    • Medical physics education and training requires the use of extensive imaging material and specific explanations. These requirements provide an excellent background for application of e-Learning. The EU projects Consortia EMERALD and EMIT developed five volumes of such materials, now used in 65 countries. EMERALD developed e-Learning materials in three areas of medical physics (X-ray diagnostic radiology, nuclear medicine and radiotherapy). EMIT developed e-Learning materials in two further areas: ultrasound and magnetic resonance imaging. This paper describes the development of these e-Learning materials (consisting of e-books and educational image databases). The e-books include tasks helping studying of various equipment and methods. The text of these PDF e-books is hyperlinked with respective images. The e-books are used through the readers' own Internet browser. Each Image Database (IDB) includes a browser, which displays hundreds of images of equipment, block diagrams and graphs, image quality examples, artefacts, etc. Both the e-books and IDB are engraved on five separate CD-ROMs. Demo of these materials can be taken from www.emerald2.net.

  • EMERALD & EMIT – worldwide computer aided education and training packages in medical physics

    • This paper describes the development of two web based education and training packages EMERALD and EMIT designed to meet the training needs of professional medical physicists. The programme has been developed over a number of years by collaboration between hospitals and universities across Europe. The programme concentrates on assisting competence development in five initial areas; diagnostic radiology, nuclear medicine, magnetic resonance tomography, ultrasound and radiotherapy. Each of the topic areas includes around 50 training tasks in 5 hypertext workbooks, which are supplemented by an image database relevant to each topic. The training materials have been extensively refereed during their development and are now in use in 65 countries across the globe. Initial evaluation has shown that the material enhances the training experience and produces a more consistent output.

  • Assessment of Transport in NCSX

    • We explore whether the energy confinement and planned heating in the National Compact Stellarator Experiment (NCSX) are sufficient to test magnetohydrodynamic (MHD) stability limits, and whether the configuration is sufficiently quasi-axisymmetric to reduce the neoclassical ripple transport to low levels, thereby allowing tokamak-like transport. A zero-dimensional model with fixed profile shapes is related to global energy confinement scalings for stellarators and tokamaks, neoclassical transport properties are assessed with the DKES, NEO, and NCLASS codes, and a power balance code is used to predict temperature profiles. Reaching the NCSX goal of = 4% at low collisionality will require HISS-95 = 3, which is higher than the best achieved in present stellarators. However, this level of confinement is actually ~10% lower than that predicted by the ITER-97P tokamak L-mode scaling. By operating near the stellarator density limit, the required HISS-95 is reduced by 35%. The high degree of quasi-axisymmetry of the configuration and the self-consistent "ambipolar" electric field reduce the neoclassical ripple transport to a small fraction of the neoclassical axisymmetric transport. A combination of neoclassical and anomalous transport models produces pressure profile shapes that are within the range of those used to study the MHD stability of NCSX. We find that = 4% plasmas are "neoclassically accessible" and are compatible with large levels of anomalous transport in the plasma periphery.

  • An integrated analysis of five double-blind, randomized controlled trials evaluating the safety and efficacy of a hyaluronan product for intra-articular injection in osteoarthritis of the kneel

    • Objective: Five double-blind, randomized, saline-controlled trials (RCTs) were included in the United States marketing application for an intra-articular hyaluronan (IA-HA) product for the treatment of osteoarthritis (OA) of the knee. We report an integrated analysis of the primary Case Report Form (CRF) data from these trials. Method. Trials were similar in design, patient population and outcome measures - all included the Lequesne Algofunctional Index (LI), a validated composite index of pain and function, evaluating treatment over 3 months. Individual patient data were pooled; a repeated measures analysis of covariance was performed in the intent-to-treat (ITT) population. Analyses utilized both fixed and random effects models. Safety data from the five RCTs were summarized. Results: A total of 1155 patients with radiologically confirmed knee OA were enrolled: 619 received three or five IA-HA injections; 536 received. "placebo" saline injections. In the active and control groups, mean ages were 61.8 and 61.4 years; 62.4% and 58.8% were women; baseline total Lequesne scores 11.03 and 11.30, respectively. Integrated analysis of the pooled data set found a statistically significant reduction (P < 0.001) in total Lequesne score with hyaluronan (HA) (-2.68) vs placebo (-2.00); estimated difference -0.68 (95% CI: -0.56 to -0.79), effect size 0.20. Additional modeling approaches confirmed robustness of the analyses. Conclusions: This integrated analysis demonstrates that multiple design factors influence the results of RCTs assessing efficacy of intra-articular (IA) therapies, and that integrated analyses based on primary data differ from meta-analyses using transformed data.   • Toroidal and poloidal momentum transport studies in tokamaks

    • The present status of understanding of toroidal and poloidal momentum transport in tokamaks is presented in this paper. Similar energy confinement and momentum confinement times, i.e. tau(E)/tau(phi)approximate to 1 have been reported on several tokamaks. It is more important though, to study the local transport both in the core and edge plasma separately as, for example, in the core plasma, a large scatter in the ratio of the local effective momentum diffusivity to the ion heat diffusivity chi(phi eff)/chi(i.eff) among different tokamaks can be found. For example, the value of effective Prandtl number is typically around chi(phi eff)/chi(i.eff)approximate to 0.2 on JET while still tau(E)/tau(phi)approximate to 1 holds. Perturbative NBI modulation experiments on JET have shown, however, that a Prandtl number chi(phi)/chi(i) of around 1 is valid if there is an additional, significant inward momentum pinch which is required to explain the amplitude and phase behaviour of the momentum perturbation. The experimental results, i.e. the high Prandtl number and pinch, are in good qualitative and to some extent also in quantitative agreement with linear gyro-kinetic simulations. In contrast to the toroidal momentum transport which is clearly anomalous, the poloidal velocity is usually believed to be neo-classical. However, experimental measurements on JET show that the carbon poloidal velocity can be an order of magnitude above the predicted value by the neo-classical theory within the ITB. These large measured poloidal velocities, employed for example in transport simulations, significantly affect the calculated radial electric field and therefore the E x B flow shear and hence modify and can significantly improve the simulation predictions. Several fluid turbulence codes have been used to identify the mechanism driving the poloidal velocity to such high values. CUTIE and TRB turbulence codes and also the Weiland model predict the existence of an anomalous poloidal velocity, peaking in the vicinity of the ITB and driven dominantly by the flow due to the Reynold's stress. It is worth noting that these codes and models treat the equilibrium in a simplified way and this affects the geodesic curvature effects and geodesic acoustic modes. The neo-classical equilibrium is calculated more accurately in the GEM code and the simulations suggest that the spin-up of poloidal velocity is a consequence of the plasma profiles steepening when the ITB grows, following in particular the growth of the toroidal velocity within the ITB.

  • Mining ventilation control : a new industrial case for wireless automation

    • This paper describes a new industrial case on wireless automation, for a large scale system with high environmental impact: the mining ventilation. We do not pretend to solve the global control problem but rather propose a novel approach based on distributed sensing/actuation capabilities, which can be allowed by the installation of an heterogeneous network in the underground process. Several key issues, such as the centralized vs. decentralized control, the macroscopic physical models for the main components and the network structure, are introduced to motivate future research along this line. We particularly highlight new grounds for the development of global control strategies that would allow for the energy consumption optimization of this large-scale system.
