  • How does a physical activity programme in elementary school affect fracture risk? : A prospective controlled intervention study in Malmo, Sweden

    • Objectives: Recent evidence from the 7-year followup of the Pediatric Osteoporosis Prevention (POP) study indicates an inverse correlation between years of participation in a physical activity (PA) intervention and fracture risk in children. However, we could not see a statistically significant reduction in fracture risk, which urged for an extension of the intervention. Setting: The study was conducted in 4 neighbouring elementary schools, where 1 school functioned as intervention school. Participants: We included all children who began first grade in these 4 schools between 1998 and 2012. This resulted in 1339 children in the intervention group and 2195 children in the control group, all aged 6-8 years at the state of the study. Intervention: We launched an 8-year intervention programme with 40 min of moderate PA per school day, while the controls continued with the Swedish national standard of 60 min of PA per week. Primary outcome measure: We used the regional radiographic archive to register objectively verified fractures and we estimated annual fracture incidences and incidence rate ratios (IRRs). Results: During the first year after initiation of the intervention, the fracture IRR was 1.65 (1.05 to 2.08) (mean 95% CI). For each year of the study, the fracture incidence rate in the control group compared with the intervention group increased by 15.7% (5.6% to 26.8%) (mean 95% CI). After 8 years, the IRR of fractures was 52% lower in the intervention group than in the control group (IRR 0.48 (0.25 to 0.91) (mean 95% CI))]. Conclusions: Introduction of the school-based intervention programme is associated with a higher fracture risk in the intervention group during the first year followed by a gradual reduction, so that during the eighth year, the fracture risk was lower in the intervention group.

  • A seven-year physical activity intervention for children increased gains in bone mass and muscle strength

    • Aim: This study evaluated the musculoskeletal effects of increased physical activity on children, starting at six to nine years of age. Methods: In one school we increased the physical education of 72 girls and 100 boys to 200 minutes per week over seven years. In three other schools, 45 girls and 47 boys continued to receive 60 minutes per week. We measured areal bone mineral density (aBMD) with dual energy X-ray absorptiometry and muscle strength with computerised dynamometer at baseline and after seven years and tibial cortical thickness with peripheral quantitative computed tomography after seven years. Results: Girls in the intervention group gained 0.04 g/cm2 (0.01-0.08) more total spine aBMD (p <0.05) and 6.2Nm (1.6, 10.7) more knee flexion strength (p <0.01) than control group girls and had a 0.1 mm (0.0, 0.3) higher tibial cortical thickness at follow-up (p <0.05). Boys in the intervention group gained 7.3Nm (0.4, 14.2) more knee extension strength (p <0.05) and 7.4Nm (2.3, 12.4) more knee flexion strength (p <0.01) than the control group boys, but their aBMD was no higher than the control group. Conclusion: A seven-year, population-based moderately intense exercise intervention enhanced gains in spine bone mass in girls and knee muscle strength in both genders.

    • One aim of the Bunkeflo project is to evaluate if daily school physical activity (PA) is associated with higher final school grades and improved eligibility for upper secondary school. One elementary school extended PA throughout elementary school to 200 minutes/week while control schools continued with in mean 60 minutes/week. We then registered final school grades and eligibility for upper secondary school in children who began first grade in the intervention school during the years before the intervention (1994-1997) and in the children who started 1998-2003 (that is, who had daily PA during the 9 compulsory school years). We also registered final school grades and eligibility for upper secondary school in all Swedish children who started school 1998-2003. We found that boys in the intervention group had higher eligibility rates and higher grade scores than boys who finished the same school before the intervention was implemented as well as higher in all Swedish boys. These differences in were not seen in girls. Daily school PA through elementary school is associated with higher final grade scores and higher eligibility rates for upper secondary school in boys.

