  • Exercise intolerance in adult congenital heart disease : comparative severity, correlates, and prognostic implication.

    • BACKGROUND: Although some patients with adult congenital heart disease (ACHD) report limitations in exercise capacity, we hypothesized that depressed exercise capacity may be more widespread than superficially evident during clinical consultation and could be a means of assessing risk.METHODS AND RESULTS: Cardiopulmonary exercise testing was performed in 335 consecutive ACHD patients (age, 33+/-13 years), 40 non-congenital heart failure patients (age, 58+/-15 years), and 11 young (age, 29+/-5 years) and 12 older (age, 59+/-9 years) healthy subjects. Peak oxygen consumption (peak VO2) was reduced in ACHD patients compared with healthy subjects of similar age (21.7+/-8.5 versus 45.1+/-8.6; P<0.001). No significant difference in peak VO2 was found between ACHD and heart failure patients of corresponding NYHA class (P=NS for each NYHA class). Within ACHD subgroups, peak VO2 gradually declined from aortic coarctation (28.7+/-10.4) to Eisenmenger (11.5+/-3.6) patients (P<0.001). Multivariable correlates of peak VO2 were peak heart rate (r=0.33), forced expiratory volume (r=0.33), pulmonary hypertension (r=-0.26), gender (r=-0.23), and body mass index (r=-0.19). After a median follow-up of 10 months, 62 patients (18.5%) were hospitalized or had died. On multivariable Cox analysis, peak VO2 predicted hospitalization or death (hazard ratio, 0.937; P=0.01) and was related to the frequency and duration of hospitalization (P=0.01 for each).CONCLUSIONS: Exercise capacity is depressed in ACHD patients (even in allegedly asymptomatic patients) on a par with chronic heart failure subjects. Lack of heart rate response to exercise, pulmonary arterial hypertension, and impaired pulmonary function are important correlates of exercise capacity, as is underlying cardiac anatomy. Poor exercise capacity identifies ACHD patients at risk for hospitalization or death.

  • MORPHOMETRY OF ALLUVIAL FANS IN A POLAR DESERT (SVALBARD, NORWAY): IMPLICATIONS FOR INTERPRETING MARTIAN FANS.

    • Alluvial fan-like landforms have been identified on Mars. Since alluvial fans are recorders of processes that are sensitive to climatic conditions, the investigation of possible Martian fans can reveal information about the planet`s climate. In lieu of direct observations of active depositional processes on Martian fans, comparisons with terrestrial analogues can constrain models of Martian fan formation derived from remote sensing data. Since present-day Mars is cold and dry, alluvial fans formed in cold deserts should be considered as useful analogues. The probably closest climatic analogue to Mars on Earth are the Antarctic Dry Valleys , but polar deserts can also be found in the Arctic. We report on our field work in summer 2008 and a simultaneous flight campaign with an airborne version (HRSC-AX) of the High Resolution Stereo Camera (HRSC) onboard Mars Express. The results are compared with measurements of Martian fans, based on HRSC DEM.

    • Small alluvial fan-like landforms have been observed on Mars in association with recent gullies. Such fans are relatively small (km-scale) when compared to large alluvial fans on Mars, which can reach dimensions of tens of kilometers. The small fans are part of an erosion-transportsedimentation system (alcove-channel-apron). Morphologically very similar systems can be studied on Svalbard (Norway), which is a cold and dry polar desert and, therefore, a potentially useful terrestrial analogue for Mars` climate and climaterelated landforms. Here we give an update on our field work in summer 2008 and a simultaneous flight campaign with an airborne version (HRSCAX) of the High Resolution Stereo Camera (HRSC) onboard Mars Express. We also present very preliminary results from the field campaign in summer 2009.

