    • Lithium is regarded as the first-line treatment for bipolar disorder (BD), a severe and disabling mental disorder that affects about 1% of the population worldwide. Nevertheless, lithium is not consistently effective, with only 30% of patients showing a favorable response to treatment. To provide personalized treatment options for bipolar patients, it is essential to identify prediction biomarkers such as polygenic scores. In this study, we developed a polygenic score for lithium treatment response (Li+PGS) in patients with BD. To gain further insights into lithium's possible molecular mechanism of action, we performed a genome-wide gene-based analysis. Using polygenic score modeling, via methods incorporating Bayesian regression and continuous shrinkage priors, Li+PGS was developed in the International Consortium of Lithium Genetics cohort (ConLi+Gen: N=2,367) and replicated in the combined PsyCourse (N=89) and BipoLife (N=102) studies. The associations of Li+PGS and lithium treatment response - defined in a continuous ALDA scale and a categorical outcome (good response vs. poor response) were tested using regression models, each adjusted for the covariates: age, sex, and the first four genetic principal components. Li+PGS was positively associated with lithium treatment response in the ConLi+Gen cohort, in both the categorical (P=9.8×10-12, R2=1.9%) and continuous (P=6.4×10-9, R2=2.6%) outcomes. Compared to bipolar patients in the 1st decile of the risk distribution, individuals in the 10th decile had 3.47-fold (95%CI: 2.22-5.47) higher odds of responding favorably to lithium. The results were replicated in the independent cohorts for the categorical treatment outcome (P=3.9×10-4, R2=0.9%), but not for the continuous outcome (P=0.13). Gene-based analyses revealed 36 candidate genes that are enriched in biological pathways controlled by glutamate and acetylcholine. Li+PGS may be useful in the development of pharmacogenomic testing strategies by enabling a classification of bipolar patients according to their response to treatment.

    • Collections of human postmortem brains gathered in brain banks have underpinned many significant developments in the understanding of central nervous system (CNS) disorders and continue to support current research. Unfortunately, the worldwide decline in postmortem examinations has had an adverse effect on research tissue procurement, particularly from control cases (non-diseased brains). Recruitment to brain donor programmes partially addresses this problem and has been successful for dementing and neurodegenerative conditions. However, the collection of brains from control subjects, particularly from younger individuals, and from CNS disorders of sudden onset, remains a problem. Brain banks need to adopt additional strategies to circumvent such shortages. The establishment of brain bank networks allows data on, and access to, control cases and unusual CNS disorders to be shared, providing a larger resource for potential users. For the brain banks themselves, inclusion in a network fosters the sharing of protocols and development of best practice and quality control. One aspect of this collective experience concerns brain bank management, excellence in which is a prerequisite not only for gaining the trust of potential donors and of society in general, but also for ensuring equitable distribution to researchers of high quality tissue samples. This review addresses the legal, ethical and governance issues, tissue quality, and health and safety aspects of brain bank management and data management in a network, as well as the needs of users, brain bank staffing, donor programs, funding issues and public relations. Recent developments in research methodology present new opportunities for researchers who use brain tissue samples, but will require brain banks to adopt more complex protocols for tissue collection, preparation and storage, with inevitable cost implications for the future.

  • The resistome and microbiome of wastewater treatment plant workers - The AWARE study.

    • Urban wastewater treatment plants harbor a large collection of antibiotic resistant enteric bacteria. It is therefore reasonable to hypothesize that workers at such plants would possess a more diverse set of resistant enteric bacteria, compared to the general population. To address this hypothesis, we have compared the fecal microbiome and resistome of 87 workers at wastewater treatment plants (WWTPs) from Romania and the Netherlands to those of 87 control individuals, using shotgun metagenomics. Controlling for potential confounders, neither the total antibiotic resistance gene (ARG) abundance, nor the overall bacterial composition were significantly different between the two groups. If anything, the ARG richness was slightly lower in WWTP workers, and in a stratified analysis the total ARG abundance was significantly lower in Dutch workers compared to Dutch control participants. We identified country of residence, together with recent antibiotic intake in the Dutch population, as the largest contributing factors to the total abundance of ARGs. A striking side-finding was that sex was associated with carriage of disinfectant resistance genes, with women in both Romania and the Netherlands having significantly higher abundance compared to men. A follow up investigation including an additional 313 publicly available samples from healthy individuals from three additional countries showed that the difference was significant for three genes conferring resistance to chemicals commonly used in cosmetics and cleaning products. We therefore hypothesize that the use of cosmetics and, possibly, cleaning products leads to higher abundance of disinfectant resistance genes in the microbiome of the users. Altogether, this study shows that working at a WWTP does not lead to a higher abundance or diversity of ARGs and no large shifts in the overall gut microbial composition in comparison to participants not working at a WWTP. Instead, other factors such as country of residence, recent antibiotic intake and sex seem to play a larger role.

