    • Nowadays, it is already a truism to state that digital media are effective tools used by citizens, civil society organizations (CSOs) and social movementsto protest, to demand, to push and (sometimes) achieve social and political change. No question, they are. This has been observed and registered time and again in a luminous series of academic contributions (some notable examples include Castells 2012; Cottle 2011; De Luca. Lawson and Sun 2012; Howard and Muzammil 2011; Tremayne 2014; Tufekci 2017). Exactly how these media have been taken up by civic actors in specific contexts has been studied and discussed in fascinating detail (Cammaerts, 2018; Gerbaudo 2012; Treré 2019). This book goes one step further to ask a broader question: Has the use of digital media by civic actors improved (or depleted) the quality of democratic life understood as broad and effective citizen participation in public affairs and decision-making?

    • Present-day, noisy, small or intermediate-scale quantum processors-although far from fault tolerant-support the execution of heuristic quantum algorithms, which might enable a quantum advantage, for example, when applied to combinatorial optimization problems. On small-scale quantum processors, validations of such algorithms serve as important technology demonstrators. We implement the quantum approximate optimization algorithm on our hardware platform, consisting of two superconducting transmon qubits and one parametrically modulated coupler. We solve small instances of the NP (nondeterministic polynomial time)-complete exact-cover problem, with 96.6% success probability, by iterating the algorithm up to level two.

  • A hazards approach to the biometric analysis of infant mortality

    • A variation of the Bourgeois-Pichat biometric analysis of infant mortality is suggested. In the original model, cumulative mortality in the last eleven months of infancy is assumed to follow a uniform distribution given a log-cube transformation of age. Instead, we assume an exponential distribution. The difference is that while the denominator is constant in the Bourgeois-Pichat model, equal to the number of births, in our model, the denominator is the current population at risk. We argue that our assumption is more satisfactory from a theoretical point of view, since it focus on the conditional probability of dying. Our model gives different estimates of endogenous and exogenous mortality and, in addition, the model fit is slightly better, especially in cases with higher levels of infant mortality.

  • Brachytherapy and osteoradionecrosis in patients with base of tongue cancer

    • Background: Base of tongue cancer incidence and patient survival is increasing why treatment sequelae becomes exceedingly important. Osteoradionecrosis (ORN) is a late adverse effect of radiotherapy and brachytherapy (BT) could be a risk factor. Brachytherapy is used in three out of six health care regions in Sweden. Aims: Investigate if patients treated in regions using BT show an increased risk for ORN and whether brachytherapy has any impact on overall survival. Material and Methods: We used data from the Swedish Head and Neck Cancer Register between 2008–2014. Due to the nonrandomized nature of the study and possible selection bias we compared the risk for ORN in brachy vs non-brachy regions. Results: Fifty out of 505 patients (9.9%) developed ORN; eight of these were treated in nonbrachy regions (16%), while 42 (84%) were treated in brachy regions. Neither age, sex, TNM-classification/stage, p16, smoking, neck dissection, or chemotherapy differed between ORN and no-ORN patients. The risk for ORN was significantly higher for patients treated in brachy regions compared to non-brachy regions (HR = 2,63, p =.012), whereas overall survival did not differ (HR = 0.95, p =.782). Conclusions and Significance: Brachytherapy ought to be used cautiously for selected patients or within prospective randomized studies.

  • Fast Multiqubit Gates through Simultaneous Two-Qubit Gates

    • Near-term quantum computers are limited by the decoherence of qubits to only being able to run low-depth quantum circuits with acceptable fidelity. This severely restricts what quantum algorithms can be compiled and implemented on such devices. One way to overcome these limitations is to expand the available gate set from single- and two-qubit gates to multiqubit gates, which entangle three or more qubits in a single step. Here, we show that such multiqubit gates can be realized by the simultaneous application of multiple two-qubit gates to a group of qubits where at least one qubit is involved in two or more of the two-qubit gates. Multiqubit gates implemented in this way are as fast as, or sometimes even faster than, the constituent two-qubit gates. Furthermore, these multiqubit gates do not require any modification of the quantum processor, but are ready to be used in current quantum-computing platforms. We demonstrate this idea for two specific cases: simultaneous controlled-Z gates and simultaneous iswap gates. We show how the resulting multiqubit gates relate to other well-known multiqubit gates and demonstrate through numerical simulations that they would work well in available quantum hardware, reaching gate fidelities well above 99%. We also present schemes for using these simultaneous two-qubit gates to swiftly create large entangled states like Dicke and Greenberger-Horne-Zeilinger states.

