    • Among the basic protective measures against COVID-19, the need to wash hands frequently and in a prolonged way using soap, and to regularly use alcohol-based hand sanitizers is well established for the whole population. Healthcare workers in general, and particularly those involved in the direct care of COVID-19 patients, have to wear personal protective equipment (PPE) daily for many hours and also accomplish general preventive measurements outside their work. Cutaneous adverse reactions can develop that need to be prevented, identified and therapeutically managed. According to the data reported by Lin et al 1, based in the experience from healthcare workers in Wuhan, adverse skin reactions were reported in 74% of responders (n=376) to a general survey. The most commonly reported types of eruptions were skin dryness or desquamation (68.6%), papules or erythema (60.4%) and maceration (52,9%).

    • Aims: To allow timely initiation of anticoagulation therapy for the prevention of stroke, the European guidelines on atrial fibrillation (AF) recommend remote monitoring (RM) of device-detected atrial high-rate episodes (AHREs) and progression of arrhythmia duration along pre-specified strata (6 min...<1h, 1 h...<24 h, >= 24h). We used the MATRIX registry data to assess the capability of a single-lead implantable cardioverter-defibrillator (ICD) with atrial sensing dipole (DX ICD system) to follow this recommendation in patients with standard indication for single-chamber ICD.Methods and results: In 1841 DX ICD patients with daily automatic RM transmissions, electrograms of first device-detected AHREs per patient in each duration stratum were adjudicated, and the corresponding positive predictive values (PPVs) for the detections to be true atrial arrhythmia were calculated. Moreover, the incidence and progression of new-onset AF was assessed in 1451 patients with no AF history. A total of 610 AHREs >= 6min were adjudicated. The PPV was 95.1% (271 of 285) for episodes 6min...<1h, 99.6% (253/254) for episodes 1 h...<24h, 100% (71/71) for episodes >= 24h, or 97.5% for all episodes (595/610). The incidence of new-onset AF was 8.2% (119/1451), and in 31.1% of them (37/119), new-onset AF progressed to a higher duration stratum. Nearly 80% of new-onset AF patients had high CHA(2)DS(2)-VASc stroke risk, and 70% were not on anticoagulation therapy. Age was the only significant predictor of new-onset AF.Conclusion: A 99.7% detection accuracy for AHRE >= 1h in patients with DX ICD systems in combination with daily RM allows a reliable guideline-recommended screening for subclinical AF and monitoring of AF-duration progression.

  • Stellar Characterization of M Dwarfs from the APOGEE Survey : A Calibrator Sample for M-dwarf Metallicities

    • We present spectroscopic determinations of the effective temperatures, surface gravities, and metallicities for 21 M dwarfs observed at high resolution (R similar to 22,500) in the H band as part of the Sloan Digital Sky Survey (SDSS)-IV Apache Point Observatory Galactic Evolution Experiment (APOGEE) survey. The atmospheric parameters and metallicities are derived from spectral syntheses with 1D LTE plane-parallel MARCS models and the APOGEE atomic/molecular line list, together with up-to-date H2O and FeH molecular line lists. Our sample range in T-eff from similar to 3200 to 3800 K, where 11 stars are in binary systems with a warmer (FGK) primary, while the other 10 M dwarfs have interferometric radii in the literature. We define an M-KS-radius calibration based on our M-dwarf radii derived from the detailed analysis of APOGEE spectra and Gaia DR2 distances, as well as a mass-radius relation using the spectroscopically derived surface gravities. A comparison of the derived radii with interferometric values from the literature finds that the spectroscopic radii are slightly offset toward smaller values, with Delta= -0.01 +/- 0.02 R star/R-circle dot. In addition, the derived M-dwarf masses based upon the radii and surface gravities tend to be slightly smaller (by similar to 5%-10%) than masses derived for M-dwarf members of eclipsing binary systems for a given stellar radius. The metallicities derived for the 11 M dwarfs in binary systems, compared to metallicities obtained for their hotter FGK main-sequence primary stars from the literature, show excellent agreement, with a mean difference of [Fe/H](M dwarf - FGK primary) = +0.04 +/- 0.18 dex, confirming the APOGEE metallicity scale derived here for M dwarfs.
