Denna uråldriga och till synes naiva fråga pockar allt ihärdigare på ett svar i en tid där den sociala ojämlikheten ökar. Den som är det minsta intresserad av arbetsliv och hälsa och som läser de kapitel som nu följer kommer att bli inspirerad, kanske provocerad och troligen väldigt imponerad av svensk arbetslivsforskning. Flera av Sveriges mest framträdande arbetslivsforskare medverkar i boken.

Implementation of care managers for patients with depression: A cross-sectional study in Swedish primary care

- Objectives To perform an analysis of collaborative care with a care manager implementation in a primary healthcare setting. The study has a twofold aim: (1) to examine clinicians' and directors' perceptions of implementing collaborative care with a care manager for patients with depression at the primary care centre (PCC), and (2) to identify barriers and facilitators that influenced this implementation. Design A cross-sectional study was performed in 2016-2017 in parallel with a cluster-randomised controlled trial. Setting 36 PCCs in south-west Sweden. Participants PCCs' directors and clinicians. Outcome Data regarding the study's aims were collected by two web-based questionnaires (directors, clinicians). Descriptive statistics and qualitative content analysis were used for analysis. Results Among the 36 PCCs, 461 (59%) clinicians and 36 (100%) directors participated. Fifty-two per cent of clinicians could cooperate with the care manager without problems. Forty per cent regarded to their knowledge of the care manager assignment as insufficient. Around two-thirds perceived that collaborating with the care manager was part of their duty as PCC staff. Almost 90% of the PCCs' directors considered that the assignment of the care manager was clearly designed, around 70% considered the priority of the implementation to be high and around 90% were positive to the implementation. Facilitators consisted of support from colleagues and directors, cooperative skills and positive attitudes. Barriers were high workload, shortage of staff and extensive requirements and demands from healthcare management. Conclusions Our study confirms that the care manager puts collaborative care into practice. Facilitators and barriers of the implementation, such as time, information, soft values and attitudes, financial structure need to be considered when implementing care managers at PCCs. © © Author(s) (or their employer(s)) 2020. - Qualitative interviews are generally conducted in person. As the coronavirus pandemic (COVID-19) prevents in-person interviews, methodological studies which investigate the use of the telephone for persons with different illness experiences are needed. The aim was to explore experiences of the use of telephone during semi-structured research interviews, from the perspective of participants and researchers. Data were collected from mobile phone interviews with 32 individuals who had common mental disorders or multimorbidity which were analyzed thematically, as well as field notes reflecting researchers' experiences. The findings reveal several advantages of conducting interviews using mobile phones: flexibility, balanced anonymity and power relations, as well as a positive effect on self-disclosure and emotional display (leading to less emotional work and social responsibility). Challenges included the loss of human encounter, intense listening, and worries about technology, as well as sounds or disturbances in the environment. However, the positive aspects of not seeing each other were regarded as more important. In addition, we present some strategies before, during, and after conducting telephone interviews. Telephone interviews can be a valuable first option for data collection, allowing more individuals to be given a fair opportunity to share their experiences.

- Running with others is motivational but finding running partners is not always accessible due to constraints such as being in remote locations. We present a novel concept of augmenting remote runners' experience by mediating a running group using drone-projected visualisations. As an initial step, a team of five interaction designers apply a user-centred design approach to scope out possible visual prototypes that can mediate a running group. The design team specifically focused on visual prototypes through a series of workshops. We report on the impressions of the visual prototypes from six potential users and present future directions to this project.

Does employer involvement in primary health care enhance return to work for patients with stress-related mental disorders? a cluster randomized controlled trial

- BackgroundStress-related disorders have become a major challenge for society and are associated with rising levels of sick leave. The provision of support to facilitate the return to work (RTW) for this patient group is of great importance. The aim of the present study was to evaluate whether a new systematic procedure with collaboration between general practitioners (GPs), rehabilitation coordinators (RCs) and employers could reduce sick leave days for this patient group.MethodEmployed patients with stress-related diagnoses seeking care at primary health care centres (PHCCs) were included in either the intervention group (n = 54), following the systematic intervention procedure, or the control group (n = 58), receiving treatment as usual (TAU). The intervention included a) a training day for participant GPs and RCs, b) a standardised procedure for GPs and RCs to follow after training, c) the opportunity to receive clinical advice from specialist physicians in the research group. Outcome measures for RTW were sick leave days.ResultsThe median number of registered gross sick leave days was lower for the control group at six, 12 and 24 months after inclusion, but the difference was not statistically significant. The control group had significantly fewer net sick leave days at three months (p = 0.03) at six months (p = 0.00) and at 12-months follow-up (p = 0.01). At 24 months, this difference was no longer significant.ConclusionsThe PRIMA intervention, which applied a standardized procedure for employer involvement in the rehabilitation process for patients with stress-related disorders, actually increased time to RTW compared to TAU. However, at 24 months, the benefit of TAU could no longer be confirmed. The study was registered on 16/01/2017 (ClinicalTrials.gov, NCT03022760).