    • In the Bunkeflo project, one elementary school increased duration of school physical activity (PA) to 200 minutes/week while 3 control schools continued with 60 minutes/week throughout the nine elementary school years. We then registered fractures in 3534 children, and evaluated the duration of PA, bone mass and muscle strength in a subsample (n=140) during the intervention and 3 years after. The PA intervention was associated with higher duration of PA both during and 3 years after the intervention. With each year of intervention, the fracture incidence rate ratio (IRR) declined in the intervention children so that it was 0.48 (95% CI 0.25, 0.91) the eighth year of intervention. These findings were accompanied by beneficial gain in musculoskeletal traits in both intervention girls and boys (p<0.05). Nine years of daily school-based PA is associated with a progressive reduction in fracture risk, accompanied by beneficial musculoskeletal gains and a more physically active lifestyle.

    • Understanding leukemia heterogeneity is critical for the development of curative treatments as the failure to eliminate therapy-persistent leukemic stem cells (LSCs) may result in disease relapse. Here we have combined high-throughput immunopheno-typic screens with large-scale single-cell gene expression analysis to define the heterogeneity within the LSC population in chronic phase chronic myeloid leukemia (CML) patients at diagnosis and following conventional tyrosine kinase inhibitor (TKI) treatment. Our results reveal substantial heterogeneity within the putative LSC population in CML at diagnosis and demonstrate differences in response to subsequent TKI treatment between distinct subpopulations. Importantly, LSC subpopulations with myeloid and proliferative molecular signatures are proportionally reduced at a higher extent in response to TKI therapy compared with subfractions displaying primitive and quiescent signatures. Additionally, cell surface expression of the CML stem cell markers CD25, CD26, and IL1RAP is high in all subpopulations at diagnosis but downregulated and unevenly distributed across subpopulations in response to TKI treatment. The most TKI-insensitive cells of the LSC compartment can be captured within the CD45RA(-) fraction and further defined as positive for CD26 in combination with an aberrant lack of cKIT expression. Together, our results expose a considerable heterogeneity of the CML stem cell population and propose a Lin(-) CD34(+) CD38(-/low) CD45RA(-) cKIT(-) CD26(+) population as a potential therapeutic target for improved therapy response.

    • Here we describe the SweGen data set, a comprehensive map of genetic variation in the Swedish population. These data represent a basic resource for clinical genetics laboratories as well as for sequencing-based association studies by providing information on genetic variant frequencies in a cohort that is well matched to national patient cohorts. To select samples for this study, we first examined the genetic structure of the Swedish population using high-density SNP-array data from a nation-wide cohort of over 10 000 Swedish-born individuals included in the Swedish Twin Registry. A total of 1000 individuals, reflecting a cross-section of the population and capturing the main genetic structure, were selected for whole-genome sequencing. Analysis pipelines were developed for automated alignment, variant calling and quality control of the sequencing data. This resulted in a genome-wide collection of aggregated variant frequencies in the Swedish population that we have made available to the scientific community through the website https://swefreq.nbis.se. A total of 29.2 million single-nucleotide variants and 3.8 million indels were detected in the 1000 samples, with 9.9 million of these variants not present in current databases. Each sample contributed with an average of 7199 individual-specific variants. In addition, an average of 8645 larger structural variants (SVs) were detected per individual, and we demonstrate that the population frequencies of these SVs can be used for efficient filtering analyses. Finally, our results show that the genetic diversity within Sweden is substantial compared with the diversity among continental European populations, underscoring the relevance of establishing a local reference data set.

    • Styrketräning kan utövas som en egen idrott i form av tyngdlyftning och styrkelyft. Styrketräning bedrivs också i prestationshöjande syfte som ett viktigt komplement till andra träningsformer eller idrotter. Många styrketränar för att orka mer och klara vardag och fritid bättre. Sist men inte minst spelar styrketräning en viktig roll i att förebygga och behandla skador och sjukdomar.Styrketräning är inkluderande och kan utformas så att vem som helst, oberoende av ålder och fysisk status kan utföra den. Styrketräning är också skonsam och anpassningsbar och därför alltid ett alternativ när andra träningsformer blivit för krävande och inte längre fungerar att utföra. Styrketräning är dessutom en effektiv och utmanande träningsform, på kort tid kan man pressas till total utmattning.Det är alltså inte så konstigt att styrketräning är en av de vanligaste träningsformerna och idag har mer än två miljoner utövare i Sverige.     • The technology and techniques for bio-sensors are rapidly evolving. Accordingly, there is significant business interest to identify upcoming technologies and new targets for the near future. Text information from internet reflects much of the recent information and public interests that help to understand the trend of a certain field. Thus, we utilize Dirichlet process topic modeling on different media sources containing short text (e.g., blogs, news) which is able to self-adapt the learned topic space to the data. We share the observations from the domain experts on the results derived from topic modeling on wearable biosensors from multiple media sources over more than eight years. We analyze the topics on wearable devices, forecast and market analysis, and bio-sensing techniques found from our method.

