  • The authors present the clinical scenario of a 75-year-old female patient presenting with a recurrent liposarcoma on the right cheek. As the oncological margins comprised more than 60% of the cheek aesthetic subunit, the defect was extended to the subunit boundaries. The reconstruction was successfully achieved by total subunit reconstruction of the cheek with a free anterolateral thigh flap.Reconstruction of a total facial aesthetic subunit with free flaps yields better final aesthetic outcomes by avoiding the patch appearance. Secondary reconstructions built on top of a solid primary reconstruction are planned from the start.
