  • Towards a European health research and innovation cloud (HRIC)

    • The European Union (EU) initiative on the Digital Transformation of Health and Care (Digicare) aims to provide the conditions necessary for building a secure, flexible, and decentralized digital health infrastructure. Creating a European Health Research and Innovation Cloud (HRIC) within this environment should enable data sharing and analysis for health research across the EU, in compliance with data protection legislation while preserving the full trust of the participants. Such a HRIC should learn from and build on existing data infrastructures, integrate best practices, and focus on the concrete needs of the community in terms of technologies, governance, management, regulation, and ethics requirements. Here, we describe the vision and expected benefits of digital data sharing in health research activities and present a roadmap that fosters the opportunities while answering the challenges of implementing a HRIC. For this, we put forward five specific recommendations and action points to ensure that a European HRIC: i) is built on established standards and guidelines, providing cloud technologies through an open and decentralized infrastructure; ii) is developed and certified to the highest standards of interoperability and data security that can be trusted by all stakeholders; iii) is supported by a robust ethical and legal framework that is compliant with the EU General Data Protection Regulation (GDPR); iv) establishes a proper environment for the training of new generations of data and medical scientists; and v) stimulates research and innovation in transnational collaborations through public and private initiatives and partnerships funded by the EU through Horizon 2020 and Horizon Europe.

  • Prevalence of Subclinical Coronary Artery Atherosclerosis in the General Population

    • Background: Early detection of coronary atherosclerosis using coronary computed tomography angiography (CCTA), in addition to coronary artery calcification (CAC) scoring, may help inform prevention strategies. We used CCTA to determine the prevalence, severity, and characteristics of coronary atherosclerosis and its association with CAC scores in a general population.Methods: We recruited 30 154 randomly invited individuals age 50 to 64 years to SCAPIS (the Swedish Cardiopulmonary Bioimage Study). The study includes individuals without known coronary heart disease (ie, no previous myocardial infarctions or cardiac procedures) and with high-quality results from CCTA and CAC imaging performed using dedicated dual-source CT scanners. Noncontrast images were scored for CAC. CCTA images were visually read and scored for coronary atherosclerosis per segment (defined as no atherosclerosis, 1% to 49% stenosis, or ≥50% stenosis). External validity of prevalence estimates was evaluated using inverse probability for participation weighting and Swedish register data.Results: In total, 25 182 individuals without known coronary heart disease were included (50.6% women). Any CCTA-detected atherosclerosis was found in 42.1%; any significant stenosis (≥50%) in 5.2%; left main, proximal left anterior descending artery, or 3-vessel disease in 1.9%; and any noncalcified plaques in 8.3% of this population. Onset of atherosclerosis was delayed on average by 10 years in women. Atherosclerosis was more prevalent in older individuals and predominantly found in the proximal left anterior descending artery. Prevalence of CCTA-detected atherosclerosis increased with increasing CAC scores. Among those with a CAC score >400, all had atherosclerosis and 45.7% had significant stenosis. In those with 0 CAC, 5.5% had atherosclerosis and 0.4% had significant stenosis. In participants with 0 CAC and intermediate 10-year risk of atherosclerotic cardiovascular disease according to the pooled cohort equation, 9.2% had CCTA-verified atherosclerosis. Prevalence estimates had excellent external validity and changed marginally when adjusted to the age-matched Swedish background population.Conclusions: Using CCTA in a large, random sample of the general population without established disease, we showed that silent coronary atherosclerosis is common in this population. High CAC scores convey a significant probability of substantial stenosis, and 0 CAC does not exclude atherosclerosis, particularly in those at higher baseline risk.

  • Evaluating P-Band TomoSAR for Biomass Retrieval in Boreal Forest

    • P-band synthetic aperture radar (SAR) is sensitive to above-ground biomass (AGB) but retrieval accuracy has been shown to deteriorate in topographic areas. In boreal forest, the signal penetrates through the canopy to interact with the ground producing variations in backscatter depending on ground topography, forest structure, and soil moisture. Tomographic processing of multiple SAR images Tomographic SAR (TomoSAR) provides information about the vertical backscatter distribution. This article evaluates the use of P-band TomoSAR data to improve AGB retrievals from backscattered intensity by suppressing the backscattered signal from the ground. This approach can be used even when the tomographic resolution is insufficient to resolve the vertical backscatter profile. The analysis is based on P-band data from two campaigns: BioSAR-1 (2007) in Remingstorp, southern Sweden, and BioSAR-2 (2008) in Krycklan (KR), northern Sweden. BioSAR airborne data were also processed to correspond as closely as possible to future BIOMASS TomoSAR acquisitions, with BioSAR-2-based results shown. A power law AGB model using volumetric HV polarized backscatter performs best in KR, with training residual root mean-squared error (RMSE) of 30%-36% (27-33 t/ha) for airborne data and 38%-39% for simulated BIOMASS data. Airborne TomoSAR data suggest that both vertical and horizontal tomographic resolution are of importance and that it is possible to greatly reduce AGB retrieval bias when compared with airborne P-band SAR backscatter intensity-based retrievals. A lack of significant ground slopes in Remningstorp reduces the benefit of using TomoSAR data which performs similar to retrievals based solely on P-band SAR backscatter intensity.