  • Burden of disease attributable to risk factors in European countries: a scoping literature review

    • Objectives: Within the framework of the burden of disease (BoD) approach, disease and injury burden estimates attributable to risk factors are a useful guide for policy formulation and priority setting in disease prevention. Considering the important differences in methods, and their impact on burden estimates, we conducted a scoping literature review to: (1) map the BoD assessments including risk factors performed across Europe; and (2) identify the methodological choices in comparative risk assessment (CRA) and risk assessment methods. Methods: We searched multiple literature databases, including grey literature websites and targeted public health agencies websites. Results: A total of 113 studies were included in the synthesis and further divided into independent BoD assessments (54 studies) and studies linked to the Global Burden of Disease (59 papers). Our results showed that the methods used to perform CRA varied substantially across independent European BoD studies. While there were some methodological choices that were more common than others, we did not observe patterns in terms of country, year or risk factor. Each methodological choice can affect the comparability of estimates between and within countries and/or risk factors, since they might significantly influence the quantification of the attributable burden. From our analysis we observed that the use of CRA was less common for some types of risk factors and outcomes. These included environmental and occupational risk factors, which are more likely to use bottom-up approaches for health outcomes where disease envelopes may not be available. Conclusions: Our review also highlighted misreporting, the lack of uncertainty analysis and the under-investigation of causal relationships in BoD studies. Development and use of guidelines for performing and reporting BoD studies will help understand differences, avoid misinterpretations thus improving comparability among estimates.

  • Free heme and hemopexin in acute kidney injury after cardiopulmonary bypass and transient renal ischemia

    • Free heme is released from hemoproteins during hemolysis or ischemia reperfusion injury and can be pro-inflammatory. Most studies on nephrotoxicity of hemolysis-derived proteins focus on free hemoglobin (fHb) with heme as a prosthetic group. Measurement of heme in its free, non-protein bound, form is challenging and not commonly used in clinical routine diagnostics. In contrast to fHb, the role of free heme in acute kidney injury (AKI) after cardiopulmonary bypass (CPB) surgery is unknown. Using an apo-horseradish peroxidase-based assay, we identified free heme during CPB surgery as predictor of AKI in patients undergoing cardiac valve replacement (n = 37). Free heme levels during CPB surgery correlated with depletion of hemopexin (Hx), a heme scavenger-protein. In mice, the impact of high levels of circulating free heme on the development of AKI following transient renal ischemia and the therapeutic potential of Hx were investigated. C57BL/6 mice were subjected to bilateral renal ischemia/reperfusion injury for 15 min which did not cause AKI. However, additional administration of free heme in this model promoted overt AKI with reduced renal function, increased renal inflammation, and reduced renal perfusion on functional magnetic resonance imaging. Hx treatment attenuated AKI. Free heme administration to sham operated control mice did not cause AKI. In conclusion, free heme is a predictor of AKI in CPB surgery patients and promotes AKI in transient renal ischemia. Depletion of Hx in CPB surgery patients and attenuation of AKI by Hx in the in vivo model encourage further research on Hx therapy in patients with increased free heme levels during CPB surgery.

    • Postmortem human brain tissue is widely used in neuroscience research, but use of tissue originating from different brain bank centers is considered inaccurate because of possible heterogeneity in sample quality. There is thus a need for well-characterized markers to assess the quality of postmortem brain tissue. Toward this aim, we determined tryptophan (TRP) concentrations, phosphofructokinase-1 and glutamate decarboxylase activities in 119 brain tissue samples. These neurochemical parameters were tested in samples from autopsied individuals, including control and pathological cases provided by 10 different brain bank centers. Parameters were assessed for correlation with agonal state, postmortem interval, age and gender, brain region, preservation and freezing methods, storage conditions and storage time, RNA integrity, and tissue pH value. TRP concentrations were elevated significantly (p = 0.045) with increased postmortem interval; which might indicate increased protein degradation. Therefore, TRP concentration might be one useful and convenient marker for estimating the quality of human postmortem brain tissue.

