  • "I can make more from selling medicine when breaking the rules" : understanding the antibiotic supply network in a rural community in Viet Nam

    • BACKGROUND: As in many other low and middle income countries (LIMCs), Vietnam has experienced a major growth in the pharmaceutical industry, with large numbers of pharmacies and drug stores, and increasing drug expenditure per capita over the past decade. Despite regulatory frameworks that have been introduced to control the dispensing and use of prescription-only drugs, including antibiotics, compliance has been reported to be strikingly low particularly in rural parts of Vietnam. This qualitative study aimed to understand antibiotic access and use practices in the community from both supplier and consumer perspectives in order to support the identification and development of future interventions.METHODS: This qualitative study was part of a project on community antibiotic access and use (ABACUS) in six LMICs. The focus was Ba Vi district of Hanoi capital city, where we conducted 16 indepth interviews (IDIs) with drug suppliers, and 16 IDIs and 6 focus group discussions (FGDs) with community members. Drug suppliers were sampled based on mapping of all informal and formal antibiotic purchase or dispensing points in the study area. Community members were identified through local networks and relationships with the field collaborators. All IDIs and FGDs were audio-taped, transcribed and analysed using content analysis.RESULTS: We identified a large number of antibiotic suppliers in the locality with widespread infringements of regulatory requirements. Established reciprocal relationships between suppliers and consumers in drug transactions were noted, as was the consumers' trust in the knowledge and services provided by the suppliers. In addition, antibiotic use has become a habitual choice in most illness conditions, driven by both suppliers and consumers.CONCLUSIONS: This study presents an analysis of the practices of antibiotic access and use in a rural Vietnamese setting. It highlights the interactions between antibiotic suppliers and consumers in the community and identifies possible targets for interventions.

  • N,2,6-Trisubstituted 1H-benzimidazole derivatives as a new scaffold of antimicrobial and anticancer agents : design, synthesis, in vitro evaluation, and in silico studies

    • Compounds containing benzimidazole moiety occupy privileged chemical space for discovering new bioactive substances. In continuation of our recent work, 69 benzimidazole derivatives were designed and synthesized with good to excellent yields of 46-99% using efficient synthesis protocol i.e. sodium metabisulfite catalyzed condensation of aromatic aldehydes with o-phenylenediamines to form 2-arylbenzimidazole derivatives followed by N-alkylation by conventional heating or microwave irradiation for diversification. Potent antibacterial compounds against MSSA and MRSA were discovered such as benzimidazole compounds 3k (2-(4-nitrophenyl), N-benzyl), 3l (2-(4-chlorophenyl), N-(4-chlorobenzyl)), 4c (2-(4-chlorophenyl), 6-methyl, N-benzyl), 4g (2-(4-nitrophenyl), 6-methyl, N-benzyl), and 4j (2-(4-nitrophenyl), 6-methyl, N-(4-chlorobenzyl)) with MIC of 4-16 mu g mL(-1). In addition, compound 4c showed good antimicrobial activities (MIC = 16 mu g mL(-1)) against the bacteria strains Escherichia coli and Streptococcus faecalis. Moreover, compounds 3k, 3l, 4c, 4g, and 4j have been found to kill HepG2, MDA-MB-231, MCF7, RMS, and C26 cancer cells with low mu M IC50 (2.39-10.95). These compounds showed comparable drug-like properties as ciprofloxacin, fluconazole, and paclitaxel in computational ADMET profiling. Finally, docking studies were used to assess potential protein targets responsible for their biological activities. Especially, we found that DHFR is a promising target both in silico and in vitro with compound 4c having IC50 of 2.35 mu M.

    • Dengue fever/dengue hemorrhagic fever (DF/DHF) has been an important public health challenge in Viet Nam and worldwide. This study was implemented in 2016-2017 using retrospective secondary data to explore associations between monthly DF/DHF cases and climate variables during 2008 to 2015. There were 48 175 DF/DHF cases reported, and the highest number of cases occurred in November. There were significant correlations between monthly DF/DHF cases with monthly mean of evaporation (r = 0.236, P < .05), monthly relative humidity (r = −0.358, P < .05), and monthly total hours of sunshine (r = 0.389, P < .05). The results showed significant correlation in lag models but did not find direct correlations between monthly DF/DHF cases and monthly average rainfall and temperature. The study recommended that health staff in Hanoi should monitor DF/DHF cases at the beginning of epidemic period, starting from May, and apply timely prevention and intervention measures to avoid the spreading of the disease in the following months. A larger scale study for a longer period of time and adjusting for other potential influencing factors could better describe the correlations, modelling/projection, and developing an early warning system for the disease, which is important under the impacts of climate change and climate variability.

