  • Danish Health Professional Students' Attitudes Toward Addressing Sexual Health : A Cross-Sectional Survey

    • Introduction: Danish health professional (HP) students' attitudes toward addressing sexual health are unknown. Aim: To investigate Danish HP students' attitudes toward addressing sexual health in their future professions, and to assess differences in perceived competences and preparedness between professional programs. Methods: A Danish national survey of nursing, occupational therapy, and physiotherapy students was conducted. Totally, 1,212 students were invited to respond to an online questionnaire “The Students' Attitudes toward Addressing Sexual Health. Main Outcome Measure: The main outcome measures investigated were Danish HP students' attitudes toward addressing sexual health in their future professions, and differences in perceived competences and preparedness depending on the professional program. Results: A total of 584 students (48%; nursing 44%, occupational therapy 70%, physiotherapy 43%) responded. Mean total score ranged between 63.7 and 66.3 (±8.3–8.8) classifying students in the low-end of the class: “comfortable and prepared in some situations". No clinically relevant differences were determined between the professional programs with respect to perceived competences and preparedness to address sexual health. Conclusion: In the field of addressing sexual health, most Danish HP students reported positive attitudes and a need for basic knowledge, competences, communication training, and education.

  • Activity repertoires and time use in people living with chronic obstructive pulmonary disease

    • Background:Understanding the dynamics and aspects of how activity choices impact health and well-being in people living with chronic obstructive pulmonary disease (COPD) is important to inform rehabilitation. Aim:To describe, firstly, how much time people living with COPD spend on work, daily living tasks, recreation and rest; secondly, how this population perceived competence, importance and enjoyment related to these activities; thirdly, if differences in such perceptions and time use were associated with the living situation and COPD severity. Material and methods:This cross-sectional study involved 76 participants (+45 years, COPD, living in ordinary homes), who completed the Occupational Questionnaire (OQ). Descriptive statistics and group comparisons were performed. Results:Most of the participants' time were spent on daily living activities and recreational activities. Participants spent approx. 80% of their recorded time in OQ on activities they valued, enjoyed and in which they felt competent. Participants living alone scored significantly lower on enjoyment in restful activities than those living in couples (p < 0.05). No statistically significant difference in perceived competence, importance or enjoyment was found in relation to COPD severity. Conclusions and significance:Findings underscore the importance of targeting overall daily activity repertoires including compositions of activity types, time use and perceived competence, importance and enjoyment.

    • Two separate apps that address the increasingly important issue of accessible housing for senior citizens have been developed in different project settings. One of the apps was developed to facilitate the process for professional raters to assess housing accessibility in the context of individual housing adaptations. The other app was developed for senior citizens to raise their awareness of possible accessibility problems in their current dwelling and in other apartments within the available housing stock. Both apps were developed with a high degree of active user involvement in processes utilizing multiple state of the art methods. The results are two well accepted prototype apps perceived as user-friendly and appropriate for the intended user groups. By combining these two apps, our ambition is for the professional raters to benefit by gaining knowledge of their clients' perceived needs and desires, and for senior citizens to benefit by getting access to a database of professionally rated dwellings. The ultimate goal is the generation of sound knowledge reflecting the needs and desires of senior citizens and professional requirements regarding accessible housing as a means to inform and influence housing provision policies.

    • For decades, accessibility to the physical housing environment for people with functional limitations has been of interest politically, professionally and for the users Guidelines and norms on accessible housing design have gradually been developed, however the built environment shows serious deficits when it comes to accessibility This study addresses development of a content valid cross Nordic version of the Housing Enabler and investigation of inter-rater reliability when used in occupational therapy practice The instrument was translated from the original Swedish version and adapted according to accessibility norms and guidelines for housing design in Sweden, Denmark, Iceland and Finland This iterative process involved occupational therapists, architects, building engineers and professional translators, resulting in the Nordic Housing Enabler For reliability testing, the sample strategy and data collection procedures were the same in all countries In total, twenty voluntary occupational therapists collected data from 106 cases by means of the Nordic Housing Enabler Inter-rater reliability was calculated by means of percentage agreement and kappa statistics Overall good percentage agreement for all parts of the instrument was shown, indicating that the Nordic Housing Enabler is sufficiently reliable for application in practice and research in the Nordic context The kappa results varied and possible explanations are discussed which should be kept in mind when interpreting the results

  • Housing Accessibility Methodology Targeting Older Poeple - Reliable Assessments and Valid Standards

    • Older people have more functional limitations than younger people and they form a segment of the population that is more dependent on mobility devices than other age groups. Since they spend most of their time at home, their dwelling is an important environmental arena for performing everyday activities. Barriers in the environment can limit or hinder activity performance. Therefore, older people are sensitive to the accessibility of the environment. To ensure housing accessibility, it is critical that professionals are provided with valid and reliable assessment instruments to identify accessibility problems before the planning of housing intervention strategies. It is also critical that housing standards addressing accessibility intended to accommodate people with functional limitations are valid in the sense that their definitions truly support accessibility. However, there is a paucity of valid and reliable assessment instruments targeting housing accessibility, and in-depth analysis of factors potentially impacting on reliability in complex assessment situations is remarkably absent. Moreover, the knowledge base informing the housing standards appears to be vague. We may therefore reasonably question the validity of the housing standards addressing accessibility. This thesis addresses housing accessibility methodology in general and the reliability of assessment and the validity of standards targeting older people with functional limitations and a dependence on mobility devices in particular. The overarching aim of the thesis was to develop and explore methods applicable for improving housing accessibility assessments and to explore feasible approaches to create housing standards that truly support accessibility and accommodate older people. A main methodological contribution of the present thesis is the development of the content-valid Nordic HE instrument which is deemed sufficiently reliable in Nordic countries, and the recommendations for in-depth examination of inter-rater agreement for the improvement of reliability. A second main contribution of the present thesis is that it explores the consequences of the housing standard definitions in terms of accessibility and provides estimates of the proportion of dwellings considered accessible and the proportion of persons defined as having accessibility problems. Collectively, these results have the potential to improve and influence research, practice and policy in a global context for the benefit of the health and well-being of older people with functional limitations. Moreover, the results provide new knowledge and invite reflections on central concepts and methodology relevant to psychometrics and research on person-environment fit.

    • OBJECTIVE: To increase the understanding of how definitions of standards for housing design influence the proportion of dwellings not meeting the standards and the proportion of individuals defined as having accessibility problems. METHODS: The sample included old people and their dwellings in three European countries (N = 1,150). Frequencies and percentages were reported and empirical distribution functions were used. RESULTS: Depending on the functional profile and standards in question, the magnitude of influence of the standards differs in extent, e.g., the existing standard for door openings at the entrance (defined ≥75 cm) implied that the proportion of dwellings not meeting it was 11.3% compared to 64.4%, if the standard was set to ≥83 cm. The proportion of individuals defined as having accessibility problems for profiles not using mobility devices was 4-5, 57% for profiles using them and 1-3% for the total sample if the standard was set to 90 cm. CONCLUSION: Research-based standard definitions for housing design are necessary to ensure that they actually lead to enhanced accessibility, which is a prerequisite for the independence and health of persons with functional limitations.