  • Fifty years’ follow up of childhood epilepsy - medical and psycho-social outcomes

    • Purpose: To describe the long-term prognosis of childhood epilepsy in general, with special emphasis on seizure remission, relapse, medication, associated neurological impairment, mortality rate, cause of death, the experience of a childhood diagnosis of epilepsy and its consequences for the experiences of daily life.Methods: A prospective longitudinal study on a population-based total cohort of 195 children with epilepsy in 1962-1964. Data were collected from medical records, questionnaires and additional interviews were conducted. Results: Follow-up data from 94% of the initial cohort showed the best long-term prognosis for seizure freedom for children with no intellectual or neurological impairment. These children had later seizure onset, shorter total duration of epilepsy and, they were more often medication free. Generalized, rather than focal, epilepsy had a better outcome. Ninety per cent, with onset during the inclusion period 1962-1964, were seizure-free after 50 years. No one in this group died in SUDEP. The standardized mortality ratio (SMR) was 2. 61. Death at young age was due to neurological impairment or epilepsy-related conditions whereas later deaths often followed non–epilepsy-related conditions. Few persons reported that the childhood diagnosis of epilepsy had affected their choice of education, work or leisure activities. However, 20% reported that the diagnosis had caused problems in school or at work and had restricted their activities of daily living. Almost all reported no anxiety (82%) and no depression (90%). The interviews revealed a good balance between ‘Controlling and managing the situation’ and ‘Not being restricted by the condition’.Conclusion: This 50-year, long-term, follow-up of a cohort of persons with childhood epilepsy in general demonstrates a better outcome for seizure freedom, relapses and minor consequences for education, work and leisure activities than earlier reported. Most of the participants had developed strategies to manage their situation.

  • Pulmonary function and atherosclerosis in the general population : causal associations and clinical implications

    • Reduced lung function is associated with cardiovascular mortality, but the relationships with atherosclerosis are unclear. The population-based Swedish CArdioPulmonary BioImage study measured lung function, emphysema, coronary CT angiography, coronary calcium, carotid plaques and ankle-brachial index in 29,593 men and women aged 50–64 years. The results were confirmed using 2-sample Mendelian randomization. Lower lung function and emphysema were associated with more atherosclerosis, but these relationships were attenuated after adjustment for cardiovascular risk factors. Lung function was not associated with coronary atherosclerosis in 14,524 never-smokers. No potentially causal effect of lung function on atherosclerosis, or vice versa, was found in the 2-sample Mendelian randomization analysis. Here we show that reduced lung function and atherosclerosis are correlated in the population, but probably not causally related. Assessing lung function in addition to conventional cardiovascular risk factors to gauge risk of subclinical atherosclerosis is probably not meaningful, but low lung function found by chance should alert for atherosclerosis.

  • The effect of electrode composition on bimetallic AgAu nanoparticles produced by spark ablation

    • A flexible way to generate bimetallic nanoparticles with high control of their composition is to use spark ablation of alloyed electrodes. It has been generally accepted and stated that particles produced using spark ablation of alloyed electrodes obtain the same chemical composition as the electrodes. However, we identify a lack of studies fully supporting the connection between electrode and particle composition, presented in a small literature survey. The aim of the study is, hence, to explore the validity of the statement by analysing the relation between alloyed electrodes and their resulting particle composition using three sets of AgAu electrodes containing Au and 25, 50, and 75 atomic % Ag, respectively. The resulting composition is thoroughly investigated using both single particle (scanning- and transmission electron microscopy) and ensemble particle techniques (inductive coupled plasma-mass spectroscopy, x-ray photoelectron spectroscopy, x-ray fluorescence, and optical measurements of surface plasmon resonance. We also investigate how sample size (e.g., the number of particles analysed) affects the reliability of the resulting sample mean. For single-particle measurements of a sample with a compositional standard deviation of a few atomic percentage points, a sample size of 20 particles is a good benchmark for obtaining reliable results of the sample mean. Furthermore, this article aims to challenge the practice in which the composition of nanoparticles is measured, presented, and interpreted, to improve and facilitate future research related to this topic. From the results of this study, it could be concluded that for the investigated Ag–Au material system, the particles obtained a composition very similar to the alloyed AgAu electrodes.

