  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

    • The experimental methodology achieved in the present study signified that the adsorption and precipitation were main mechanisms occurred together in the removal of nickel from aqueous solution by sorption using cement kiln dust (CKD) byproduct as sorbent. Finding the contribution of each mechanism in the removal process and derivation an analytical model for finding the portion of precipitation were the focal points of this work. Results proved that pure precipitation was increased with the increase of CKD dosage and metal concentration where total removal (adsorption-precipitation) ranged from 45 to 100%. The SEM micrographs of the CKD sorbent before and after sorption process certified that there was crystal precipitates on the surface of the CKD. Also, these graphs in combination with FT-IR tests proved that [Ni(OH2)n]+2 (n=4-6) species were bonded with CKD and insoluble hydroxide species may be precipitated onto the CaO surfaces by co-precipitation, while K–O, Si-O and Ca-O groups enhanced the adsorption mechanism.

    • Wastewater treatment and subsequent effluent recycling for non-drinking purposes such as irrigation contributes to the mitigation of the pressure on freshwater resources. In this study, two vertical sub-surface flow constructed wetland (VSSF-CW) pilot plants were operated to treat municipal wastewater and their effluents were reused for irrigation purposes. One of the wetlands was vegetated with Phragmites australis (Cav.) Trin. ex Steud. (common reed) to compare its efficiency of pollutant removals with the non-vegetated system, which had the same design. COMSOL Multiphysics 3.5a was operated for the Activated Sludge Model 2 (ASM2) to predict the chemical oxygen demand (COD) and ammonia-nitrogen (NH4-N) concentrations. The effluent quality of both treatment systems was assessed for several parameters. Computer simulations show a good compliance between the measured and predicted values of COD and NH4-N for the vegetated system. The calibrated model could be effectively used to predict the behaviours of those parameters as a function of time. Moreover, the effluents of both vegetated (VFp) and non-vegetated (VF) VSSF-CW were significantly (p < 0.05) improved compared to influent. Significant (p < 0.05) effects due to the presence of P. australis were observed for removals of total suspended solids (TSS), 5-day biochemical oxygen demand (BOD5), COD, NH4-N and ortho-phosphate-phosphorus (PO4-P). However, significant increases (p < 0.05) were noted for electrical conductivity (EC), total dissolved solids (TDS), nitrate-nitrogen (NO3-N) and sulphate (SO4) of both effluents compared to the raw wastewater. Except for EC, NH4-N and SO4, all water quality parameters complied with irrigation water standards.

  • Effect of COVID-19 Anthropause on Water Clarity in the Belize Coastal Lagoon

    • The Coronavirus disease 2019 (COVID-19) pandemic halted human activities globally in multiple sectors including tourism. As a result, nations with heavy tourism, such as Belize, experienced improvements in water quality. Remote sensing technologies can detect impacts of “anthropauses” on coastal water quality. In this study, moderate resolution imaging spectroradiometer (MODIS) satellite data were employed along the Belizean coast to investigate impacts of the COVID-19 shutdown on water quality. The attenuation coefficient at 490 nm, Kd(490), was used as an indicator of water quality, with a lower Kd(490) indicating increased water clarity. Four Coastal Management Zones were characterized by marine traffic as high traffic areas (HTAs) and two as low traffic areas (LTAs). Monthly composites for two periods, 2002–2019 (baseline) and 2020 were examined for Kd(490). For months prior to the COVID-19 shutdown in Belize, there was generally no significant difference in Kd(490) (p &gt; 0.05) between 2020 and baseline period in HTAs and LTAs. Through the shutdown, Kd was lower in 2020 at HTAs, but not for LTAs. At the LTAs, the Kd(490)s observed in 2020 were similar to previous years through October. In November, an unusually active hurricane season in 2020 was associated with decreased water clarity along the entire coast of Belize. This study provides proof of concept that satellite-based monitoring of water quality can complement in situ data and provide evidence of significant water quality improvements due to the COVID-19 shutdown, likely due to reduced marine traffic. However, these improvements were no longer observed following an active hurricane season.

    • The present study develops the sorption model for simulating the effects of pH and temperature on the uptake of cadmium from contaminated water using waste foundry sand (WFS) by allowing the variation of the maximum adsorption capacity and affinity constant. The presence of two acidic functional groups with the same or different affinity is the basis in the derivation of the two models; Model 1 and Model 2 respectively. The developed Bi-Langmuir model with different affinity (Model 2) has a remarkable ability in the description of process under consideration with coefficient of determination > 0.9838 and sum of squared error < 0.08514. This result is proved by FTIR test where the weak acids responsible of cadmium ions removal using WFS sorbent can be represented by surface silanol (O-H) functional groups.

