  • Design of a national patient-centred clinical pathway for sepsis in Sweden

    • BACKGROUND: The World Health Organization has adopted a resolution on sepsis and urged member states to develop national processes to improve sepsis care. In Sweden, sepsis was selected as one of the ten first diagnoses to be addressed, when the Swedish government in 2019 allocated funds for patient-centred clinical pathways in healthcare. A national multidisciplinary working group, including a patient representative, was appointed to develop the patient-centred clinical pathway for sepsis.METHODS: The working group mapped challenges and needs surrounding sepsis care and included a survey sent to all emergency departments (ED) in Sweden, and then designed a patient-centred clinical pathway for sepsis.RESULTS: The working group decided to focus on the following four areas: (1) sepsis alert for early detection and management optimisation for the most severely ill sepsis patients in the ED; (2) accurate sepsis diagnosis coding; (3) structured information to patients at discharge after sepsis care and (4) structured telephone follow-up after sepsis care. A health-economic analysis indicated that the implementation of the clinical pathway for sepsis will most likely not drive costs. An important aspect of the clinical pathway is implementing continuous monitoring of performance and process indicators. A national working group is currently building up such a system for monitoring, focusing on extraction of this information from the electronic health records systems.CONCLUSION: A national patient-centred clinical pathway for sepsis has been developed and is currently being implemented in Swedish healthcare. We believe that the clinical pathway and the accompanying monitoring will provide a more efficient and equal sepsis care and improved possibilities to monitor and further develop sepsis care in Sweden.

  • Fatigue, psychological distress, coping and quality of life in patients with uterine cancer.

    • BACKGROUND: Cancer-related fatigue (CRF) is a subjectively experienced symptom that is multidimensional and multifactorial. Patients with cancer have identified fatigue as one of the major troubling symptoms and the primary cause of distress in their lives. AIMS: The major aim of the study was to examine how patients diagnosed with uterine cancer describe their experience of fatigue, psychological distress, coping resources and quality of life. A secondary aim was to describe the relationship between selected variables. METHOD: A descriptive and correlational design was used and the study was conducted at a university hospital in Sweden. The study population consisted of women, diagnosed with uterine cancer, who where scheduled to receive curative external radiation therapy. Sixty women participated in the study and data were collected through self-report instruments. Demographic and clinical data were extracted from patient records. The data were collected during year 2000-2002. The Conceptual Model of Symptom Management was used as a framework to guide the study. FINDINGS: Patients experienced a low grade of fatigue and psychological distress, but their functional status and global quality of life was high. Significant correlations were found between general fatigue and anxiety and also between general fatigue and depression. There was a significant negative correlation between general fatigue and coping resources. Depression explained 44% of the variance in general fatigue. CONCLUSIONS: The findings provide knowledge about predictors of CRF in women with uterine cancer, and can serve as a basis for future longitudinal studies in which different prophylactic strategies against therapy-related fatigue are prospectively studied.

    • The two breast cancer genes BRCA1 and BRCA2 were identified more than 10 years ago and, depending on population, mutations in these genes are responsible for a varying percentage of familial breast cancer. In more than half the families, the increased risk of breast cancer cannot be explained by mutations in these genes, and the goal of this study was to locate novel susceptibility genes. One of the main difficulties in identifying the cause of hereditary non-BRCA1/BRCA2 breast cancer is genetic heterogeneity, possibly due to multiple, incompletely penetrant susceptibility genes, along with ethnic and geographic differences. In this study, one large family and 13 small to medium-sized families with multiple cases of breast cancer were analyzed by genome-wide linkage analysis. The genome scan was performed by genotype analysis of 10,000 SNP markers on microarrays. The strongest evidence of linkage (HLOD 2.34) was obtained on chromosome region 10q23.32-q25.3. A further two regions were identified, with LOD scores above 2.10 on 12q14-q21 and 19p13.3-q12. In a subset of families of western Swedish origin, two regions generated LOD scores exceeding 1.8: 10q23.32-q25.3 and 19q13.12-q13.32. The large family in the study exceeded LOD 1.5 in three regions: 10q23.32-q25.3, 19q13.12-q13.32, and 17p13. Our results indicate that one or more of the suggested regions may harbor genes that are involved in the development of breast cancer.

