    • The Metalock method is a mechanical joining technique most commonly employed in cracked castings of iron, aluminum and steel. The marine diesel engine designer MAN Diesel & Turbo uses the Metalock method for crack patching in large cast iron components. The service life of these components can thereby be extended, reducing both replacement costs and environmental impact. The purpose of this study is to investigate the mechanical behavior of the Metalock method. This was studied by full scale testing, using a non-contact deformation measurement technique known as digital image correlation (DIC), of cast iron specimens joined together by the Metalock method. Furthermore, finite element (FE) simulations were performed, and verified by experimental results, in order to study the mechanical behavior in detail and to carry out a parametric study on some of the components included in the joint. Experimental tests show that using the DIC technique gives a good possibility to verify the strain-field achieved by FE simulations and improving the understanding of the Metalock joining technique.

  • Genetic architecture of tameness in a rat model of animal domestication

    • A common feature of domestic animals is tameness - i.e., they tolerate and are unafraid of human presence and handling. To gain insight into the genetic basis of tameness and aggression, we studied an intercross between two lines of rats (Rattus norvegicus) selected over >60 generations for increased tameness and increased aggression against humans, respectively. We measured 45 traits, including tameness and aggression, anxiety-related traits, organ weights, and levels of serum components in >700 rats from an intercross population. Using 201 genetic markers, we identified two significant quantitative trait loci (QTL) for tameness. These loci overlap with QTL for adrenal gland weight and for anxiety-related traits and are part of a five-locus epistatic network influencing tameness. An additional QTL influences the occurrence of white coat spots, but shows no significant effect on tameness. The loci described here are important starting points for finding the genes that cause tameness in these rats and potentially in domestic animals in general.

    • Even though ammonia is considered to underlie nervous system symptoms of dysfunction during hyperammonemia, lactate, which increases as a metabolic consequence of high ammonia levels, might also be a contributing factor. The data presented here show that NH4Cl (5 mM) mediates astroglial cell swelling, and that treatment with NH4Cl or lactate (25 mM) causes rearrangements of actin filaments and reduces astroglial glutamate uptake capacity. Co-application with BaCl2, which blocks astroglial uptake of NH4+, prevents NH4Cl-mediated cell swelling and rearrangement of actin filaments, but does not reduce NH4Cl-induced glutamate uptake capacity inhibition. Neither NH4Cl nor lactate affected glutamate uptake or protein expression in microglial cultures, indicating that astroglial cells are more susceptible to the neurotoxic affects of ammonia. Our results suggest that ammonium underlies brain edema, but that lactate can contribute to some of the cellular dysfunctions associated with elevated cerebral levels of ammonia.

  • Long-Term Function of Single-Implant Restorations: A 17-to 19-Year Follow-Up Study on Implant Infraposition Related to the Shape of the Face and Patients' Satisfaction

    • Background: Various levels of infraposition of single-implant restorations have been observed in long-term follow-up studies, but little knowledge is available on the biological mechanism behind this pattern. Purpose: The primary aim of this study is to report the frequency and severeness of implant infraposition in the anterior single-implant application after 17 to 19 years in function and, secondly, to try to relate these observations to anatomical appearance of the shape of the face of the patient. Materials and Methods: The present study comprised of 57 patients who were provided with 65 CeraOne (TM) single-tooth restorations (Nobel Biocare AB, Gothenburg, Sweden) between 1989 and 1991. Altogether 46 of these patients were treated with single implants in the anterior region. Besides clinical and radiographic data, clinical photographs, study casts, and patient's assessment of the long-term aesthetic result (visual analog scale) was collected at the termination of the present study. The degree of implant crown infraposition was related to assessed facial shape and to patient and clinical assessment of the aesthetic result by means of Pearson's correlation test. To increase the numbers of patients, another group of 25 patients presented in another similar study were pooled with the present material for prevalence calculations. Results: Altogether 47 patients showed up for the final examination after an average of 18 years (82%). Two implants failed (18 years cumulative survival rates [CSR] - 96.8%) and eight original single-crown restorations were replaced (CSR 83.8%). Three of the replaced crowns were replaced because of infraposition of the crowns. About 40% of the patients showed signs of infraposition, similar in younger and older age groups, but more frequently observed in female patients at termination of the study (p < 0.05). There was a weak trend indicating an association between "long-face" appearance and infraposition of the crown restoration (p > 0.05), and patients were more satisfied with the aesthetic clinical result than the participating clinicians (p < 0.05). Conclusion: Single-implant restorations in the anterior upper jaw may present small degrees of infraposition in long-term perspectives. Female patients seem to be at a higher risk of infraposition (p < 0.05), but no clear relationship between age at implant placement or facial shape and degree of infraposition was possible to establish (p > 0.05). Patients were more satisfied with the aesthetic result, as compared with the clinicians (p < 0.05), and patients seemed to pay less attention to the degree of infraposition in their aesthetic assessments, as compared with most of the clinicians.

