Vitamin D deficiency as a risk factor for cystic fibrosis-related diabetes in the Scandinavian Cystic Fibrosis Nutritional Study

- Aims/hypothesis Many cystic fibrosis patients are vitamin D-insufficient. Cystic fibrosis-related diabetes is a major complication of cystic fibrosis. The literature suggests that vitamin D might possess certain glucose-lowering properties. We aimed to assess the relationship between vitamin D and cystic fibrosis-related glucose intolerance. Methods We enrolled 898 cystic fibrosis patients from Sweden, Norway and Denmark. Vitamin D intake was assessed using a seven-day food record. Serum 25-hydroxyvitamin D (s25OHD) and HbA(1c) were measured, and an OGTT was carried out. Multiple linear and logistic regressions were used for HbA(1c) and cystic fibrosis-related diabetes/OGTT result as outcome variables, respectively. Each model was controlled for country, and for known cystic fibrosis-related diabetes risk factors: age, sex, genotype, liver dysfunction, long-term corticosteroid treatment, and lung and pancreatic function. Results Degree of vitamin D insufficiency (OR 1.36; p=0.032) and s25OHD<30 nmol/l (OR 1.79; p=0.042) were significant risk factors for cystic fibrosis-related diabetes. Accordingly, HbA(1c) value was positively associated with s25OHD<30 nmol/l and<50 nmol/l, as well as with degree of vitamin D insufficiency (adjusted R-2=20.5% and p<0.05 in all). In subgroup analyses, s25OHD<30 nmol/l determined the HbA(1c) value in paediatric patients (adjusted R-2=20.2%; p=0.017), but not in adults. Conclusions/interpretation Vitamin D status is associated with HbA(1c) and diabetes in cystic fibrosis, particularly in children. The study justifies prospective studies on the proposed role of vitamin D deficiency in the pathophysiology of diabetes mellitus.

- Bakgrund: Uppsatsen kom till efter en förfrågan från CSU, Centrum för skolutveckling vid Högskolan i Borås. Uppdraget var att se på nya lärares arbetssituation. De ville att jag skulle undersökta om lärarna vantrivs i arbetet och om de hoppar av yrket i förtid. Jag valde att genom en enkätundersökning ta reda på hur det ser ut i Borås, Alingsås, Bollebygd, Mark, Svenljunga, Tranemo, Vårgårda och Ulricehamn. Då jag själv är verksam som 4-9 lärare, lärarutbildare, handledare för lärarstudenter och mentor för nya lärare tyckte jag att ämnet var intressant ur många olika synvinklar. Utgångsfrågor i arbetet har varit: Hur ska arbetsgivare och lärarutbildningar agera för att behålla lärare i yrket? Hur hjälper vi de nya rent praktiskt ute på fältet? Hur motsvarar lärarutbildningen de krav som ställs på nya lärare idag? Syfte: Syftet med studien är att kartlägga hur nya lärare upplever sin arbetssituation. Överväger många att hoppa av yrket redan efter kort tid? Motsvarar lärarutbildningen de krav som ställs på lärare i dagens skola? Vantrivs de i arbetet? Metod: Min metod är en enkätundersökning med dels slutna, dels öppna frågor. Genom enkäten tar jag reda på hur nyexaminerade lärare har det i arbetet. Enkäten vill spegla många lärares uppfattningar om yrket. Med de öppna frågorna vill jag visa lärarnas känslomässiga uppfattningar. Resultat: Resultatet visar att lärarna överlag har positiva erfarenheter efter sin första tid i yrket. De nya har god kontakt med kolleger, elever och föräldrar. Att läraryrket är omväxlande och ingen dag är den andra lik uppskattas av de svarande. De ser utvecklingsmöjligheter i arbetet och har klart fastställda mål för sin verksamhet. Det som ses som mest positivt är undervisningstiden och kontakten med eleverna. Däremot ses allt arbete runt omkring och konferenstiden som mer negativt. Konfliktlösning, samtalsteknik och specialpedagogik är ämnen de saknar från sin utbildning. Vad som inte blev som de tänkt sig är stressen och tiden som inte räcker till. För övrigt verkar deras högt ställda förväntningar infrias i hög grad.

