    • A comprehensive high-spin decay scheme has been deduced for 57Co using the Gammasphere Germanium detector array coupled to the 4π charged-particle detector system Microball. Following the fusion-evaporation reaction of 28Si(36Ar,1α3p)57Co at 136 MeV beam energy about 90 excited states have been observed in 57Co, which are connected by some 230 γ-ray transitions. The proposed level scheme reaches 16 MeV excitation energy at a spin of 17ħ–18ħ. With its complexity the level scheme of 57Co appears to be “complete” up to 1 or 2 MeV above the yrast line. This aspect is further investigated through a comparison of the data with spherical shell-model calculations.

    • Gamma-ray lines from Sn-103 have been identified for the first time using EUROBALL and ancillary detectors. The level scheme of Sn-103 has been established by means of particle-gated gamma gamma coincidences. The energy spacing between the g(7/2) and d(5/2) neutron single-particle orbitals is determined from the excited states in Sn-103.

    • Excited states of nuclei in the vicinity of Sn-100 have been studied using: the EUROBALL detector array. Gamma-ray lines from Sn-103 have been identified for the first time, and a level scheme of low-lying excited states of Sn-103 has been established. New constraints on energies of single particle orbitals with respect to the Sn-100 core are obtained.

  • Prompt Alpha Decay of a Well-deformed Band in 58Ni

    • Two excited well-deformed bands have been observed in the semi-magic nucleus Ni-58. One of the bands was observed to partially decay by emission of a prompt discrete alpha particle that feeds the 2949 keV 6(+) spherical yrast state in the daughter nucleus Fe-54. This constitutes the first observation of prompt alpha emission from states lying in the deformed secondary minimum of the nuclear potential. gamma -ray linking transitions via several parallel paths establish the spin. parity, and excitation energy of this deformed band in Ni-58.

    • BACKGROUND: While there are data to support the use of comprehensive non-pharmacological intervention programs in patients with heart failure (HF), other studies have not confirmed these positive findings. Substantial differences in the type and intensity of disease management programs make it impossible to draw definitive conclusions about the effectiveness, optimal timing and frequency of interventions. AIMS: 1. To determine the effectiveness of two interventions (basic support vs. intensive support) compared to 'care as usual' in HF patients, on time to first major event (HF readmission or death), quality of life and costs. 2. To investigate the role of underlying mechanisms (knowledge, beliefs, self-care behaviour, compliance) on the effectiveness of the two interventions. METHODS: This is a randomised controlled trial in which 1050 patients with heart failure will be randomised into three treatment arms: care as usual, basic education and support or intensive education and support. Outcomes of this study are; time to first major event (HF hospitalisation or death), quality of life (Minnesota Living with HF Questionnaire, RAND36 and Ladder of Life) and costs. Data will be collected during initial admission and then 1, 6, 12, and 18 months after discharge. In addition, data on knowledge, beliefs, self-care behaviour and compliance will be collected. RESULTS: The study started in January 2002 and results are expected at the end of 2005. CONCLUSIONS: This study will help health care providers in future to make rational and informed choices about which components of a HF management program should be expanded and which components can possibly be deleted.

  • High-angular-momentum structures in Zn-64

    • High-angular-momentum states in Zn-64 were populated in the Ca-40(Si-28,4p) reaction at a beam energy of 122 MeV. Evaporated, light, charged particles were identified by the Microball, while gamma rays were detected using the Gammasphere array. The main focus of this paper is on two strongly coupled, collective bands. The yrast band, which was previously known, has been linked to lower-lying states establishing the excitation energies and angular momenta of in-band states for the first time. The newly identified excited band decays to the yrast band but firm angular-momentum assignments could not be made. In order to interpret these structures cranked-Nilsson-Strutinsky calculations have been performed. The calculations have been extended to account for the distribution of nucleons within a configuration. The yrast collective band is interpreted as based on the pi(f(7/2))(-1)(p(3/2)f(5/2))(2)(g(9/2))(1)nu(p(3/2)f(5/2))(4)(g(9/2))(2) configuration. There are several possible interpretations of the second band but it is difficult to distinguish between the different possibilities.

    • PURPOSE: The feasibility and utility of assessing quality of life (QoL) and disease-related symptoms in patients with advanced cancer have been evaluated in two Phase I clinical trials of p.o. administered ZD1839 ('Iressa'), an epidermal growth factor receptor tyrosine kinase inhibitor, in patients with advanced cancer. EXPERIMENTAL DESIGN: Functional Assessment of Cancer Therapy (FACT) questionnaires, including disease-specific subscales for lung, head and neck, colorectal, prostate, and ovarian cancer, were completed by patients in two open-label, Phase I, escalating multiple-dose safety and tolerability trials. RESULTS: In 157 patients, 92% of whom had received prior therapy, compliance in returning FACT questionnaires was 87% (European/Australian trial) and 57% (United States trial). This did not appear to be influenced by dose level or tumor type. For patients with colorectal, prostate, or ovarian cancer, median QoL [FACT and Trial Outcome Index (TOI)] scores deteriorated over time. In contrast, for patients with non-small cell lung cancer (NSCLC) or head and neck cancer, median FACT and TOI scores did not deteriorate significantly, and in the United States trial, head and neck cancer scores improved significantly over time. In patients with NSCLC, symptom-related scores measured by the Lung Cancer Subscale of FACT-L appeared sensitive to clinical change. CONCLUSIONS: QoL (FACT-L) questionnaires were used successfully in the Phase I clinical trials of ZD1839. They appeared to be a sensitive tool to monitor clinical changes for the five tumor types in these trials and showed that ZD1839 has the potential to improve patients' QoL.