  • Enterovirus Infections of β-Cells : A Mechanism of Induction of Type 1 Diabetes?

    • The process of β-cell destruction that leads to type 1 diabetes (T1D) is incompletely understood and it is believed to be a result of both genetic and environmental factors. Enterovirus (EV) infections of the β-cells have been proposed to be involved, however, the effects of EV infections on human β-cells have been little investigated. This thesis summarises studies of three different Coxsackie B4 virus strains that have previously been shown to infect human islets. The effects of infections with these EV were studied in vitro in human islets and in a rat insulin-producing cell line. In addition, a pilot study was performed on isolated human islets to investigate the ability to treat such infections with an antiviral compound.It was found that one of the virus strains replicated in human β-cells without affecting their main function for at least seven days, which in vivo may increase a virus’s ability to persist in islets.Nitric oxide was induced by synthetic dsRNA, poly(IC), but not by viral dsRNA in rat insulinoma cells in the presence of IFN-γ, suggesting that this mediator is not induced by EV infection in β-cells and that poly(IC) does not mimic an EV infection in this respect.All three virus strains were able to induce production of the T-cell chemoattractant interferon-γ-inducible protein 10 (IP-10) during infection of human islets, suggesting that an EV infection of the islets might trigger insulitis in vivo.Antiviral treatment was feasible in human islets, but one strain was resistant to the antiviral compound used in this study.To conclude, a potential mechanism is suggested for the involvement of EV infections in T1D. If EV infections induce IP-10 production in human islet cells in vivo, they might recruit immune cells to the islets. Together with viral persistence and/or virus-induced β-cell damage, this might trigger further immune-mediated β-cell destruction in vivo.

    • ABSTRACT Background: Knowledge on long-term clinical performance of more than 5 years on the single-implant CeraOne (Nobel Biocare AB, Gothenburg, Sweden) concept is limited. Purpose: The aim of this study is to report the long-term clinical performance of the first CeraOne single-implant restorations, installed 17 to 19 years ago. Materials and Methods: The group comprised 57 patients provided with 65 CeraOne single-tooth restorations. Sixty-two all-ceramic and three metal-ceramic crowns were cemented between 1989 and 1991. Patients were followed up clinically and with intraoral radiographs at placement, after 1, 5, and between 17 and 19 years after placement. Results: Data were available for altogether 48 patients, followed up on an average time of 18 years. Excluding deceased patients (n = 2) and failed implant patients (n = 2), only five patients were lost to follow-up (8.8%). Two implants failed, resulting in an 18-year implant cumulative success rate (CSR) of 96.8%, and altogether eight original single-crown restorations were replaced (CSR 83.8%). The most common reason for crown replacement was infra-position of the implant crown (n = 3). Many of the remaining original crowns showed various signs of implant crown infraposition at the termination of the study. In general, the soft tissue at the restorations was assessed to be healthy and comparable with the gingiva at the adjacent natural teeth. Bone levels were on an average stable with only few patients exhibiting bone loss of more than 2 mm during 18 years in function. Conclusion: This long-term follow-up study of single-implant restorations shows encouraging results with few implant failures and minimal bone loss over an 18-year period. Original single-crown restorations were replaced more frequently, because of, for example, implant crown infraposition and veneer fractures. The CeraOne concept proved to be a highly predictable and safe prosthodontic treatment.

    • Acceptance and commitment therapy (ACT) interventions for persons with chronic pain have recently received empirical support. ACT focuses on reducing the disabling influences of pain through targeting ineffective control strategies and teaches people to stay in contact with unpleasant emotions, sensations, and thoughts. The aim of the present study was to investigate the effect of a guided internet-delivered ACT intervention for persons with chronic pain. A total of 76 patients with chronic pain were included in the study and randomized to either treatment for 7 weeks or to a control group that participated in a moderated online discussion forum. Intent-to-treat analyses showed significant increases regarding activity engagement and pain willingness. Measurements were provided with the primary outcome variable Chronic Pain Acceptance Questionnaire which was in favour of the treatment group. Reductions were found on other measures of pain-related distress, anxiety and depressive symptoms. A six month follow-up showed maintenance of improvements. We conclude that an acceptance based internet-delivered treatment can be effective for persons with chronic pain.

