- This document reports the activities and impressions of 30 Swedish adult educators who exchanged ideas, experiences, and research results concerning adult education and learning at more than 20 conferences and seminars organized in the United States and Canada. The foreword briefly summarizes some impressions. (Among them are that adult education is more market-oriented in the United States than in Sweden, which has a more policy-oriented system; that Sweden may have more adults being educated per capita, whereas the United States and Canada may offer a wider variety of programs; and that Sweden may be more concerned with equality as it relates to adult education, whereas personal fulfillment may be a more common value in North America.) Part 1 of the document compares and contrasts the North American and Swedish systems, including such topics as study circles, study finance, outreach activities, workplace learning, public broadcasting, and adult learning, the status of research and development in adult education and the infrastructure of adult learning. Ten references are provided for Part 1. Part 2 contains a set of black and white photographs of the Swedish educators and of people and places they encountered during their visits to North America and Canada. Part 3 provides a smorgasbord of ideas, programs, notes, and references about the trip and the ideas exchanged there

- An association between low serum vitamin D levels and poorer melanoma survival has been reported. We have studied inheritance of a polymorphism of the GC gene, rs2282679, coding for the vitamin D-binding protein, which is associated with lower serum levels of vitamin D, in a meta-analysis of 3137 melanoma patients. The aim was to investigate evidence for a causal relationship between vitamin D and outcome (Mendelian randomization). The variant was not associated with reduced overall survival (OS) in the UK cohort, per-allele hazard ratio (HR) for death 1.23 (95% confidence interval (CI) 0.93, 1.64). In the smaller cohorts, HR in OS analysis was 1.07 (95% CI 0.88, 1.3) and for all cohorts combined, HR for OS was 1.09 (95% CI 0.93, 1.29). There was evidence of increased melanoma-specific deaths in the seven cohorts for which these data were available. The lack of unequivocal findings despite the large sample size illustrates the difficulties of implementing Mendelian randomization.

Inherited variation in the PARP1 gene and survival from melanoma

- We report the association of an inherited variant located upstream of the poly(adenosine diphosphate-ribose) polymerase 1 (PARP1) gene (rs2249844), with survival in 11 BioGenoMEL melanoma cohorts. The gene encodes a protein involved in a number of cellular processes including single-strand DNA repair. Survival analysis was conducted for each cohort using proportional hazards regression adjusting for factors known to be associated with survival. Survival was measured as overall survival (OS) and, where available, melanoma-specific survival (MSS). Results were combined using random effects meta-analysis. Evidence for a role of the PARP1 protein in melanoma ulceration and survival was investigated by testing gene expression levels taken from formalin-fixed paraffin-embedded tumors. A significant association was seen for inheritance of the rarer variant of PARP1, rs2249844 with OS (hazard ratio (HR) = 1.16 per allele, 95% confidence interval (CI) 1.04-1.28, p=0.005, eleven cohorts) and MSS (HR=1.20 per allele, 95% CI 1.01-1.39, p=0.03, eight cohorts). We report bioinformatic data supportive of a functional effect for rs2249844. Higher levels of PARP1 gene expression in tumors were shown to be associated with tumor ulceration and poorer OS. What's new? Although staging systems predict outcome fairly well for melanoma, survival still varies among individual patients. In this meta-analysis, the authors found that inheritance of a rare genetic variant of PARP1 was associated with improved survival of melanoma patients. Increased expression of PARP1 has been associated with poorer outcome, and depletion of PARP1 may reduce both melanoma growth and angiogenesis. The identification of this and other germline variants that affect survival may help to identify key biological pathways active in host/tumor interactions, which may lead to the discovery of new therapeutic targets for treating advanced melanoma. - Artikeln fokuserar på hälso- och sjukvårdskuratorns arbete med att å ena sidan bemöta de känslor som barn och familjer kan känna efter att barnet diagnostiserats med den kroniska sjukdomen diabetes, å andra sidan hantera olika byråkratiska processer som ska sörja för att barnets vårdbehov tillgodoses i familjens nya vardag. Arbetet riktas mot två olika världar som är inbördes beroende av varandra, men som många gånger innebär en känslig balansakt för kuratorn att förena. Syftet är att undersöka hur kuratorn förhåller sig till, och balanserar, dessa två världar i mötet med barnet och familjen. Balansakten undersöks etnografiskt genom att vi studerar blanketters betydelse i vårdmötet. Ett viktigt resultat av vår undersökning är blankettens roll när kuratorn översätter familjens nya vardag till ett språk som kan generera ett välfärdsstöd.