    • This review summarizes the status of prognostic and predictive markers in brain tumors with a focus on the most frequent tumors, gliomas. Brain tumors are a heterogeneous group of different tumors with a huge variation in outcome. Although the most common tumor, high-grade malignant glioma, still has a dismal prognosis, the last years have seen a significant improvement in the management in this tumor as well as in most other brain tumors. Age, tumor grade and KPS are still the most reliable prognostic and predictive variables available for patients with brain tumors. Although chromosome 1p/19q co-deletion and methylation status of the promoter of the MGMT gene (encoding O6-methylguanine-DNA methyl transferase) have been identified as the most promising potential predictors of response to chemotherapy in malignant gliomas, there are as yet no reliable biomarkers for tumour grading or tumour monitoring in the clinical setting.

  • Cardiac effects of endothelin receptor antagonism in endotoxemic pigs.

    • Myocardial depression in sepsis is frequently encountered clinically and contributes to morbidity and mortality. Increased plasma levels of endothelin-1 (ET-1) have been described in septic shock, and previous reports have shown beneficial effects on cardiovascular performance and survival in septic models using ET receptor antagonists. The aim of the current study was to investigate specific cardiac effects of ET receptor antagonism in endotoxicosis. Sixteen domestic pigs were anesthetized and subjected to endotoxin for 5 h. Eight of these pigs were given tezosentan (dual ET receptor antagonist) after 3 h. Cardiac effects were evaluated using the left ventricular (LV) pressure-volume relationship. Endotoxin was not associated with any effects on parameters of LV contractile function [end-systolic elastance (Ees), preload recruitable stroke work (PRSW), power(max)/end-diastolic volume (PWR(max)/EDV) and dP/dt(max)/end-diastolic volume (dP/dt(max)/EDV)] but with impairments in isovolumic relaxation (time constant for pressure decay, tau) and mechanical efficiency. Tezosentan administration decreased Ees, PWR(max)/EDV, and dP/dt(max)/EDV, while improving tau and LV stiffness. Thus, dual ET receptor antagonism was associated with a decline in contractile function but, in contrast, improved diastolic function. Positive hemodynamic effects from ET receptor antagonism in acute endotoxemia may be due to changes in cardiac load and enhanced diastolic function rather than improved contractile function.

    • The morphology of gullies on Mars indi-cates that they were formed by the action of water. How-ever, it is not clear what the dominant gully forming process is. On Earth, gullies might have formed by the processes of overland flow or debris flows, or a combination of both. In this work we compare the morphologic and morphometric observations and measurements (slopes, depth-width ratios and sinuosities) of Martian gullies with terrestrial analogs from Svalbard, Norway, in order to constrain the formation process (fluvial and/or debris flow).

    • This paper provides a detailed overview of developments in transducer materials technology relating to their current and future applications in micro-scale devices. Recent advances in piezoelectric, magnetostrictive and shape-memory alloy systems are discussed and emerging transducer materials such as magnetic nanoparticles, expandable micro-spheres and conductive polymers are introduced. Materials properties, transducer mechanisms and end applications are described and the potential for integration of the materials with ancillary systems components is viewed as an essential consideration. The review concludes with a short discussion of structural polymers that are extending the range of micro-fabrication techniques available to designers and production engineers beyond the limitations of silicon fabrication technology.

    • A bistridentate RuII-polypyridine complex [Ru(bqp)2]2+ (bqp = 2,6-bis(8'-quinolinyl)pyridine) has been prepared, which has a coordination geometry much closer to a perfect octahedron than the typical Ru(terpyridine)2-type complex. Thus, the complex displays a 3.0 mus lifetime of the lowest excited metal-to-ligand charge transfer (3MLCT) state at room temperature. This is, to the best of our knowledge, the longest MLCT state lifetime reported for a RuII-polypyridyl complex at room temperature. The structure allows for the future construction of rod-like, isomer-free molecular arrays by substitution of donor and acceptor moieties on the central pyridine units. This makes it a promising photosensitizer for applications in molecular devices for artificial photosynthesis and molecular electronics.