    • Children with lesbian, gay, bisexual, trans* and/or queer (LGBTQ) parents have received heightened attention during the processes of policy making regarding adoption and donor insemination legislation in Sweden during the 1990s and early 2000s. Yet, very little academic knowledge exists about children and young people in LGBTQ families and their experiences in schools. We engage the idea of "culturally queer" as a potentially useful framework for understanding the experiences of children and young people with LGBTQ parents. School, with its major impact on young people's lives, is one of the sites where family as discourse and practise is negotiated. This is why a group of researchers from Humboldt University, Berlin (Germany), University of Ljubljana (Slovenia) and Lund University (Sweden) are undertaking a study on the school experiences of children and young people with LGBTQ parents. Our first aim is to discuss how these kids are recognized within queer communities. Our second aim is to analyze what we see as a problematic silence in school around children and young people with LGBTQ parents. This is about the powerful and everyday workings of normative understandings of who family should be. Schools and teachers might not see the need to actively include non-normative family-structures in their work. This seemingly innocent-looking process in fact re-constructs ideals of family, childhood, sexuality and gender. It is necessary to actively work against the silencing of kids with LGBTQ parents in the context of school. This is important for these kids; it is equally important for children and young people with heterosexual parents to see that family can be more than mum-dad-child. We see this as a part of the long-established debate in queer theorizing and activism about the meaning and practises of 'family'. We see a need to further theorize the currently ongoing normalization of queer families. At the same time, there is an equally ongoing need for struggle against indirect homophobia and transphobia.

Moreover, research with lesbian, gay, bisexual, transgender and queer (LGBTQ) kids is tightly connected to anti-homophobia, anti-transphobia and norm-critical activism. One of the key contentions within this field is what researchers and activists mean by “queer” in the context of education: is it a focus on queer/ed subjectivities? Is it about using queer theories to critique forms and norms of education in a given sociopolitical context? Who is queer/ed in schools? Is the language of homophobia and transphobia the best or even correct way to describe and analyse normative educational settings and frameworks?

    • AIMS: Most brain diseases are complex entities. Although animal models or cell culture experiments mimic some disease aspects, human post mortem brain tissue remains essential to advance our understanding of brain diseases using biochemical and molecular techniques. Post mortem artefacts must be properly understood, standardized, and either eliminated or factored into such experiments. Here we examine the influence of several premortem and post mortem factors on pH, and discuss the role of pH as a biochemical marker for brain tissue quality. METHODS: We assessed brain tissue pH in 339 samples from 116 brains provided by 8 different European and 2 Australian brain bank centres. We correlated brain pH with tissue source, post mortem delay, age, gender, freezing method, storage duration, agonal state and brain ischaemia. RESULTS: Our results revealed that only prolonged agonal state and ischaemic brain damage influenced brain tissue pH next to repeated freeze/thaw cycles. CONCLUSIONS: pH measurement in brain tissue is a good indicator of premortem events in brain tissue and it signals limitations for post mortem investigations.

Here we relate to Butler's analysis and suggestions for thinking about individual lives and society's formation of livability. An important point is that livability refers to something that goes beyond survival.

    • The development of new molecular and neurobiological methods, computer-assisted quantification techniques and neurobiological investigation methods which can be applied to the human brain, all have evoked an increased demand for post-mortem tissue in research. Psychiatric disorders are considered to be of neurobiological origin. Thus far, however, the etiology and pathophysiology of schizophrenia, depression and dementias are not well understood at the cellular and molecular level. The following will outline the consensus of the working group for neuropsychiatric brain banking organized in the Brainnet Europe II, on ethical guidelines for brain banking, clinical diagnostic criteria, the minimal clinical data set of retrospectively analyzed cases as well as neuropathological standard investigations to perform stageing for neurodegenerative disorders in brain tissue. We will list regions of interest for assessments in psychiatric disorder, propose a dissection scheme and describe preservation and storage conditions of tissue. These guidelines may be of value for future implementations of additional neuropsychiatric brain banks world-wide.

