  • Ventricular arrhythmias in arrhythmic mitral valve syndrome-a prospective continuous long-term cardiac monitoring study

    • AimsArrhythmic mitral valve syndrome is linked to life-threatening ventricular arrhythmias. The incidence, morphology and methods for risk stratification are not well known. This prospective study aimed to describe the incidence and the morphology of ventricular arrhythmia and propose risk stratification in patients with arrhythmic mitral valve syndrome.MethodsArrhythmic mitral valve syndrome patients were monitored for ventricular tachyarrhythmias by implantable loop recorders (ILR) and secondary preventive implantable cardioverter-defibrillators (ICD). Severe ventricular arrhythmias included ventricular fibrillation, appropriate or aborted ICD therapy, sustained ventricular tachycardia and non-sustained ventricular tachycardia with symptoms of hemodynamic instability.ResultsDuring 3.1 years of follow-up, severe ventricular arrhythmia was recorded in seven (12%) of 60 patients implanted with ILR [first event incidence rate 4% per person-year, 95% confidence interval (CI) 2–9] and in four (20%) of 20 patients with ICD (re-event incidence rate 8% per person-year, 95% CI 3–21). In the ILR group, severe ventricular arrhythmia was associated with frequent premature ventricular complexes, more non-sustained ventricular tachycardias, greater left ventricular diameter and greater posterolateral mitral annular disjunction distance (all P < 0.02).ConclusionsThe yearly incidence of ventricular arrhythmia was high in arrhythmic mitral valve syndrome patients without previous severe arrhythmias using continuous heart rhythm monitoring. The incidence was even higher in patients with secondary preventive ICD. Frequent premature ventricular complexes, non-sustained ventricular tachycardias, greater left ventricular diameter and greater posterolateral mitral annular disjunction distance were predictors of first severe arrhythmic event.

  • A TDD Distributed MIMO Testbed Using a 1-bit Radio-Over-Fiber Fronthaul Architecture

    • We present the uplink and downlink of a time-division duplex distributed multiple-input multiple-output (D-MIMO) testbed, based on a 1-bit radio-over-fiber architecture, which is low cost and scalable. The proposed architecture involves a central unit (CU) that is equipped with 1-bit digital-to-analog and analog-to-digital converters, operating at 10 GS/s. The CU is connected to multiple single-antenna remote radio heads (RRHs) via optical fibers, over which a binary radio frequency (RF) waveform is transmitted. In the uplink, a binary RF waveform is generated at the RRHs by a comparator, whose inputs are the received RF signal and a suitably designed dither signal. In the downlink, a binary RF waveform is generated at the CU via bandpass sigma-delta modulation. Our measurement results show that low error-vector magnitude (EVM) can be achieved in both the uplink and the downlink, despite 1-bit sampling at the CU. Specifically, for point-to-point over-cable transmission between a single user equipment (UE) and a CU equipped with a single RRH, we report, for a 10-MBd signal using single-carrier (SC) 16 quadratic-amplitude modulation (QAM) modulation, an EVM of 3.3% in the downlink, and of 4.5% in the uplink. We then consider a CU connected to three RRHs serving over the air two UEs, and show that, after over-the-air reciprocity calibration, a downlink zero-forcing precoder designed on the basis of uplink channel estimates at the CU achieves an EVM of 6.4% and 10.9% at UE 1 and UE 2, respectively. Finally, we investigate the ability of the proposed architecture to support orthogonal frequency-division multiplexing (OFDM) waveforms, and its robustness against both in-band and out-of-band interference.

  • A human factors intervention in a hospital-evaluating the outcome of a TeamSTEPPS program in a surgical ward