- The article focuses on the work of clinical social workers who, on the one hand, respond to the emotions that children and families experience after the child is diagnosed with the chronic disease diabetes, on the other hand, deal with various bureaucratic processes aimed at ensuring that the child’s medical needs are met in the family’s new life. The work is directed towards two different worlds that are interdependent, yet bringing them together fre-quently necessitates a balancing act. The purpose is to examine how the social worker deal with these two worlds in the encounter with the child and the family. A result is the document’s role when the social worker translates the family’s new everyday life into a language that can generate assistance or allowance from the state.

Enabling and Coercive Control : Coexistence in the Case of Banking

- This thesis focuses on subjects of control and attitudinal outcomes of formalized control in organizations. Previous research have concluded contradicting results of whether formalized control is positive or negative for the employees and propose that not only degree of formalization, but also type of formalization, can explain attitudinal outcomes.With the theoretical perspective of Adler and Borys’ concepts of enabling and coercive types of control, this thesis explore the concepts and practices of enabling and coercive control, and their relationships with attitudinal outcomes. This is done with a focus on the banking industry, which serve as a case of an extensively regulated context. An assumption put forth in this thesis is that the context in which individuals are part must be considered in the study of attitudinal outcomes of control.The analysis show that both enabling and coercive control can be found in banking, for example in business plans and regulations. However, the picture emerging is more complex than enabling control leading to positive attitudes, and coercive control leading to negative attitudes. Also, coexistence of enabling and coercive control is responded to with decoupling and acquiescence, and by drawing on global transparency.Based on these findings, together with theoretical elaborations, this thesis contribute to the literature of enabling and coercive control in a number of ways. First, it make explicit central concepts and relationships within the theory, such as design vs. perception, the role of zone of indifference as an outcome of control, and enabling and coercive control as dual roles or qual-ities of control. Moreover, this thesis suggests that multiple-level explanations to attitudinal outcomes of control, where contextual and institutional structures are considered, helps us understand attitudes to control in this context. Lastly, this thesis contribute to the notion of coexistence of enabling and coercive control by showing that coexistence can be simultaneous systems, and simultaneous cognitions, where a control can be perceived as both enabling and coercive at the same time.

- I detta nummer ges en rad exempel där det påvisas att föreställningar, normer och värderingar spelar en avgörande roll i medicinsk praktik. Allt ifrån barnhälsovårdens fyraårskontroller till habiliteringsprocesser för handikappade. I en framåtblickande artikel analyseras problematiken om friskt och sjukt utifrån kartläggning av genetiska markörer. Om den genetiska kartläggningen utsäger att någon med stor sannolikhet kommer att bli sjuk, men kanske 20 år senare. Hur skall vi förhålla oss till detta? I takt med mikroteknologins utveckling kommer allt fler dilemman av detta slag att prägla vården och preventionens vardag. Etnologin kan ge en större förståelse för kulturella processer som finns inom olika medicinska praktikformer, vilket illustreras genom en rad empiriska ingångar i numret.

- I det här temanumret av Socialmedicinsk tidskrift undersöker etnologer den kulturella grund som medicinen vilar på. Etnologin studerar de föreställningar om liv, död, sjukdom och bot som skapats, och skapas, i en viss historisk kontext; vilken syn på samhället, individen och kroppen som den medicinska blicken genererar och hur människors vardag med sjukdom och ohälsa ser ut. Medicinska förklaringsmodeller och praktiker formas i mötet mellan olika personalgrupper eller mellan patient och personal, men även i förhållande till de förändringar som sker i samhället. Centralt är att beskriva hur synen på hälsa och sjukdom förhandlas fram, omtolkas eller anpassas från situation till situation. I den kulturella process som vårdmötet utgör tydliggörs de olika roller och positioner som är verksamma och som antingen underlättar eller försvårar möjligheten till kommunikation. Genom att avtäcka och beskriva de föreställningar, normer och värderingar som präglar såväl patienter som vårdpersonal kan etnologin bidra till en större förståelse för medicinens kulturella beståndsdelar.