  • Forest biodiversity and ecosystem services from spruce-birch mixtures : The potential importance of tree spatial arrangement

    • There is increasing empirical support for the biodiversity and ecosystem service (ES) benefits of mixed-species production forests. However, few studies control for the spatial arrangement of the trees within mixtures to determine the influence that clustering the tree species (patch scale mixtures), versus evenly dispersing them (intimate scale mixtures), may have for biodiversity and ES outcomes. To highlight the potential implications of altering tree spatial arrangement in mixtures, and the need to fill related knowledge gaps, here we provide a qualitative multi-disciplinary overview of ecological and socio-economic drivers with the potential to alter biodiversity, ecosystem services, and management-related outcomes from patch versus intimate scale mixtures. We focused our overview on even-aged mixtures of Norway spruce (Picea abies) and birch (Betula pendula or B. pubescens) in Sweden, which enabled us to contrast findings within a biogeographical and silvicultural setting. Specifically, we targeted implications for biodiversity (understory vascular plants, epiphytic lichens, saproxylic beetles, birds), biomass production, harvesting costs, management ease, recreation and aesthetics, cervid game, as well as abiotic and biotic risks (wind, fire, pathogens, pests, browsing damage). In the absence of direct empirical evidence, we primarily relied on expert inference from theory and relevant empirical studies sourced from the Fennoscandian region, and further afield if needed. Collectively these efforts allowed us to develop a number of informed hypotheses indicating that for spruce-birch mixtures in this region, patch scale mixtures may have the potential to favour the diversity of several forest dependant taxonomic groups, cervid game and reduce harvesting costs, whereas intimate mixtures may have the potential to reduce pathogen and pest damage, and likewise, potentially benefit production outcomes. Current knowledge was too limited, inconsistent or context dependant to even tentatively infer outcomes for fire risk, wind damage, browsing damage, management ease, recreational and aesthetic outcomes. We emphasize that our hypotheses require testing, but are sufficient to (1) highlight the likely importance of spatial-scale to biodiversity and ecosystem services outcomes in mixed-species production forests, (2) caution against generalization from mixture studies that lack scale considerations, and (3) motivate the targeted consideration of spatial grain in future mixture studies.

    • CD169(+) resident macrophages in lymph nodes of breast cancer patients are for unknown reasons associated with a beneficial prognosis. This contrasts CD169(+) macrophages present in primary breast tumors (CD169(+) TAMs), that correlate with a worse prognosis. We recently showed that these CD169(+) TAMs were associated with tertiary lymphoid structures (TLSs) and T-regs in breast cancer. Here, we show that CD169(+) TAMs can be monocyte-derived and express a unique mediator profile characterized by type I IFNs, CXCL10, PGE(2) and inhibitory co-receptor expression pattern. The CD169(+) monocyte-derived macrophages (CD169(+) Mo-M) possessed an immunosuppressive function in vitro inhibiting NK, T and B cell proliferation, but enhanced antibody and IL6 secretion in activated B cells. Our findings indicate that CD169(+) Mo-M in the primary breast tumor microenvironment are linked to both immunosuppression and TLS functions, with implications for future targeted Mo-M therapy.

  • Detailed analysis of the characteristics of sample volume in blood culture bottles

    • Blood volume is the most important variable, for detection of microorganisms in blood cultures (BC). Most standards recommend 40-60 ml blood, collected in several BC bottles filled up to 10 ml. We measured blood volume in individual BC bottles, and analysed the association of hospital, bottle type, day of the week, daily sampling time, and age and gender of the patient, with sampling volume and BC result. The variation in blood volume per BC bottle was analysed in a mixed linear model using hospital, bottle type, weekday, sampling time, age and gender as fixed factors, and patient ID and episode as random factors to control for repetitive sampling of individual patients. Only 18 % of all bottles were filled with the recommended 8-10 ml, and 47 % were filled with less than 8 ml. The mean (±SE) volume was larger in positive 9.09 (±0.15) compared to negative bottles 8.47 (±0.07) (p<0.001). Blood volume was larger in BacT/ALERT-FA Plus than in -FN Plus BC bottles (p<0.001). There was significantly lower volumes collected during the night (p<0.001). The volume of blood collected decreased significantly with increasing patient age (p<0.001). Larger volumes were collected from males compared to females, 8.78 (±0.06) vs. 8.36 (±0.06) ml (mean ± SE) respectively (p<0.001). The odds of detecting a positive patient increases with 13 % for each additional ml blood drawn. Our results show that we need to work actively with development of blood sampling routines to overcome age and gender effects, and to optimize blood sampling volumes.

