    • In a health-care service with the emphasis on improvement related to functioning and well-being, the communication process between patient and professionals is essential. There is a lack of research on this matter. Aim: The aim was to investigate, in a group of severely mentally ill persons, whether the use of a simple communication tool could influence the sense of empowerment, satisfaction with care, therapeutic alliance and unmet needs. Method: The study had a cluster randomized design. The intervention was a communication tool (2-COM) applied in two teams during one year. In a comparison group of two other teams, the treatment was as usual. At baseline, after six months, and after one year, assessments were made. Results: After one year the 2-COM groups seemed to have a larger reduction in unmet needs compared to the treatment-as-usual group. However, there were large problems with attrition in the study, and it was not possible to draw relevant conclusions. Conclusions: The methodological problems were substantial, and the study may be considered as a pilot study. In a main study the researchers ought to take control over the selection of patients on the basis of the experiences from this study.

    • PEF curves are a useful but cumbersome tool in diagnosing work-related asthma. Using a digital spirometer and smartphone app, time to clinical decision could be shortened by 6-7 weeks. Physician's time spent analysing PEF data is also shortened.

    • In Sweden, disabled people with comprehensive support needs have the right to personal assistance (PA) since 1994. Disabled people were expected to take on roles as experts and managers of their own on assistance and were assumed to oppose professional and/or family interference in their lives. However, official statistics show that many assistant users have PA from one or several family members, often in combination with non-family assistance, a situation we don’t know much about. Thus, further knowledge is needed about family PA (FPA) and how it may promote or obstruct the possibilities to achieve the policy intentions of individual support, independence and participation in society for disabled people. In this study we have explored how Swedish PA users experience and manage having family members employed as personal assistants, with a special focus on autonomy, dependence and interdependence. We draw upon theory and research on controversies around FPA, the shifting views on disabled people from passive dependants to active agents, as well as individual and relational conceptions of autonomy. We have performed 17 qualitative interviews with users of FPA, both men and women of different ages, and who has a father or mother, a partner and/ or an adult son or daughter as FPA. Preliminary findings about ‘Family as a prison or a free-zone?, ‘Strategies for managing FPA’ and ‘Dealing with dependency’ will be presented.

    • Background: The right to personal assistance is intended to enable people with severe disabilities to increased influence and self-determination in their daily lives. The number of personal assistants in Sweden is estimated at 70,000, of which 20-25% consists of family members of persons with disabilities. Family members working as personal assistants are a controversial phenomenon that has traits of both traditional family care and modern disability policy. Concerns are raised about the complexity of combining informal relationships within the family with formal employment, social and economic interdependence, and how different interests are met. The study highlights the intersection between formal and informal care, the complexity of family care, and the opportunity for people with disabilities and their families to “manage” their everyday lives and to influence their living situations. Aim: to study various aspects of the living situations, interdependence and strategies of users and providers of assistance, with different family ties. Methods: Thematic qualitative interviews have been conducted with users and providers of assistance in four types of family ties: Parents who work as assistants to adult children; Siblings who work as assistants to an adult sibling; Spouses where one works as assistant to the other; Adult children working as assistants to a parent. Results: In this presentation, the experiences and strategies of parents working as personal assistants to their adult children will be focused on. How is independence, dependence and interdependence negotiated? What dilemmas do they face? What strategies do they develop to deal with potential role conflicts and ambivalence?

    • The aim of this article was to explore how users experienced and managed personal assistance from family members who are employed as personal assistants. Seventeen users of personal assistance provided by one or several family members, often in combination with external personal assistants, participated. Thematic qualitative interviews were conducted. The participants were between 19 and 58 years old, and were living with various impairments: mobility, sensory and/or learning disabilities. The results showed that personal assistance from family members could both promote and be a barrier to disabled people’s control over their own lives and participation in society. It became evident that the situation could have both advantages and disadvantages for both parties, which puts the focus on the interdependency between the participants and their family assistants. However, the results also points to a potential risk that the welfare state (re)passes its responsibilities back on to disabled people and their families.

  • Agricultural breadbaskets shift poleward given adaptive farmer behavior under climate change

    • Modern food production is spatially concentrated in global "breadbaskets". A major unresolved question is whether these peak production regions will shift poleward as the climate warms, allowing some recovery of potential climaterelated losses. While agricultural impacts studies to date have focused on currently cultivated land, the Global Gridded Crop Model Intercomparison Project (GGCMI) Phase 2 experiment allows us to assess changes in both yields and the location of peak productivity regions under warming. We examine crop responses under projected end-of-century warming using 7 process-based models simulating 5 major crops (maize, rice, soybeans, and spring and winter wheat) with a variety of adaptation strategies. We find that in no-adaptation cases, when planting date and cultivar choices are held fixed, regions of peak production remain stationary and yield losses can be severe, since growing seasons contract strongly with warming. When adaptations in management practices are allowed (cultivars that retain growing season length under warming and modified planting dates), peak productivity zones shift poleward and yield losses are largely recovered. While most growing-zone shifts are ultimately limited by geography, breadbaskets studied here move poleward over 600 km on average by end of the century under RCP8.5. These results suggest that agricultural impacts assessments can be strongly biased if restricted in spatial area or in the scope of adaptive behavior considered. Accurate evaluation of food security under climate change requires global modeling and careful treatment of adaptation strategies.
