  • Robust treatment planning of dose painting for prostate cancer based on ADC-to-Gleason score mappings : what is the potential to increase the tumor control probability?

    • Background and Purpose The aim of this study was to evaluate the potential to increase the tumor control probability (TCP) with ‘dose painting by numbers’ (DPBN) plans optimized in a treatment planning system (TPS) compared to uniform dose plans. The DPBN optimization was based on our earlier published formalism for prostate cancer that is driven by dose-responses of Gleason scores mapped from apparent diffusion coefficients (ADC).Material and MethodsFor 17 included patients, a set of DPBN plans were optimized in a TPS by maximizing the TCP for an equal average dose to the prostate volume (CTVT) as for a conventional uniform dose treatment. For the plan optimizations we applied different photon energies, different precisions for the ADC-to-Gleason mappings, and different CTVT positioning uncertainties. The TCP increasing potential was evaluated by the DPBN efficiency, defined as the ratio of TCP increases for DPBN plans by TCP increases for ideal DPBN prescriptions (optimized without considering radiation transport phenomena, uncertainties of the CTVT positioning, and uncertainties of the ADC-to-Gleason mapping).ResultsThe median DPBN efficiency for the most conservative planning scenario optimized with a low precision ADC-to-Gleason mapping, and a positioning uncertainty of 0.6 cm was 10%, meaning that more than half of the patients had a TCP gain of at least 10% of the TCP for an ideal DPBN prescription. By increasing the precision of the ADC-to-Gleason mapping, and decreasing the positioning uncertainty the median DPBN efficiency increased by up to 40%.ConclusionsTCP increases with DPBN plans optimized in a TPS were found more likely with a high precision mapping of image data into dose-responses and a high certainty of the tumor positioning. These findings motivate further development to ensure precise mappings of image data into dose-responses and to ensure a high spatial certainty of the tumor positioning when implementing DPBN clinically.

The story of the company and its business events is presented as episodes during a financial year and are discussed in practical and theoretical terms. The episodes are first dealt with from a book-keeping point of view, which in turn forms the foundation for the closing of the books, and, finally, preparation of the financial statements and the annual report.     • Följande ULF-projekt (Utveckling, lärande, forskning) är ett samarbete mellan en kommunal gymnasieskola i en mellanstor stad samt Linköpings Universitet. Syftet är att öka förståelsen för hur gymnasieelever upplever prestationssituationer. Genom en narrativ ansats har 261 elevberättelser analyserats och kategoriserats. De känslor som kan knytas till erfarenheter av att bli bedömda kallas i studien för prestationsemotioner. En prestation kan definieras som en aktivitet som mäts mot någon form av standard och prestationsemotioner är emotioner som kopplas till en sådan aktivitet. Vanligt förekommande positiva prestationsemotioner är stolthet, nöje, hopp, tacksamhet, avslappning, belåtenhet och lättnad. Negativa prestationsemotioner är oro, ångest, stress, skam, tristess, hopplöshet eller besvikelse (Pekrun, 2007; 2014). I studien finns en rad positiva och negativa emotioner som eleverna tillskriver sina prestationer, samt att elevernas känslor ofta förändras i samband med viktiga prestationer. Eleverna gör känsloresor som verkar ha betydelse för deras vilja och förmåga att prestera i framtiden. En mer utförlig resultatredovisning görs i en annan artikel i Venue. Utöver dessa resultat har projektet skapat ett kollegialt lärande som lyfter och bidrar till en mer hållbar bedömningspraktik.

    • I denna artikel presenteras resultat från ett ULF-projekt där lärare på en gymnasieskola, i samverkan med forskare från LiU, samlat in och analyserat elevers skrivna berättelser om sina erfarenheter av emotioner i relation till prestationer under de år som de gått på gymnasiet. En slutsats vi drar är att fler skolor bör analysera sin egen praktik angående emotioner i relation med bedömningstillfällen och att den metod lärare och forskare utvecklat i detta projekt ger gott stöd i ett sådant arbete. Det kan få stor betydelse för lärares arbete och för elevers lust att lära och prestera i skolan.

  • Cataract surgery in patients with uveitis : Data from the Swedish National Cataract Register

    • PURPOSE: To investigate the surgical and pharmacological management and outcomes of patients with cataract and concurrent uveitis.METHODS: Data from the Swedish National Cataract Register, 2018-2019, were collected and analysed. Uveitic eyes were identified and eyes without uveitis were used as controls. Generalized estimating equations were used to adjust for intra-individual correlation.RESULTS: The study included 719 eyes with and 256 360 without uveitis. The mean age was 66.0 ± 13.5 (standard deviation [SD]) years in the uveitis group and 74.3 ± 8.7 years in the control group (p < 0.001). Surgery was associated with more intraoperative difficulties in eyes with uveitis (27.0%) than in control eyes (7.1%; p < 0.001). Posterior capsule rupture/zonular complications were registered in nine eyes with uveitis (1.3%) and in 1464 eyes without uveitis (0.6%; p = 0.02). Hydrophilic acrylic intraocular lenses (uveitis 3.6%, controls 1.2%) and subconjunctival steroids (uveitis 17.4%, controls 6.1%) were more frequently used in eyes with uveitis (p < 0.001). post-operative best-corrected visual acuity (BCVA) was 0.16 ± 0.38 logarithm of the minimum angle of resolution (logMAR, mean ± SD) in eyes with uveitis (n = 52) and 0.08 ± 0.20 in control eyes (n = 14 489; p = 0.008).CONCLUSION: In this large registry-based Swedish cohort study, the findings demonstrate that cataract surgery in patients with uveitis poses more challenges and requires special surgical precautions. Eyes with concurrent uveitis had worse BCVA prior to and following surgery. Despite the intraoperative challenges, the visual improvement was greater in the uveitic group.