    • Att forska med och om ungdomar och i skolan är spännande och skrämmande. När jag forskade om ungdomar i en tysk (och sedan även i en kanadensisk) skola hade jag bestämt mig för att inte komma ut i forskningsprocessen. Vad jag inte hade väntat mig var hur det kändes att återigen befinna sig ’i garderoben’, som jag dessutom kom att reproducera. Det var å andra sidan väldigt produktivt forskningsmässigt. I denna artikel diskuterar jag den spänning som uppstod mellan de forskningsmässiga fördelarna med att undvika att komma ut för de man studerar och problemen med att reproducera heteronormativitet. Jag diskuterar möjligheten, kanske framför allt i ungdoms- och skolforskning, av en queer positionalitet som inte enbart reduceras till sexualitet utan snarare omfattar en kritisk analys av forskarens och forskningsdeltagarens position i samhället. Det handlar inte om en re-essentialisering, utan om att tydliggöra hur specifika positioner o/synliggörs. Anammandet av en queer positionalitet aktualiserar frågan om vilka förväntningar vi som forskare har på våra forskningsdeltagare. I min studie fanns en underliggande förväntan om att speciellt muslimska elever skulle komma att lämna projektet om de fick reda på min sexualitet. Denna problematiska förväntning använder jag som utgångspunkt för en analys av det ömsesidiga beroende som jag, genom konceptet queer positionalitet, försöker förverkliga. Den queera positionaliteten innebär också ett synliggörande av forskarens sexualitet – inte bara queerforskarens – i forskningsprocessen. Slutligen handlar det om hur vi som forskare använder oss av vår egen position som metodologiskt verktyg, och hur vi förstår och analyserar de begränsningar och möjligheten den innebär.

    • Despite extensive policy, regulation, and activism, heteronormativity and cisnormativity, and other forms of oppression, flourish in schools. Based on interviews with educators in schools, queer social movement organizations, school boards and teachers’ unions in Vancouver and Toronto, Canada, this chapter analyzes the strategies of queer educational justice work in schools. Working with Gilbert’s call to do more than ‘being on the right side’ and Rasmussen, Sanjakdar, Allen, Quinlivan, and Bromdal’s complications of how we think responsibility, I argue that it is possible to differentiate three strategies of queer educational justice work: reflexive identity politics, intersectional systems critique, and individual humanism. I also contend that responsibility for queer educational justice work is attached to queer educators through fear. It is necessary to analyze this attachment of responsibility to understand how it can undermine the work of QSM and queer educational justice work in schools.

    • Being trans in different ways is not an emergency and does not have to be dramatic. Instead of reacting only in the moment, politicians, principals, teachers, counselors, nurses and other adults can create a school where fewer spontaneous solutions are needed because there is well thought-out pedagogical and administrative work, what is called the 'whole-of-school approach' (Bartholomaeus & Riggs, 2017a, 2017b; Brömdal et al., 2020). Doing this work is not optional, it is part of the school's mission to create a learning environment free from discrimination, and being prepared saves time and effort.Based on the material from a study with 13 adolescents and young adults discussed in the article "'You really have to change the textbooks to change the society' - gender and gender identity in schools" in this report, material created by activists and advocacy organizations, and the international research, I suggest that schools can and need to change in several areas, and I structure the suggestions according to these areas.

  • Gendered knowledges in schools – trans* young people’s experiences and schools’ strategies

    • What knowledges and understandings of gender are done and producedin schools in Sweden today? Through interviews with trans* young peopleand young adults, this study analyzes schools and adults’ abilities and knowledges to understand and meet the needs of trans* students and to address cisnormative structures in school. The participants’ narratives give a richand complex image of what it can mean to be trans* and go to school inSweden and show that many students take it upon themselves to educatethe adults in their schools about trans*-inclusion.Many participants discuss administrative violence (Spade) and culturalcis-genderism (Kennedy) that affect the mundane aspects of being inschool and the knowledges that are given space and produced. Youngtrans* people can become the bearers and sharers of knowledge aboutwhat it means to be trans* and about embodying abject(ed) identity formations, and about the necessity to manage interlocking oppressions. Theylearn about the workings of un/justice in spaces of knowledge productionand about creating support for themselves and others. On the other hand,there are experiences of embeddedness of trans* as unequivocal part ofsocial and academic knowledge production in schools.Which this presentation, I am interested in following Engel’s suggestionto analyze political struggle and changes as paradoxical, rather than linear,by exploring the paradox that such different experiences can be locatedin one person’s experiences of school, if not in the same school. I wantto argue that we need to strengthen both a rights-based approach and asubject knowledge-based approach to make schools better places beyondcis-normativity.

