    • Objective: The objective was to analyse relationships between inspiratory and expiratory static and dynamic elastic pressure-volume (P-el/V) curves in healthy pigs. Design: The modulated low flow method was developed to allow studies also of the expiratory P-el/V curves. Static P-el/V (P-el,P-st/V) and dynamic P-el/V (P-el,P-dyn/V) loops were studied in healthy pigs. Setting: Animal research laboratory in a university hospital. Material: Ten healthy anaesthetised and paralysed pigs. Interventions and measurements: A computer controlled a Servo Ventilator 900C with respect to respiratory rate, inspiratory flow and expiratory pressure to achieve a sinusoidal modulation of inspiration and expiration for determination of P-el,P-dyn/V loops from zero end-expiratory pressure (ZEEP) and from a positive end-expiratory pressure (PEEP) of 6 cmH(2)O to 20, 35 and 50 cmH(2)O. The same system was used for studies of P-el,P-st/V loops with the flow-interruption method from ZEEP and PEEP to 35 cmH(2)O. Recordings were analysed with an iterative technique. Results: The feasibility of automated determination of P-el,P-dyn/V loops was demonstrated. Differences between P-el,P-dyn/V and P-el,P-st/V loops were explained by viscoelastic behaviour. P-el,P-st/V loops recorded from PEEP to 35 cmH(2)O showed no significant hysteresis, indicating a non-significant surface tension hysteresis. P-el,P-dyn/V loops from PEEP and both P-el,P-st/V and P-el,P-dyn/V loops from ZEEP to 35 cmH(2)O showed hysteresis. This indicates that lung collapse/re-expansion caused P-el/V loop hysteresis which, in P-el,P-dyn/V loops, was augmented by viscoelastic behaviour. Conclusions: Viscoelasticity influences P-el,P-dyn/V curves. Hysteresis caused by surface tension merits re-evaluation. Lung collapse and re-expansion may be indicated by hysteresis of P-el/V loops.

    • Introduction To prevent further lung damage in patients with acute respiratory distress syndrome ( ARDS), it is important to avoid overdistension and cyclic opening and closing of atelectatic alveoli. Previous studies have demonstrated protective effects of using low tidal volume ( VT), moderate positive end-expiratory pressure and low airway pressure. Aspiration of dead space ( ASPIDS) allows a reduction in VT by eliminating dead space in the tracheal tube and tubing. We hypothesized that, by applying goal-orientated ventilation based on iterative computer simulation, VT can be reduced at high respiratory rate and much further reduced during ASPIDS without compromising gas exchange or causing high airway pressure. Methods ARDS was induced in eight pigs by surfactant perturbation and ventilator-induced lung injury. Ventilator resetting guided by computer simulation was then performed, aiming at minimal VT, plateau pressure 30 cmH(2)O and isocapnia, first by only increasing respiratory rate and then by using ASPIDS as well. Results VT decreased from 7.2 +/- 0.5 ml/kg to 6.6 +/- 0.5 ml/kg as respiratory rate increased from 40 to 64 +/- 6 breaths/min, and to 4.0 +/- 0.4 ml/kg when ASPIDS was used at 80 +/- 6 breaths/min. Measured values of arterial carbon dioxide tension were close to predicted values. Without ASPIDS, total positive end-expiratory pressure and plateau pressure were slightly higher than predicted, and with ASPIDS they were lower than predicted. Conclusion In principle, computer simulation may be used in goal-oriented ventilation in ARDS. Further studies are needed to investigate potential benefits and limitations over extended study periods.

  • CO2 elimination at varying inspiratory pause in acute lung injury

    • Previous studies have indicated that, during mechanical ventilation, an inspiratory pause enhances gas exchange. This has been attributed to prolonged time during which fresh gas of the tidal volume is present in the respiratory zone and is available for distribution in the lung periphery. The mean distribution time of inspired gas (MDT) is the mean time during which fractions of fresh gas are present in the respiratory zone. All ventilators allow setting of pause time, T-P, which is a determinant of MDT. The objective of the present study was to test in patients the hypothesis that the volume of CO2 eliminated per breath, VTCO2, is correlated to the logarithm of MDT as previously found in animal models. Eleven patients with acute lung injury were studied. When T-P increased from 0% to 30%, MDT increased fourfold. A change of T-P from 10% to 0% reduced VTCO2 by 14%, while a change to 30% increased VTCO2 by 19%. The relationship between VTCO2 and MDT was in accordance with the logarithmic hypothesis. The change in VTCO2 reflected to equal extent changes in airway dead space and alveolar PCO2 read from the alveolar plateau of the single breath test for CO2. By varying T-P, effects are observed on VTCO2, airway dead space and alveolar PCO2. These effects depend on perfusion, gas distribution and diffusion in the lung periphery, which need to be further elucidated.