  • From simple metrics to cervid forage : Improving predictions of ericaceous shrub biomass

    • Common understory vegetation species such as the ericaceous shrubs bilberry (Vaccinium myrtillus), cowberry (V. vitis-idaea) and heather (Calluna vulgaris), are key forage plant species for moose and other large herbivores, as well as fulfilling many additional ecosystem functions and services. Here we developed models to predict above-ground biomass of these ericaceous species in coniferous forests, using data on their percentage cover, height, and different stand characteristics. We also built models to understand how the aforementioned variables affect the proportion of the shrubs commonly utilized as forage by large herbivores. We found that the percentage cover of shrubs was the most important explanatory variable when predicting above-ground biomass, explaining 51%, 47% and 71% of the variation (marginal R2) in bilberry, cowberry and heather biomass, respectively. By adding ramet height to the model with percentage cover, the variation explained increased to 77% for bilberry, 75% for cowberry and 87% for heather. The best outcome for candidate models was obtained by adding stand site index and spruce basal area to the model, improving the variation explained in bilberry to 83%, to 81% for cowberry, and 91% for heather. When modelling the proportion of the shrubs commonly utilized as forage by large herbivores, stand site index and spruce basal area often played important roles. Some of the best fitting models for forage biomass explained 51% of the variation in bilberry, 59% in cowberry and 30% in heather. Site location did not have a major role in improving the variability explained in either type of model, which indicated the applicability of the models regardless of study location. Our models therefore have a high potential to be implemented in forestry decision support systems. Their inclusion should provide better large-scale estimations of forage resources, aiding forest management, and thereby taking an important step forward to determine the ecosystem carrying capacity of large herbivores. © 2023 The Author(s)

    • Förbättrade produktionsresultat inom svensk grisproduktion, användning av biproduk-ter och djurhälsans betydelse har analyserats i en livscykelanalys. Klimatavtrycket för kött från en svensk medelgris är 2,54 kg koldioxidekvivalenter/kg slaktvikt, vilket är bland de lägsta jämfört med tillgängliga internationella studier. Produktionen av foder står för 54 % av klimatavtrycket och hanteringen av stallgödsel för 36 %. Av foderstaten till den svenska integrerade medelslaktgrisen utgjorde biprodukter 10 % och soja 4 %. Baserat på antalet dagars förlängd uppfödningstid för sjuka grisar, visar vi att 3,4 % av klimatavtrycket beror på ökad foderförbrukning orsakad av fyra utvalda sjukdomar i svenska grisbesättningar. Produktionshöjande åtgärder såsom exempelvis friska grisar och hög fodereffektivitet, övergång till förnybara bränslen inom såväl odling av foder som inom grisuppfödning är viktiga åtgärder för att minska klimatavtrycket givet att det inte påverkar andra miljöaspekter, djurhälsa eller djurvälfärd negativt. En central aspekt är fortsatt utveckling mot välbalanserade foderstater med val av foderråvaror med lågt klimatavtryck, användning av biprodukter och inhemska fodergrödor odlade på ett hållbart sätt.

  • An animal model to study health effects during continuous low-dose exposure to the nerve agent VX

    • In the present study, we have developed an animal model to study long-term health effects of continuous exposure of toxic chemical agents, in awake, freely moving rats. The aim was to evaluate the effect of low-dose exposure of the nerve agent VX, and to find specific biomarkers for intoxication. To exclude the influence of stress, we used an implanted radio-telemetric device for online registration of physiological parameters, and an osmotic pump, implanted subcutaneously, for continuous exposure of the toxic agent. Our results showed that the lowest observable effect dose of VX in Wistar rats was 5 microg/kg/24 h, after continuous exposure by the osmotic pump. Although we observed significant inhibition of acetylcholinesterase (AChE) in blood and a significant decrease in body weight gain at this dose, no change in blood pressure, heart rate or respiratory rate was registered. However, a significant decrease in the thyroid hormone, free T4, was measured in blood after 8 weeks, indicating that low doses of VX might affect the thyroid function. Rats given repeated daily injections were more sensitive to VX and needed only 1/10 of the concentration to reach a similar level of AChE inhibition, compared to animals exposed by the osmotic pump. Moreover, the results showed that exposure of VX in our experimental design, does not induce an increase in corticosterone blood levels. Thus, the model used in this investigation renders minimal stress and will not cause unnecessary pain to the animals, indicating that this model could be a useful tool to study long-term effects of various toxic substances in freely moving rats.