    • Denna text beskriver hur normer kring kön påverkar elever som är trans på olika sätt, alltså till exempel killar med transerfarenhet, ickebinära och transtjejer. Texten bygger på en intervjustudie med 13 unga och unga vuxna som berättar om sina erfarenheter av att vara trans på olika sätt i skolan. Det är den första studien av sitt slag i en svensk kontext. Deltagarna i studien var mellan 16 och 25 år när vi möttes, och kom från både stora städer och mindre orter i olika delar av landet. De flesta gick fortfarande i skolan. Deltagarnas analyser av skolan lyfter både konkreta situationer och övergripande strukturer där skolan brister, och deltagarna berättar om klassrum och skolor där personal aktivt arbetar på ett medvetet sätt med könsnormer och stödjer könsnormbrytande elever. Utifrån deltagarnas berättelser kommer jag med denna text att beskriva fyra olika typer av skolmiljöer och klassrumssituationer som är mer eller mindre inkluderande för elever som är trans på olika sätt. Texten ”Förändring pågår! Förslag på förändringsarbete kring kön och könsidentitet i skolan” kan läsas som komplement till denna text. Där belyser jag sex områden där enskilda lärare, lärarlag och hela skolor kan arbeta aktivt för en skola där elever som bryter mot könsnormer inte utsätts för exkludering, minoritetsstress och våld.

    • Young people create differentiated models of belonging. Their strategies reflect contexualized competences - the capacity to understand and negotiate the influence of national frameworks in specific situations. Theories that understand belonging as processual and intersectional offer useful frameworks with which to analyse this. This article uses data from empirical research with young people in a German secondary school and a Canadian junior high school to highlight young people's situated competences and their critique of the respective frameworks of belonging.

    • I detta kapitel argumenterar jag för normkritiska och icke-förtryckande sätt att tänka inom undervisning och lärande. Efter en diskussion av argument som förs för och emot antidiskriminerings- och normkritisk arbete i skolan presenterar jag några av normkritiska pedagogikers teoretiska utgångspunkt. Det följs av en överblick över de regler som gäller för icke-diskriminering och inkludering i skollagen och läroplanen och en diskussion av deras begränsningar. Att lära sig om normkritiska och icke-förtryckande pedagogiker kan ge framtida lärare verktyg för analys av såväl sina ämnen som sina arbetsplatser och redskap för solidaritet med både elever och andra lärare samt för motstånd där det behövs. Det kan vara ett sätt att ta på allvar den homogeniteten som lärare finner i sin arbetsvardag. Det är elevernas och studenternas rättighet att få tillgång till metoder att lära och till kunskap som ingår som en del av ett demokratiskt samhälle, lokalt, regionalt och globalt.

    • Policy documents are part of university’s positioning within national and globalized negotiations. Who is written in as belonging to academia, and how do we describe and ascribe belonging? Since 2011 Lund University has an integrated policy for gender equality, equal treatment and diversity work. With this text, I analyze this policy and related documents to understand which meanings are attached to these three concepts and which bodies thess meanings get attached to in the process. I approach this analysis through Carol Bacchi’s analytical approach asking “what is the problem presented to be” as well as through Sara Ahmed’s notion of how meaning is attached, gets “stuck” to certain words, and how terms are attached to certain bodies. I argue that gender equality, equal treatment and diversity work are hierarchized in these texts. I also argue that there is an increase in conceptualizing gender equality, equal treatment and diversity work in terms of profitability and tolerance, both relations that manifest instead of challenge unequal relations of power.

    • Queer studies of education and research with lesbian, gay, bisexual, transgender and queer (LGBTQ) kids, tightly connected to anti-homophobia, anti-transphobia and norm-critical activism, have become a growing field with a range of theoretical and political positions and methodological approaches. One of the key contentions within this field is what researchers and activists mean by “queer” in the context of education: is it a focus on queer/ed subjectivities? Is it about using queer theories to critique forms and norms of education in a given sociopolitical context? Who is queer/ed in schools? Is the language of homophobia and transphobia the best or even correct way to describe and analyse normative educational settings and frameworks? In this issue of Confero, we highlight both ethnographic investigations of queer and queered kids in school and critical views of school’s policy making and normative frameworks. Queer education research is a rapidly growing area of study. Where researchers and activists insist on the entanglements between not least sexual, gendered and racialised structural formations, we also insist on our expectation that principal values in schools meet the increasing challenges from queer activism and research.

    • I analyze the role of gender-sex-sexuality in school policies in Sweden; Canada; and Germany. Policies concerning young people’s negotiations of gender-sex-sexuality in schools draw on specific understandings of societal belonging and cohesion. Invariably such policies are part of national discourses regarding values and norms of gender and relationships; informed by concepts of ethnicized belonging. Furthermore; the analysis of school policy texts allows insights into conceptualizations of young people and their societal role in the context of ongoing neo-liberal reconfigurations of schools and societies.