    • Introduction: The inspiratory flow pattern influences CO2 elimination by affecting the time the tidal volume remains resident in alveoli. This time is expressed in terms of mean distribution time (MDT), which is the time available for distribution and diffusion of inspired tidal gas within resident alveolar gas. In healthy and sick pigs, abrupt cessation of inspiratory flow (that is, high end-inspiratory flow (EIF)), enhances CO2 elimination. The objective was to test the hypothesis that effects of inspiratory gas delivery pattern on CO2 exchange can be comprehensively described from the effects of MDT and EIF in patients with acute respiratory distress syndrome (ARDS). Methods: In a medical intensive care unit of a university hospital, ARDS patients were studied during sequences of breaths with varying inspiratory flow patterns. Patients were ventilated with a computer-controlled ventilator allowing single breaths to be modified with respect to durations of inspiratory flow and postinspiratory pause (T-P), as well as the shape of the inspiratory flow wave. From the single-breath test for CO2, the volume of CO2 eliminated by each tidal breath was derived. Results: A long MDT, caused primarily by a long TP, led to importantly enhanced CO2 elimination. So did a high EIF. Effects of MDT and EIF were comprehensively described with a simple equation. Typically, an efficient and a less-efficient pattern of inspiration could result in +/- 10% variation of CO2 elimination, and in individuals, up to 35%. Conclusions: In ARDS, CO2 elimination is importantly enhanced by an inspiratory flow pattern with long MDT and high EIF. An optimal inspiratory pattern allows a reduction of tidal volume and may be part of lung-protective ventilation.

    • The Advanced GAmma Tracking Array (AGATA) is a European project to develop and operate the next generation γ-ray spectrometer. AGATA is based on the technique of γ-ray energy tracking in electrically segmented high-purity germanium crystals. This technique requires the accurate determination of the energy, time and position of every interaction as a γ ray deposits its energy within the detector volume. Reconstruction of the full interaction path results in a detector with very high efficiency and excellent spectral response. The realisation of γ-ray tracking and AGATA is a result of many technical advances. These include the development of encapsulated highly segmented germanium detectors assembled in a triple cluster detector cryostat, an electronics system with fast digital sampling and a data acquisition system to process the data at a high rate. The full characterisation of the crystals was measured and compared with detector- response simulations. This enabled pulse-shape analysis algorithms, to extract energy, time and position, to be employed. In addition, tracking algorithms for event reconstruction were developed. The first phase of AGATA is now complete and operational in its first physics campaign. In the future AGATA will be moved between laboratories in Europe and operated in a series of campaigns to take advantage of the different beams and facilities available to maximise its science output. The paper reviews all the achievements made in the AGATA project including all the necessary infrastructure to operate and support the spectrometer. © 2011 Elsevier B.V. All rights reserved.

    • The O-15(2p, gamma)Ne-17 cross section has been studied by the inverse reaction, the Coulomb dissociation of Ne-17. The experiment has been performed at the GSI. The Ne-17 excitation energy prior to decay has been reconstructed by using the invariant-mass method. The preliminary differential and integral Coulomb dissociation cross sections (sigma(Coul)) have been extracted, which provide a photoabsorption (sigma(photo)) and a radiative capture cross section (sigma(cap)). Additionally, important information about the Ne-17 nuclear structure will be obtained. The analysis is in progress.

  • Lithium isotopes beyond the drip line

    • The unbound isotopes 10Li, 12Li and 13Li have been observed after nucleon-knockout reactions at relativistic energies with 11Li and 14Be beams impinging on a liquid hydrogen target. The channels , and were analysed in the ALADIN-LAND setup at GSI. The 10Li data confirm earlier findings, while the 12Li and 13Li nuclei were observed for the first time. The relative-energy spectrum shows that the ground state of 12Li can be described as a virtual s-state with a scattering length of -13.7(1.6) fm. A broad energy spectrum was found for the channel. Based on the assumption that the relative-energy spectrum is dominated by a correlated background presumably stemming from initial correlations in the 14Be ground-state, evidence for a 13Li resonance at 1.47(31) MeV above the threshold with a width around 2 MeV has been found.