    • Background Chronic pain can be treated with cognitive behavioural therapy delivered in multidisciplinary settings. However, relapse is likely, and there is a need for cost-effective secondary interventions for persons with residual problems after rehabilitation. The aim of the present study was to investigate the effects of a guided Internet-delivered cognitive behavioural intervention for patients who had completed multidisciplinary treatment at a pain management unit. Methods A total of 72 persons with residual pain problems were included in the study and were randomized to either treatment for 8 weeks or to a control group who were invited to participate in a moderated online discussion forum. The participants had different chronic pain conditions, and a majority were women (72%). Twenty-two percent of the participants dropped out of the study before the post-treatment assessment. Results Intent-to-treat analyses demonstrated differences on the catastrophizing subscale of the Coping Strategies Questionnaire (Cohens d=0.70), in favour of the treatment group but a small within-group effect. Differences were also found on other measures of pain-related distress, anxiety and depressive symptoms. A 6-month follow-up exhibited maintenance of improvements. Conclusions We conclude that Internet-delivered treatment can be partly effective for persons with residual problems after completed pain rehabilitation.

    • Depression and anxiety are commonly seen in patients with chronic pain which affects the patient´s daily life functioning. Although considerable attention has been devoted to explain why depression and anxiety are frequent comorbid with chronic pain, little empirical work has been conducted on interventions that target depression and anxiety and chronic pain. The present study was designed to test an individualized cognitive-behavioral treatment delivered through the internet for persons with chronic pain and emotional distress. A total of 52 patients with chronic pain and depression were included and randomized to either treatment for 8 weeks or to a control group that participated in a moderated online discussion forum. Intent-to-treat analyses showed significant decreases regarding depressive symptoms and pain disability in the treatment group. Results on the primary outcomes of depression and anxiety were in favour of the treatment group. Reductions were also found on pain catastrophizing. One year follow-up showed maintenance of improvements. We conclude that an individualized guided internet-delivered treatment based on cognitive behaviour therapy can be effective for persons with chronic pain comorbid emotional distress.

  • Internet interventions for chronic pain including headache: A systematic review

    • Chronic pain is a major health problem and behavioral based treatments have been shown to be effective. However, the availability of these kinds of treatments is scarce and internet-based treatments have been shown to be promising in this area. The objective of the present systematic review is to evaluate internet-based interventions for persons with chronic pain. The specific aims are to do an updated review with a broad inclusion of different chronic pain diagnoses and to assess disability and pain and also measures of catastrophizing, depression and anxiety. A systematic search identified 891 studies and 22 trials were selected as eligible for review. Two of the selected trials included children/youth and five included individuals with chronic headache and/or migraine. The most frequently measured domain reflected in the primary outcomes was interference/disability, followed by catastrophizing. Result across the studies showed a number of beneficial effects. Twelve trials reported significant effects on disability/interference outcomes and pain intensity. Positive effects were also found on psychological variable such as catastrophizing, depression and anxiety. Several studies (n = 12) were assessed to have an unclear level of risk bias. The attrition levels ranged from 4% to 54% where the headache trials had the highest drop-out levels. However, findings suggest that internet-based treatments based on cognitive behavioural therapy (CBT) are efficacious measured with different outcome variables. Results are in line with trials in clinical settings. Meta-analytic statistics were calculated for interference/disability, pain intensity, catastrophizing and mood ratings. Results showed that the effect size for interference/disability was Hedge's g = − 0.39, for pain intensity Hedge's g = − 0.33, for catastrophizing Hedge's g = − 0.49 and for mood variables (depression) Hedge's g = − 0.26.

