimed to use 16S rRNA sequencing to describe the composition and temporal stability of the bacterial microbiota in bovine milk from healthy udder quarters, an environment previously believed to be sterile. Sequencing of the 16S rRNA gene is a technique commonly used to describe bacterial composition and diversity in various environments. With the increased use of 16S rRNA gene sequencing, awareness of methodological pitfalls such as biases and contamination has increased although not in equal amount. Evaluation of the composition and temporal stability of the microbiota in 288 milk samples was largely hampered by background contamination, despite careful and aseptic sample processing. Sequencing of no template control samples, positive control samples, with defined levels of bacteria, and 288 milk samples with various levels of bacterial growth, revealed that the data was influenced by contaminating taxa, primarily Methylobacterium. We observed an increasing impact of contamination with decreasing microbial biomass where the contaminating taxa became dominant in samples with less than 10 4 bacterial cells per mL. By applying a contamination filtration on the sequence data, the amount of sequences was substantially reduced but only a minor impact on number of identified taxa and by culture known endogenous taxa was observed. This suggests that data filtration can be useful for identifying biologically relevant associations in milk microbiota data.

    • The aim of this study was to evaluate the effect of sampling technique and milk fraction on bovine milk microbiota data and to compare the microbiota in milk to microbiota on the teat end and in the teat canal. Representative milk samples are highly important for assessment of bacteriological findings and microbiota in milk. Samples were obtained from 5 healthy lactating dairy cows at udder quarter level during 1 milking. Swab samples from the teat. end and teat canal, and milk samples collected using different techniques and in different milk fractions were included. Milk was collected by hand stripping and through a teat canal cannula before and after machine milking, through a trans-teat wall needle aspirate after milking, and from udder quarter composite milk. The microbiota of the samples was analyzed with sequencing of the V1-V3 region of the 16S rRNA gene. In addition, somatic cell counts and bacterial cultivability were analyzed in the milk samples. Microbiota data were analyzed using multivariate methods, and differences between samples were tested using analysis of similarity (ANOSIM). Differences between samples were further explored via individual studies of the 10 most abundant genera. The microbiota on the teat end, in the teat canal, and in udder quarter composite milk, collected using a milking machine, differed in composition from the microbiota in milk collected directly from the udder quarter. No differences in milk microbiota composition were detected between hand-stripped milk samples, milk samples taken through a teat canal cannula, or milk samples taken as a trans-teat wall needle aspirate before or after milking. We conclude that for aseptic milk samples collected directly from the lactating udder quarter, sampling technique or milk fraction has minor effect on the microbiota composition.

    • BACKGROUND: The World Health Organisation (WHO) 2013 diagnostic criteria for gestational diabetes mellitus (GDM) has been criticised due to the limited evidence of benefits on pregnancy outcomes in different populations when switching from previously higher glycemic thresholds to the lower WHO-2013 diagnostic criteria. The aim of this study was to determine whether the switch from previous Swedish (SWE-GDM) to the WHO-2013 GDM criteria in Sweden following risk factor-based screening improves pregnancy outcomes.METHODS AND FINDINGS: A stepped wedge cluster randomised trial was performed between January 1 and December 31, 2018 in 11 clusters (17 delivery units) across Sweden, including all pregnancies under care and excluding preexisting diabetes, gastric bypass surgery, or multifetal pregnancies from the analysis. After implementation of uniform clinical and laboratory guidelines, a number of clusters were randomised to intervention (switch to WHO-2013 GDM criteria) each month from February to November 2018. The primary outcome was large for gestational age (LGA, defined as birth weight >90th percentile). Other secondary and prespecified outcomes included maternal and neonatal birth complications. Primary analysis was by modified intention to treat (mITT), excluding 3 clusters that were randomised before study start but were unable to implement the intervention. Prespecified subgroup analysis was undertaken among those discordant for the definition of GDM. Multilevel mixed regression models were used to compare outcome LGA between WHO-2013 and SWE-GDM groups adjusted for clusters, time periods, and potential confounders. Multiple imputation was used for missing potential confounding variables. In the mITT analysis, 47 080 pregnancies were included with 6 882 (14.6%) oral glucose tolerance tests (OGTTs) performed. The GDM prevalence increased from 595/22 797 (2.6%) to 1 591/24 283 (6.6%) after the intervention. In the mITT population, the switch was associated with no change in primary outcome LGA (2 790/24 209 (11.5%) versus 2 584/22 707 (11.4%)) producing an adjusted risk ratio (aRR) of 0.97 (95% confidence interval 0.91 to 1.02, p = 0.26). In the subgroup, the prevalence of LGA was 273/956 (28.8%) before and 278/1 239 (22.5%) after the switch, aRR 0.87 (95% CI 0.75 to 1.01, p = 0.076). No serious events were reported. Potential limitations of this trial are mainly due to the trial design, including failure to adhere to guidelines within and between the clusters and influences of unidentified temporal variations.CONCLUSIONS: In this study, implementing the WHO-2013 criteria in Sweden with risk factor-based screening did not significantly reduce LGA prevalence defined as birth weight >90th percentile, in the total population, or in the subgroup discordant for the definition of GDM. Future studies are needed to evaluate the effects of treating different glucose thresholds during pregnancy in different populations, with different screening strategies and clinical management guidelines, to optimise women's and children's health in the short and long term.TRIAL REGISTRATION: The trial is registered with ISRCTN (41918550).