    • One-neutron knockout reactions from Borromean nuclei are analyzed using a profile function analysis technique. The profile function, which is derived as the square root of the variance of the measured fragment + neutron momenta as a function of their relative energy, is shown to be very sensitive to the angular momentum of the knocked out neutron. Three cases are studied here: He-7, where the profile function analysis shows a presence of (s(1/2))(2) component in the He-8 ground-state wave-function, Li-10, where the presence of a 11(2)% d-wave contribution to the relative energy spectrum above 1.5 MeV is found and, finally, the presence of a major s contribution around 0.5 MeV relative energy in the Be-13 case and that the observed decay to the Be-12 2(+) state originates in a d state in Be-13.

  • Properties of the 7He ground state from 8He neutron knockout

    • The unbound nucleus 7He, produced in neutron-knockout reactions with a 240 MeV/u 8He beam in a liquid-hydrogen target, has been studied in an experiment at the ALADIN-LAND setup at GSI. From an R-matrix analysis the resonance parameters for 7He as well as the spectroscopic factor for the 6He(0+) + n configuration in its ground-state have been obtained. The spectroscopic factor is 0.61 confirming that 7He is not a pure single-particle state. An analysis of 5He data from neutron-knockout reactions of 6He in a carbon target reveals the presence of an s-wave component at low energies in the α+n relative energy spectrum. A possible low-lying exited state in 7He observed in neutron knockout data from 8He in a carbon target and tentatively interpreted as a Iπ=1/2− state, could not be observed in the present experiment. Possible explanations of the shape difference between the 7He resonance obtained in the two knockout reactions are discussed in terms of target-dependence or different reaction mechanisms at relativistic energies.

  • Structure of the unbound nucleus Be-13: One-neutron knockout reaction data from Be-14 analyzed in a holistic approach

    • At the ALADIN-LAND setup at GSI the unbound nucleus Be-13 has been produced in one-neutron knockout reactions from a 304 MeV/nucleon relativistic beam of Be-14 ions impinging on a liquid hydrogen target. An analysis of the data including all available information about Be-13, and in particular recent data from a similar experiment performed at RIKEN, has been performed. A consistent description is reached. It is found that the excitation spectrum is dominated by s-waves at low energy, which solves problems from previous seemingly contradictory interpretations. A possible interference between two s-states in Be-13 is also discussed. The results indicate that the ground-state wave function of Be-14 is dominated by valence neutrons in the s-shell contributing with 60-75% of the total neutron knockout cross section.

  • Study of the Be14 continuum: Identification and structure of its second 2+ state

    • The coupling between bound quantum states and those in the continuum is of high theoretical interest. Experimental studies of bound drip-line nuclei provide ideal testing grounds for such investigations since they, due to the feeble binding energy of their valence particles, are easy to excite into the continuum. In this Letter, continuum states in the heaviest particle-stable Be isotope, Be14, are studied by employing the method of inelastic proton scattering in inverse kinematics. New continuum states are found at excitation energies E*=3.54(16) MeV and E*=5.25(19) MeV. The structure of the earlier known 21+ state at 1.54(13) MeV was confirmed with a predominantly (0d5/2)2 configuration while there is very clear evidence that the 22+ state has a predominant (1s1/2, 0d 5/2) structure with a preferential three-body decay mechanism. The region at about 7 MeV excitation shows distinct features of sequential neutron decay via intermediate states in Be13. This demonstrates that the increasing availability of energetic beams of exotic nuclei opens up new vistas for experiments leading towards a new understanding of the interplay between bound and continuum states. © 2013 American Physical Society.

    • A complete kinematics study of the B-10(He-3,p alpha alpha alpha) and B-11(He-3,d alpha alpha alpha) reactions has been performed to study the multi-particle break-up of C-12 resonances above the triple-alpha threshold. Four-particle coincidence detection gives us complete information on the direction and energy of the individual alpha particles from the decay of C-12, allowing us to extract new information on the structure of C-12 which we shall present in this contribution. We have observed gamma de-excitation of the T=1 15.11 MeV resonance using charged particle detectors, and have constructed Dalitz plots of the individual resonances in C-12 using the complete kinematics information of the alpha particles which come from their break-up.