  • Evaluation of the first agrivoltaic system in Sweden

    • Photovoltaic (PV) systems in Sweden have primarily been seen as an energy efficiency measure to reduce the amount of purchased electricity for buildings, both residential and commercial. Only recently utility-scale solar systems have begun to increase their share of the solar market to support national energy and emissions targets. Due to the economies of scale, conventional ground-mounted PV (CGMPV) installations represent the best solution for producing electricity at the lowest specific initial investment costs. This relatively new solar market segment, with large-scale ground-mounted solar farms on agricultural land, has faced several challenges with the permitting process. Agricultural land that is suitable for cultivation is of "national importance" according to the Swedish Environmental Code. Cultivable agricultural land may be exploited for other purposes on a permanent basis only if it is necessary to satisfy essential societal interests and there is no other possible land to use within the area in question. Traditionally, ground-mounted solar farms have increased competition for land resources for food production and drawn criticism in the so-called "food-versus-fuel (electricity)" debate over whether agricultural land should be used for electricity generation or food production. Agrivoltaic (APV) systems represent an intelligent solution to avoid land use competition by combining arable farming and electricity production on the same agricultural land. The main objective of this project was to study how APV systems perform from an energy, agricultural and economic perspective compared to CGMPV systems and agriculture production. The project aimed to highlight advantages and disadvantages of APV systems at northern latitudes with an energy-food-water perspective. The aim was pursued by establishing an APV test site, the first APV system in Sweden, monitoring its performance both from an energy and agricultural point of view, and developing new techno-economic models. Data from the APV test site were used to better understand how APV systems at northern latitudes affect: 1) the efficiency of the solar modules; 2) crop productivity, and 3) the financial return for ground-based solar PV systems. The first agrivoltaic system in Sweden has been built on a permanent ley grass field, at Kärrbo Prästgård, Västerås, and research activities have been carried out on the ley grass during 2021 and 2022. As in previous research studies in other countries, we defined three sub-fields: 1) a sub-field is covered only by the ley grass (reference area), 2) a sub-field is a CGMPV system 11.8 kWp solar PV system with two rows of solar modules with a 30° tilt and 3) the last subfield is a  22.8 kWp APV system with three rows of vertically mounted solar modules, with ley grass between the modules. This field set-up allowed for comparisons between practices (agriculture and electricity generation) and technologies (CGMPV systems versus APV systems). The calculated specific electricity production during a typical meteorological year for the APV system and the CGMPV system was 1,067 kWh/kWp/year and 1,116 kWh/kWp/year, respectively. Nevertheless, the APV system tends to have higher efficiency than the CGMPV systems due to the solar irradiation patterns on the solar cell surfaces and wind cooling of the PV modules. The main results of the project in terms of shadow effects on the ley grass showed that the APV system did not significantly affect the productivity of the forage grass in 2021-2022. There was no statistically significant difference between the yield of the samples taken in the APV system and the reference area. Even so, the yield per hectare is reduced by approximatively 10%, when the distance between the vertically mounted solar modules is 10 meters, due to the area under the solar modules that cannot be mechanically harvested. The measurements performed at the test site allowed us to validate the earlier developed model for both electricity production and the effects of shading on crop production. Having a model to assess crop yields under APV systems is of utmost importance to be able to pre-assess the system's effects on food production, which is one of the main goals of APV system regulations worldwide. From an economic perspective, APV systems cannot compete with CGMPV systems due to lower electricity production per hectare, lower density of the solar modules per hectare, and higher investment costs per hectare. Nevertheless, APV systems can be the solution to overcome the legal obstacles that prohibit or hinder the use of agricultural land for electricity generation with PV systems.

  • Very low risk of short bowel after Roux-en-Y gastric bypass : a large nationwide Swedish cohort study

    • BACKGROUND: Roux-en-Y gastric bypass (RYGB) is one of the most common bariatric procedures. Internal herniation may lead to small bowel ischemia requiring small bowel resection, resulting in short bowel syndrome. OBJECTIVE: To determine the incidence of extensive small bowel resection in patients operated with RYGB. We also aimed to look for early clinical warning signs among patients requiring extensive small bowel resection.SETTING: Cohort from national quality registers.METHODS: All patients having undergone RYGB between January 2007 to June 2019 were analyzed in the Scandinavian Obesity Surgery Registry (SOReg). We identified patients with small bowel obstruction (SBO) for whom small bowel resection was necessary. Additionally, we assessed clinical signs in these patients.RESULTS: The study included 57,255 patients having undergone RYGB. Closure of the mesenteric openings was performed in 78%. Surgery for SBO was required in 3659 (6%) of patients, and small bowel resection in 188 (.3%). Extensive small bowel resection, resulting in less than 1.5 meters of remaining small bowel, was required in 7 patients (.01%). All patients with extensive small bowel resection presented with abdominal pain and had confirmed internal herniation as the cause of the small bowel resection, and 2 of 7 patients died. Closure of mesenteric defects was not associated with a reduction in overall small bowel resection rates (P = .89)CONCLUSION: Surgery for SBO after RYGB was common (6%). The risk of extensive small bowel resection leading to short bowel was low (.01%). Patients with abdominal pain after RYGB should be assessed for internal hernia, as it can be devastating.