    • Deficiencies of C2 and other components of the classical pathway of complement are associated with increased risk of infections with encapsulated bacteria, such as Haemophilus (H.) influenzae. Defense against H. influenzae is dependent on specific antibodies and complement, which mediate serum bactericidal activity (SBA) and opsonization. Due to lack of normal classical and lectin complement pathway function in C2 deficiency (C2D), SBA would have to depend either on the alternative pathway or on C2 bypass mechanisms. Here we studied SBA against H. influenzae type b (Hib) before and after vaccination in a group of C2-deficient persons, as the bactericidal capacity of antibodies in autologous complement in relation to vaccination has not been investigated at group level in C2D. Sera from 22 persons with C2D and 26 healthy controls were available. Out of these, 18 persons with C2D and all controls had been vaccinated with Act-HIB®. SBA against Hib bacteria was analyzed with autologous serum as the only complement source. Antibodies to Hib capsular polysaccharide had been analyzed previously. Concentrations of mannose-binding lectin (MBL) and other complement components were measured in serum. SBA of both C2-deficient persons and controls was significantly more efficient after vaccination (p = 0.002 and p < 0.0001, respectively). After vaccination, all but two C2-deficient sera and one control serum showed sufficient SBA (<50% surviving bacteria). Before vaccination, SBA of C2-deficient sera was negatively correlated to serum concentrations of MBL (lower proportion of surviving bacteria with higher MBL concentration; r = −0.55, p = 0.008). After vaccination, SBA of C2-deficient sera was negatively correlated to serum concentrations of IgG Hib antibodies (r = −0.56, p = 0.01). In conclusion, SBA against Hib in autologous serum is increased after vaccination in persons with C2D. In unvaccinated C2-deficient persons SBA was correlated to MBL concentration, providing further support for an MBL-dependent C2 bypass mechanism operating in C2D.

    • Aims/hypothesis Chronic stimulation of beta(2)-adrenoceptors, opposite to acute treatment, was reported to reduce blood glucose levels, as well as to improve glucose and insulin tolerance in rodent models of diabetes by essentially unknown mechanisms. We recently described a novel pathway that mediates glucose uptake in skeletal muscle cells via stimulation of beta(2)-adrenoceptors. In the current study we further explored the potential therapeutic relevance of beta(2)-adrenoceptor stimulation to improve glucose homeostasis and the mechanisms responsible for the effect.Methods C57Bl/6N mice with diet-induced obesity were treated both acutely and for up to 42 days with a wide range of clenbuterol dosages and treatment durations. Glucose homeostasis was assessed by glucose tolerance test. We also measured in vivo glucose uptake in skeletal muscle, insulin sensitivity by insulin tolerance test, plasma insulin levels, hepatic lipids and glycogen.Results Consistent with previous findings, acute clenbuterol administration increased blood glucose and insulin levels. However, already after 4 days of treatment, beneficial effects of clenbuterol were manifested in glucose homeostasis (32% improvement of glucose tolerance after 4 days of treatment,p < 0.01) and these effects persisted up to 42 days of treatment. These favourable metabolic effects could be achieved with doses as low as 0.025 mg kg(-1) day(-1)(40 times lower than previously studied). Mechanistically, these effects were not due to increased insulin levels, but clenbuterol enhanced glucose uptake in skeletal muscle in vivo both acutely in lean mice (by 64%,p < 0.001) as well as during chronic treatment in diet-induced obese mice (by 74%,p < 0.001). Notably, prolonged treatment with low-dose clenbuterol improved whole-body insulin sensitivity (glucose disposal rate after insulin injection increased up to 1.38 +/- 0.31%/min in comparison with 0.15 +/- 0.36%/min in control mice,p < 0.05) and drastically reduced hepatic steatosis (by 40%,p < 0.01) and glycogen (by 23%,p < 0.05).Conclusions/interpretation Clenbuterol improved glucose tolerance after 4 days of treatment and these effects were maintained for up to 42 days. Effects were achieved with doses in a clinically relevant microgram range. Mechanistically, prolonged treatment with a low dose of clenbuterol improved glucose homeostasis in insulin resistant mice, most likely by stimulating glucose uptake in skeletal muscle and improving whole-body insulin sensitivity as well as by reducing hepatic lipids and glycogen. We conclude that selective beta(2)-adrenergic agonists might be an attractive potential treatment for type 2 diabetes. This remains to be confirmed in humans.
