    • Purpose: The purpose of this study was to illuminate experiences of obtainingsupport among male adolescents with congenital heart disease and their mothers,and nurses’ experiences of providing support within pediatric nursing care.Methods: This descriptive qualitative study used an inductive approach. Datawere collected from interviews with six participants: two adolescents with congenital heart defects-mother dyads, and two nurses at a pediatric cardiac outpatient clinic in Sweden. Data were analyzed through content analysis.Results: Results are divided into three domains: desired support, lacking support,and undesirable support. Within these three respective domains, support is furtherillustrated from three categories of perspectives: adolescents, mothers, and nurses.Conclusions: Study findings show adolescents and their mothers desire support,such as family-system nursing. Nurses strive to provide support in the form ofinformative care like the approach based on the philosophy of pediatric familycentered care.Implications: When highlighting experiences of support from different perspectives, it is important for the provided support to be adapted to families’ needs.

  • Children's perceptions of injuries: a qualitative study in Sweden

    • To understand one of the major public health problems for children, it is important to consider the children's perspective. The purpose of this qualitative study was to explore, describe, and categorize children's perceptions of injury severity and children's explanations of the injuries they experience. A total of 29 students from six randomly selected schools were interviewed in age groups of 9, 13, and 17 years. Manifest content analysis according to Graneheim and Lundman (2004) was used to categorize children's own statements. Need of medical attention, long-term consequences, and familiarity with the injury risk situation were identified as important determinants of children's perception of injury severity. Three categories emerged from children's explanations of their injuries: "Because of Me" (beliefs, lack of concentration, health conditions, and lack of awareness of risk), "Because of the Situation" (rain, ice, wind, animals, inanimate objects, constructions, and the children's games), and "Just Inexplicable" to the children. Findings suggest that children have a wide perception of injury severity and that children's beliefs of injury causation, as well as children's familiarity with injury risk situations, need to be considered in future studies focusing on the development of childhood injury prevention strategies. Additionally, results suggest that sometimes children cannot or do not want to explain their injuries.

  • Children Need to Know : A Follow-Up Study Two Decades Later on Informing and Preparing Children for Clinical Examinations and Procedures

    • Purpose: To describe the current practice of how children and their parents are informed and prepared for medical procedures at Swedish pediatric units, and to follow up on practices reported previously in 17 units.Design and Methods: A cross-sectional survey using a semi-structured questionnaire was performed in 2000, defined as Trial 1 (T1), with 58 responses, and repeated from 2017 to 2018, defined as Trial 2 (T2), with 26 responses. Potential differences between T1 and T2 were calculated with Pearson's Chi-square, and open-ended question responses were subjected to content analysis.Results:  In T1, 95% of respondents reported regular preparation services and 65% reported this in T2; most practices were equal between T1 and T2. However, prior to admission, conversations with nurses (p < 0.05) and video shows on the Internet or web-based programs (p < 0.001) were more frequently reported in T2 than in T1. Additionally, written information about psychological needs at discharge (p < 0.001) and communication with nurses and physicians increased in the present study (p < 0.01). Checking the child's level of understanding was reported less in T2 (p < 0.01). Open-ended responses gave the impression that the units had ambition to inform and prepare children properly but were hindered because of time constraints, a strained staff situation, and lack of routines.Conclusions: In Swedish Pediatric Care, preparation services are emphasized; however, increased outpatient care and decreased hospital stays challenged the practice of preparation and information. The child's perspective of preparation and information needs further illumination.

  • Practices for preparing children for clinical examinations and procedures in Swedish pediatric wards.

    • This study sought to elucidate Swedish pediatric wards concerning the practice of informing children and their parents about clinical examinations and procedures. A semi-structured questionnaire was distributed to all 36 pediatric departments in Sweden, comprising of 83 wards. Fifty-eight (70%) of the wards responded. The results showed that 55 (95%) of the wards provided regular planned information programs. Twenty-seven (47%) of the wards had formulated quality goals for their information program, but in only nine (16%) of the wards were the goals quality assured. Although the results showed that most pediatric wards in Sweden inform children about hospitalization, formulated quality goals remain uncommon. In some wards, economic cutbacks had led to reduction of information for preparation. Despite obstacles, nurses try to give priority to the giving of information. Further studies should focus on children's and parents' experience and satisfaction.

  • Being the parent of a child with asthma

    • The aim of this study is to describe what it means to be parents of a child with asthma. Unstructured interviews were carried out with 12 mothers and 12 fathers of children with asthma living in Sweden. The parents' accounts were analyzed using a phenomenological-hermeneutic approach. The results revealed that parents were living a strenuous life and their actions involved both protecting and liberating. Parents also reported feelings of sadness and acceptance. In most cases, mothers acted in a protecting manner and expressed feelings of sadness; fathers acted in a liberating manner and expressed feelings of acceptance. To gain a deeper understanding of the parents' actions and feelings, study results were interpreted through philosophical perspectives described by Ruddick (1989), Mayeroff (1965), and Hegel (1975). These interpretations show that the feelings and actions of these parents exist in a dialectical relation with one another. Results emphasize the importance of a good partnership between the parents and the nurse, where the nurse shows consideration for the parents' unique actions and feelings and understands and supports parents in the care of their child with asthma.

  • Parents' experience of hospitalization: different strategies for feeling secure

    • Twenty parents of boys (ages 2-14 years) hospitalized for hypospadias repair in a pediatric surgery department in Sweden, were interviewed concerning their experience when their child was hospitalized. A qualitative analysis of the interviews indicated that the most important issue to the parents was finding security at the hospital. Parents manifested one of three different strategies that enabled them to feel secure at the hospital; (a) relinquishing the care of their children to the nursing staff; (b) obtaining a measure of control over their children's care; and (c) relying on knowing their child best. The parental strategy adopted to feel secure was found to correspond with the way parents experienced the hospitalization. Differences were found in their children's experiences of pain and the alleviation of the pain during the hospitalization.

  • Experiencing neonatal death: an ambivalent transition into motherhood

    • OBJECTIVE: To illuminate the lived experience of women facing the threat of lossing their newborn child and then experiencing the reality of their infant's death. STUDY DESIGN: Sixteen women were interviewed approximately 2 years after the death of their infant using a hermeneutic phenomenological approach. FINDINGS: The main theme was ambivalent transition into motherhood. The women went through the whole life-span of their infant during a very short time. The women's narratives were interpreted as broken expectations, total confusion, reality awareness, consciously leave-taking, and elusive grief. CONCLUSIONS: Women need the opportunity to evolve their own patterns and rhythms in the ambivalent transition from expected motherhood to experiencing neonatal dying and death. Knowledge about the individuality of this process may assist nurses in improving the quality of care.