    • ABSTRACT: Rapid changes in lifestyle in Vietnam have had a significant impact on young people with increasing numbers suffering from sexually transmitted diseases. In addition to changes in lifestyle, Vietnamese adolescents face many other problems including reproductive health. It is therefore important to examine their understanding of these topics. The purpose of this study is to assess Vietnamese adolescents’ views on puberty and reproductive health and the resources that may benefit them. The study consisted of interviews with 24 children aged 12-13 years old in two secondary schools, using an open interview guide and adopting qualitative content analysis. Experiences of puberty, the concepts of sexual intercourse, friendship and love were found to reflect adolescents’ understanding of these issues. The children’s knowledge was limited. Girls, more often than boys, had sought information from movies, books and from more experienced friends. Teachers, parents, and media played an important role in educating adolescents and encouraging them to make the right decisions. The adolescents’ perception of puberty and reproductive health was immature. Adolescents need more information to be able to prepare for their sexuality in addition to cope with the bodily changes.

  • Climate Services For Infectious Disease Control: A Nexus Between Public Health Preparedness and Sustainable Development, Lessons Learned From Long-Term Multi Site Time Series Analysis of Dengue Fever in Vietnam

    • Background: Climate Services provide valuable information for making actionable, data-driven decisions to protect public health in a myriad of manners. There is mounting global evidence of the looming threat climate change poses to human health, including the variability and intensity of infectious disease outbreaks in Vietnam and other low-resource and developing areas. In light of the Sustainable Development Goals, lessons learned from time-series analysis may inform public health preparedness strategies for sustainable urban development in terms of dengue epidemiology, surveillance, control, and early warnings.Subjects and Methods: Nearly 40 years of spatial and temporal (times-series) dataset of meteorological records, including rainfall, temperature, and humidity (among others) which can be predictors of dengue were assembled for all provinces of Vietnam and associated with case data reported to General Department of Preventive Medicine, Ministry of Health of Vietnam during the same period. Time series of climate and disease variables was analyzed for trends and changing patterns of those variables over time. The time-series statistical analysis methods sought to identify spatial (when possible) and temporal trends, seasonality, cyclical patterns of disease, and to discover anomalous outbreak events, which departed from expected epidemiological patterns and corresponding meteorological phenomena, such as El Nino Southern Oscillation (ENSO).Results: Analysis yielded largely conserved finding with other locations in South East Asia for larger Outbreak years and events such as ENSO. Seasonality, trend, and cycle in many provinces were persistent throughout the dataset, indicating strong potential for Climate Services to be used in dengue early warnings.Conclusion: Even public health practitioners, having adequate tools for dengue control available must plan and budget vector control and patient treatment efforts well in advance of large scale dengue epidemics to curb such events overall morbidity and mortality. Similarly, urban and sustainable development in Vietnam might benefit from evidence linking climate change, and ill-health events spatially and temporally in future planning. Long term analysis of dengue case data and meteorological records, provided a cases study evidence for emerging opportunities that on how refined climate services could contribute to protection of public health.

    • A highly efficient method for the direct construction of amide bonds via a selective cleavage of C-H and C=C bonds in indole structures using an iodine-promoted approach was developed. Mechanistic studies indicated the formation of superoxide radicals obtained from molecular oxygen activation as a key intermediate step, which provided a precursor for subsequent oxidative ring-opening and intermolecular cyclization. A broad range of quinazolin-4(3H)-ones and tryptanthrins were synthesized in moderate to good yields under mild and environmentally benign conditions.

  • Osteoporosis, a major health problem in Vietnam : life style factors and determinants of bone mass