    • Background B-cell depleting therapies are highly efficacious in relapsing-remitting multiple sclerosis but one such therapy, rituximab, is not approved for multiple sclerosis and no phase 3 trial data are available. We therefore examined the safety and efficacy of rituximab compared with dimethyl fumarate in patients with relapsing-remitting multiple sclerosis to obtain data that might allow inclusion of rituximab in treatment guidelines. Methods RIFUND-MS was a multicentre, rater-blinded, active-comparator, phase 3, randomised controlled trial done at 17 Swedish university and community hospitals. Key inclusion criteria for participants were: age 18-50 years; relapsing-remitting multiple sclerosis or clinically isolated syndrome according to prevailing McDonald criteria; 10 years or less since diagnosis; untreated or only exposed to interferons or glatiramer acetate; and with clinical or neuroradiological disease activity in the past year. Patients were automatically randomly assigned (1:1) by the treating physician using a randomisation module in the Swedish multiple sclerosis registry, without stratification, to oral dimethyl fumarate 240 mg twice daily or to intravenous rituximab 1000 mg followed by 500 mg every 6 months. Relapse evaluation, Expanded Disability Status Scale rating, and assessment of MRI scans were done by examining physicians and radiologists masked to treatment allocation. The primary outcome was the proportion of patients with at least one relapse (defined as subacute onset of new or worsening neurological symptoms compatible with multiple sclerosis with a duration of more than 24 h and preceded by at least 30 days of clinical stability), assessed in an intention-to-treat analysis using log-binomial regression with robust standard errors. This trial is registered at ClinicalTrials.gov, NCT02746744. Findings Between July 1, 2016, and Dec 18, 2018, 322 patients were screened for eligibility, 200 of whom were randomly assigned to a treatment group (100 assigned to rituximab and 100 assigned to dimethyl fumarate). The last patient completed 24-month follow-up on April 21, 2021. 98 patients in the rituximab group and 97 patients in the dimethyl fumarate group were eligible for the primary outcome analysis. Three (3%) patients in the rituximab group and 16 (16%) patients in the dimethyl fumarate group had a protocol-defined relapse during the trial, corresponding to a risk ratio of 0.19 (95% CI 0.06-0.62; p=0.0060). Infusion reactions (105 events [40.9 per 100 patient-years]) in the rituximab group and gastrointestinal reactions (65 events [47.4 per 100 patient-years]) and flush (65 events [47.4 per 100 patient-years]) in the dimethyl fumarate group were the most prevalent adverse events. There were no safety concerns. Interpretation RIFUND-MS provides evidence that rituximab given as 1000 mg followed by 500 mg every 6 months is superior to dimethyl fumarate in preventing relapses over 24 months in patients with early relapsing-remitting multiple sclerosis. Health economic and long-term safety studies of rituximab in patients with multiple sclerosis are needed.

    • Many researchers are interested in using natural fibres to treat due to recent advancements in polymer characteristics. The mechanical properties of three types of bars are studied in this paper: Cotton Fibre-Reinforced Polymer bars, Cotton/Carbon Fibre-Reinforced Polymer bars, and Cotton/Glass Fibre-Reinforced Polymer bars. The goal of the paper was to create low-cost bars with comparable mechanical performance and corrosion resistance to steel reinforcement. The bars were made using two methods: fibres immersed in polymer and fibres coated with polymer by repeated tension and relaxation of fibres. The second method produced better results in terms of the tensile strength of Cot.CFRP, Cot.GFRP, and Cot.FRP bars, which were 688, 477, and 284 MPa, respectively, and the stress–strain curve revealed brittle behaviour for all bars and modulus of elasticity of 43, 31 and 22 GPa. When sand was put on the bar’s surface, the bars demonstrated a good connection with the concrete. It also showed good resistance to moisture, alkaline solutions and acids, as well as heat resistance at temperatures below 200°C.

    • We aimed to obtain magnesium/iron (Mg/Fe)-layered double hydroxides (LDHs) nanoparticles-immobilized on waste foundry sand-a byproduct of the metal casting industry. XRD and FT-IR tests were applied to characterize the prepared sorbent. The results revealed that a new peak reflected LDHs nanoparticles. In addition, SEM-EDS mapping confirmed that the coating process was appropriate. Sorption tests for the interaction of this sorbent with an aqueous solution contaminated with Congo red dye revealed the efficacy of this material where the maximum adsorption capacity reached approximately 9127.08 mg/g. The pseudo-first-order and pseudo-second-order kinetic models helped to describe the sorption measurements, indicating that the physical and chemical forces governed the removal process.

    • Re-use of the byproduct wastes resulting from different municipal and industrial activities in the reclamation of contaminated water is real application for green projects and sustainability concepts. In this direction, the synthesis of composite sorbent from the mixing of waterworks and sewage sludge coated with new nanoparticles named “siderite” (WSSS) is the novelty of this study. These particles can be precipitated from the iron(II) nitrate using waterworks sludge as alkaline agent and source of carbonate. Characterization tests using X-ray diffraction (XRD), scanning electron microscopy (SEM) and energy dispersive spectroscopy (EDS) mapping revealed that the coating process was correctly achieved and siderite nanoparticles were planted on the surface of the composite sorbent. Interaction between Congo Red dye and composite sorbent was evaluated through a set of batch tests under the variation of agitation time, pH of aqueous solution, and sorbent dosage. The results proved that the prepared sorbent had a high ability in the treatment of water contaminated with Congo Red dye in comparison with previous studies and the maximum adsorption capacity reached to maximum value i.e. 9416 mg/g. The sorption process was governed by electrostatic attractions; however, Sips and Pseudo-second-order models described this process with coefficient of determination greater than 0.99.