To elude on this positron emission tomography combined with computed tomography (PET/CT) was performed for 60 min during islet transplantation in five patients receiving six transplants. A fraction of the islets (23%) were labeled with 18F-fluorodeoxyglucose ([(18)F]FDG) and carefully mixed with unlabeled islets just prior to intraportal transplantation. The peak radioactivity concentration in the liver was found at 19 min after start of islet infusion and corresponded to only 75% of what was expected, indicating that islets are lost during the transplantation procedure. No accumulation of radioactivity was found in the lungs. A nonphysiological peak of C-peptide was found in plasma during and immediately after transplantation in all subjects. Distribution in the liver was heterogeneous with wide variations in location and concentration. Islets found in areas with concentrations of >400 IEQ/cc liver tissue varied between 1% and 32% of the graft in different subjects. No side effects attributed to the PET/CT procedure were found. Clinical outcome in all patients was comparable to that previously observed indicating that the [(18)F]FDG labeling procedure did not harm the islets. The technique has potential to be used to assess approaches to enhance islet survival and engraftment in clinical transplantation.

    • A rotating measurement system has been designed and commissioned for a cold flow test turbine and tested under the influence of partial admission. A shrouded turbine rotor of impulse design is equipped with miniature pressure transducers and strain gauges. This paper discusses the selected experimental design and procedure. Overall, the first test runs went well and necessary data were collected and could be evaluated accordingly. Encountered specific measurement technique problems are addressed where the importance of high redundancy is stressed. Results demonstrate one effect that imbedded sensor technology may encounter as regards of dynamic measurements and calibrations.

    • Comorbid psychological problems are commonly related to chronic pain but addressing heterogeneous comorbidities in traditional settings is often difficult. Delivering individually tailored treatment using the internet could be a viable alternative. The present study investigates whether a guided, individually tailored and internet-delivered cognitive behavioral therapy (ICBT) could improve mood and reduce disability in individuals suffering from chronic pain and comorbid psychological distress. Participants were recruited from a pain clinic and randomized to either ICBT or waiting list. The participants (n = 187) individually tailored treatments included 6-13 modules targeting different types of psychological distress. Modules were designed to be completed weekly, and feedback was provided by clinicians. Participants completed an average of 5.1 (49.7%) modules, with 22.9% completing all assigned modules. Intention-to-treat analyses showed significantly larger improvements in depression, disability, pain acceptance, catastrophizing, and quality of life in the ICBT-group compared to the control group. Between-group effect sizes were very small or small at post for the primary outcomes depression (d = 0.18) and pain interference (d = 0.22). Other effect sizes ranged from very small to small, with the largest effect being improvements in pain acceptance (d = 0.3). All significant changes were stable at 12-month follow up.

    • PURPOSE: This study aimed to evaluate and relate visual function, ocular dimensions and neuropaediatric findings in adoptees from Eastern Europe. METHODS: We studied 72 of 99 children, born during 1990-95 and adopted from Eastern Europe to western Sweden during 1993-97. The children (mean age 7.5 years, range 4.8-10.5 years; 41 boys, 31 girls) were examined after a mean period of 5 years post-adoption by a multidisciplinary team. Correlations between ophthalmological findings and neuropaediatric data were analysed. RESULTS: Bivariate and regression analyses indicate a significant positive correlation between visual acuity (VA) and perceptual organization (p < 0.001), as well as between strabismus and verbal comprehension (p < 0.02). Fetal alcohol syndrome (FAS) was correlated with low VA (p < 0.02), subnormal stereovision (p < 0.009) and small optic discs (p < 0.02). Small head circumference was related to low VA (p < 0.015) and small optic discs (p < 0.03). Furthermore, small optic discs were related to low birthweight (p < 0.005) and preterm birth (p < 0.01). Large optic cups were correlated with poorer perceptual organization (p < 0.02). CONCLUSIONS: In this group of adoptees from Eastern Europe, ophthalmological findings were correlated to neuropaediatric findings, especially those arising from prenatal adverse events resulting in growth deficiency and central nervous system damage. Therefore, it is important and valuable with an ophthalmological examination in children adopted from Eastern Europe.

