- Primary adrenal insufficiency (PAI) may be less prevalent in South Africa and specifically among blacks. There are a number of possible reasons for this, with under-diagnosis among South African blacks, being the most likely explanation. Deaths from undiagnosed PAI still occur. © 2012 Blackwell Publishing Ltd.