    • Kapacitetstilldelningen för basunderhåll samt koordineringen med tågtrafiken fokuserar ofta på linjesträckorna, men en stor anläggningsmassa finns även på större driftplatser och knutpunkter. På sådana stationer behöver arbetsområden utformas som både medger ett effektivt underhåll och en rimlig trafikpåverkan. Trafikverket uppmärksammade denna fråga i projektet ”Öka nyttjandegrad av servicefönster” och lyfte där fram behovet av att dela in större driftplatser i arbetsområden (öar) som kan nyttjas effektivt. Tanken är att sådana förutbestämda områden ska underlätta både för entreprenörer, trafik-ledning, och planering, exempelvis vid utformning av servicefönster, samt ge en tydlighet i hur tågtrafiken påverkas, vilket gynnar operatörerna.  Trafikverket har arbetat med så kallade ”öar” sedan 2015. Initiativet togs av trafikledningen i Göteborg där man hade återkommande problem med att störningar i underhållsarbeten kunde ge följdförseningar på en stor mängd linjer och stråk. Genom att definiera spårindelningar anpassade efter trafikanvändningen ville man minska störningarna, men också skapa större områden som var utprovade i förväg och medgav arbete på flera anläggningsdelar.  Erfarenheterna från Göteborg är goda och successivt har detta arbetssätt börjat användas på flera ställen i landet. Däremot har Trafikverket ibland haft svårigheter att utforma väl fungerande och stabila arbets-områden/öar. Det saknas också en samlad kunskapsgrund, systematiska metoder och ett verktygsstöd för utformningen av arbetsområden samt hur själva framtagningen av dessa bör gå till, förutom att forskningsläget inte varit kartlagt. Dessa frågeställningar har nu utretts i förstudie-projektet UFO (Utformning av underhållsområden på större driftplatser), vilket redovisas i denna presentation. Arbetet har bestått av tre delar: en verksamhetsstudie för att ta reda på krav- och behovsbilden; en litteraturstudie för att undersöka forskningsläget och publicerade metoder för liknande frågeställningar; samt en modellstudie för att undersöka olika lösnings- och arbetsmetoder som kan användas för att utforma underhållsområden på större stationer.  Verksamhetsstudien har resulterat i en drygt 30-sidig forskningsrapport, baserad på en serie intervjuer och granskad av berörda parter på Trafikverket. Kravbilden vad gäller allmänna egenskaper, underhåll, planering, trafikledning, samt målkonflikter och värdering, visar på den komplexitet och stora mängd aspekter som behöver hanteras.

  • Complete and long-lasting clinical responses in immune checkpoint inhibitor-resistant, metastasized melanoma treated with adoptive T cell transfer combined with DC vaccination

    • Development of T cell-directed immune checkpoint inhibitors (ICI) has revolutionized metastatic melanoma (MM) therapy, but <50% of treated patients experience durable responses. This phase I trial (NCT01946373) investigates the safety/feasibility of tumor-infiltrating lymphocyte (TIL) adoptive cell therapy (ACT) combined with dendritic cell (DC) vaccination in MM patients progressing on ICI. An initial cohort (5 patients) received TIL therapy alone to evaluate safety and allow for optimization of TIL expansion protocols. A second cohort (first-in-man, 5 patients) received TIL combined with autologous tumor lysate-loaded DC vaccination. All patients received cyclophosphamide/fludarabine preconditioning prior to, and intravenous (i.v.) IL-2 after, TIL transfer. The DC vaccine was given as five intradermal injections after TIL and IL-2 administration. [F-18]-FDG PET/CT radiology was performed to evaluate clinical response, according to RECIST 1.1 (on the CT part). Immunological monitoring was performed by flow cytometry and T-cell receptor (TCR) sequencing. In the safety/optimization cohort, all patients had a mixed response or stable disease, but none durable. In the combination cohort, two patients experienced complete responses (CR) that are still ongoing (>36 and >18 months, respectively). In addition, two patients had partial responses (PR), one still ongoing (>42 months) with only a small bone-lesion remaining, and one of short duration (<4 months). One patient died early during treatment and did not receive DC. Long-lasting persistency of the injected TILs was demonstrated in blood. In summary, we report clinical responses by TIL therapy combined with DC vaccination in 4 out of 4 treated MM patients who previously failed ICI.