    • Quinolone-resistant Escherichia coli (QREC) is common in feces from young calves, but the prevalence and genetic diversity of QREC in groups of cattle of other ages and the farm environment are unknown. The aims of the study were to obtain knowledge about the occurrence of QREC on dairy farms, the genetic diversity of QREC within and between farms, and how these relate to the number of purchased animals and geographic distances between farms. We analyzed the within-sample prevalence of QREC in individual fecal samples from preweaned dairy calves and postpartum cows and in environmental samples from 23 Swedish dairy farms. The genetic diversity of the QREC isolates on 10 of these farms was assessed. In general, QREC was more prevalent in the dairy farm environment and in postpartum cows if QREC was commonly found in calves than if QREC was rare in calves. In particular, we found more QREC organisms in feed and water troughs and in environments that may come into contact with young calves. Thus, the results suggest that QREC circulates between cattle and the farm environment and that calves are important for the maintenance of QREC. Some genotypes of QREC were widespread both within and between farms, indicating that QREC has spread within the farms and likely also between farms, possibly through purchased animals. Farms that had purchased many animals over the years had greater within-farm diversity than farms with more closed animal populations. Finally, animals on more closely located farms were more likely to share the same genotype than animals on farms located far apart.

  • Risk factors for antimicrobial resistance in fecal Escherichia coli from preweaned dairy calves

    • The primary objective of this study was to investigate calf and farm factors associated with antimicrobial-resistant Escherichia coli in the feces of preweaned dairy calves in Sweden. In particular, we investigated the effects of feeding calves colostrum and milk from cows treated with antimicrobials. The secondary objective was to describe the prevalence of resistant E. coli in feces of preweaned dairy calves in Sweden. Fecal samples from 3 calves, aged 7 to 28 d, from 243 farms were analyzed for the within-sample prevalence of E. coli resistant to nalidixic acid, streptomycin, and cefotaxime using selective agars supplemented with antimicrobials. In addition, resistance to 12 antimicrobials was tested in one randomly selected E. coli isolate per calf. Information was collected from the farmers via questionnaires regarding the use of colostrum and milk from cows treated with antimicrobials as calf feed and other uses of antimicrobials in the herd. Multivariable zero-inflated negative binomial and logistic regression models were used to assess the effect of various risk factors for shedding of resistant E. coli. Escherichia coli resistant to streptomycin, nalidixic acid, or cefotaxime were isolated from 90, 49, and 11% of the calves, respectively. Resistance to at least one antimicrobial was found in a random isolate of E. coli from 48% of the calves. Feeding colostrum from cows treated with antimicrobials at drying off did not affect the prevalence of resistant E. coli. In contrast, feeding milk from cows treated with antimicrobials during lactation resulted in significantly more nalidixic acid- and streptomycin-resistant E. coli than when such milk was discarded; no significant effect was seen for other resistance traits. Furthermore, an interaction was found between feeding milk from cows treated with antimicrobials and use of fluoroquinolones in cows. In general, the prevalence of resistance was lower for older calves and calves on small farms. Other factors that were associated with the shedding of resistant E. coli were administration of oral dihydrostreptomycin to calves, administration of systemic tetracycline and ceftiofur to cows and calves, housing of the calves, predominant breed of the herd, and geographic location of the farm. The presence of resistant E. coli in calves was clearly due to multiple factors, but minimizing the feeding of milk from cows treated with antimicrobials during lactation should lower the prevalence of resistant E. coli in the gastrointestinal tract of the calves.