  • Advanced simulation of a 750 t/d municipal solid waste grate boiler to better accommodate feedstock changes due to waste classification

    • To better accommodate changes in municipal solid waste (MSW) properties due to waste classification, advanced computational fluid dynamics (CFD) simulations are carried out for a 750 t/d MSW moving-grate boiler. A moving-grate bed model is developed and iteratively coupled to the freeboard simulation which is performed in Ansys Fluent. The model is first validated by the measurement data for the daily operation case incinerating current feedstock. Then, the model is deployed to investigate the impacts of feedstock change and adjust boiler operation for better accommodating the new MSW. The results indicate incineration of the new MSW leads to irrational utilization of oxygen, non-uniform temperature distribution and low mixing, while maintaining current operation conditions. Subsequently, adjustments of air supply and thermal input are proposed and conducted by the model, which address the potential issues and benefit boiler operation and energy recycling. Finally, the uniformity of velocity and turbulent kinetic energy which indicates mixing, are compared for different cases. The latter is increased by 51.39% and 81.04% after the adjustments of air supply and thermal input. The investigation provides solid references for incinerating new MSW in the current boiler.

    • Assessing forced response is crucial during the design phase of turbomachines. Since the analyses are computationally expensive and time-consuming when using full models, Reduced Order Models (ROM) are utilized to decrease the number of Degrees Of Freedom (DOF) and consequently analysis time and cost. The ROM used in the current analysis belongs to the component mode synthesis (CMS) method with a free-interface approach known as Craig-Chang. A transonic high pressure turbine is investigated featuring large ranges of disk and blade dominated modes depending in the Nodal Diameter (ND). The free-interface approach will be assessed in the disk and blade dominated regions with a detailed study of the frequencies and mode shapes. In addition, a forced response analysis within the blade dominated region is evaluated in the paper. Moreover, a study of the amount of modes required in the basis for the reduced order transformation matrix is presented.

    • Syncytiotrophoblast and invasive extravillous trophoblast arise from a common stem cell, namely villous cytotrophoblast, but have very different characteristics. The study of the differentiation process relies on the availability of suitable markers for these different cell types of developing placenta. In this work, we have produced monoclonal antibodies that are specific to human villous cytotrophoblast. Monoclonal antibody (MAb) MG2 was specific to villous cytotrophoblast across gestation,                     and recognizes hepatocyte growth factor activator inhibitor type 1. MAb MD10 stained villous cytotrophoblast across gestation and also some endothelial cells, particularly in the second or third trimester. MAb MD10 recognizes human integrin α6β4. As a test for specificity, the novel MAbs were also used for staining of frozen tissue from human colon carcinoma. The results show that the two antibodies can be used as tools to study human villous cytotrophoblasts and also human tumors. The MG2 antibody seems most specific and promising for the study of various aspects of human villous cytotrophoblast.

    • Background: Laparoscopic gastric bypass (LGBP) is a well-documented surgical intervention for severe obesity. Recently, laparoscopic sleeve gastrectomy (LSG) has gained increased popularity. Short-term follow-up in limited-sized randomized trials comparing LGBP and LSG show no major differences in weight-loss, adverse events, or effect on comorbidities; however, there is a lack of sufficiently powered, pragmatic, randomized controlled trials comparing the mid- and long-term results of the two methods. Method: BEST is a randomized, registry-based, multicenter trial comparing LGBP and LSG. The trial has two primary outcomes; rates of substantial complications (SC) and total body weight loss. We hypothesize that patients treated with LSG will experience 35% fewer substantial complications during the 5-year follow-up compared to patients treated with LGBP, and that the efficacy of LSG will remain within a non-inferiority margin of 5% in terms of weight loss. Our sample size calculation, using data from the Scandinavian Obesity Surgery Registry (SOReg), shows a power of 80% for SC and > 95% for weight loss at p < .025 with a total of 2100 included patients. The design of the trial will also enable comparisons within several relevant patient subgroups. Conclusions: As a large-sized, pragmatic, randomized trial, BEST will provide robust data comparing LGBP with LSG by generating long-term results on weight loss and SC's, as well as secondary outcomes and comparisons within patient subgroups. The use of a well-established registry for registration of all data facilitates a large multicenter trial, and combines the strengths of registry studies with those of a randomized trial.   • Comparison of Sleeve Gastrectomy vs Roux-en-Y Gastric Bypass : A Randomized Clinical Trial