    • Större delen av de läkemedel och andra mikroföroreningar som används i samhället hamnar i våra avloppsreningsverk. Där bryts en del ner effektivt, men många föroreningar passerar i stort sett opåverkade genom verken. Vid låg utspädning i närrecipienten kan vissa substanser nå så höga halter att man inte kan utesluta effekter på vattenlevande organismer. Kompletterande rening av avloppsvattnet vid avloppsreningsverk kan därför vare ett alternativ för att avlägsna dessa mikroföroreningar. De mest lovande teknikerna för att minska utsläppen av ett brett spektrum av läkemedelssubstanser och andra svårnedbrytbara substanser är behandling med starka oxidationsmedel, främst ozon, adsorption till aktivt kol och en utökad biologisk nedbrytning. Denna rapport redovisar resultat från fortsatta försök med avskiljning av särskilt läkemedelsrester i avloppsvatten inom projektet SystemLäk, och kompletterar tidigare genomförda försök inom samma projekt. I första hand har 24 olika läkemedel, som ofta förekommer i avloppsvatten och som representerar olika läkemedelsgrupper, analyserats. Tekniker som studerats innefattar: § Granulerat aktivt kol som filtermaterial med biologisk aktivitet - BAF(GAK) § Rening med biokol framställt från bioslam/organiskt substrat - BAK § Användning av pulveriserat aktivt kol i MembranBioReaktor (MBR) -PAK § Oxidation med ozon § Oxidation med klordioxid Genomförda tester, i kombination med tidigare försök, visar tydligt på att de flesta studerade teknikerna ger ökad avskiljning av de flesta studerade ämnena. I många fall får man en mycket bra effekt med en rimlig insats. Testerna har även lyft fram begränsningar hos de studerade teknikerna: BAF(GAK) - Granulerat aktivt kol som biofilter, BAK – Biokol framställt från bioslam, PAK-MBR – Pulveriserat aktivt kol (PAK) i membranbioreaktor, Resurseffektiv ozonering, Klordioxid (ClO2) För mer detaljer kring de ämnen som studerats, analysmetoder för dessa, vilken effekt dessa har i recipienten samt till vilken nivå de bör reduceras hänvisas till den första delrapporten i SystemLäk (Baresel m fl., 2015a). För en samlad bedömning av olika reningssystem inklusive miljöpåverkan och kostnader samt vilka av tekniker som rekommenderas hänvisas till slutrapporten i SystemLäk (Baresel m fl., 2017).

    • Population growth, increasing living standards, but also environmental hazards with global climate change as the most significant are all contributing to an increasing water stress in many parts of the world. While access to fresh water for drinking water is getting more costly due to environmental pollution, uses of drinking water conflicts with water needs for agricultural and industrial use, which are in need of substantial water quantities. The use of reclaimed wastewater for non-potable purposes provides a solution for this. This is not new and has in fact been applied in many regions as the main water management approach. As water scarcity becomes more severe, also the need for more sustainable and holistic approaches to deal with our limited fresh water resources becomes more and more obvious. The traditional one-way water handling approach, with end-of-pipe treatment releasing “clean” effluent water to nature, has to be converted into a society-internal water reuse scheme where different water qualities and water uses are considered as an integral part of the water cycle. The present report presents activities and results from an international project that aimed at developing and optimizing water treatment processes and systems for sustainable reuse of treated wastewater. The starting point is to combine the sequential batch treatment (SBR, sequencing batch reactors) with different conventional and emerging secondary and tertiary treatment techniques in various combinations, optimized from an overall sustainability perspective. Evaluation and optimization is achieved using life cycle assessment and life cycle cost assessment and their combination. Den här rapporten finns endast på engelska.
