Prostate Cancer Registration in Four Swedish Regions 1996 : Differences in Incidence, Age Structure and Management

- Introduction: In 1996 registration of prostate cancer in four of the six Swedish regions was started to facilitate evaluation of geographical variations in incidence and treatment.Material and methods: For all cases of prostate cancer, personal identification number, tumour stage, tumour grade and primary treatment were registered.Results: In the four regions covered by the register, 3541 cases of prostate cancer were registered. Altogether there were 5795 cases of prostate cancer diagnosed in Sweden the same year. The age-standardized incidence varied from 89/100 000 to 169/100 000 among counties. The proportion of localized tumours correlated positively to the incidence (p < 0.05) and negatively to mean age at diagnosis (p < 0.01). There was also a significant positive correlation between the proportion of localized tumours and the percentage of patients given curative treatment. All registered variables showed large geographical variations, especially concerning percentage of T1c tumours, treatment of localized tumours and choice of palliative treatment.Conclusion: Diagnostic activity varied considerably among counties, resulting in large variation in age-standardized incidence. High incidence is associated with a larger proportion of localized tumours, which, in turn, is associated with early age at diagnosis. In counties where a policy of detecting tumours early is practised, curative treatment is also given more often. Treatment of localized tumours and preference for palliative treatment seem to depend on local traditions. The lack of cytological and histopathological standards makes geographical comparisons based on tumour grade impossible.

- Banks are known to have stable and long lasting relations with their customers,which is why development of customer relations is a crucial strategic issue inbanking. However, a basic problem in banking is that the customer base isheterogeneous, ranging from homogenous segments of customers with the samepreferences to customers with individual demands for customized services. A keyissue in banking is thus how bank managers view different strategies to develop theircustomer relations. This paper investigates how bank branch managers perceive theheterogeneity of their market and how the bank’s relationship strategy affect thedevelopment of customer relations. To this end, 135 branch bank managersanswered a questionnaire, which was processed by LISREL. Results suggest thathow bank managers perceive customer relationship development is determined bythe heterogenous market and the bank’s relationship strategy. Contrary to ourexpectations, bank managers consider that more difficult relationship developmentleads to a more centralized locus of realized strategy. This suggests that banksrelationship development need to focus more on customer orientation, and thatrealized strategies need to support this.

- The subject of this thesis is the interpretation of tax legislation, particularly the use ofpreparatory works. The purpose of the thesis is to devise appropriate principles for theinterpretation of tax legislation in general and principles for the use of preparatoryworks in particular.The author discusses the different goals involved in interpreting tax legislation, such as achieving predictability, uniformity and facilitating reasonable courtdecisions. In this context, the effect. of the EC-membership on the use of Swedishpreparatory works in the interpretation of tax legislation is analysed.Subsequently, the use of preparatory works in the interpretation of tax legislationin the Swedish Supreme Administrative Court is examined, from the viewpoint of thegoals for the interpretation of tax legislation.Thereafter, the author discusses German doctrine on the sources of law and theuse of preparatory works in the highest German court of law for tax cases (Bundes-finanzhof). The focus is put on the question of whether any of the distinctive featuresof the German doctrine on the sources of law could contribute toattaining the goals ofthe interpretation of Swedish tax legislation.In the last chapter, the prerequisites of putting forward principles for theinterpretation of tax legislation are discussed and a proposal for an overall structurefor the interpretation of tax legislation is presented. This structure is based on analysisat two different levels. The first level consists of a prima facie analysis of basicprinciples governing the interpretation of tax legislation. The result of the prima facieanalysis is supplemented and tested against the goals of the interpretation of taxlegislation.