    • A complete kinematics study of the 10B(3He, pααα) and the 11B(3He, dααα) reactions has been performed to study the multi-particle break-up of 12C resonances above the triple-alpha threshold. The values of energy and widths of some states has been improved, and in states of natural parity partial branches of decay through the ground state of 8Be have been extracted. The influence of the "ghost" of the 8Be ground state has been taken into account in order to clarify the partial branches.

  • Halo excitations in fragmentation of He-6 at 240 MeV/u on carbon and lead targets

    • Dissociation of a 240 MeV/u beam of He-6, incident on carbon and lead targets, has been studied in kinematically complete experiments to investigate low-lying excitation modes in the halo nucleus He-6. It is shown that alignment effects characterize the inelastic scattering and allow an unambiguous assignment of the spin of a narrow resonance observed in the excitation energy spectrum. The differential cross sections for the He-6 inelastic scattering on carbon and lead targets were deduced from the measured moments of the two neutrons and the a-particle. An analysis of these distributions shows that quadrupole and, possibly, monopole excitations characterize the hadronic interaction, while the dipole mode is dominating in Coulomb dissociation. Neither theoretically predicted new resonance states in He-6 nor nuclear excitation of a dipole mode were found. Direct evidence has been obtained for strong suppression of Coulornb post-acceleration in direct Coulomb breakup in a lead target.

  • Invariant mass spectrum and alpha-n correlation function studied in the fragmentation of He-6 on a carbon target

    • Momentum distributions and invariant mass spectra from the breakup of He-6 ions with an energy of 240 MeV/u interacting with a carbon target have been studied. The data were used to extract information about the reaction mechanism which is influenced by the structure of He-6. It is found that the dominant reaction mechanism is a two-step process: knock out of one neutron followed by the decay of the He-5 resonance. The shape of the (alpha+n) two-body invariant mass spectrum is interpreted as mainly reflecting the 5He ground state which is a J(pi) = 3/2(-) resonance. However, no evidence for correlations between cu particles and neutrons is observed in the momentum widths of the distributions. It is demonstrated that a combined analysis of the two-body invariant mass spectrum and an appropriate correlation function may be used to determine the properties of the intermediate resonance.   • Performance of simultaneous emission-transmission systems for attenuation-corrected SPEct: a method for validation applied to two camera systems

    • Several commercially available systems for attenuation correction in single photon emission computed tomography (SPECT) based on a transmission scan have been introduced that vary in performance. A test procedure for attenuation correction in SPECT is described and applied to two principally different gamma camera systems (the Siemens Multispect 3 triple-headed system [3HS] and the ADAC Genesys Vertex double-headed system [2HS]). The test procedure was based on geometrically well-defined phantoms. A torso phantom was used to illustrate the attenuation correction methods. The test procedure can be used without detailed knowledge of or access to the algorithms used for attenuation correction. The influence on the transmission measurement of radioactivity in a phantom was higher for the 2HS than for the 3HS. The 3HS produced satisfactory attenuation maps and corrected emission count rates to a constant value independent of phantom density and size. With the 2HS, there was a progressive decrease in the correction of emission count rates with increasing phantom density, and about 30% lower corrected count rates in the large compared with the small phantom. A decrease in measured attenuation coefficients in the vicinity of an emission source was demonstrated in large but not small phantoms. A likely explanation is erroneous correction of downscatter into the transmission energy window. This study demonstrates the need for independent evaluation of systems for attenuation correction in SPECT.

    • A method for measuring attenuation coefficients in single-photon emission tomography (SPECT) is described and evaluated, together with a method for attenuation correction using these measured attenuation coefficients. Build-up, caused by scattered photons, is corrected for by a simple substitution in the algorithms. Transmission studies are performed with a 99mTc- or 57Co flood source, and emission phantom studies with 99mTc line sources. The method is evaluated with variable but well-defined phantoms. The result is accurate attenuation coefficients for different densities, dimensions and geometries, and an accuracy of corrected emission activities of better than +/- 10% in most cases. The present limitations of the method for attenuation correction are discussed.
