  • Twelve years of fluconazole in clinical practice : Global-trends in species distribution and fluconazole susceptibility of bloodstream isolates of Candida

    • We determined the species distribution and in-vitro susceptibility of 6082 bloodstream infection (BSI) isolates of Candida spp. collected from 250 medical centres in 32 nations over a 10-year period from 1992 through 2001. The species included 3401 C. albicans, 984 C. glabrata, 796 C. parapsilosis, 585 C. tropicalis, 153 C. krusei, 67 C. lusitaniae, 48 C. guilliermondii, 10 C. famata, 10 C. kefyr, six C. pelliculosa, five C. rugosa, four C. lipolytica, three C. dubliniensis, three C. inconspicua, two C. sake and one isolate each of C. lambica, C. norvegensis and C. zeylanoides. Minimum inhibitory concentration determinations were made using the National Committee for Clinical Laboratory Standards reference broth microdilution method. Variation in the rank order and frequency of the different species of Candida was observed over time and by geographic area. The proportion of BSI due to C. albicans and C. glabrata increased and C. parapsilosis decreased over time in Canada, the USA and Europe. C. glabrata was an infrequent cause of BSI in Latin America and the Asia-Pacific region. Very little variation in fluconazole susceptibility was observed among isolates of C. albicans, C. tropicalis and C. parapsilosis. These species accounted for 78% of all BSI and remained highly susceptible (91-100% susceptible) to fluconazole from 1992 to 2001 irrespective of geographic origin. The prevalence of fluconazole resistance among C. glabrata isolates was variable both over time and among the various countries and regions. Resistance to fluconazole among C. glabrata isolates was greatest in the USA and varied by US census region (range 0-23%). These observations are generally encouraging relative to the sustained usefulness of fluconazole as a systemically active antifungal agent for the treatment of candida BSI. © 2004 Copyright by the European Society of Clinical Microbiology and Infectious Diseases.

    • Dynamic voltage scaling and adaptive body biasing have been shown to reduce dynamic and leakage power consumption effectively. In this paper, we optimally solve the combined supply voltage and body bias selection problem for multi-processor systems with imposed time constraints, explicitly taking into account the transition overheads implied by changing voltage levels. Both energy and time overheads are considered. We investigate the continuous voltage scaling as well as its discrete counterpart, and we prove NP-hardness in the discrete case. Furthermore, the continuous voltage scaling problem is formulated and solved using nonlinear programming with polynomial time complexity, while for the discrete problem we use mixed integer linear programming. Extensive experiments, conducted on several benchmarks and a real-life example, are used to validate the approaches.

    • In this paper, we propose a new technique for the combined voltage scaling of processors and communication links, taking into account dynamic as well as leakage power consumption. The voltage scaling technique achieves energy efficiency by simultaneously scaling the supply and body bias voltages in the case of processors and buses with repeaters, while energy efficiency on fat wires is achieved through dynamic voltage swing scaling. We also introduce a set of accurate communication models for the energy estimation of voltage scalable embedded systems. In particular, we demonstrate that voltage scaling of bus repeaters and dynamic adaption of the voltage swing on fat wires can significantly influence the system's energy consumption. Experimental results, conducted on numerous generated benchmarks and a real-life example, demonstrate that substantial energy savings can be achieved with the proposed techniques.

    • We have applied the Medipix2 pixel CMOS chip as direct anode readout for a TPC. For the gas amplification two options have been investigated: (i) a three-stage GEM system and (ii) a Micromegas mesh. The structure of the cloud of primary electrons, left after interactions of 55Fe quanta with the gas is visible with unprecedented precision. This proof-of-principle is an essential step in our project to realize a monolithic pixel sensor with integrated Micromegas, to be developed specially for the readout of TPCs, and applicable for drift chambers in general.
