  • Designated officials’ approaches to judging serious incidents: an analysis of incident reports in care for older people, disability services and family services in Sweden

    • There are various systems for reporting incidents that have harmed service users depending on the country and legislation. In Sweden, the duty of staff to report mistreatment covers three areas: care for older people, disability services, and individual and family care. This study examines Designated Officials’ judgements concerning reported incidents of serious mistreatment in social services in Sweden. A qualitative analysis of incident reports covering all three areas shows that investigators used five approaches to judge the seriousness of an incident. The ‘individual approach’ is seen as a blaming strategy of individual staff, and the ‘quality development approach’ highlighted systematic failures, with little focus on protecting individual service users. The ‘medical approach’, predominantly used in care for older people, measured physical injuries but neglected service users’ own experiences. The ‘empathic approach’ incorporated service users’ perspectives as interpreted by staff, often resulting in service users being stereotyped as victims. The ‘legal and human rights approach’ was least common in the analysis, despite echoing the core values of social work that are highly important for social workers. We conclude that the assessment process for reports needs to be reconsidered and the meaning of harm in the context of social work re-evaluated.

    • While previous drug treatment research has focused on the importance of supportive social relationships for recovery from alcohol and drugs, less is known about how this theme relates to compulsory drug treatment. This study analyzes how staff at four compulsory treatment institutions for adult drug users in Sweden rationalize the importance of maintaining contact with concerned significant others (CSOs) during the client’s treatment. Four focus groups (22 participants) were carried out and analyzed thematically. This study shows that staff perceives the client’s initial isolation as a necessity, primarily making CSOs a problem or distraction in relation to the recovery process. Moreover, staff position CSOs in two broad categories, as either ‘problematic’ or ‘resourceful’ in relation to a client’s recovery. The two categories are used to justify individual regulation between clients and their CSOs, which varies between strictness and leniency. In addition, staff position themselves as guides, finding it necessary to help clients regulate problematic CSOs themselves and strengthen the link to those who are seen as resources. These two forms of regulatory work – setting rules and providing guidance – are analyzed in terms of disciplinary and pastoral power.

    • Mandatory reporting of elder abuse aims to detect and prevent mistreatment and improve services. Service users and their relatives can raise concerns, but only staff can file mandatory reports. This article examines how the concerns of service users and relatives were managed by designated officials in reports of mistreatments in care for older adults in Sweden. We lean on sociological theories of “interpersonal trouble” and organizational “disputes domains.” The thematic analysis is based on 28 incident reports initiated by service users or their relatives. The analysis shows that the reports were managed in one of three ways: asymmetrically, by 1) dismissing or 2) supporting the complainant’s position, or symmetrically, by 3) treating complainants’ accounts as credible but minimizing their seriousness. There were differences between reports initiated by service users and relatives. Dismissing concerns about abuse, mainly those made by relatives, risks support for service users failing.

  • Animerad kemi : Elever i grundskolans tidiga år förklarar kemiska samband

    • Kemi har under stora delar av 1900-talet varit ett ämne som elever tidigast mötte i årskurs 5. Då behandlades ofta fenomen som smältning, stelning, kokning och avdunstning. Dessutom studerades egenskaper hos luft och vatten. Att elever innan de börjar högstadiet skulle resonera kring dessa fenomen i termer av vad som sker på submikronivå, med hjälp av antaganden om enkla partikelmodeller, har aldrig varit ett uttalat mål för undervisningen i kemi förrän introduktionen av Lgr 11. I den här studien har en grupp bestående av två lärare, en rektor och fyra forskare utformat och genomfört undervisning om egenskaper hos luft samt fenomenen smältning och stelning med elever i årskurs 3 och 4. Eleverna har inte bara beskrivit det som skett med egna ord, utan också lärt sig att beskriva det som sker på submikronivå i ord och bild. För att kunna göra detta har de, tillsammans med läraren, utvecklat enkla partikelmodeller. Projektet har kallats "Animerad kemi” och är ett forskningsprojekt finansierat av Skolforskningsinstitutet.

  • Locked Up and Locked Out: Client Perspectives on Personal Relationships While in Compulsory Drug Treatment

    • This qualitative study explores clients’ perspectives on their personal relationships while in compulsory drug treatment. Interviews with 31 participants (14 female and 17 male) were conducted at four compulsory treatment institutions for adults who use drugs in Sweden. Taken together, our study reveals that clients in general had to struggle to maintain social relationships due to strict restrictions on their interpersonal contact and communication. Feelings of isolation and anxiety characterized much of their relationships during the treatment period, with emotional withdrawal commonly described as a way to cope. Moreover, some participants expressed shame and guilt over the pain and suffering they had subjected their family members to through their drug use, feelings that put additional strain on the contact. The emotionally and socially significant relationships described by our interviewees provide links to other personal roles and settings than those prescribed by the institution. At the studied institutions, however, little attention was given to this relational dimension of the clients’ situation. Based on the results of the present study, possibilities for improvement of compulsory drug treatment are discussed.

  • Att diagnostiseras med graviditetsdiabetes - påverkar det upplevelsen av graviditet, förlossning och amning?

    • Graviditetsdiabetes uppkommer på grund av en ökad insulinresistens hos den gravida kvinnan. Sedan 2013 förespråkar WHO striktare diagnoskriterier för graviditetsdiabetes vilka även Socialstyrelsen rekommenderar sedan 2015. Trots rekommendationen används olika diagnoskriterier runtom i Sverige och förekomsten av graviditetsdiabetes varierar stort, allt från 1–22% (nationellt 6,2%). Att få diagnosen graviditetsdiabetes kan upplevas stigmatiserande och medföra ett ökat fokus på risker, övervakning och medicinska interventioner. Hur detta påverkar kvinnans upplevelse av graviditet, förlossning och amning är inte tillräckligt studerat. Syftet med studien är att undersöka om diagnosen graviditetsdiabetes påverkar kvinnans tillit till egen förmåga och känsla av sammanhang, hennes förlossningsupplevelse och tillit till egen förmåga att amma. Studien är en del av multicenterstudien CDC4G (ISRCTN 41918550) och förlossningsklinikerna i Örebro, Lund, Uppsala, Göteborg och Falun deltar. De kvinnor som enligt lokala riktlinjer för screening planeras för oralt glukostoleranstest (OGTT), kan tillfrågas om medverkan. Studien har etiktillstånd (2019–02080, 2020-00122). Studien pågår med insamling av enkäter som mäter känsla av sammanhang (SOC-13), tilltro till egen förmåga (GSE), förlossningsupplevelse (CEQ2) och tillit till egen förmåga att amma (BSE). Mätningarna sker före OGTT samt cirka åtta veckor postpartum. Jämförande analyser kommer att göras mellan svarande kvinnor som fått diagnosen (studiegrupp) med de som inte fått diagnosen (kontrollgrupp). Då diagnoskriterierna varierar på studieorterna kommer en andra kontrollgrupp bestå av kvinnor som inte fick diagnosen men vars resultat från OGTT överskred de rekommenderade diagnoskriterierna från Socialstyrelsen. Resultatet kommer att ge en ökad förståelse för om diagnosen graviditetsdiabetes påverkar kvinnans känsla av sammanhang och tilltro till egen förmåga under graviditet och amning samt fylla kunskapsluckan om diagnosen påverkar kvinnans förlossningsupplevelse. Slutsatserna kan bli ett viktigt bidrag i diskussionen om när och hur diagnosen graviditetsdiabetes ska användas under graviditet.