    • Quinolone resistance may emerge in gut bacteria (e.g., in Escherichia coli) of animals. Such bacteria could cause infections in the animal itself or be transmitted to humans via the food chain. Quinolone resistance is also observed in fecal E. coli of healthy dairy cattle, but the prevalence varies between farms, not solely as a result of varying degree of fluoroquinolone exposure. The objective of this study was to identify risk factors for the fecal shedding of quinolone-resistant E. coli (QREC) from dairy calves and postpartum cows. Rectal swabs from 15 preweaned calves and 5 postpartum cows per farm were collected on 23 Swedish dairy farms to determine the prevalence of QREC. Risk factors for the shedding of QREC were investigated using multivariable statistical models. Quinolone-resistant E. coli were found on all but one farm. Factors associated with QREC shedding by calves were being younger than 18 d, being fed milk from cows treated with antimicrobials, recent use of fluoroquinolones in the herd, carriage of QREC by postpartum cows, and using the calving area never or rarely as a sick pen compared with often. Factors associated with QREC shedding by cows were calving in group pens or freestalls compared with single pens or tiestalls, purchasing cattle, sharing animal transports with other farmers, and poor farm hygiene. Proper biosecurity and improved hygiene, as well as minimizing fluoroquinolone exposure and waste milk feeding, may be important factors to reduce the burden of QREC on dairy farms.

    • Since the 1990s, the Swedish education market has gone through a dramatic transformation due to the introduction of voucher-funded independent schools. We make use of data on school applications to condition on student preferences for independent versus public education, and estimate a positive relationship between independent upper secondary school attendance and grades, graduation rates, and post-secondary education. We however also find strong indications of more lenient grading standards in independent schools, especially in schools organized as for-profit entities and in schools with a low share of qualified teachers. Our results suggest that, although independent school attendance seems to benefit the individual students in terms of higher grades and increased transition to post-secondary studies, grade inflation in the Swedish upper secondary independent schools may be a serious problem.

    • Background The optimal criteria to diagnose gestational diabetes mellitus (GDM) remain contested. The Swedish National Board of Health introduced the 2013 WHO criteria in 2015 as a recommendation for initiation of treatment for hyperglycaemia during pregnancy. With variation in GDM screening and diagnostic practice across the country, it was agreed that the shift to new guidelines should be in a scientific and structured way. The aim of the Changing Diagnostic Criteria for Gestational Diabetes (CDC4G) in Sweden () is to evaluate the clinical and health economic impacts of changing diagnostic criteria for GDM in Sweden and to create a prospective cohort to compare the many long-term outcomes in mother and baby under the old and new diagnostic approaches. Methods This is a stepped wedge cluster randomised controlled trial, comparing pregnancy outcomes before and after the switch in GDM criteria across 11 centres in a randomised manner. The trial includes all pregnant women screened for GDM across the participating centres during January-December 2018, approximately two thirds of all pregnancies in Sweden in a year. Women with pre-existing diabetes will be excluded. Data will be collected through the national Swedish Pregnancy register and for follow up studies other health registers will be included. Discussion The stepped wedge RCT was chosen to be the best study design for evaluating the shift from old to new diagnostic criteria of GDM in Sweden. The national quality registers provide data on the whole pregnant population and gives a possibility for follow up studies of both mother and child. The health economic analysis from the study will give a solid evidence base for future changes in order to improve immediate pregnancy, as well as long term, outcomes for mother and child.