- An apparatus was constructed for on-line measurements of the concentration of iodine species in aqueous and gas phases using radioactive tracers. The apparatus and procedures for determining detector efficiencies and flow rates of both phases are described in detail. A complete description of experimental procedures is given and reactions between gaseous iodine and reactor containment construction materials such as copper, zinc and aluminium are investigated using the apparatus.

- Purpose: To compare, absolute cerebral blood flow (CBF) estimates obtained by dynamic susceptibility contrast MRI (DSC-MRI) and Xe-133 SPECT. Materials and Methods: CBF was measured in 20 healthy volunteers using DSC-MRI at 3T and Xe-133 SPECT. DSC- MRI was accomplished by gradient-echo EPI and CBF was calculated using a time-shift-insenisitive deconvolution algorithm and regional arterial input functions (AIFs). To improve the reproducibility of AIF registration the time integral was rescaled by use, of a venous output function. In the Xe-133 SPECT experiment, Xe-133 gas was inhaled over 8 minutes and CBF was calculated using a biexponential analysis. Results: The average whole-brain CBF estimates obtained by DSC-MRI and Xe- 133 SPECT were 85 +/- 23 mL/(min 100 g) and 40 +/- 8 mL/(min 100 g), respectively (mean +/- SD, n = 20). The linear CBF relationship between the two modalities showed a correlation coefficient of r = 0.76 and was described by the equation CBF(MRI) = 2.4 CBF(Xe) - 7.9 (CBF in units of mL/(min 100 g)). Conclusion: A reasonable positive linear correlation between MRI-based and SPECT-based CBF estimates was observed after AIF time-integral correction. The use of DSC-MRI typically results in overestimated absolute perfusion estimates and the present study indicates that this trend is further enhanced by the use of high magnetic field strength (3T).

- This presentation discusses how perception of pictures, of foremost faces, might be processed in the brain. Research in cognitive psychology suggests that pictures of faces are processed in to some extent specific areas of the brain (fusiform gyrus), than those showing other objects. Pictures of faces seem to be perceived in a holistic way, that is, dealt with in parallel (several features are processed at the same time). May ordinary pictures of faces, which are processed holistically (as Gestalt) also be said to be typical for iconic perception, in contrast to perception of index? We therefore also put the question: do we perceive so called Mooney-pictures of faces (which are flat and have only two intensity levels) in serial, and may we identify the process with perception of indexical signs? We will seek some tentative answers to those questions by relating to contemporary research in cognition and visual perception.

- PURPOSE: To compare absolute cerebral blood flow (CBF) estimates obtained by model-free arterial spin labeling (ASL) and dynamic susceptibility contrast MRI (DSC-MRI), corrected for partial volume effects (PVEs). METHODS: CBF was measured using DSC-MRI and model-free ASL (quantitative signal targeting with alternating radiofrequency labeling of arterial regions) at 3 T in 15 subjects with brain tumor, and the two modalities were compared with regard to CBF estimates in normal gray matter (GM) and DSC-to-ASL CBF ratios in selected tumor regions. The DSC-MRI CBF maps were calculated using a global arterial input function (AIF) from the sylvian-fissure region, but, in order to minimize PVEs, the AIF time integral was rescaled by a venous output function time integral obtained from the sagittal sinus. RESULTS: In GM, the average DSC-MRI CBF estimate was 150+/-45 ml/(min 100 g) (mean+/-SD) while the corresponding ASL CBF was 44+/-10 ml/(min 100 g). The linear correlation between GM CBF estimates obtained by DSC-MRI and ASL was r=.89, and observed DSC-to-ASL CBF ratios differed by less than 3% between GM and tumor regions. CONCLUSIONS: A satisfactory positive linear correlation between the CBF estimates obtained by model-free ASL and DSC-MRI was observed, and DSC-to-ASL CBF ratios showed no obvious tissue dependence.
