  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

  • Tracking development assistance for health and for COVID-19 : a review of development assistance, government, out-of-pocket, and other private spending on health for 204 countries and territories, 1990-2050

    • Background The rapid spread of COVID-19 renewed the focus on how health systems across the globe are financed, especially during public health emergencies. Development assistance is an important source of health financing in many low-income countries, yet little is known about how much of this funding was disbursed for COVID-19. We aimed to put development assistance for health for COVID-19 in the context of broader trends in global health financing, and to estimate total health spending from 1995 to 2050 and development assistance for COVID-19 in 2020. Methods We estimated domestic health spending and development assistance for health to generate total health-sector spending estimates for 204 countries and territories. We leveraged data from the WHO Global Health Expenditure Database to produce estimates of domestic health spending. To generate estimates for development assistance for health, we relied on project-level disbursement data from the major international development agencies' online databases and annual financial statements and reports for information on income sources. To adjust our estimates for 2020 to include disbursements related to COVID-19, we extracted project data on commitments and disbursements from a broader set of databases (because not all of the data sources used to estimate the historical series extend to 2020), including the UN Office of Humanitarian Assistance Financial Tracking Service and the International Aid Transparency Initiative. We reported all the historic and future spending estimates in inflation-adjusted 2020 US$, 2020 US$ per capita, purchasing-power parity-adjusted US$ per capita, and as a proportion of gross domestic product. We used various models to generate future health spending to 2050. Findings In 2019, health spending globally reached $8. 8 trillion (95% uncertainty interval [UI] 8.7-8.8) or $1132 (1119-1143) per person. Spending on health varied within and across income groups and geographical regions. Of this total, $40.4 billion (0.5%, 95% UI 0.5-0.5) was development assistance for health provided to low-income and middle-income countries, which made up 24.6% (UI 24.0-25.1) of total spending in low-income countries. We estimate that $54.8 billion in development assistance for health was disbursed in 2020. Of this, $13.7 billion was targeted toward the COVID-19 health response. $12.3 billion was newly committed and $1.4 billion was repurposed from existing health projects. $3.1 billion (22.4%) of the funds focused on country-level coordination and $2.4 billion (17.9%) was for supply chain and logistics. Only $714.4 million (7.7%) of COVID-19 development assistance for health went to Latin America, despite this region reporting 34.3% of total recorded COVID-19 deaths in low-income or middle-income countries in 2020. Spending on health is expected to rise to $1519 (1448-1591) per person in 2050, although spending across countries is expected to remain varied. Interpretation Global health spending is expected to continue to grow, but remain unequally distributed between countries. We estimate that development organisations substantially increased the amount of development assistance for health provided in 2020. Continued efforts are needed to raise sufficient resources to mitigate the pandemic for the most vulnerable, and to help curtail the pandemic for all. Copyright (C) 2021 The Author(s).   • An Experimental and Comparative Performance Evaluation of a Hybrid Photovoltaic-Thermoelectric System

    • The majority of incident solar irradiance causes thermalization in photovoltaic (PV) cells, attenuating their efficiency. In order to use solar energy on a large scale and reduce carbon emissions, their efficiency must be enhanced. Effective thermal management can be utilized to generate additional electrical power while simultaneously improving photovoltaic efficiency. In this work, an experimental model of a hybrid photovoltaic-thermoelectric generation (PV-TEG) system is developed. Ten bismuth telluride-based thermoelectric modules are attached to the rear side of a 10 W polycrystalline silicon-based photovoltaic module in order to recover and transform waste thermal energy to usable electrical energy, ultimately cooling the PV cells. The experiment was then carried out for 10 days in Lahore, Pakistan, on both a simple PV module and a hybrid PV-TEG system. The findings revealed that a hybrid system has boosted PV module output power and conversion efficiency. The operating temperature of the PV module in the hybrid system is reduced by 5.5%, from 55 degrees C to 52 degrees C. Due to a drop in temperature and the addition of some recovered energy by thermoelectric modules, the total output power and conversion efficiency of the system increased. The hybrid system's cumulative output power increased by 19% from 8.78 to 10.84 W, compared to the simple PV system. Also, the efficiency of the hybrid PV-TEG system increased from 11.6 to 14%, which is an increase of 17% overall. The results of this research could provide consideration for designing commercial hybrid PV-TEG systems.