    • Extract from seeds and bark of horse chestnut (Aesculus hippocastanum L) is used as an herbal medicine against chronic venous insufficiency. The effect and mechanism of action on veins, arteries, and platelets are not fully understood. The aim of this study was to investigate the effects and mechanisms of action of horse chestnut on the contraction of bovine mesenteric veins and arteries, and human platelet aggregation. Contraction studies showed that horse chestnut extract dose-dependently contracted both veins and arteries, with the veins being the most sensitive. Contraction of both veins and arteries were significantly inhibited by the 5-HT(2A) receptor antagonist ketanserin. No effect on contraction was seen with the cyclooxygenase inhibitor indomethacin, the alpha(1) receptor antagonist prazosin or the angiotensin AT(1) receptor antagonist saralasin neither in veins nor arteries. ADP-induced human platelet aggregation was significantly reduced by horse chestnut. A further reduction was seen with the extract in the presence of ketanserin. In conclusion, horse chestnut contraction of both veins and arteries is, at least partly, mediated through 5-HT(2A) receptors. Human platelet aggregation is reduced by horse chestnut. The clinical importance of these findings concerning clinical use, possible adverse effects, and drug interactions remains to be investigated. Copyright (c) 2010 John Wiley & Sons, Ltd.

  • Public and private networks in tourism – : barriers to network identity construction and commitmen