    • While the prevalence of osteoporosis and risk factors for low bone mineral density (BMD) has been well documented in Caucasian populations, there is a lack of data from Asia. This work was designed to clarify to what extent osteoporosis could be regarded as a major public health problem in Vietnam. Furthermore, to elucidate the prevalence of certain risk factors, such as vitamin D deficiency and other determinants of bone mass as a basis to indentify high-risk individuals among the Vietnamese women and men. The clinical studies were designed as cross-sectional investigations using a multistage sampling scheme. Within the setting of northern Vietnam (latitude 21°N), districts were selected to represent urban and rural areas. In total 612 healthy women and 222 men aged 13-83 years were investigated. BMD was measured at the lumbar spine, femoral neck and total hip in all qualified subjects with dual energy X-ray absortiometry. Serum concentrations of 25(OH)D, parathyroid hormone, estrogen and testosterone were quantified by electrochemiluminescence immunoassay. Data on clinical history and lifestyle were collected by individual face-to-face interviews. Reference values for peak BMD were defined. These data allowed the calculation of T-scores and thus for the first time, an accurate identification of osteoporosis in a Vietnamese population. As determined at the femoral neck, the prevalence of osteoporosis was 17-23% in women and 9% in men. The results clearly suggest that osteoporosis is an important public health problem. Postmenopausal women living in urban areas experienced osteoporosis more than rural residents. Serum levels of 25(OH)D and estrogen were significantly associated with bone mass in both women and men. The prevalence of vitamin D deficiency (<20 ng/mL) was very high, 30% in women and 16% in men. An experimental study on the isoflavone content of different soymilk preparations was performed by HPLC (high pressure liquid chromatography). Values of isoflavones were very low, around 60-80 mg/L, and there were only 10-20% of bioactive aglycones. This is far below the reported threshold levels to exert significant effects on bone. In the future these data will be useful as a valuable reference base to diagnose osteoporosis and for the clinical management of its consequences. The high prevalence of vitamin D deficiency should raise the awareness of potentially important health issues such as osteoporosis but also about other serious diseases within the Vietnamese society.

  • Prediction of Flash Flood Susceptibility of Hilly Terrain Using Deep Neural Network: A Case Study of Vietnam

    • Flash floods are one of the most dangerous natural disasters, especially in hilly terrain, causing loss of life, property, and infrastructures and sudden disruption of traffic. These types of floods are mostly associated with landslides and erosion of roads within a short time. Most of Vietnam is hilly and mountainous; thus, the problem due to flash flood is severe and requires systematic studies to correctly identify flood susceptible areas for proper landuse planning and traffic management. In this study, three Machine Learning (ML) methods namely Deep Learning Neural Network (DL), Correlation-based Feature Weighted Naive Bayes (CFWNB), and Adaboost (AB-CFWNB) were used for the development of flash flood susceptibility maps for hilly road section (115 km length) of National Highway (NH)-6 in Hoa Binh province, Vietnam. In the proposed models, 88 past flash flood events were used together with 14 flash floods affecting topographical and geo-environmental factors. The performance of the models was evaluated using standard statistical measures including Receiver Operating Characteristic (ROC) Curve, Area Under Curve (AUC) and Root Mean Square Error (RMSE). The results revealed that all the models performed well (AUC > 0.80) in predicting flash flood susceptibility zones, but the performance of the DL model is the best (AUC: 0.972, RMSE: 0.352). Therefore, the DL model can be applied to develop an accurate flash flood susceptibility map of hilly terrain which can be used for proper planning and designing of the highways and other infrastructure facilities besides landuse management of the area.

  • Therapeutic hypothermia after perinatal asphyxia in Vietnam : medium-term outcomes at 18 months - a prospective cohort study

    • Aim To determine neurodevelopmental outcome at 18 months after therapeutic hypothermia for hypoxic-ischaemic encephalopathy (HIE) infants in Vietnam, a low-middle-income country. Method Prospective cohort study investigating outcomes at 18 months in severely asphyxiated outborn infants who underwent therapeutic hypothermia for HIE in Hanoi, Vietnam, during the time period 2016-2019. Survivors were examined at discharge and at 6 and 18 months by a neonatologist, a neurologist and a rehabilitation physician, who were blinded to the infants' clinical severity during hospitalisation using two assessment tools: the Ages and Stages Questionnaire (ASQ) and the Hammersmith Infant Neurological Examination (HINE), to detect impairments and promote early interventions for those who require it. Results In total, 130 neonates, 85 (65%) with moderate and 45 (35%) with severe HIE, underwent therapeutic hypothermia treatment using phase change material. Forty-three infants (33%) died during hospitalisation and in infancy. Among the 87 survivors, 69 (79%) completed follow-up until 18 months. Nineteen children developed cerebral palsy (8 diplegia, 3 hemiplegia, 8 dyskinetic), and 11 had delayed neurodevelopment. At each time point, infants with a normal or delayed neurodevelopment had significantly higher ASQ and HINE scores (p<0.05) than those with cerebral palsy. Conclusion The rates of mortality and adverse neurodevelopment rate were high and comparable to recently published data from other low-middle-income settings. The ASQ and HINE were useful tools for screening and evaluation of neurodevelopment and neurological function.