    • IMPORTANCE: Laparoscopic sleeve gastrectomy (SG) and laparoscopic Roux-en-Y gastric bypass (RYGB) are widely used bariatric procedures for which comparative efficacy and safety remain unclear.OBJECTIVE: To compare perioperative outcomes in SG and RYGB.DESIGN, SETTING, AND PARTICIPANTS: In this registry-based, multicenter randomized clinical trial (Bypass Equipoise Sleeve Trial), baseline and perioperative data for patients undergoing bariatric surgery from October 6, 2015, to March 31, 2022, were analyzed. Patients were from university, regional, county, and private hospitals in Sweden (n = 20) and Norway (n = 3). Adults (aged ≥18 years) eligible for bariatric surgery with body mass indexes (BMIs; calculated as weight in kilograms divided by height in meters squared) of 35 to 50 were studied.INTERVENTIONS: Laparoscopic SG or RYGB.MAIN OUTCOMES AND MEASURES: Perioperative complications were analyzed as all adverse events and serious adverse events (Clavien-Dindo grade >IIIb). Ninety-day mortality was also assessed.RESULTS: A total of 1735 of 14 182 eligible patients (12%; 1282 [73.9%] female; mean (SD) age, 42.9 [11.1] years; mean [SD] BMI, 40.8 [3.7]) were included in the study. Patients were randomized and underwent SG (n = 878) or RYGB (n = 857). The mean (SD) operating time was shorter in those undergoing SG vs RYGB (47 [18] vs 68 [25] minutes; P < .001). The median (IQR) postoperative hospital stay was 1 (1-1) day in both groups. The 30-day readmission rate was 3.1% after SG and 4.0% after RYGB (P = .33). There was no 90-day mortality. The 30-day incidence of any adverse event was 40 (4.6%) and 54 (6.3%) in the SG and RYGB groups, respectively (odds ratio, 0.71; 95% CI, 0.47-1.08; P = .11). Corresponding figures for serious adverse events were 15 (1.7%) for the SG group and 23 (2.7%) for the RYGB group (odds ratio, 0.63; 95% CI, 0.33-1.22; P = .19).CONCLUSIONS AND RELEVANCE: This randomized clinical trial of 1735 patients undergoing primary bariatric surgery found that both SG and RYGB were performed with a low perioperative risk without clinically significant differences between groups.TRIAL REGISTRATION: ClinicalTrials.gov Identifier: NCT02767505.

    • BACKGROUND: Neisseria animaloris and Neisseria zoodegmatis, former CDC Group EF-4a and -4b, are considered to be rare zoonotic pathogens, usually associated with dog or cat bites. The aim of the study was to phenotypicaly characterize 13 EF-4 isolates from wound infections, determine their antibiotic susceptibility and to follow the clinical outcome of the patients.METHODS: 13 of the EF-4 isolates were cultured on agar plates. Conventional biochemical tests and the Biolog system were used for phenotypical identification. An arbitrary primed polymerase chain reaction (AP-PCR) was carried out to determine the genetic profiles. Minimum inhibitory concentration (MIC) values were determined for different antibiotics were determined. According to this, clinical data for the patients were recorded.RESULTS: 11 isolates were identified as N. animaloris and 2 as N. zoodegmatis due to the production of arginine dihydrolase. A majority of the patients had a history of dog bite. In 6 cases only grewth of N. animaloris or zoodegmatis was registered. When a patient received antibiotic treatment the most common drug of choice was penicillin V. Only 3 patients received treatment for which the isolated EF-4 bacterium was fully susceptible.CONCLUSION: Human infections involving N. animaloris and N. zoodegmatis usually present themselves as local wound infection, but severe complications can occur. Despite their pathogenic potentia, l N. animaloris and N. zoodegmatis are often misidentified, dismissed as skin contaminants or not recognized at all. Due to the fact that N. animaloris and N. zoodegmatis are significant pathogens in animal bites, physicians should keep these bacteria in mind when choosing antibiotic therapy.

    • Birth weight within the normal range is associated with a variety of adult-onset diseases, but the mechanisms behind these associations are poorly understood. Previous genome-wide association studies of birth weight identified a variant in the ADCY5 gene associated both with birth weight and type 2 diabetes and a second variant, near CCNL1, with no obvious link to adult traits. In an expanded genome-wide association meta-analysis and follow-up study of birth weight (of up to 69,308 individuals of European descent from 43 studies), we have now extended the number of loci associated at genome-wide significance to 7, accounting for a similar proportion of variance as maternal smoking. Five of the loci are known to be associated with other phenotypes: ADCY5 and CDKAL1 with type 2 diabetes, ADRB1 with adult blood pressure and HMGA2 and LCORL with adult height. Our findings highlight genetic links between fetal growth and postnatal growth and metabolism.

