  • A Multidisciplinary Cross-Cultural Measurement of Functioning After Stroke : Rasch Analysis of the Brief ICF Core Set for Stroke

    • Purpose: To investigate the possibility of constructing a multiprofessional cross-cultural measure of functioning after stroke across categories of the International Classification of Functioning, Disability and Health (ICF). Method: Data on 757 stroke survivors from China, Germany, Italy, and Sweden, including ratings of 15 categories from the Brief ICF Core Set for stroke, were analyzed using the Rasch model. Unidimensionality, reliability, fit of the ICF categories to the model, ordering of response options of the ICF qualifier, and presence of differential item functioning (DIF) were studied. Results: Of the 15 ICF categories, response options for 7 categories were collapsed, 5 categories were deleted due to misfit, and 4 ICF categories showed DIF for country and were accordingly split into country-specific categories. The proposed final clinical measure consists of 20 ICF categories (6 categories were country-common) with an overall fit statistic of χ2180 = 184.87, P = .386, and a person separation index of r = 0.72, which indicates good reliability. Based on an individual's functioning after stroke, the ratings across the different ICF categories can be summed on an interval scale ranging from 0 to 100. Conclusion: A construction of a cross-cultural clinical measure after stroke based on ICF categories across body functions, structures, and activities and participation was possible. With this kind of clinical measure, stroke survivors' functional levels can be compared even across countries. Despite the promising results, further studies are necessary to develop definitive measures based on ICF categories.

    • Background. Limonene is a common fragrance terpene that, in its pure form, is not allergenic or is a very weak allergen. However, limonene autoxidizes on air exposure, and the oxidation products can cause contact allergy. Oxidized R-limonene has previously been patch tested in multicentre studies, giving 2–3% positive patch test reactions in consecutive patients. Objectives. To investigate whether oxidized R-limonene 3.0% in petrolatum, with a stable concentration of the main haptens, limonene hydroperoxides (Lim-OOHs), could be a useful tool for the detection of contact allergy in an international setting. Methods. Oxidized R-limonene 3.0% (Lim-OOHs 0.33%) pet. was tested in 2900 consecutive dermatitis patients in Denmark, the United Kingdom, Singapore, Spain, Sweden, and Australia. Results. Overall, 5.2% (range 2.3–12.1%) of the patients showed a positive patch test reaction to oxidized R-limonene. Doubtful reactions were found in 7.0% of the patients (range 0–24%). Few irritant reactions were seen. Conclusions. Oxidized R-limonene at 3.0% pet. with a specified content of Lim-OOHs 0.33% is a standardized and useful tool for the detection of contact allergy in dermatitis patients. Many patients showing positive patch test reactions to oxidized R-limonene would not be informed of their fragrance allergy if this specific test had not been performed.

  • Contact between mother, child and partner and attitudes towards breastfeeding in relation to mode of delivery

    • Objective: To investigate contact between mothers and their newborn child and study if there are differences between those who requested an elective caesarean section compared to women who had a vaginal birth and those who underwent an elective caesarean section due to obstetrical indication. The psychometric properties of a scale that measure the contact between mother and child were also investigated. Design: A prospective cohort study. Setting: Danderyd Hospital, Stockholm, Sweden. Sample: 510 primiparas from three groups: women undergoing caesarean section on maternal request (n = 96), women undergoing caesarean section on obstetrical indication (n = 116) and women planning a vaginal delivery (n = 198). The later were further divided into subgroups; women who underwent assisted vaginal delivery (n = 35) and women who had an emergency caesarean section (n = 65). Methods: The instrument used was the Alliance Scale. Main outcome measure: The contact between mother and child in relation to mode of delivery. Results: The contact with the child was rated as positive on all occasions: there were no significant differences between the groups. The relation to the partner was rated as positive at all occasions. Mothers with a vaginal delivery experienced breastfeeding less stressful than the mothers with a caesarean delivery. Three and nine months after delivery the mothers with a caesarean delivery on request reported more breastfeeding problems than mothers in the other groups. Mothers with a vaginal delivery rated less sadness at every occasion. Conclusion: Mode of delivery does not seem to affect how mothers experience their contact towards the newborn child. © 2009 Elsevier B.V. All rights reserved.

  • Origin of the Bathochromic Shift of Astaxanthin in Lobster Protein: 2D Electronic Spectroscopy Investigation of beta-Crustacyanin

    • We report on ultrafast spectroscopy study of beta-crustacyanin, the carotenoprotein responsible for the coloration of the lobster shell. beta-Crustacyanin is formed by two closely positioned astaxanthin molecules encapsulated in protein. The 2D electronic spectroscopy together with two-color pump-probe was applied to investigate the electronic structure, the excited-state dynamics, and the influence of the excitonic interaction between the two carotenoids in beta-crustacyanin. By using the similar to 20 Is laser pulses tuned to absorption bands of the S-0-S-2 and S-1-S-n transitions of carotenoids, we were able to trace full excitation relaxation dynamics, starting with S-2-S-1 relaxation on the similar to 30 fs time scale and finishing with the ground-state recovery of 3.2 ps. Superimposed on the relaxation dynamics in the 2D spectra, we observed long-lived beating signals at the characteristic frequencies of astaxanthin vibrational modes. We assign these oscillations to the ground-state vibrational wavepacket dynamics. All major features of the 2ll spectra, including amplitude and phase maps of the long-lived oscillations, were reproduced by employing the exciton-vibronic model. Consistent modeling of all optical properties of beta-crustacyanin (including absorption and circular dichroism spectra) points to the relatively weak coupling between the two astaxanthin molecules (similar to 250 cm(-1)). This implies that the excitonic coupling provides insignificant contribution to the bathochromic shift in beta-crustacyanin. We discuss the origin of the shift and propose that it is caused by two major effects: conformational changes of astaxanthin molecules (increase in effective conjugation length) together with increased charge-transfer character of the S-2 state. We put the bathochromic shift in the broad perspective of other "blue" carotenoids properties.