    • Recently, the nanostructured nickel–cobalt bimetallic oxide (NiCo2O4) material with high electrochemical activity has received intensive attention. Beside this, the biomass assisted synthesis of NiCo2O4 is gaining popularity due to its advantageous features such as being low cost, simplicity, minimal use of toxic chemicals, and environment-friendly and ecofriendly nature. The electrochemical activity of spinel NiCo2O4 is associated with its mixed metal oxidation states. Therefore, much attention has been paid to the crystal quality, morphology and tunable surface chemistry of NiCo2O4 nanostructures. In this study, we have used citrus lemon juice consisting of a variety of chemical compounds having the properties of a stabilizing agent, capping agent and chelating agent. Moreover, the presence of several acidic chemical compounds in citrus lemon juice changed the pH of the growth solution and consequently we observed surface modified and structural changes that were found to be very effective for the development of energy conversion and energy storage systems. These naturally occurring compounds in citrus lemon juice played a dynamic role in transforming the nanorod morphology of NiCo2O4 into small and well-packed nanoparticles. Hence, the prepared NiCo2O4 nanostructures exhibited a new surface-oriented nanoparticle morphology, high concentration of defects on the surface (especially oxygen vacancies), sufficient ionic diffusion and reaction of electrolytic ions, enhanced electrical conductivity, and favorable reaction kinetics at the interface. The electrocatalytic properties of the NiCo2O4 nanostructures were studied in oxygen evolution reaction (OER) at a low overpotential of 250 mV for 10 mA cm−2, Tafel slope of 98 mV dec−1, and durability of 40 h. Moreover, an asymmetric supercapacitor was produced and the obtained results indicated a high specific capacitance of (Cs) of 1519.19 F g−1, and energy density of 33.08 W h kg−1 at 0.8 A g−1. The enhanced electrochemical performance could be attributed to the favorable structural changes, surface modification, and surface crystal facet exposure due to the use of citrus lemon juice. The proposed method of transformation of nanorod to nanoparticles could be used for the design of a new generation of efficient electrocatalyst materials for energy storage and conversion uses.

    • Hydrophilic drugs could be incorporated into the skin surface by manes of Lipogel. This study aimed to prepare miconazole lipogel with natural ingredients to enhance drug permeability using dimethyl Sulfoxide (DMSO). The miconazole lipogels, A1 (without DMSO) and A2 (with DMSO) were formulated and evaluated for organoleptic evaluation, pH, viscosity, stability studies, freeze -thawing, drug release profile and drug permeation enhancement. Results had stated that prepared lipogel's pH falls within the acceptable range required for topical delivery (4 to 6) while both formulations show good results in organoleptic evaluation. The A2 formulation containing DMSO shows better permeation of miconazole (84.76%) on the artificial skin membrane as compared to A1 lipogel formulation (50.64%). In in -vitro drug release studies, A2 for-mulation showed 87.48% drug release while A1 showed just 60.1% drug release from lipogel. Stability studies were performed on model formulations under environmental conditions and both showed good spreadibility, stable pH, free of grittiness and good consistency in formulation. The results concluded that A2 formulation containing DMSO shows better results as compared to DMSO-free drug lipogel.

    • A novel, frequency selective surface (FSS) inspired, fully passive, chipless data encoding circuit capable of being operated as a radio frequency identification (RFID) tag is presented. The tag is composed of finite repetitions of the unit cell realized on a grounded FR4 substrate having an overall size of 27.5 x 30mm(2). The unit cell is made up of several triangle-shaped resonators patterned in a looped fashion. Variation in the geometric structure of the tag, achieved by addition or removal of nested loops, corresponds to a specific bit sequence. Each sequence is represented in the spectral domain as a unique frequency signature of the resonators. The proposed 10-bit tag covers the spectral range from 4 to 11 GHz. The tag is compact, robust, and exhibits a stable response to impinging signals at different angles of incidence.