    • Background: Patient safety in hospitals is being jeopardized, since too many patients experience adverse events. Most of these adverse events arise from human factors, such as inefficient teamwork and communication failures, and the incidence of adverse events is greatest in the surgical area. Previous research has shown the effect of team training on patient safety culture and on different areas of teamwork. Limited research has investigated teamwork in surgical wards. The aim of this study was to evaluate the professional and organizational outcomes of a team training intervention among healthcare professionals in a surgical ward after 6 and 12 months. Systems Engineering Initiative for Patient Safety 2.0 was used as a conceptual framework for the study. Methods: This study had a pre-post design with measurements at baseline and after 6 and 12 months of intervention. The intervention was conducted in a urology and gastrointestinal surgery ward in Norway, and the study site was selected based on convenience and the leaders' willingness to participate in the project. Survey data from healthcare professionals were used to evaluate the intervention. The organizational outcomes were measured by the unit-based sections of the Hospital Survey of Patient Safety Culture Questionnaire, and professional outcomes were measured by the TeamSTEPPS Teamwork Perceptions Questionnaire and the Collaboration and Satisfaction about Care Decisions in Teams Questionnaire. A paired t-test, a Wilcoxon signed-rank test, a generalized linear mixed model and linear regression analysis were used to analyze the data. Results: After 6 months, improvements were found in organizational outcomes in two patient safety dimensions. After 12 months, improvements were found in both organizational and professional outcomes, and these improvements occurred in three patient safety culture dimensions and in three teamwork dimensions. Furthermore, the results showed that one of the significant improved teamwork dimensions "Mutual Support" was associated with the Patient Safety Grade, after 12 months of intervention. Conclusion: These results demonstrate that the team training program had effect after 12 months of intervention. Future studies with larger sample sizes and stronger study designs are necessary to examine the causal effect of a team training intervention in this context.

  • Fluctuations of anisotropic flow in Pb+Pb collisions at √sNN = 5.02 TeV with the ATLAS detector

    • Multi-particle azimuthal cumulants are measured as a function of centrality and transverse momentum using 470 μb−1 of Pb+Pb collisions at sNN = 5.02 TeV with the ATLAS detector at the LHC. These cumulants provide information on the event-by-event fluctuations of harmonic flow coefficients vn and correlated fluctuations between two harmonics vn and vm. For the first time, a non-zero four-particle cumulant is observed for dipolar flow, v1. The four-particle cumulants for elliptic flow, v2, and triangular flow, v3, exhibit a strong centrality dependence and change sign in ultra-central collisions. This sign change is consistent with significant non-Gaussian fluctuations in v2 and v3. The four-particle cumulant for quadrangular flow, v4, is found to change sign in mid-central collisions. Correlations between two harmonics are studied with three- and four-particle mixed-harmonic cumulants, which indicate an anti-correlation between v2 and v3, and a positive correlation between v2 and v4. These correlations decrease in strength towards central collisions and either approach zero or change sign in ultra-central collisions. To investigate the possible flow fluctuations arising from intrinsic centrality or volume fluctuations, the results are compared between two different event classes used for centrality definitions. In peripheral and mid-central collisions where the cumulant signals are large, only small differences are observed. In ultra-central collisions, the differences are much larger and transverse momentum dependent. These results provide new information to disentangle flow fluctuations from the initial and final states, as well as new insights on the influence of centrality fluctuations.     • A search for direct pair production of scalar partners of the top quark (top squarks or scalar third-generation up-type leptoquarks) in the all-hadronic t (t) over bar plus missing transverse momentum final state is presented. The analysis of 139 fb(-1) of root s = 13 TeV proton-proton collision data collected using the ATLAS detector at the LHC yields no significant excess over the Standard Model background expectation. To interpret the results, a supersymmetric model is used where the top squark decays via (t) over tilde -> t(*) (chi) over tilde (0)(1), with t(*) denoting an on-shell (off-shell) top quark and (chi) over tilde (0)(1) the lightest neutralino. Three specific event selections are optimised for the following scenarios. In the scenario where m((t) over tilde) > m(t) + m((chi) over tilde 10), top squark masses are excluded in the range 400-1250 GeV for (chi) over tilde (0)(1) masses below 200 GeV at 95% confidence level. In the situation where m((t) over tilde) similar to m(t) + m((chi) over tilde 10), top squarkmasses in the range 300-630 GeV are excluded, while in the case where m((t) over tilde) < m(W) + m(b) + m(<(chi)over tilde>10) (with m((t) over tilde) - m((chi) over tilde 10) >= 5 GeV), considered for the first time in an ATLAS all-hadronic search, top squark masses in the range 300-660 GeV are excluded. Limits are also set for scalar third-generation up-type leptoquarks, excluding leptoquarks with masses below 1240 GeV when considering only leptoquark decays into a top quark and a neutrino.
