The Niebauer-Cutter prosthesis in excision arthroplasty of the trapezium

- Sixteen patients had excision arthroplasty of the trapezium, using the Niebauer-Cutter prosthesis. The average period of postoperative follow-up was thirteen months. All patients had severe preoperative disabilities and the majority were relieved from resting pain and pain during active use of their hands. The postoperative range of motion with regard to the palmar and radial abduction was found to be satisfactory, while varying degrees of limitation of circumduction was the rule. There was no correlation between the patients' own judgment of the postoperative benefits and the objectively measured range of motion. One patient had a luxation of the prosthesis, while another had a subluxation when opposing the thumb. This frequency of luxation/subluxation seems to be less than that experienced with the other common type of prosthesis, the Swanson prosthesis. Although it is obvious that the ideal prosthesis for treatment of disabilities in the carpometacarpal joint of the thumb is still not available, it was concluded that the Niebauer-Cutter prosthesis is the best choice at the present.

Scintigraphy, radiography, and acid clearing in dysphagia patients after anti-reflux surgery

- Oesophageal emptying was studied with scintigraphy, radiography, and the acid clearing test (ACT) in 18 patients reporting dysphagia and previously operated on with fundoplication. Radiography with contrast medium, isodense with water, revealed abnormalities in either motility or emptying capacity in 39% (7/18). A A barium meal showed abnormalities--that is, a tight repair, disruption of the fundoplication, or recurrence of the hernia--in 56% (10/18). The ACT was prolonged in 40% (6/15) of the patients. Pathological findings at scintigraphy with a solid bolus were found in 67% (12/18). Even if scintigraphy with a solid bolus is the method that identifies the highest number of patients with impaired oesophageal function among the tests used, it cannot differentiate between functional and anatomical disorders. A barium meal examination is the method of choice when an anatomical disorder is suspected.

- In der urologischen Praxis ist die Hämaturie ein häufiges Symptom, dessen Genese Patient und Arzt geklärt haben möchten. Die üblichen diagnostischen Maßnahmen sind chemische und mikroskopische Untersuchung des Harns sowie Urinkultur, Zystoskopie mit Zytologie und Urographie evt. mit Tomographie. Im Rahmen dieser Untersuchungen werden bei etwa 22 Prozent der Patienten maligne Tumoren und in etwa 70 Prozent der Fälle andere Blutungsursachen in den Nieren oder Harnwegen gefunden. In 8 bis 10 Prozent der Fälle verbleibt die Diagnose dagegen unklar.

- A prospective, randomized trial of the effect of graded compression of the calf was done in 62 patients operated on electively for hip disease. After total hip arthroplasty, compression reduced the number of positive fibrinogen uptake tests by two-thirds, significant only in males, who seem to run a higher risk of thrombosis than females.