    • IntroductionDestinations all over the world encompass a broad range of stakeholders who all aspire to increase the tourism industry to accomplish regional development. At these destinations, there are many different stakeholders, who have mutual interest in a specific region, and there are different attempts to promote cooperation for joint efforts to attract visitors. Moreover, there is an increasing drive for municipalities to influence the tourism business as part of their regional development (Tillväxtverket, 2017).However, the aforementioned development of destinations has proven to involve competing interests between local, national and international stakeholders. Although private enterprises, the municipalities and on-governmental organizations, and residents have a common, or at least partly overlapping, interest in the development of a specific destination, competing interests will occasionally occur (c.f. Elbe et al., 2018). For instance, private businesses, like small local entrepreneurs, could be in direct competition with national or international corporations, despite the fact that they may have much to gain from joint marketing efforts (c.f. investment in a common brand). Similarly, public1organizations have a shared interest in the development of the tourism industry to increase tax revenues and job creation. However, municipalities have a greater responsibility for communities and its residents.As in many other sectors, public and private interactions are common within the tourism industry due to an overall reduction in public sector funding (Valente et al., 2015). Relationships between public and private actors are also set up to conjointly pool resources, share risks in the process of building, maintaining and developing public services (Keränen, 2017). Interactions between public-private actors have been acknowledged as important since these relationships enable firms to influence decisions within areas such as the public sector, rules and actions that can affect how the firm is perceived as legitimate or not (Hadjikhani, Lee, & Ghauri, 2008; Jansson, Saqib, & Sharma, 1995) but also as ways to develop new and existing resources. However, research has shown that uncertainty tends to be rather high in these relationships and the roles that the actors play are consequently dynamic and unclear. This in turn also affects how the actors perceive the cooperation between public and private actors (Keränen, 2017) and their commitment to these types of networks (Elbe et al., 2018). The sought after commitment would be enhanced if a mutual identity could be constituted; still the identity is established from how the stakeholders perceive their roles and relations to each other. So how could the identity of public actors merge with the identity of private actors to establish a mutual and common identity?Despite the apparent advantage to create a network to coordinate mutual, or at least overlapping, interests, it has proven to entail a number of difficult issues of which some will be addressed below. The combination of private and public stakeholders in one organization may have implications for its role and commitments (c.f. Elbe et al. 2009; 2018) i.e. it could influence the perceived identity construction of its constituents. The combination of public and private interests in the same organization could create tension; one perspective could have precedence over the other. For instance, if a publicly funded organization considers it to be its mission to only enhance businesses, there are stakeholder who could be overlooked, such as permanent residents, community services, and other lines of industry. Although the established networks are intended to work towards common goals, there may still be implicit and ambiguous goals, roles and identity formations.2The organizing of public and private interests in order to transform a place and its characteristics into a destination has proven to be a complex process. More research is needed in order to bring further clarity into factors affecting these types of relationships in terms of activities and resources (de Araujo and Bramwell, 2002) as well as how this affects the actors. Although there is a growing scholarly interest in public-private relationships, additional knowledge is needed on how the processes of these types of cooperation evolve (de Araujo and Bramwell, 2002). This is especially so in the context of created networks where the network is constructed rather than emerging and where the aim is to foster and manage activities, resource interactions and actor bonds.This paper sets out to further the understanding of interaction processes where public and private actors attempt to find ways to cooperate on a common issue. This is done by analyzing how the identity of a created network, consisting of public and private actors, develops over time. This paper focuses on explaining how a created network develops over time and why it evolves in the way it does. The purpose is to identify possible barriers to identity construction in a created network consisting of public and private actors. This is done through a case taking its point of departure in the decision of a major infrastructural investment in a destination in Sweden: the building of a new cruise quay on Gotland.Created public-private cooperation – previous research.Interactions between public-private actors have been acknowledged as important since these relationships enable firms to influence decisions within different areas such as the public sector, rules and actions (Keränen, 2017; Elbe et al., 2018) but also as ways to develop new and existing resources. These co-operations are encouraged by governments in many countries as interactions can facilitate the discussions and decisions related to how different activities evolve and are coordinated as well as how public resources are distributed. Moral responsibility and a way to recover citizens’ trust for politicians’ abilities to deliver on electoral pledge when resources are scarce has been emphasized as a reason for cooperation between public and private actors (Velotti, Botti & Vesci, 2012). In a tourism context, private-public cooperation can also increase the competitive advantage of destinations (Kotler et al., 1993).Over the last decades, a growing number of designated organizations for the coordination of tourist actors have been established using the concept of: Destination Management Organizations, DMO (c.f. Elbe et al. 2009). According to UNWTO (2018), the DMO’s3role should, “be to lead and coordinate activities under a coherent strategy in pursuit of this common goal.” The argument in research, and in practice, has been that these destination organizations can manage marketing and coordinate different actors from varying sectors in the society. Through this, it is assumed that the destination through facilitation of interactions among local actors and creation of networks can stimulate economic growth (Elbe et al., 2018). Hence, specific organizations are politically created to facilitate regional development. In addition, DMOs are proposed as a coalition of many organizations and interest although in practice, these organizations have proven to be composed in different ways, with varying performance (Tillväxtverket 2017). In order to understand the accomplishment the sought-after coordination of diverse stakeholder interests, a network approach has been suggested that encompasses inter-organizational relations (Elbe et al. 2018). The proposed network approach could take into account relations between organizations, not only limited to intra-organizational management models that do not encompass all stakeholders.Despite the numerous potential advantages of public-private interactions, they can be time- consuming and difficult as the participants come from different sectors and with deviating interests. Therefore, actors can perceive a risk that their power and influence may decrease. This can also lead to a lack of trust between the actors (de Araujo & Bramwell, 2002).Tensions and paradoxesInteraction between business actors are built around an economic rational. An assumption is that without economic incentives, interactions between business actors in networks will not come about (Håkansson, 1982; Finke et al., 2017). Different types of resources such as financial resources (capital), physical resources (time, technologies, people) and informational resources (knowledge) are exchanged and combined in new and unique ways through the interactions. Resources are through inherently dynamic and can always be used in new combinations within one relationship or in other relationships (Baraldi et al., 2012). An important condition and a factor influencing the interaction processes are the norms and perceptions of what constitutes the rules of the game. Through the interactions new norms for exchange can form and influence the continuation of the exchange. Whether to engage in the cooperation or not will be affected by previous experiences and expectations as expressed by Mousas and Ford (2009, p. 497): “Recurrent episodes are affected by the perceptions of the participants of their previous interactions and by their expectations of the future.” These exchange relationships, constitute the context in which interaction4processes between actors takes place (Easton & Håkansson, 1996; Håkansson & Ford, 2002; Håkansson & Waluszewski, 2002, 2007; Ritter, 2000).In tourist destinations, many of the resources used to attract tourists are public and common goods. The question of who owns these common goods is not always clear. Beaches, lakes, museums, culture heritage buildings are all resources that can attract tourists but that are public.     • Background: Supporting families where one person suffers from long-term severe mental illness (SMI) is essential, but seems hard to reach. The aim of this study was to examine professionals’ views of supporting relatives of persons with SMI. Material: Individual interviews mirroring personal narratives and group interviews reflecting group-processed answers were conducted among 23 professionals and analyzed thematically. Results: Three themes emerged: (a) information and group interaction reduces stigma and increases well-being, (b) professionals need to feel secure and confident about how the support structure works and (c) collaboration is difficult but required on several levels. Conclusion: Trusting relationships with families were considered important, although seldom achieved; professionals wished to feel secure in their role toward relatives of a person with SMI; and professionals wanted to feel confident when working together with other services to support families.

