    • Syftet med denna rapport har varit att kartlägga kompetensbehov som finns bland enhetschefer/verksamhetschefer och biståndshandläggare inom äldreomsorg och omsorg om funktionsnedsatta i Region Gävleborg. Kartläggningen genomfördes genom en webbenkät som besvarades av totalt 155 respondenter. Resultatet visade att, trots att Högskolan i Gävle i den nuvarande socionomutbildningen har inslag av ämnesspecifika kunskaper inom äldre- och funktionshinderområdet samt en specifik kurs inom organisation/ledarskap, finns behov av kompetensutveckling på grundläggande högskolenivå inom dessa områden. Likartade kompetensutvecklingsbehov uttrycktes även på avancerad-/masternivå. För att kunna erbjuda den kompetensutveckling som efterfrågas är det av vikt att arbetsgivare och Högskolan i Gävle samverkar kring utformning av utbildningsinsatser för att ge goda förutsättningar för medarbetare att delta och genomföra dessa.

    • Where, if, and how, should researchers draw the limit between gesture proper and semiotically less complex forms of bodily conduct that do not quite qualify as gesture? This is the question of a lower limit of gesture (Andrén 2010). In accord with a comparative semiotic approach (Kendon 2008) I suggest that the question is best understood, not as a binary distinction between gesture and non-gesture, but as a matter of several different semiotic properties that can vary independently of each other. This involves, in particular, different levels of representational complexity and communicative explicitness. These semiotic properties are both conceptually explicated and applied to empirical examples in this paper, eventually leading me to propose a family resemblance conception of gesture.

    • Efforts to apply nanotechnology in cancer have focused almost exclusively on the delivery of cytotoxic drugs to improve therapeutic index. There has been little consideration of molecularly targeted agents, in particular kinase inhibitors, which can also present considerable therapeutic index limitations. We describe the development of Accurin polymeric nanoparticles that encapsulate the clinical candidate AZD2811, an Aurora B kinase inhibitor, using an ion pairing approach. Accurins increase biodistribution to tumor sites and provide extended release of encapsulated drug payloads. AZD2811 nanoparticles containing pharmaceutically acceptable organic acids as ion pairing agents displayed continuous drug release for more than 1 week in vitro and a corresponding extended pharmacodynamic reduction of tumor phosphorylated histone H3 levels in vivo for up to 96 hours after a single administration. A specific AZD2811 nanoparticle formulation profile showed accumulation and retention in tumors with minimal impact on bone marrow pathology, and resulted in lower toxicity and increased efficacy in multiple tumor models at half the dose intensity of AZD1152, a water-soluble prodrug of AZD2811. These studies demonstrate that AZD2811 can be formulated in nanoparticles using ion pairing agents to give improved efficacy and tolerability in preclinical models with less frequent dosing. Accurins specifically, and nanotechnology in general, can increase the therapeutic index of molecularly targeted agents, including kinase inhibitors targeting cell cycle and oncogenic signal transduction pathways, which have to date proved toxic in humans.

  • E-assessment of prior learning : a pilot study of interactive assessment of staff with no formal education who are working in Swedish elderly care

    • BackgroundThe current paper presents a pilot study of interactive assessment using information and communication technology (ICT) to evaluate the knowledge, skills and abilities of staff with no formal education who are working in Swedish elderly care.MethodsTheoretical and practical assessment methods were developed and used with simulated patients and computer-based tests to identify strengths and areas for personal development among staff with no formal education.ResultsOf the 157 staff with no formal education, 87 began the practical and/or theoretical assessments, and 63 completed both assessments. Several of the staff passed the practical assessments, except the morning hygiene assessment, where several failed. Other areas for staff development, i.e. where several failed (>50%), were the theoretical assessment of the learning objectives: Health, Oral care, Ergonomics, hygiene, esthetic, environmental, Rehabilitation, Assistive technology, Basic healthcare and Laws and organization. None of the staff passed all assessments. Number of years working in elderly care and staff age were not statistically significantly related to the total score of grades on the various learning objectives.ConclusionThe interactive assessments were useful in assessing staff members' practical and theoretical knowledge, skills, and abilities and in identifying areas in need of development. It is important that personnel who lack formal qualifications be clearly identified and given a chance to develop their competence through training, both theoretical and practical. The interactive e-assessment approach analyzed in the present pilot study could serve as a starting point.

  • Synthesis and in Vitro Evaluation of Monodisperse Amino-Functional Polyester Dendrimers with Rapid Degradability and Antibacterial Properties

    • Amine functional polymers, especially cationically charged, are interesting biomacromolecules for several reasons, including easy cell membrane entrance, their ability to escape endosomes through the proton sponge effect, spontaneous complexation and delivery of drugs and siRNA, and simple functionalization in aqueous solutions. Dendrimers, a subclass of precision polymers, are monodisperse and exhibit a large and exact number of peripheral end groups in relation to their size and have shown promise in drug delivery, biomedical imaging and as antiviral agents. In this work, hydroxyl functional dendrimers of generation 1 to 5 based on 2,2-bis(methylol)propionic acid (bis-MPA) were modified to bear 6 to 96 peripheral amino groups through esterification reactions with beta-alanine. All dendrimers were isolated in high yields and with remarkable monodispersity. This was successfully accomplished utilizing the present advantages of fluoride-promoted esterification (FPE) with imidazole-activated monomers. Straightforward postfunctionalization was conducted on a second generation amino functional dendrimer with tetraethylene glycol through NHS-amidation and carbonyl diimidazole (CDI) activation to full conversion with short reaction times. Fast biodegradation of the dendrimers through loss of peripheral beta-alanine groups was observed and generational- and dose-dependent cytotoxicity was evaluated with a set of cell lines. An increase. in neurotoxicity compared to hydroxyl-functional dendrimers was shown in neuronal cells, however, the dendrimers were slightly less neurotoxic than commercially available poly(amidoamine) dendrimers (PAMAMs). Additionally, their effect on bacteria was evaluated and the second generation dendrimer was found unique inhibiting the growth of Escherichia coli at physiological conditions while being nontoxic toward human cells. Finally, these results cement a robust and sustainable synthetic route to amino-functional polyester dendrimers with interesting chemical and biological properties.

    • The past of the Baltic Sea has been intensively investigated using a wealth of techniques. By far the largest number of studies has focused on sea level and salinity changes, driven by global climate and isostatic crustal rebound after the Baltic Sea emerged underneath the Weichselian Ice Sheet ca. 15,000 cal. years BP. The post-glacial history of the Baltic has included both freshwater and brackish water stages depending on the connection of the Baltic Sea with the world’s oceans. As the Baltic is one of the most polluted sea areas in the world, many studies have also focused on both the long-term trends in nutrients and productivity and the relatively recent anthropogenic eutrophication. The long-term changes in the trophic state of the Baltic Sea have been found to be linked to changes in climate, which controls freshwater discharge from the catchment and weathering rates, as well as marine water inflow from the North Sea. The productivity of the Baltic Sea has followed major climate patterns: it was high during warm periods and lower during phases of deteriorating climate. Recent eutrophication of the Baltic Sea can mainly be explained by a marked increase in discharge of nutrients caused by a growing population and changes in the agricultural practice, although long-term climate variability also plays a part. Signs of recovery have recently been detected, however, the Baltic Sea is still far from its pre-industrial trophic state.