    • Neuropathic pain affects 1-10% of the general population and is caused by a lesion or disease of the somatosensory nervous system. Spinal cord stimulation (SCS), a method where implanted electrodes stimulate the spinal cord, has been successfully used to treat drug-resistant neuropathic pain, but the mechanism of action is largely unknown. Studies show that SCS changes the protein levels in CSF (cerebrospinal fluid) of pain patients. Several neurological conditions have been shown to alter the elemental composition of CSF. Therefore changes in the levels of ions and trace elements in the CSF may correspond to SCS use. This study used ICP-MS (Inductively coupled plasma mass spectrometry) and ICP-AES (Inductively coupled plasma atomic emission spectroscopy) to quantify 10 elements in CSF from chronic neuropathic pain patients using SCS. The element concentrations in CSF from patients with SCS treatment on/off, were measured. No effect on the element concentrations in CSF from treatment with SCS could be detected. Also, the elemental concentrations in pooled CSF from patients without chronic neuropathic pain was determined and compared to the patients using SCS. The concentration of the elements Ca, Sr, Na, K, P, Mg and Ti were, significantly higher in patients compared to the CSF-control.

  • Perception of cockpit environment among pilots on commercial aircraft

    • Impaired cockpit environment may influence both well-being and performance of pilots. Objective: To study the perception of cockpit environment among pilots, in relation to demographic factors, and type of air craft (B767-300, B737-600, DC9/21-41, MD 81/90 series). Methods: A standardized questionnaire was mailed to all pilots in one airline company; 81% participated (n = 622). All flights were non-smoking flights and the B767 was the only aircraft operated on intercontinental flights. The DC9 was the only aircraft without air recirculation. Multiple logistic regression analysis was applied, controlling for age, gender, smoking, perceived psychosocial work environment, and type of aircraft. Results: Younger age and a history of atopy and stress due to excess work were the main predictors of symptom and environmental perceptions. The most common symptoms were fatigue (14%), facial dermal (10%), and nasal symptoms (9%). Common complaints on cockpit environment were dry air (53%), dust and dirt (48%), noise (46%), and inadequate illumination (34%). Using the DC9 as a reference category, Boeing 767 pilots had more fatigue (OR 19.5; p < 0.001), throat symptoms (OR = 4.40; p < 0.05), complaints on dry air (OR = 2.93; p < 0.01), stuffy air (OR = 4.60; p < 0.01), static electricity (OR = 6.39; < 0.05), and dust (OR = 2.01; p < 0.05). Boeing 737 pilots had more complaints on noise (OR = 4.01; p < 0.001) and dust (OR = 1.81; p < 0.05). MD 81/90 pilots had more complaints on dry air (OR = 1.76; p < 0.05), dust (OR = 1.92; p < 0.05), and inadequate illumination (OR = 2.08; p < 0.05). Conclusion: Complaints on the cockpit environment were common and differed between different types of aircraft. This indicates a need to optimize the cockpit environment, e.g., increase the cleaning and relative air humidity.

    • Numerical methods, both Computational Fluid Dynamics (CFD) as well as Finite Elements (FE) methods, are widely used in industry with the purpose of predicting potential fatigue problems early in the design process. However, the uncertainty of such predictions is not clearly identified. The present paper presents the prediction of the vibration response of a rotor blisk part of 1 1/2 transonic compressor stage with comparison with experiments. Different uncertainty sources along the numerical aeromechanical chain are then identified. CFD solvers are employed for the prediction of both blade row interaction forces as well as the aerodynamic damping determination. Mistuning is assessed by the use of Reduced Order Modeling analyses and results compared with tip timing data. The peak amplitude response of a resonance mode of interest is determined for two different inlet conditions and thus the accuracy dependence on the excitation level is discussed. Results show that the largest uncertainties come from the unsteady aerodynamics, in which both aerodynamic damping and forcing estimations are critical. The structural dynamic models seem to capture the vibration response and mistuning effects well. Additionally, the challenges of tip timing data processing for detailed one-to-one validation of the tools are highlighted.