    • Severe mental illness causes suffering for the patient as well as the patient’s immediate family. The Swedish National Board of Health and Welfare has recommended the implementation of multifamily psychoeducation in order to assist patient and family in the recovery process. The aim of this study was to determine how introducing multifamily psychoeducation in Sweden has been viewed by professionals. Semi-structured interviews were conducted with 11 service providers, who were involved in evaluating multifamily psychoeducation. Our main findings fell under the headings of defensive culture and unsuitable model. Resistance to introducing the new intervention was found on multiple levels. The model proposed was considered too rigid for both the target group and the organizations because it could not be adjusted to the needs of patients, families, or facilitators. Despite good evidence for the effectiveness of the intervention, there were difficulties introducing the multifamily psychoeducation model in clinical practice. The feasibility of an intervention needs to be evaluated before adopting it as a national guideline.

  • Towards an evidence-based assessment of early motor performance in the child health services : Psychometric properties and clinical utility of the Structured Observation of Motor Performance in Infants

    • The Swedish child health services (CHS) have a unique position in that they reach almost all children 0-6 years of age. The child health nurse has the main responsibility for developmental surveillance. Twelve scheduled visits with the nurse during the child’s first year of life make this an ideal setting to systematically identify infants with motor problems, ranging from asymmetric head positioning that may lead to plagiocephaly to more severe forms such as cerebral palsy (CP). However, the CHS lack evidence-based methods to assess motor development.The Structured Observation of Motor Performance in Infants (SOMP-I) assesses motor performance in two domains, i.e. level of motor development and quality of motor performance. SOMP-I is quick, non-invasive, requires minimal handling, and is suitable for a busy clinical setting when applied by physiotherapists. Given the importance of early detection, the increased likelihood of detecting motor problems when using evidence-based assessment methods and the key role of nurses within the CHS, the overall aim of this thesis was investigate the psychometric properties and clinical utility of SOMP-I when used by child health nurses. Furthermore, we aimed to establish the ability of SOMP-I to detect CP.Our results show that child health nurses can reliably assess the level of motor development in infants using SOMP-I. More variability was found when they assessed the infants’ quality of motor performance. Although the nurses expressed concern about introducing a more time-consuming assessment in an already tight schedule, they were able to integrate the SOMP-I assessment in routine care. The nurses reported that barriers to using SOMP-I were mostly logistic and practical in nature, and they pointed out the necessity of education and practice in order to become proficient assessors. Using SOMP-I appears to have supported the nurses in the decision-making process regarding motor performance in routine care. SOMP-I detected CP during the first months of life in neonatal intensive care recipients.To our knowledge, these studies are the first to evaluate child health nurses’ assessment of early motor performance using an evidence-based assessment method in routine care. The results are promising, but further research is warranted.

Rapporten redovisar mätresultat från de båda nationella övervakningsnäten EMEP-nätet och Luft- och nederbördskemiska nätet. I rapporten redovisas även de spridnings- och depositionberäkningar som gjorts med MATCH-modellen, med 1995 års mätresultat som grund. Tidigare år har mätresultat och modellberäkningar rapporterats i tre skilda rapporter, men har här samlats i en rapport. Samtliga mätningar drivs av IVL, på uppdrag av Naturvårdsverket, med undantag av ozonmätningarna i Aspvreten som genomförs av ITM. Modellberäkningarna med MATCH-Sverige har utförts av SMHI. Verksamheten finansieras av Miljöövervakningsenheten vid Naturvårdsverket. I rapportens bilagor redovisas provtagnings- och analysmetoder, mätprogram och kvalitetssäkringsrutiner utförligt. Denna samlade redovisning av information rörande mätverksamheten är tänkt att fungera som referens till kommande årsrapporter.