    • It is now widely accepted that adaptation will be necessary if we are to manage the risks posed by climate change. What we know about adaptation, however, is limited. While there is a well established body of scholarship proposing assessment approaches and explaining concepts, few studies have examined if and how adaptation is taking place at a national or regional level. This deficit in understanding is particularly pronounced in developed nations which have typically been assumed to have a low vulnerability to climate change. Yet as recent research highlights, this assumption is misplaced: developed nations are experiencing the most pronounced changes in climatic conditions globally and have significant pockets of vulnerability. Chapters in this book profile cases from different sectors in developed nations where specific adaptation measures have been identified, implemented, and evaluated. The contributions provide practical advice and guidance that can help guide adaptation planning in multiple contexts, identifying transferable lessons. It is a comprehensive and timely piece of work on an emerging body of literature that is critical for both academics and policy makers to be aware of and learn from in regards to the importance of adaptation and adaptation needs associated with climate change and variability.     • This study analyzes processes of adaptation to climate change through participatory research in Sweden’s two largest cities, Stockholm and Gothenburg. Perceptions of climate risks and constraints to adaptation are discussed. Practitioners from the public and private sector have identified stakeholders who are, and who should be, giving attention to adaptation, including the risks and threats facing the regions and how and which factors hinder the implementation of adaptation. In this study, it is found that those issues where adaptation is considered most difficult are mainly related to response capacity.

    • Background: Biosecurity routines at herd level may reduce the probability of introduction of disease into the herd, but some measures may be regarded as expensive and cumbersome for the farmers. Custom-made measures based on individual farm characteristics may aid in improving the actual application of on-farm biosecurity. The aim of the study was to provide a tool for calculating the effects of different biosecurity measures and strategies on the individual farm level.A simple model was developed to assess the risk of disease introduction and the need for biosecurity measures in individual farms. To illustrate the general applicability of the tool, it was applied to theoretical examples of Swedish cattle and pig farms and diseases endemic in those animal species in the EU, in two scenarios with different between-farm contact patterns.Results: The model illustrated that the most important factors affecting the risk, and the effect of biosecurity measures such as quarantine routines and protective clothing, were the frequency of between- farm contacts and prevalence of the disease. The risk of introduction as well as the effect of biosecurity measures differed between farm types and disease transmission routes. Adapting contact patterns to mitigate a specific disease risk was as important as biosecurity measures for some farm types, but the largest effect was seen when combining biosecurity measures with more planned contact patterns.Conclusions: The risk assessment model proved useful for illustrating the risk of introduction of endemic diseases and the mitigating effect of different biosecurity measures on farm level. Model outputs could be used to justify prioritisation of measures or adapting contact patterns. The theoretic exercise of adjusting model inputs and comparing outputs may help veterinary advisors to understand farm-specific risks and motivate farmers to improve biosecurity in their individual farm, as it can be tailored to each farmer's needs and preferences.

    • Universal parental support intended to enhance parents' capacity for parenting is an important aspect of public health strategies. However, support has mostly been aimed at parents, especially mothers, of younger children. There is a gap in the research concerning parents of adolescents and fathers' interest in parenting support. Aim: To investigate and compare the interest in parenting support of parents of adolescents and younger children, potential differences between mothers and fathers, and their knowledge of what is being offered to them already, and to explore their requirements for future universal parental support. Methods: Telephone interviews were conducted with a random sample of 1336 parents. Quantitative methods were used to analyze differences between groups and qualitative methods were used to analyze open-ended questions in regard to parents' requirements for future universal parental support. Results: About 82% of the parents of adolescents interviewed think that offering universal parental support is most important during child's adolescence. There is a substantial interest, particularly among mothers, in most forms of support. Despite their interest, parents have limited awareness of the support available. Only 7% knew about the local municipality website, although 70% reported a possible interest in such a website. Similarly, 3% knew that a parent phone line was available to them, while 59% reported a possible interest. Conclusions: It poses a challenge but is nevertheless important for municipalities to develop support targeted at parents of adolescents which is tailored to their needs, and to reach out with information.