    • A robustness-driven hybrid descriptor (RDHD) for noise-deterrent texture classification is presented in this paper. This paper offers the ability to categorize a variety of textures under challenging image acquisition conditions. An image is initially resolved into its low-frequency components by applying wavelet decomposition. The resulting low-frequency components are further processed for feature extraction using completed joint-scale local binary patterns (CJLBP). Moreover, a second feature set is obtained by computing the low order derivatives of the original sample. The evaluated feature sets are integrated to get a final feature vector representation. The texture-discriminating performance of the hybrid descriptor is analyzed using renowned datasets: Outex original, Outex extended, and KTH-TIPS. The experimental results demonstrate a stable and robust performance of the descriptor under a variety of noisy conditions. An accuracy of 95.86%, 32.52%, and 88.74% at noise variance of 0.025 is achieved for the given datasets, respectively. A comparison between performance parameters of the proposed paper with its parent descriptors and recently published paper is also presented.

    • In this paper, a compact and fully passive bit encoding circuit, capable of operating as a chipless radio frequency identification (RFID) tag is presented. The structure consists of novel concentric trefoil-shaped slot resonators realized using Rogers RT/duroid (R) 5880 laminate, occupying a physical footprint of 13.55 x 13.55 mm(2). Each resonating element is associated with a particular data bit, having a 1:1 resonator-to-bit correspondence. Bit sequences are configured through introducing modifications in the geometric structure either by addition or exclusion of each nested slot resonator. Such changes manifest directly in the electromagnetic signature of the tag as presence or absence of corresponding resonant peaks. The proposed 10-bit tag offers minimized inter-resonator mutual coupling and insensitivity to changes in polarization and incident angles thereby demonstrating orientation independent functionality. Moreover, error-free encoding is achieved through stabilizing the shift in resonant frequencies for a variety of different geometric configurations and orientation of the structure. The tag operates within the license-free ultrawideband ranging from 5.4 to 10.4 GHz, providing spectral bit capacity and bit density of 2 bits/GHz and 5.44 bits/cm(2) respectively.

    • This paper ideates a novel texture descriptor that retains its classification accuracy under varying conditions of image orientation, scale, and illumination. The proposed Overlapped Multi-oriented Tri-scale Local Binary Pattern (OMTLBP) texture descriptor also remains insensitive to additive white Gaussian noise. The wavelet decomposition stage of the OMTLBP provides robustness to photometric variations, while the two subsequent stages - overlapped multi-oriented fusion and multi-scale fusion - provide resilience against geometric transformations within an image. Isolated encoding of constituent pixels along each scale in the joint histogram enables the proposed descriptor to capture both micro and macro structures within the texture. Performance of the OMTLBP is evaluated by classifying a variety of textured images belonging to Outex, KTH-TIPS, Brodatz, CUReT, and UIUC datasets. The experimental results validate the superiority of the proposed method in terms of classification accuracy when compared with the state-of-the-art texture descriptors for noisy images.

    • In this research work, bromine (Br) is successfully doped into ZnO nanostructures using solvothermal method. The morphology, crystalline features, and composition of Br doped ZnO nanostructures were studied by scanning electron microscopy (SEM), X-ray diffraction (XRD) and energy dispersive X ray spectroscopy (EDX) respectively. These newly prepared nanostructured materials were tested as photocatalysts for the photodegradation of methylene blue (MB) in aqueous solution under UV light. The kinetic rate constants were observed in the order (20% Br/ZnO > 15% Br/ZnO > 10% Br/ZnO > 5% Br/ZnO >pristine ZnO), thus they are indicating that the increasing Br dopant level has linear effect on the photodegradation of MB. The photocatalytic degradation efficiency of 60% was achieved for the pristine ZnO during the irradiation of UV light for 5 h, however 20% Br doped ZnO nanostructures has shown enhanced degradation efficiency of 97.63% during the irradiation of UV light for short interval of time of 2.2 h. The 20% Br/ZnO describes the highest rate constant value of (24.13 x 10(-3) min(-1)), for time period of 2.2 h and this values is about 8 and 4 times higher than the pristine ZnO (3.72 x 10(-3) min(-1)) and 5% Br/ZnO (6.13 x 10(-3) min(-1)), respectively. The obtained results of 20% Br doped ZnO sample are superior or equal in performance than the recently reported works. The catalytic mechanism is also proposed and it indicates the role of electrons coming from the bromine ion might act as radical for the degradation of MB. The present approach is simpler, environment friendly, scalable and could be of great consideration for the diverse energy and environment related applications. Graphic