    • I föreliggande rapport redovisas resultat från 1996 års mätningar av luftens och nederbördens sammansättning i bakgrundsmiljö i Sverige. Rapporten redovisar mätresultat från de båda nationella övervakningsnäten EMEP-nätet och Luft- och nederbördskemiska nätet. I rapporten redovisas även de spridnings- och depositionberäkningar som gjorts med Sverigemodellen. Samtliga mätningar drivs av IVL, på uppdrag av Naturvårdsverket, med undantag av ozonmätningarna i Aspvreten som genomförs av ITM. Modellberäkningarna med Sverigemodellen utförs av SMHI. Verksamheten finansieras av Miljöövervakningsenheten vid Naturvårdsverket. Mätningarna har under 1996 omfattat dygnsprovtagning av svaveldioxid, kvävedioxid och partikelbundet sulfat i luft vid sex EMEP-stationer, av sot vid fem stationer samt dygnsprovtagning av totalnitrat och totalammonium vid fyra av stationerna. Månadsprovtagning av lufthalter av svaveldioxid och kvävedioxid har skett vid tolv stationer i landet. Registrering av halter av marknära ozon har skett på timbasis vid sammanlagt sex stationer. Nederbördsprovtagning, dygnsvis med lock-provtagare, har utförts hela året vid en station och veckovis vid tre stationer. Månadsprovtagning av nederbörd med öppna insamlare har skett vid 28 stationer. Nederbördsproverna har analyserats med avseende på pH, sulfat, nitrat, ammonium, klorid, kalcium, magnesium, natrium, kalium och konduktivitet. Vid fyra stationer vardera har nederbörd samlats in för analys av tungmetaller respektive av total-kvicksilver och metylkvicksilver. Genomgående visar mätresultaten ett spridningsmönster med högre halter och deposition i södra Sverige och avtagande värden mot norr. Över södra Halland, inre Skåne och östra Blekinge, där den största totaldepositionen av svavel (exkl. havssalt) uppträdde, var den maximala beräknade depositionen knappt 900 mg S/m2. I Göteborgstrakten erhölls den största totaldepositionen för oxiderat kväve, ca 800 mg N/m2, medan depositionen av reducerat kväve var störst över inre Skåne, ca 850 mg N/m2. En beräknad luftföroreningsbudget för 1996 visar en nettoimport av föroreningar till Sverige, liksom under tidigare år. Importen var för svavel och reducerade kväveföreningar 4-5 gånger så stor som exporten, medan importen av oxiderat kväve endast var ca 10% större än exporten. Årsmedelhalten av marknära ozon var, liksom tidigare år, högst i Esrange i norr, 67 µg/m3. Antalet tillfällen med timmedelhalter överskridande 120, 150 respektive 180 µg/m3 var generellt högre än 1995 men lägre än 1994. Det högsta uppmätta timvärdet under året var 210 µg/m3 i Vavihill i juni månad. Halter i nederbörd och deposition av tungmetaller och kvicksilver uppvisar, med smärre undantag, sjunkande gradienter från söder till norr i landet

    • OBJECTIVE: To assess the impact of infertility treatment with causes of infertility on incidence of breast cancer. STUDY DESIGN: Historical prospective cohort study of 1135 women attending major university clinics for treatment of infertility in Sweden, 1961-1976. Women were classified as users of clomiphene citrate or gonadotropins, or a combination of both therapies. Standardized incidence ratios were calculated to estimate relative risk of breast cancer. RESULTS: We observed 54 cases of breast cancer during 1961-2004, which did not significantly exceed those expected. Users of high-dose clomiphene citrate had an almost 2-fold increased risk (standardized incidence ratio, 1.90; 95% confidence interval, 1.08-3.35). This association was more pronounced among women referred for nonovulatory factors, with 3-fold increased risk (standardized incidence ratio, 3.00; 95% confidence interval, 1.35-6.67). CONCLUSION: No overall increased risk for breast cancer was shown with infertility treatment. Women with nonovulatory causes treated with high-dose clomiphene citrate therapy may have an elevated risk for breast cancer.