Co-localization of CD169+ macrophages and cancer cells in lymph node metastases of breast cancer patients is linked to improved prognosis and PDL1 expression

- Breast cancer is the most common form of cancer in women worldwide. Although the survival among breast cancer patients has improved, there is still a large group of patients with dismal prognosis. One of the most important prognostic factors for poor prognosis is lymph node metastasis. Increasing knowledge concerning the lymph nodes of breast cancer patients indicates that they are affected by the primary tumor. In this study we show that presence of CD169+ subcapsular sinus macrophages in contact with lymph node metastases in breast cancer patients, is related to better prognosis after adjuvant tamoxifen treatment, but only in patients with PDL1+ primary tumors. This is in contrast to the prognostic effect of CD169+ primary tumor-associated macrophages (TAMs). We further show that CD169+ macrophages were spatially associated with expression of PDL1 on nearby cells, both in primary tumors and metastatic lymph node, although PDL1 expression in metastatic lymph node as such did not have further prognostic impact. Our data suggest that CD169+ resident lymph node macrophages have a unique function in targeting immune responses against breast cancer and should be further investigated in detail.

- I kapitlet behandlas Annika Härenstams forskning med avseende på drivkrafter, metoder och perspektiv. Karaktäristiskt är att arbetslivet ses som en arena där betydelsen av klass, genus och etnicitet är under ständig omförhandling. Organisationer utgör arbetets närmaste kontext och den psykosociala arbetsmiljön är sammanlänkad med hur organisationer utformas och styrs. Organisationer är ett slags filter mellan skeenden på makro- och mikronivå. En uppgift för forskningen är att systematiskt fånga, mäta och korrelera fenomen på olika nivåer. Den arbetande människan finns i ett yrke, på en arbetsplats, som är en del av en avdelning på ett företag, som finns i en bransch. På konkreta arbetsplatser är det omöjligt att isolera enskilda variabler från varandra. Många faktorer samverkar och åstadkommer de fenomen som kan observeras i enkäter och register. För att fånga komplexiteten krävs holistiska ansatser. Kluster- och flernivåanalyser är metoder för detta genom att de inte fragmentiserar arbetssituationer, utan håller samman aspekter i den konkreta verklighet som utgör individers arbetsliv. I Annika Härenstams genusperspektiv är den grundläggande hypotesen att de betydelsefulla skillnaderna snarare finns mellan kvinno- och mansdominerade yrken, sektorer och branscher, än mellan kvinnor och män, vilket synsätt har klara implikationer för hur arbetsmiljöarbete kan och bör bedrivas.

- For several decades, modal shift within the freight transport sector has been promoted by policy makers as an important basis for achieving lower transport emission targets. Even though the literature on mode choice is well established, there is less consensus about the volume of freight with the potential to shift and, more importantly, the actual contribution of freight modal shift to achieving less climate and air pollution impacts. The climate contribution of freight modal shift can be increasingly questioned, as decarbonization is taking place at increasing rates within road freight transport. In this paper, the role of modal shift policies in realizing climate objectives is scrutinized by focusing on the case of Sweden, which serves to illustrate some general insights. We highlight how modal shift is often analyzed and discussed in isolation, even though it forms an important part of a policy mix in which it may contribute to achieving climate objectives. Treating modal shift as an objective in its own right may render less effective and cost-efficient policy instruments.

Freight modal shift in Sweden : means or objective?

- Modal shift within the freight transport sector has been promoted by policy makers as one of the means to achieving climate and environmental targets for several decades. The European Union has also had modal split objectives in place since 2011. In the Green Deal the ambition of shifting freight away from road transports is repeated. Even though the literature on mode choice is well established, there is less consensus about the volume of freight with potential to shift and, more importantly, the actual contribution of modal shift to achieving environmental and climate objectives. Based on a literature review to determine the potential of freight modal shifts within different segments in Sweden, this paper discuss the underlying assumptions explaining how freight modal shift is interpreted both as a political means to achieving overarching policy objectives and as a policy objective in itself.