    • The overarching aim of this thesis was to explore the general interest in universal parental support, the circumstances under which this interest is stronger, and how universal support groups for parents of adolescents could be developed according to those parents’ perceived needs. Study I examined parents´ interest in municipal parental support. The results showed that mothers were more interested than fathers in all forms of parental support except a webpage, and that frequent use of the Internet as a source of parenting information was associated with high interest in parental support. Study II explored interest in existing and possible uni-versal parental support in parents of adolescents compared with parents of younger children. About 82% of the parents of adolescents inter-viewed considered universal parental support most important during the child’s adolescence. There was substantial interest, in most forms of support. Despite their interest, parents had limited awareness of available support. Study III explored the factors associated with interest in univer-sal parental support and found it was linked to parents’ own anxious mood, lower perceived parental capacity, perception of their child as having psychiatric problems, perception of their adolescents’ openness about things, and perception of their adolescent’s overall difficulties in daily life due to psychiatric symptoms. Study IV explored what kind of support parents of adolescents’ request from universal parent support groups and what practical requirements would enable parents to partici-pate. Parents could give each other emotional support and develop better parenting skills together. Reaching out with support universally requires that various information channels be utilized to reach out to all parents regardless of gender and social status, and various forms of support be offered. The Internet is an important but challenging information chan-nel for reaching out to parents, especially fathers. Supports should be developed that are targeted to parents of adolescents, tailored to their needs, and well-advertised. Schools are important arenas for offering universal parental supports such as support groups moderated by trained professionals. Lighter support forms should be accompanied by more resource-demanding forms of support. to prevent increasing inequalities between parents with different social situations.

    • Parenting a child through adolescence can be a challenge for many parents; adolescents go through notable developmental, physical, and psychological changes that affect the relationships around them. Most parents find parental support during these years important. These young people's relationships with their parents are important to their mental health, but although parental support is universally available to parents of younger children, it is still scarce for parents of adolescents. The aim of this study was to explore what factors are associated with interest in universal parental support through telephone interviews and questionnaires with 223 parents of 13- to 17-year-olds. Parents’ interest in parental support was linked to their own anxious mood, their lower perceived parental capacity, their perception of the child as having psychiatric problems, the parents’ perception of their adolescents’ openness about things and their perception of the adolescent's overall difficulties in daily life due to psychiatric symptoms. The results show that lighter forms of support such as lectures or seminars were more appealing to parents with higher social status. Offering community-based individual counseling and leader-led parent training groups therefore has the potential to reach parents with difficulties more equally, while offering support only through lectures and seminars could increase the inequality between parents in different social situations.

    • Ekologisk produktion bygger på en ekocentrisk grundsyn där man värderar naturlighet högt. Detta ledertill ökat fokus på att skapa produktionssystem där djuren kan få utlopp för sina naturliga beteenden. Tillexempel ska djuren ha större ytor att röra sig på, tillgång till bete och utevistelse och ha tillgång till grovfoder.Men det innebär vissa utmaningar. Till exempel leder utevistelse till att djuren i högre grad utsättsför parasiter och vissa moderna raser kan få benproblem när de rör sig mycket i ekologiska system. Dessaoch andra utmaningar behöver den ekologiska branschen adressera, och forskningen fortsätta att belysa,för att ytterligare förbättra djurhälsa och djurvälfärd i den ekologiska djurproduktionen.Den här rapporten sammanfattar den senaste forskningen kring djurvälfärd inom ekologisk djurproduktion.Den behandlar ekologisk mjölk- och nötproduktion, ekologisk får- och lammproduktion, ekologiskgrisproduktion samt ekologisk ägg- och slaktkycklingsproduktion. Sammanfattningsvis diskuterar denvilka målkonflikter som kan uppstå inom ekologisk djurproduktion. Ambitionen är att ge främst forskareoch rådgivare en uppdaterad inblick i hur forskningsbehovet och kunskapsläget ser ut inom området menäven att öka insikten hos beslutsfattare om vilka målkonflikter som finns inom ekologisk djurproduktionoch varför de uppstår.SLU Ekologisk produktion och konsumtion, Epok, har finansierat arbetet med rapporten. Huvudförfattareär Torun Wallgren, SLU, Niels Andresen, HIR Skåne AB och Magdalena Åkerfeldt, SLU. Rapportenbygger på en kunskapssammanställning som påbörjades 2019 av Magdalena. I januari 2020 hölls ett seminariumpå KSLA med titel ”Hur mår djuren i ekologisk produktion?” som gav en del inspel till fortsattarbete med sammanställningen. En vetenskaplig artikel med titel ”Health and welfare in organic livestockproduction systems – a systematic mapping of current knowledge” publicerades senare 2020 i OrganicAgriculture med Magdalena som huvudförfattare.