    • Evidence suggests that cocoa from the bean of Theobroma cacao L. has beneficial effects on cardiovascular disease. The aim of this study was to investigate if cocoa extract and dark chocolate influence angiotensin-converting enzyme (ACE) and nitric oxide (NO) in human endothelial cells (in vitro) and in healthy volunteers (in vivo). ACE activity was analyzed with a commercial radioenzymatic assay and measured in human endothelial cells from umbilical veins (HUVEC) after 10 minutes of incubation with cocoa extract. NO was measured after 24 hours of incubation. ACE activity and NO were measured at baseline and after 30, 60, and 180 minutes in 16 healthy volunteers after a single intake of 75 g of dark chocolate containing 72% cocoa. Significant inhibition of ACE activity (P < 0.01) and significant increase of NO (P < 0.001) were seen in HUVEC. In the study subjects, a significant inhibition of ACE activity (mean 18%) 3 hours after intake of dark chocolate was seen, but no significant change in NO was seen. According to ACE genotype, significant inhibition of ACE activity was seen after 3 hours in individuals with genotype insertion/insertion and deletion/deletion (mean 21% and 28%, respectively). Data suggest that intake of dark chocolate containing high amount of cocoa inhibits ACE activity in vitro and in vivo.

    • OBJECTIVE: Tea has been reported to reduce cardiovascular mortality, but the underlying mechanisms are largely unknown. The aim of the current project was to investigate the effect of green tea (Japanese Sencha), black tea (Indian Assam B.O.P.) and Rooibos tea (South Africa) on angiotensin-converting enzyme (ACE) and nitric oxide (NO). DESIGN: Seventeen healthy volunteers received a single oral dose of 400 ml green tea, black tea or Rooibos tea in a randomized, three-phase, crossover study. ACE activity and NO concentration were measured (at 0, 30, 60 and 180 min) in all phases. ACE activity was analysed by means of a commercial radioenzymatic assay. Nitrite was analysed as a marker of NO concentration. In addition, ACE genotype was determined using a PCR method. RESULTS: Oral intake of a single dose of Rooibos tea significantly inhibited ACE activity after 30 min (P < 0.01) and after 60 min (P < 0.05). A significant inhibition of ACE activity was seen with green tea for the ACE II genotype 30 min after intake of the tea (P < 0.05) and for the ACE ID genotype 60 min after intake (P < 0.05). A significant inhibition of ACE activity was also seen with Rooibos tea for the ACE II genotype 60 min after intake (P < 0.05). No significant effect on NO concentration was seen. CONCLUSIONS: These results suggest that green tea and Rooibos tea may have cardiovascular effects through inhibition of ACE activity.

    • Gender, male and female has always been of great interest and present in almost all fiction and films, within different perspectives. Ever since the dawn of time, man has taught ethics, morals, values and existential questions using fictional stories in oral and later in written form. The aim of analyzing the concept of male and female from fiction and film is to reach deeper insight and understanding of human beings with special reference to gender in both individual, group and community perspective. The general aim is to create security in our gender and professional roles. Whole or parts of books and films are used, chosen according to the specific learning outcome. Different aspects are then discussed in lectures, seminars or groups. Aspects include projections, culture and behavior. Students learn to reach a deeper insight and understanding of gender regarding behavior, communication, attitudes, values, culture and ethnicity. Peeling off the illusion of male and female created by our social structure, and making our values and prejudices conscious, we might discern the eternally human, the inner core of being human, regardless of sex. Fiction and film make conscious behavior, communication and gender, and merge cognition and emotions. This way of teaching may lead to better treatment of patients by health care personnel

    • This study investigates the effects of the Panax ginseng (Araliaceae) extract G115 on angiotensin-converting enzyme (ACE) activity and nitric oxide (NO) in cultured human endothelial cells from umbilical veins (HUVEC) and bovine mesenteric arteries (BMA). In HUVEC, ACE activity was significantly reduced after 10 min incubation with aqueous extract of ginseng 5.0 and 10 mg/ml. This effect was additative with the inhibition of the traditional ACE inhibitor enalaprilat. No effect was seen on NO production from the cells. Angiotensin I-induced contraction of BMA was significantly attenuated by 0.1 and 0.5 mg/ml ginseng, while no endothelium-dependent or -independent relaxation was seen. In conclusion, extract of Panax ginseng (G115) inhibits ACE activity, but does not affect NO production in HUVEC and BMA. © 2005 Elsevier Ireland Ltd. All rights reserved.
